Readeveloping Turbulent Boundary Layer after Separation-Reattachment(I) (박리-재부착 이후의 재발달 난류경계층 I)

  • An experimental study has been performed to investigate the process from nonequilibrium state to equilibrium state in redeveloping turbulent boundary layer beyond separation-reattachment using pitot tube and hot-wire anemometer. The model sued in the experiment has the form of a backward facing step which is assembled by a two-dimensional 4:1 half elipse and a plate. Measurements are carried out up to a distance of about 50 step height downstream of the step, where the reattachment observed at about x/h=6.5. The profiles of the shape factor H the Clauser parameter G and the coefficient of friction $C^{f}$ exhibited the characteristics similar to those of the equilibrium turbulent boundary layer from x/h=25, and the profiles of the trubulent quantities did from x/h=35. However, the wake region of the boundary layer does not seem to recover the equilibrium turbulent boundary layer even at x/h=50. By considering the distributions of the intermittency factor it has been noted that the turbulence structure changes gradually from a mixing layer to a turbulent boundary layer along downstream direction after reattachment. This becomes clearer as we analyse the one-dimensional energy spectra and the dissipation energy spectra which are measured and caculated at various downstream positions after the backward facing step.p.

Effects of Acupotomy on Pain and Functional Improvement in Acute Low Back Pain Patients: A Retrospective Study (도침치료가 급성 요추 염좌 환자의 통증 및 기능개선에 미치는 영향: 후향적 연구)

  • Objectives The aim of this study is to investigate the effect of Acupotomy on the pain reduction and functional improvement of patients with Acute low back pain. Methods We studied 60 patients with Acute low back pain with Numeric Rating Scale (NRS) 5 or higher in admission and had admitted to Department of Korean Medicine Rehabilitation of Daejeon University from March 1, 2017 to October 31, 2017. 30 patients had received Acupotomy combined Korean Medicine treatment(such as acupuncture, moxabustion, herbal medicine, etc) and the other 30 had received Korean Medicine treatment only. The analysis was conducted as a retrospective study which analyzes the patient's medical records. Statistical analysis was performed using the IBM SPSS Statistics 24 program. We used NRS to evaluate pain reduction, used Range of Motion (ROM) and Roland Morris Disability Questionnaire (RMDQ) to evaluate function improvement, and used EuroQol-5 Dimension (EQ-5D) and EuroQoL-Visual Analogue Scale (EQ-VAS) to assess quality of life. Finally, a five-point Likert scale was used to assess treatment satisfaction. Results The analysis revealed that patients who were treated with Acupotomy showed statistically significant NRS reduction, improvement of ROM and RMDQ, improvement of EQ-5D and EQ-VAS and satisfaction compared to those who just recevied Korean Medicine treatment. Conclusions In conclusion, we found that the Acupotomy showed a positive effect on pain resolving, functional rehabilitation and quality of life in patients with Acute low back pain.

Forty-one Cases of Rotator Cuff Injuries Treated by Complex Korean Medicine Treatment: A Retrospective Review (한방 복합치료를 진행한 회전근개 파열 환자 치험 41예: 후향적 관찰 연구)

  • Objectives The purpose of this study is to report the improvement of complex Korean Medicine treatment for patients with rotator cuff injuries and determine the clinical improvement by number of damaged lesion. Methods Forty-one patients who diagnosed with rotator cuff injury were included in the retrospective study. Patients were treated with acupuncture, electroacupuncture and pharmacopuncture. Clinical outcomes were assessed using Numeric Rating Scale (NRS), Shoulder Pain and Disablity Index (SPADI) and EuroQol-5 Dimension Index (EQ-5D Index). Results Both NRS and SPADI scores were significantly reduced after treatment (p<0.01), but EQ-5D for assessing quality of life and clinical improvement by number of damaged lesion were no significant improvement (p>0.05). Conclusions These results show that patients with rotator cuff injuries could gain improvement from complex Korean Medicine. Further research is required to confirm the effectiveness of Korean Medicine treatment.

Prevalence and Risk Factors of Colonic Polyps in Colonoscopy Examinees (대장내시경 수검자에서 관찰된 대장용종의 유병률 및 위험인자)

  • This study was performed to evaluate the prevalence and risk factors of colonic polyps which is an established premalignant lesion and to provide basic data for the management of colon cancer in asymptomatic adults. The subjects of this study were 995 persons who underwent screening colonoscopy between January to December 2010 in health promotion center of an University Hospital located in Busan, and structured questionnaire, medical questionnaire, medical records were reviewed retrospectively. The data were analyzed by SPSS 18.0 program. Colonic polyps were found in 63.2% of the subjects. The prevalence rate of colonic polyps was significantly higher in males and it increased with age. Age, sex and exercise were found to be related independently to colonic polyps. Early detection of colonic polyps by colonoscopy is important for adults over 40 years, and it is necessary to advise such subjects to correct a wrong life-style in order to primarily prevent the development of colonic polyps.

Retrospective Study about the Effectiveness of a Korean Medicine Treatment on 45 Facial Atopic Dermatitis Patients (얼굴아토피 피부염환자 45명의 한의학적 치료효과에 대한 후향적 연구)

  • Objective : The purpose of this study is to report the effectiveness of a Korean Medicine Treatment on 45 facial atopic dermatitis patients. Methods : Total 45 facial atopic dermatitis patients, who has visited korean medical clinic in year 2011 were analyzed by Objective SCORAD Index(OSI) and Investigator's Global Assessment(IGA). Subanalysis of OSI and IGA were done according to topical ointment user/non-user, treatment period and change in IGA distribution. Results : 1. Male and female percent was 20%:80%. 17.8% were teens, 55.6% were twenties, 13.3% were thirties, 13.3% were above age forty. 64.4% were on topical ointment treatment of corticosteroid or calcineurin inhibitor, and 35.6% were not at the initial visit. Patients with family history were 44.4% and 62.2% had atopic dermatitis past history. 2. OSI and IGA were significantly lower after 1~3, 3~6, 6~9 month of treatment. Average post-treatment score was lower in longer-treated group. 3. IGA distribution has changed from average 3.42 at the first visit to 1.76 at final visit. 91.1% of total patients reported decrease in IGA at the final visit. 4. OSI and IGA improvement rate were significantly higher in non-topical ointment user than the user. Age, treatment period, initial OSI and IGA score difference were not significantly different. Conclusion : A significant percent of 45 facial atopic dermatitis patients who were treated with Korean Medicine Treatment reported decrease in OSI and IGA. The difference increased with the treatment period. Non-topical ointment users' improvement was significantly higher than topical ointment user.

Retrospective Study about the Effectiveness of Korean Medical Treatment on 101 Psoriasis Patients for 6 months (건선 환자 101명의 6개월간의 한의학적 치료효과에 대한 후향적 연구)

  • Objectives : The purpose of this study is to report the effectiveness of Korean Medicine Treatment on 101 psoriasis patients for 6 months. Methods : Psoriasis Area and Severity Index(PASI) is used for measuring the degree of psoriasis with photos of total 101 psoriasis patients who have visited Korean medical clinic from October 11, 2013 to August 31, 2017. And we analyzed the PASI score according to gender, age, period of treatment, being used steroid or not. Results : 1. The ratio of male to female was 44.55 : 55.45. The mean age of patients was 34.65 years old(All patients were constituted with 9.9% of 10~19 year-old group, 31.7% of 20~29 year-old group, 26.7% of 30~39 year-old group, 12.9% of 40~49 year-old group, 15.8% of 50~59 year-old group and 3.0% of 60~69 year-old group). 2. PASI score was significantly higher in men than in women before treatment, but there was no significant difference of PASI score improvement after 2, 4 and 6 months treatment in both men and women. 3. PASI score improvement according to age after 2, 4 and 6 months treatment was no significant difference. 5. In initial visit, the steroid group patients were 61 and non-steroid group patients were 40. PASI score in initial visit between steroid group and non-steroid group was no significant difference. And PASI score after 2, 4 and 6 months treatment was no significant difference. Conclusions : PASI score was significantly decreased after 2, 4 and 6 months treatment compared with before treatment. PASI score improvement was significantly increased in proportion to the period of treatment.

The Removal of Plates after Craniomaxillofacial Surgery: A Retrospective Study (두개 악안면 수술 후 플레이트 제거에 관한 후향적 연구)

  • Purpose: Plate systems have been used for osteosynthesis of cranial and oromaxillofacial fracture. However, there is no consensus on the need for routine removal of plate and the question about indications of removal. Therefore, we present the retrospective study to clarify the indications and consensus of removal. Methods: The medical records of patients who were treated with rigid internal fixation using plates after craniofacial trauma were reviewed. Study variables included age, gender, type of fracture, type of plate, seniority of the operator, causes of removal, and time between insertion and removal. All results amendable to statistics were analyzed using SPSS 10.0 to determine which set of variables might affect the fate of the plates. Results: For a period of 10 years (March 1, 1994 through July 31, 2004), total of 41 plates(6.7%) were removed among 609 plates inserted into 419 patients; 27 plates were removed from 15 patients for infection, which is the most common cause of removal accounting for 65.8%. Mean time between insertion and removal is 35.2 months and mean age is 41.4 years. Most plates were removed from combined fracture(14.92%) and facial fracture(8.47%) and these were statistically significant. The age, gender, seniority of the operator and other variables were not statistically associated with plate removal. Conclusion: This retrospective study shows that routine removal does not appear to be clinically indicated due to respectively low removal rate and that the commonest indications for removal were infection.

Effects of Korean Medicine on Patients with Idiopathic Parkinson’s Disease: A Retrospective Study (특발성 파킨슨병 환자에 대한 한의치료의 효과 : 후향적 연구)

  • Objectives: The purpose of this study was to investigate the effects of Korean medicine on patients with idiopathic Parkinson’s disease.Methods: The charts of 47 patients diagnosed with idiopathic Parkinson’s disease in the Department of Internal Korean medicine, Stroke and Neurological Disorders Center, Kyung Hee University Hospital, Gangdong between August 2012 and July 2016 were reviewed. The Unified Parkinson’s disease rating scale (UPDRS) was administered before and after treatment with Korean medicine.Results: Thee average UPDRS Ⅱ+Ⅲ, UPDRS Ⅱ, and UPDRS Ⅲ of the 47 patients decreased significantly from 22.26±15.15, 10.19±7.53, and 12.06±8.35, respectively, pretreatment to 16.96±13.63, 7.47±6.44, and 9.49±7.73, respectively, post-treatment. The average postural instability and gait difficulty (PIGD), tremor, and bradykinesia also significantly improved post-treatment.Conclusions: These results provide evidence that Korean medicine can improve the activities of daily living and motor function of patients with idiopathic Parkinson’s disease. In particular, Korean medicine may be effective for the treatment of PIGD, tremor, and bradykinesia.

A Retrospective Study: Influence of Beck Depression Inventory Score to a Patient under the Early Stages of Oriental Medical Treatment for Low Back Pain (Beck 우울 척도 점수가 요통 환자의 한방 치료 초기에 미치는 영향에 대한 후향적 연구)

  • Objectives : The purpose of this study is to know that Beck Depression Inventory(BDI) score to effect on improvement degree of low back pain after oriental medical treatment. Methods : We analyzed charts of 33 inpatients who were treated in Ja-Seng Hospital of Oriental Medicine and Bu-Cheon Ja-seng Hospital of Oriental Medicine. Subject is divided into Group A(BDI<10) and Group B(BDI${\geqq}$10). Numerical Rating Scale(NRS) and Oswestry Disability Index(ODI) were checked on admission, after 1 week, NRS and ODI were rechecked to estimate the efficacy of treatments. Results : 1. In both groups, NRS was significantly decreased(p<0.05). Analyzed result of the interaction between the two groups didn't show significant difference(p=0.197). 2. In Group A, ODI was significantly decresed(p<0.05). In Group B, ODI wasn't significantly decreased(p=0.058). The interaction analysis between the two groups didn't show significant difference(p=0.173). 3. Analyzing patients with chronic back pain longer than 6 months by the presence of depression tendency and NRS, ODI decrease after 1 week of ambulation, while the NRS reduction showed relevancy by the depression tendency (p<0.05), the ODI reduction didn't(p=0.272). Conclusions : 1. Although improvement of pain before and after the treatment and degree of functional recovery in high-level BDI group was less than low-level BDI group, but it was stastically insignificant. 2. High BDI score of patients with chronic low back pain is the factor of disturbing pain improvement.

Changes in Pathogens and Antibiotic Sensitivities in very Low Birth Weight Infants with Neonatal Sepsis (극소저출생체중아에서 신생아 패혈증의 원인균 및 항생제 감수성의 변화)

  • Purpose : To set up an appropriate treatment plan for neonatal sepsis by investigating changes in pathogens and antibiotic sensitivities. Methods : The medical records of very low birth weight infants (VLBWI) admitted to the neonatal intensive care unit (NICU) of the Eulji University Hospital between January 2000 and June 2006 were retrospectively reviewed. The culture reports were analyzed for causative microorganisms and antibiotic sensitivities. Results : Among 164 neonates, 19 neonates (11.6%) had 26 episodes of culture-proven sepsis. Very late onset sepsis was the most common type. The dominant pathogens of sepsis included Klebsiella pneumoniae, Streptococcus spp., coagulase negative Staphylococcus and Enterobacter spp., and were mostly multi-drug resistant. Conclusion : For the appropriate treatment of neonatal sepsis, proper use of antibiotics through the periodic review and understanding of changes of the microorganisms and antimicrobial sensitivities is necessary to prevent multi-drug resistant microorganisms and to avoid excessive use of broad-spectrum empiric antibiotics.
