COMPARATIVE STUDY ON THE CLINICAL AND RADIOGRAPHIC FINDINGS OF TEMPOROMANDIBULAR JOINT DYSFUNCTION PATIENTS (악관절 기능장애 환자의 임상적 방사선학적 소견에 관한 비교 연구)

  • 악관절 기능장애 환자 118명을 대상으로 하여 임상적으로 악관절 잡음, 개구량, 동통 및 병력을 조사하고 골변화 양상 및 악관절내에서의 과두위치 등을 방사선학적으로 관찰하였으며, 조영술식을 이용하여 악관절내의 원판 위치 및 천공등을 관찰하여 이들 조사결과를 비교 검토한 결과 다음과 같은 결론을 얻었다 1. 악관절 기능장애 환자에서 악관절부위의 골변화는 환자의 병력과 밀접한 관계가 있었다. 2. 최대 개구시 40㎜미만을 개구하는 환자에서 더욱 심한 골변화를 보였다. 3. 악관절 기능장애 환자의 교합상태에서 과두가 악관절강의 후방에 위치한 경우 개구시 더욱 쉽게 과두가 관절융기 전방으로 이동하였으며, 교합시 과두가 전방에 위치한 경우 후방에 위치한 경우보다 더욱 심한 골변화양상을 보였다. 4. 교합시 관절간격의 감소는 과두의 악관절내 어떠한 위치보다 병변이 진행된 상태였다. 5. 5. 비환원성 내장증 환자의 특징적 증상은 clicking후 개구장애를 나타내었으며 비환원성 내장증을 환원성 내장증보다 더욱 진행된 상태였다

Arthroscopic Reconstruction of Posterior Cruciate Ligament with Achilles Tendon Allograft (동종 이식 아킬레스 건을 이용한 관절경적 후방 십자 인대 재건술)

  • Purpose : We have evaluated the outcome of the operative treatment of PCL rupture using the achilles tendon allograft. Materials and Methods : A retrospective study was completed for 30-PCL reconstruction cases, using the achilles tendon allograft from september 1996 to march 1998. There was an average follow up of 24 months, with range of 12 to 50 months. Results : The active range of motion was improved postoperatively. The Lysholm Knee Score was improved from a mean of 54 points preoperatively to 87 points postoerative 18 months. The "Cybex 340 isometric test" revealed satisfactory results compared with autograft. Conclusions : Use of allograft for recostruction of the PCL is an attractive option because it precludes the harvesting of autogenous tissue in a knee that is already at risk for patellofemoral and tibiofemoral osteoarthrosis.

  • Localized forms of giant cell tumor are known to arise commonly in the synovial membrane of the finger joints. Multinucleated giant cells are its characteristic pathology finding, giant cell tumor shows a low rate of recurrence after complete excision. When occurring at the knee joints, giant cell tumor manifests a wide form of symptoms, from no symptom at all, to intermittent locking. Complete excision is possible by arthroscopy, but if done incompletely, it is reported to recur in 45% of cases. We present here a case of giant cell tumor that has arisen from the anterior portion of the posterior cruciate ligament, excised by arthroscopy and followed by pathologic confirmation.

  • Insertional bone cysts of the knee joint mainly appear in the form of small cysts at the bone attachment of the cruciate ligaments, and are usually not associated with symptoms. The authors report a 31-year-old man diagnosed with a huge insertional bone cyst connected to the femoral attachment of the posterior cruciate ligament in the knee joint, who obtained good results after curettage and bone cement filling.

  • Purpose: The results of the repair for the superior labrum lesions attaching at the superior glenoid have not been satisfactory in every cases. We wanted to analyze the shoulder MRI and the anatomical morphology and pattern of the superior glenoid at which the superior labrum attaches to get anatomical information helpful in treating the superior labrum. Materials and Methods: We analyzed the coronal images of the shoulder MRI of 108 cases taken at our hospital. Average age was 52 years (range, 17~71 years), 55 males and 53 females. On two coronal images behind the attachment of the long head of biceps that the repair of the SLAP was performed at, the length of the attachment of superior labrum and the angle of the supero-lateral glenoid were measured. Results: The average length of the attachment of superior labrum was $9.78{\pm}1.64\;mm$. The average length was $10.1{\pm}1.61mm$ in male, $9.43{\pm}1.6\;mm$ in female. The angle of the supero-lateral glenoid was $89.6{\pm}7.6$ degrees. Conclusion: The attachment of the superior labrum in coronal plane was shorter in posterior spot than anterior. The angle of the supero-lateral glenoid was less in posterior spot.

  • 후방십자인대 및 후외측인대 재건술 후 결과는 많은 발전이 있었지만, evidence medicine의 측면에서 재활 방법에 관한 전향적 연구는 없다. 현재까지의 재활 방법은 다양하게 소개되었지만 전방십자인대 재건술처럼 가속 재활을 하는 것은 바람직스럽지 않다. 후방십자인대, 후외측인대 그리고 다발성 인대 재건술 후 재활은 수술 방법과 환자의 재건된 인대의 상태, 하지 정렬 등에 따라서 개별화하여 점진적으로 진행하여야 할 것이고, 재활기간 동안 의사의 세심한 관찰이 필요하다.

Posterior cruciate ligament reconstruction using fresh-frozen Achilles tendon allograft with preservation of ligament remnant (남아 있는 인대를 보존하고 신선 동결 동종 아킬레스건을 이용한 후방십자인대 재건술)

  • Puropse: To evaluate the outcome of arthroscopic posterior cruciate ligament (PCL) reconstruction using fresh-frozen achilles allograft tendon with preservation of ligament remnant or elongated ligament. Materials and Methods: From October 2004 to March 2010, we performed PCL reconstruction with Achilles tendon allografts in 22 complete rupture patients. Mean age was 31.5 years. 14 cases were male and 8 cases were female. Average follow-up period was 3 years and 7 months, range from 1 year to 6 years and 4 months. Subjective and objective parameters were utilized in analyses, such as the mean range of motion, post. drawer test, Lysholm knee score, Tegner activity score, IKDC score, and second look arthroscopic examination. Results: Postoperative Lysholm knee score, IKDC scores, Tegner activity scale, and posterior displacement by the Telos stress test demonstrated statistically significant improvement compared to the preoperative state. Conclusion: Arthroscopic PCL reconstruction using fresh-frozen achilles allograft tendon with preservation of ligament remnant showed good clinical results and posterior stability.

Natural History of Conservatively Treated Posterior Cruciate Ligament Injury (보존적으로 치료된 급성 단독 후방십자인대 손상의 자연 경과)

  • Purpose: We investigated the natural history of acute, isolated posterior cruciate ligament injuries treated conservatively. Materials and Methods: Between February 1999 and October 2006, we evaluated retrospectively the results of acute, isolated posterior cruciate ligament injuries treated conservatively. The subjects consisted of 21 patients. At initial and follow up visits physical examination, $KT-2000^{TM}$ arthrometer and MRI were performed to assess improvement on the knee stability and continuity of the posterior cruciate ligament. IKDC (International Knee Documentation Committee) knee scoring and quadriceps muscle strength were also checked in all population. Comparing the results of initial and follow up examinations we assessed the natural history of acute, isolated posterior cruciate ligament injuries treated conservatively. The mean follow up period was 22.7 months. Results: The posterior drawer test showed 14 cases of Grade I, 6 cases of Grade II and 1 case of Grade III progressing 18 cases of Grade I and 3 cases of Grade II. The mean difference of 5.7mm by $KT-2000^{TM}$ arthrometer was changed Into 2.7mm and the continuity of posterior cruciate ligament initially checked by 48.1% on MRI increased to 69.7%. The mean quadriceps muscle strength was grade 'Good' and mean IKDC knee score was nearly grade 'A'. Conclusion: Our study suggests that patients with acute, isolated posterior cruciate ligament injuries treated conservatively may get good clinical outcomes on clinical situation and MRI.

Effect of Backward Walking Exercise on ROM, VAS score and Proprioception in Anterior Cruciate Ligament Reconstruction Patients (전방십자인대 재건술 환자의 후방보행 재활운동이 관절의 가동범위 및 통증점수, 고유수용성감각에 미치는 영향)

  • This study examined the effects of 4-weeks of backward walking exercise on ROM, VAS (Visual Analogue Scale) score and proprioception in anterior cruciate reconstruction patients. Fourteen subjects were randomly assigned to either a backward walking group (BWG, n=7) or a control group (CON, n=7) and then participated in rehabilitation program for 4 weeks. The ROM of the flexion significantly decreased in both groups (BWG, p<0.001 vs CON, p<0.01), while ROM of extension significantly decreased only in the BWG (p<.05) post exercise, but no significant difference was observed between groups. Additionally, VAS score decreased significantly in both groups (P<0.001) and BWG was lower than CON post exercise (p<0.001). Finally, proprioception decreased significantly in both groups (BWG, p<0.001 vs CON, p<0.05), but did not differ significantly between groups. These results suggest that the 4 week backward walking rehabilitation program improves ROM, VAS score, and proprioception and has positive effects on functional recovery of anterior cruciate ligament reconstruction patients.
