A Profitability Analysis of Sixteen Oriental Medicine Hospitals (한의과대학부속 한방병원의 손익분석 - 2007~2009년도 7개 대학의 16개 부속한방병원을 중심으로 -)

  • The purpose of this paper is to review the profitability of sixteen university hospitals of oriental medicine during 2007~2009. Data was collected from 16 hospitals that reveal financial statements to the public and the hospitals were classified into 7 groups. Net sales and COGS per 100 beds of the 7 groups were analyzed to measure profitability and as a percentage of net sales, the rates of personnel expenses, maintenance costs, and material costs were evaluated. The results showed that six groups had consecutively recorded net loss in medical services and differences in profitability among 7 groups were substantial. The analysis showed the profitability was significant for medium-sized hospitals. The rates of personnel expenses in net sales were above 50% in 6 groups with net loss and one group with net profit was 45%. This result indicate the rate of personnel expenses in net sales could be the main factor affecting profitability and further studies are recommended to analyze the determinants of profitability in oriental medical hospitals.

A study on the Current of Hospital Practices in Clinical Dietetics (임상영양사의 업무수행 현황에 관한 연구)

  • A study was undertaken to elucidate the clinical dietetic performance level and to identify the difference between the actual role performance and perception of important level of the clinical dietitian. The survey was carried out for the 21 general hospital over 100 beds in Seoul, Pusan and Daegu area. The results of the study can be summarized as follows. 1. One dietitian was responsible for the feeding and clinical dietetic activity of 166 in-patients on the average. 2. In the actual role performance of clinical dietetic practices, of the 26 items listed, only 3 were always or almost or almost performed by 75% of the dietitians, 4 additional items were always or almost performed by 60% of them. 3. In the perception of importance level of clinical dietetic practices, more than 75% of the dietitians indicated that 23 of the 26 tasks were important practices. 4. The performance average score of nutrition assessment was 15.85, of nutrition care plan development was 19.72, of patient counseling was 12.22, of research activity was 19.62, and of education was 21.27 in the dietetic department was a division for the clinical dietetic activity, while 10.96, 15.24, 9.54, 15.97, 18.50 respectively in the dietetic department has not a division for it.

  • The purpuse of this study is to find factors on the recognition by people involved in occupational safety and health of KOSHA 18001, the need of KOSHA 18001 certification, and the compliance by each medical institution with KOSHA 18001 review standards to help medical institutions introduce and use OSHMS. There was a survey of people involved in occupational safety and health at 300 local general hospitals with 100 or more beds that were registered with the Korean Hospital Association in 2012. The survey included the recognition of KOSHA 18001, the need of certification and the compliance by medical institutions of the occupational safety and health. A total of 132 people responded, of which an analysis of 126 cases was conducted except 6 cases poor in information.

Influence of Leadership Perception of Hospital Employees on Organizational Efficacy and Controlling Effects of Individual and Organizational Characteristics (병원 종사자의 리더십 지각이 조직효과성에 미치는 영향과 개인특성 및 조직특성의 조절효과)

  • The main purpose of this study is to derive the implications for managing effectively the leadership of hospital organization, by analyzing the effect of hospital employees' perception of the types of leadership on the organizational efficacy, and the controlling effect of individual and organizational characteristics. To test the model, survey was conducted using structural and self-administering questionnaires for 400 employees of the hospitals that have over 100 beds, in Seoul, Busan, Daegu, and Ulsan, from 29 March to 17 April, 2010, and 310 were used in final analysis. The results of the study suggest that the management methods to build the innovating leadership of managers are required in order to raise the organizational efficacy, and that searching of the methods to improve the achievement desire, self-esteem, and locus of control is required in order to maximize the effect of the innovating leadership.

Knowledge and Attitude Toward BLS and Provided CPR Education among Nurses at General Wards in Pusan (일부 일반병동 간호사의 기본 심폐소생술에 대한 지식, 태도 및 교육수혜 실태)

  • Purpose: A survey was done on knowledge and attitude toward CPR and provision of CPR education to nurses on general wards. Method: A survey of 280 nurses on general wards in four university hospitals with 700 beds or more in the city of Pusan was done. The data were collected from September 10, to September 22, 2007. Results: The mean score for knowledge of Basic Life Support was 12.71 (mean converted to 100: 63). About 76% of the nurses believed that they had a responsibility to perform CPR, but 53.3% of the nurses were not confident to perform CPR. About 94% of the nurses had received education on CPR but 32.3% of those took it 6 to 12 years ago. About 41% of the nurses spent 3-4 hours for the education and 73.2% of those took simulation education. Conclusion: Although most of the nurses had received CPR education, they were not knowledgeable or confident. Therefore, better CPR education programs including up-dated knowledge are needed. More reinforced education should be offered every six months or on a yearly basis in order to enhance lasting efficiency.

  • Pulmonary arteriovenous fistulas(PAVFs) is a rare disorder that occurs in two to three children per 100,000 population. It is presented as absence of intervening capillary beds between the pulmonary artery and vein with resultant persistent right to left shunt. Other causes include trauma, liver cirrhosis, malignancy and schistosomiasis. It is mostly asymptomatic, but it may present with respiratory difficulty, cyanosis, clubbed fingers induced by right to left shunt or hemoptysis, polycythemia and epistaxis. Major complications, such as brain abscess, brain embolism, paradoxical embolism and subacute infective endocarditis can be devastating, so therapeutic intervention is recommended in all patients. However, removal of low-resistance fistulas can aggrevate pulmonary hypertension, so detection of increased pulmonary pressure is important. We report two patients : One a 42 year-old male with PAVFs treated with coil embolization, and a 42 year-old female who was treated with anticoagulants due to pulmonary hypertension.

  • This study was conducted to develop a vehicle, leading or following a worker at a certain distance to assist laborious transporting works in greenhouses. A prototype vehicle, which consisted of the rear driving, the front steering and the console units, was designed and tested in the ideal indoor conditions. Results of this study were summarized as following: 1. The driving unit was designed to travel at the speed ranges of 0.3∼0.8 m/sec depending on the operating modes with a maximum payload of 100 kg. 2. The console unit consisted of a main-board including a 80C196KC microprocessor and peripheral devices, a power-board and safety interlock. Worker-leading, and following modes were available in automatic and manual modes. 3. Steering was achieved by turning the steering motor against the sensed direction. Proper steering angles for correcting travel direction were determined as 5 and 9 degrees when sensing cultivation beds and plants, respectively.

A Study of Human Resource Efficiency in Public Corporation Medical Centers (지방공사의료원의 인적자원 효율성평가)

  • This study applied Data Envelopment Analysis(DEA) and Ratio Analysis and Regression Analysis to a set of Korean Public Corporation Medical Centers to evaluate their relative human resource efficiencies. The output measure used in this study was based on health insurance system which was used in both in-patient departments and out-patient departments. Inputs included working time of the doctors, nurses, technicians, and managerial department staff. Based on the data provided on the inputs and outputs, the analysis showed 23 of the 34 hospitals to be relatively inefficient. Each hospital with an efficiency rating of less than 1 was considered relatively inefficient. In addition, managerial strategies based on dual variables were constructed to indicate the manner In which inefficient hospitals may be made efficient. A subsequent analysis of t-test revealed that the bed occupancy rate, medical revenue per 100beds, value added revenue per staff, medical revenue per staff were statistically significant. The results of this study suggest the DEA is a promising tool for evaluating relative human resource efficiency in hospitals which have multiple inputs and outputs and where the efficient production function is not specifiable with any precision. But it is considered that efficiency evaluations may be most effective]y accomplished by Incorporating a combination of methodologies such as ratio analysis and regression analysis.

A Study on Factors of Job Satisfaction and Burnout of Hospital Dietitians (병원 영양사의 직무만족도 및 소진에 영향을 미치는 요인)

  • This research aims to examine the experiences of hospital dietitians' burnout and the factors of their burnout, such as demographic factors, unique characteristics of hospitals, their self-respect, and organizational factors. In order for this, a questionnaire survey was conducted with hospital dietitians working in general hospitals of over 400 beds in the Seoul Metropolitan area. The average point of self-respect of dietitians is 3.67 out of 5.00. Their experience of burnout turned out to be 3.17 out of 7, the extent of job satisfaction is high, that is 3.35 out of 5.00. The decrease of personal accomplishment is shown to be closely related to the burnout of dietitians. The emotional exhaustion in burnout at the significant level 0.001 is increasing as the annual gross salary is low, their age is young, extra-working hours are long, and is high to the dietitians serving meals and working at entrusted companies. The factors affected by burnout experience vary with individual circumstances. So, to prevent the emotional exhaustion, depersonalization, and reduced personal accomplishment, hospital dietitians' spiritual and physical health needs to be kept up by their self-development and investment.

  • Since the ending of the IMF period, the Korean health industry has experienced a number of changes in its environment such as hospital bankruptcy, enforcement of SDF(Separation of Dispensing and Prescription), opening of the medical market by WHO, evaluation of medical institutions with more than 100 sick beds, and limited approval for medical service advertisement. Furthermore, the concept of medical service has changed from a beneficent and vortical one to a hi lateral and righteous one. These changes in medical service have required medical institutions to provide customers with medical service options and adopt market principles actively, while considering customer satisfaction. Thus, this paper aims to investigate the service quality and service value of medical institutions, to understand the mutual relationship between customer satisfaction and repeat visits, and to suggest better solutions for the improvement of service quality. For future studies, It will be necessary to overcome the limitations of this research and develop proper measurement tools on service quality in the Korean medical system.
