The Association Between Neurodegenerative Diseases and Development of Type 2 Diabetes (신경퇴행성 질환과 제2형 당뇨병 발생의 연관성)

  • Objectives : A growing body of evidence links type 2 diabetes (T2D) with a neurodegenerative disease (ND) such as Alzheimer's disease and Parkinson's disease. The purpose of this study is to investigate the relationship between NDs and the development of T2D by comparing the incidence of T2D in a group of various NDs (ND group) and control group. Methods : A population-based 10-year follow-up study was conducted using the Korean National Health Information Database for 2002-2015. We used a retrospective cohort study design to investigate the association of ND with T2D occurrence. The study population included ND (n=8,814) and control (n=37,970) groups, all aged 60 years or over. The Kaplan-Meier method was used to estimate the risk of developing T2D as a function of time. Cox proportional hazards regression models were used to evaluate the relationship between ND and T2D. Results : T2D was developed in a significantly higher percentage of patients in the ND group (53.6%) than in the control group (44.7%). The ND group increased the risk of T2D (HR, 1.43; 95% CI, 1.38-1.47). About one-third of patients in both groups were additionally diagnosed with another ND before the occurrence of T2D during a 10-year follow-up period. When compared to those who did not have another ND during the follow-up period, the incidence of T2D in those who were additionally diagnosed with another ND was higher in both the ND and control groups. Conclusions : The ND group had about 1.4 times higher risk of developing T2D than the control group. Our results showed a positive association between ND and T2D.

Antihyperglycemic Effects of Green Tea Extract on Alloxan-Induced Diabetic and OLETF Rats (Alloxan 당뇨쥐(제1형 당뇨병 모델)와 OLETF 쥐(제2형 당뇨병 모델)에서 녹차 추출물의 고혈당 억제 효과)

  • This study was performed to investigate the antihyperglycemic effect of green tea extracts (GTE) in diabetic rats. Experimental animals used were alloxan-induced diabetic Sprague-Dawley (SD) rats; as a model of type 1 diabetes mellitus and Otsuka Long Evans Tokushima fatty (OLETF) rats, and as a model of type 2 diabetes mellitus. Animals were randomly assigned either to continue the ad libitum diet or begin a green tea extracts (GTE) contained diet. GTE extracted from green tea was supplemented in the diet (2%). Body weight, food intake, and blood glucose concentration were recorded for 4 weeks. Animals were killed and glucose, triglyceride, and asparate aminotransferase (AST) were analysed in blood. Food intake was not affected by GTE in both alloxan.induced diabetic and OLETF rats but body weight was slightly decreased by GTE in OLETF rats. The blood glucose concentration was markedly decreased by GTE supplementation in both alloxan-induced diabetic and OLETF rats; however, triglyceride and AST levels in serum of GTE treated animals were not changed. This study shows that GTE beneficially modulate blood glucose concentration in diabetic animals. Dietary supplementation with GTE could potentially contribute to nutritional strategies for the treatment of diabetes mellitus.

개와 고양이의 당뇨병 진단과 치료

  • 대부분의 당뇨병이 있는 개와 고양이는 쉽게 진단할 수 있고 초기에 적절한 식이관리와 중간형 또는 지속형 인슐린을 하루에 1회 또는 2회 투여함으로써 적절히 관리할 수 있다. 당뇨병의 장기간 관리는 체중, 물섭취량과 연속적인 혈당치의 감시에 의하여 적절히 수행할 수 있다. 연속적인 혈당치 측정은 인슐린 치료와 관련된 잠재적인 문제점(저혈당증, 인슐린 유발성 과혈당증, 인슐린 내성)을 확인하거나 피할 수 있는 가능성을 높여 주며, 혈당을 적절한 수준으로 유지시켜 만성 당뇨병의 합병증 발생율을 줄이거나 지연시킬 수 있게 해준다.

  • 인슐린 펌프는 첨단이라는 단어가 붙으며 당뇨병 환자들이나 의료인들에게 모든 것이 알아서 해결되는 인공췌장(artificial pancreas)으로 잘못 오인되기도 하지만 앞으로 가까운 장래에 좀 더 완벽하게 첨단 인공췌장의 역할이 기대되는 분야이기도 하다. 본 장에서는 최근에 이루어진 첨단 인슐린 펌프들로 1) 연속혈당 측정기와 결합형 펌프 (glucose sensor-augmented insulin pump), 2) 인슐린 주입 알고리듬 (artificial pancreas software)이 탑재된 인공췌장형 펌프 연구들을 소개하고 앞으로 해결해야 할 과제와 전망에 대해서 알아보고자 한다.

  • 대혈관 합병증은 중간크기 이상의 혈관(동맥 및 정맥)에서 동맥경화증이나 죽상경화증에 의한 변화로 발생하는 질환으로 관상동맥, 뇌혈관 및 말초혈관에 주로 발생하며 당뇨병환자의 주된 사망원인과 관련이 있기 때문에 매우 중요합니다. 당뇨병환자에서 대혈관 합병증인 죽상경화증은 가장 흔하고 중대한 만성합병증으로 관상동맥질환에 의한 관상동맥질환, 말초혈관질환 및 뇌혈관질환이 발생합니다. 발생률은 당뇨병이 없는 사람에 비해 2$\sim$5배 높으며 제 2형 당뇨병뿐만 아니라 제 1형 당뇨병환자의 주된 사망원인이 됩니다. 대혈관 합병증과 관련된 질환은 유병기간이 오래된 당뇨병환자에서 이환율, 장애 정도, 사망률 및 치료비용에 심각한 영향을 미칩니다. 당뇨병환자의 사망은 대부분이 대혈관 합병증과 관련이 있으며, 이중에서도 관상동맥질환으로 인한 사망률이 2$\sim$4배 증가합니다. 당뇨병성 만성합병증은 건강관리를 위한 지출에 상당히 영향을 미칩니다. 총 의료관리 예산의 약 25%가 당뇨병 및 당뇨병성 만성합병증의 치료에 지출됩니다. 위험요소를 감소시키거나 제거하면 만성합병증의 85%가 지연되거나 진행을 늦출 수 있습니다. 이것으로 미국의 경우 매년 170억 달러의 의료관리 예산을 절감할 수 있다고 합니다.

  • 당뇨병 환자에 있어서 산화 스트레스의 증가는 당뇨병 합병증의 발생을 증가시키는 것으로 널리 알려져 있으며, 식사 요법을 통한 혈당관리의 중요성 때문에 영양교육이 타 질병에 비해 강조되고 있다. 그러나 당뇨병 환자의 영양교육에 있어서 합병증 발생의 주요 원인인 산화스트레스와 이를 감소시키는 항산화 영양소에 대한 교육은 거의 이루어지지 않고 있다. 이에, 일반당뇨교육에 항산화 영양소를 소개하고 이의 섭취를 강조하는 내용을 포함한 교육을 추가적으로 실시하여 일반당뇨교육과 항산화교육의 효과를 비교하였으며, 이를 토대로 당뇨 관리의 효율성을 높일 수 있는 방안을 모색하고자 하였다.(중략)

  • Today, the environment in which type 1 Diabetes children learn how to cope with their Diabetes is very vulnerable. Therefore, it is necessary to create an environment that can respond to changes in blood sugar level of children. The purpose of this study is to suggest game-based educational content on how to deal with changes in blood sugar level through behavioral methods for children with type 1 Diabetes. The purpose of this study is to suggest education type game contents using hybrid method which enhances efficiency in learning children 's expert knowledge. Based on the results of the interviews conducted with diabetes specialists for this study, we suggested the contents of coping with blood sugar change composed of education contents required for the children. As a result of this study, it was found that the hybrid factor was useful in learning diabetes through games. These game contents are expected to provide an environment that children with diabetes can learn more efficiently.

Effects of Different Intensity Aerobic and Resistance Exercise on Anti-diabetic and Lipid Profile Improvement in Type 2 Diabetic mice (다른 강도의 유산소성 및 저항성 운동이 제2형 당뇨 마우스의 항당뇨, 지질 개선에 미치는 영향)

  • The purpose of this study was to examine the effects of aerobic and resistance exercise of different intensity on anti-diabetic and lipid profile improvement in type 2 diabetic mice. C57BL/6 mice were divided into six groups (n=8, in each group): normal group (Normal), type 2 diabetes (DM), type 2 diabetes+VO2max 50% aerobic exercise group (DM50A), type 2 diabetes+VO2max 75% aerobic exercise group (DM75A), type 2 diabetes+1RM 50% resistance exercise group (DM50R), and type 2 diabetes+1RM 75% resistance group (DM75R). DM50A and DM75A were subjected to treadmill exercise 40 min/day, 5 days/week, during 8 weeks (DM50A, at the speed of 8 m/min for 1-4 weeks and 8~10 m/min for 5-8 weeks; DM75A, 12 m/min for 1-4 weeks and 12~14 m/min for 5-8weeks). DM50R (1RM50%) and DM75R (1RM75%) were subjected to ladder-climbing exercise with weights secured to their tails, 8 set/day, 5 days/week, during 8 weeks. After 8 weeks of exercise, fasting blood glucose and HOMA-IR was significantly lower in DM group than in DM group. HbA1c showed significantly lower DM50R and DM75R groups than DM group. HDL-C showed the highest level in DM75A group and triglyceride was lowest in DM75R group. The cardiovascular risk index was lowest in the Normal and DM75A groups. Therefore, moderate intensity exercise in T2DM mice showed better improvement in blood glucose and insulin resistance control, and moderate intensity aerobic exercise was effective in reducing the cardiovascular risk index by increasing HDL-C levels.

Relationship between Insomnia and Depression in Type 2 Diabetics (2형 당뇨병 환자에서 불면증과 우울 증상의 관련성)

  • Objectives : Many of the patients with type 2 diabetes are associated with sleep problems, and the rate of insomnia is known to be higher in the general population. The aims of this study were to know the frequency and clnical characteristics of insomnia, and related variables to insomnia in patients diagnosed with type 2 diabetes. Methods : For 99 patients from 18 to 80 years of age (65 males and 34 females) with type 2 diabetes, interviews were performed. Total sleep time and sleep latency was evaluated. Insomnia was evaluated using the Korean Version of the Insomnia Severity Index (ISI-K). Severity of depressive symptoms were evaluted using the Korean version of the Hamilton Depression Scale (K-HDRM). According to the cutoff score of 15.5 on the ISI-K, subjects were divided into the group of type 2 diabetics with insomnia (N=34) and those without insomnia (N=65) at first, and then statistically analyzed. Results : TInsomnia could be found in 34.34% of type 2 diabetics. Type 2 diabetics with insomnia had significantly more single or divorced (respectively 11.8%, p<0.05), higher total scores of the K-HDRS ($11.76{\pm}5.52$, p<0.001), shorter total sleep time ($5.35{\pm}2.00hours$, p<0.001), and longer sleep latency ($50.29{\pm}33.80minutes$, p<0.001). The all item scores of the ISI-K in type 2 diabetics with insomnia were significantly higher than those in type 2 diabetics without insomnia, that is, total ($18.38{\pm}2.69$), A1 (Initial insomnia) ($2.97{\pm}0.76$), A2 (Middle insomnia) ($3.06{\pm}0.69$), A3 (Terminal insomnia) ($2.76{\pm}0.61$), B (Satisfaction) ($3.18{\pm}0.72$), C (Interference) ($2.09{\pm}0.97$), D (Noticeability) ($2.12{\pm}1.09$) and E (Distress) ($2.21{\pm}0.81$) (respectively p<0.001). Variables associated with insomnia in type 2 diabetics were as following. Age had significant negative correlation with A3 items of the ISI-K (${\beta}=-0.241$, p<0.05). Total scores of the K-HDRS had significant positive correlation, while total sleep time had significant negative correlation with all items of the ISI-K (respectively p<0.05). Sleep latency had significant positive correlation with total,, A1, B and E item scores of the ISI-K (respectively p<0.05). Conclusions : Insomnia was found in about 1/3 of type 2 diabetics. According to the presence of insomnia, clinical characteristics including sleep quality as well as quantity seemed to be different. Because depression seemed to be correlated with insomnia, clinicians should pay attention to early detection and intervention of depression among type 2 diabetics.
