Primary tumors of trachea are rather uncommon, and few cases of direct surgical excision were reported in the literature. Recently we had the opportunity to see a patient with a benign obstructing tumor of the trachea which was confirmed as fibroma. The patient has complained of intermittent dyspnea, especially during inspiratory phase, dry cough and wheezing of a strident character for last 8 years. Bronchoscopy or bronchography were not attempted because of severe dyspnea. Trachea tomogram revealed oval mass at the terminal trachea. The right posterolateral thoracotomy was performed. Tumor, $2.5{\times}1.7cm$ in size, was located at terminal trachea and removed through right lateral tracheotomy without difficulty. Postoperatively all the symptoms and signs disappeared.

The present treatment of respiratory failure, using cuffed endotracheal and tracheostomy tube has produced, apparently with increasing frequency, three lesions which have serious ceminical manifestations such as tracheal stenosis, tracheomalasia, and localized tracheal erosion. Extensive resection and reconstruction of the trachea must be necessary because conservative treatment has generally failed in the fully developed stenotic lesion. of the mediastinal trachea following extensive resection is best accomplished by direct anastomosis of the patient`s own tracheobronchial tissue. Any replacement of the mediastinal trachea must be air tight and laterally rigid, and must heal dependably. A variety of materials has been used for substitution following circumferential excision of tracheal segments within the mediastinum. These attempts have often failed because of early leak or late stenosis. We have successfully performed circumferential resection and primary end-to-end anastomosis of the trachea for 4 cases of post-intubation tracheal stenosis located a few centimeter below the tracheostomy stoma in the period of 3 years between 1974 and 1976. The lesion in one patient was found in the upper trachea which was approached anteriorly through a cervicomediastinal incision with division of the upper sternum. Other three located in the lower half of the trachea were operated through a high transthoracic incision with appropriate hilar mobilization in addition to cervical flexion for the development of the cervical trachea into the mediastinum. There were no hospital death, but suture line granulations occurred in two patients were managed by bronchoscopic removal of granulations without difficulties.

Effects of Haenginbanhatang Extract on the Contraction of Isolated Guinea Pig Trachea Smooth Muscle. This study was carried out to investigate the effect of Haenginbanhatang extract on the contractile force of the isolated guinea pig trachea smooth muscle and elucidate its mechanism. The results were obtained as follows: 1. The contratile response of the trachea smooth muscle of the guinea pig to histamine was significantly inhibited by Haenginbanhatang. 2. Effects of Haenginbanhatang extract on the contractile response of the isolated guinea pig smooth muscle pretreated propranolol was not significant. 3. The contractile response of isolated guinea pig smooth muscle pretreated methylene blue was significantly inhibited by Haenginbanhatang, 4. The contractile response of the trachea smooth muscle of the guinea pig to prostaglandin F2a was significantly inhibited by Haenginbanhatang. 5. Effect of prostaglandin F2a on the contractile response of guinea pig smooth muscle pretreated Haenginbanhatang was not significant. According to the above results, it was suggested that the contractile response mechanism of the guinea pig smooth muscle to Haenginbanhatang was related to cyclic GMP.

"Saber-sheath" trachea is a deformity of lower cervical or intrathoracic trachea. The configuration of the deformity is marked coronal narrowing associated with sagittal widening. This deformity is associated with chronic obstructive pulmonary disease. We report a case of patient with saber-sheath who underwent total laryngectomy. Although the patient had no tracheal collapse after the total laryngectomy, crusted discharge was increased. Diagnosis of the saber-sheath trachea, possible causes, and clinical implications are discussed.

Tumors of trachea are uncommon and benign tumors are rare in adults. Tracheal tumors are overlooked as a cause of pulmonary symptoms. Neurilemmoma is originated from Schwann cells of nerve sheath, which are characterized by benign, solitary, and encapsulated mass. Although it may occur most frequently in the head and neck region, the neurilemmoma of trachea is extremely rare. We report a case in a 55-year-old man who presented with mild exertional dyspnea and foreign body sensation. The round encapsulated mass was demonstrated in posterior wall of trachea and removed by tracheofissure.

The bronchogenic cyst is not so rare in incidence, but it is rare for this cyst to cause compression of trachea and main bronchi. A 6-year old female child was evaluated for coughing, fever and left total atelectasis. This patient had a history of frequent upper respiratory symptom like bronchial asthma since 6month after birth. Fiberoptic bronchoscopy revealed external compression at distal trachea and both main bronchi. Computed Tomography revealed well demarcated homogeneous mass compressing distal trachea and main bronchi. The pathological examination showed bronchogenic cyst and this patient had uneventful recovery.

Myoepithelioma of the trachea is an extremely rare disease. The neoplasm shows histologic features identical to those described in myoepithelioma of salivary glands. The myoepithelioma cells demonstrate the numerous myofilament, desmosomal cellular attachment, cytoplasmic glycogen and pinocytotic vesicles. In immunohistochemical study, myoepithelioma cells show the positive antibody reactions to actin, keratin, vimentin and S-100 protein. A case of myoepithelioma arising from trachea in the seventy-two-year old male diagnosed by microscopic finding and immunohistochemistry is presented.

This study has been carried out to investigate the effects of Banhaonpaetang extract and its constituent herbs on the contractile force of Isolated Guinea pig trachea smooth muscle and to elucidate its mechanism. The results were as follows; 1. Banhaonpaetang significanly inhibitd the contractile force of isolated guinea pig trachea smooth muscle pretreated by histamine. 2. Banhaonpaetang significantly inhibited the contractile force of isolated guinea pig trachea smooth muscle pretreated by acetylcholine. 3. Radix Asari(細辛), Flos Inulae(旋覆花), Pericarpium Citri Nobilis(陳皮) and Radix Ginseng(人蔘) extract significantly inhibited the contractile force of isolated guinea pig trachea smooth muscle pretreated by histamine. 4. Radix Asari(細辛), Cortex Cinnamomi(桂心), Flos Inulae(旋覆花), Pericarpium Citri Nobilis(陳皮) and Radix Ginseng(人蔘) extract significantly inhibited the contractile force of isolated guinea pig trachea smooth muscle pretreated by acetylcholine. 5. The effects of Banhaonpaetang extract on the changes of plasma ACTH in rats were shown to be insignificant. 6. The effects of Banhaonpaetang extract on the changes of serum electrolytes were shown to be significant in $K^+$ and $Ca^{2+}$.

This study was carried out to investigate the effect of Heapyoyangjintang and Haepyoejintang extract on the contractile force of the isolated guinea pig trachea smooth muscle and elucidate its mechanism. The results were obtained as follows : 1. The isolated trachea smooth muscle was suspended in the organ bath with oxygenated kreb's Henseleit bicarbonate buffer solution at $37^{\circ}C$, and the developed tension by the drug was recorded with Isometric transducer(nacro F-60). The resting tension was approximately 0.5g. 2. The trachea smooth muscle of the isolated guinea pig was significantly relaxed by the administration of Haepyoyangjintang and Haepyoejintang extract. 3. The contractile response of the trachea smooth muscle of the isolated guinea pig to histamine$10^4M$ Was significantly inhibited by Heapyoyangjintang and Haepyoejintang extract. 4. The contractile response of the trachea smooth muscle of the isolated guinea pig to acetylcholine$10^4M$ was considerably inhibited by Haepyoyangjintang and Haepyoejintang extract. 5. The contractile response of the trachea smooth muscle of the isolated guinea pig to 5-hydorxytrtrypptamine $10^4M$ was considerably inhibited by Haepyoyangjintang and haepyoejintang extract.
