Isometry of the Posterior Cruciate Ligament (후방 십자 인대의 등장성)

  • Less has been written about the PCL than the ACL. There has, however, been an increasing amount of the interest in the PCL recently. Surgical reconstructions using grafts are often performed. However, these procesures often fail to provide long-term stability and function. Graft attachment sites are critical determinants of success in the PCL reconstruction. The clinical literature contains conflicting recommendations for graft attachment sites. We present a review of the isometry of the PCL.

Effects of 8week Plyometric training on power, dynamic balance and joint position sensory in Taekwondo demonstrator (8주간의 플라이오메트릭 훈련이 대학 태권도 시범선수의 순발력, 동적 평형성 및 관절 위치감각에 미치는 영향)

  • The purpose of this study investigate the effects of 8 weeks plyometric training on power, dynamic balance and joint position sensory in Taekwoondo demmonstrators. Twenty healthy collegiate Taekwoondo demonstrators were randomly assigned to either an exercise group (Ex=10), and a control group (Con=10), Ex group completed the plyometric training in three times a week, 60 minute for 8 weeks. Testing before and after training include the Sargent jump, standing long jump, back muscle strength, Dynamic balance and joint position sensory. Sargent jump, standing long jump and back muscle strength were significantly development(p<.05). And dynamic balance were significantly exercise effect in posterolateral(p<.01) and posteromedial(p<.05). Not only but also, joint position sensroy were significantly exercise effect in left food 15°(p<.01) and 45°(p<.05). The results of the study showed that 8 weeks of plyometric training had a positive effect on the power, dynamic balance and joint position sensory in Taekwondo demonstrator.

Endoscopic Treatment of Extensive Deep Abscess in Distal Posterior Thigh - A Case Report - (원위 대퇴부 후방에 발생한 광범위 심부 농양의 내시경적 치료 - 증례 보고 -)

  • At the posterior aspect of distal thigh, major nerves, vessels, muscles and tendons were located. So, if abscess occurs within deep muscular fascia of posterior aspect of distal thigh, it can be widely and deeply spread to proximal thigh, popliteal fossa, posterior proximal leg, surrounding areas of knee joint along deep fascia, muscles and tendons. In that case, it is difficult to eradicate the abscess using antibiotics without surgical drainage and debridement. But, it is often impossible to obtain satisfactory view of operative field with conventional technique and critical damage to major nerves and vessels in the popliteal fossa during operation may occur. We performed endoscopic treatment for extensive deep abscesses occurred in posterior aspect of distal thigh in 64-year-old man, and obtained satisfactory result without injury to the normal structures including major nerves and vessels. So we report this case with a review of relevant literatures.

  • Purpose: A triple labral lesion represents a combination of injuries of the anterior, posterior and superior glenoid labrum. The injury mechanism and symptoms is not completely understood. Materials and Methods: We encountered a triple labral injury in a 39-year-old male complaining pain and active abduction difficulty after a motor vehicle accident. Currently, he does not complain any instability symptoms. The labrum was repaired using bio-absorbable suture anchors and a suture hook. Results: Eighteen months following surgery, the active range of motion was restored and he had no difficulty in his daily activities.

The Results of Primary Repair in Acute Injuries of the Posterior Cruciate Ligament (후방 십자 인대 급성 손상에서 일차 복원술의 결과)

  • Background: We analyze the functional evaluation, posterior stability after surgery of acute injuries in the femoral detachment and tibial avulsion fracture of posterior cruciate ligament. Materials and Methods: Twenty two patients who underwent primary repair were followed more than one year after operation (mean follow-up 33.7 months). The patients were evaluated with KT 2000 arthrometer and posterior stress test for posterior stability. Lysholm knee score was used for knee function. Results: Lysholm score revealed 85.3 in femoral detachment group and 91.1 in tibial avulsion fracture group (P<0.05). Posterior displacement of the posterior stress test showed 17.2 mm at initial injury and 8.6 mm at last follow up in femoral detachment group, 16.8 mm at initial injury and 7.1 mm at last follow up in tibial avulsion fracture group. There revealed the tendency of the improved posterior stability of the tibial avulsion group compared with femoral detachment group, but there showed no statistical significance in KT 2000 arthrometer and posterior stress x-ray. Conclusion: Functional results of tibial avulsion fracture group revealed the better outcome compared with femoral detachment group, but the degree of posterior stability in tibial avulsion fracture group showed no statistical significance in posterior stability.

Usefulness of Ultrasound for the Loose Body in Knee (슬관절내 유리체에 대한 초음파 검사의 유용성)

  • Purpose: We wished to evaluate the clinical usefulness of ultrasound, when loose body is palpated and is shown radiographically in knee. Material and Methods: Twenty-five knees of twenty-two patients with loose body in knee radiographically from March, 2007 to July, 2009 were involved in this study, there were 8 males and 14 females, there were 11 right knees, 8 left knees and 3 both knees, the average age was 56.3 years (range, 32~76 years). The location of loose bodies were 13 cases at suprapatella pouch, 1 case at infrapatella anterior compartment and 11 cases at posterior compartment. In loose bodies of suprapatella pouch, we examined the mobility of loose bodies longitudinally with ultrasound placed at superior patella pole while flexing and extending the knee. In loose bodies of infrapatella anterior and posterior compartment, we examined the mobility of loose bodies longitudinally and transversely with ultrasound while flexing and extending the knee. Results: All cases of suprapatella pouch and infrapatella anterior compartment were diagnosed to movable loose bodies and were removed surgically, all cases of posterior compartment were diagnosed to immovable loose bodies and were not removed. Conclusion: When we determined the surgical removal according to mobility of loose body in knee, ultrasound may be helpful clinically.

Posterior Cruciate Ligament Reconstruction by Tibial Inlay Technique (Tibial Inlay 방법을 이용한 후방 십자 인대 재건술의 결과)

  • Purpose : To evaluate the clinical and radiological results of posterior cruciate ligament reconstruction(PCL) by tibial inlay technique. Materials and Methods : Twenty four patients(25 cases), who underwent reconstruction of posterior cruciate ligament by tibial inlay technique and were followed up for at least 2 years, were reviewed retrospectively. Mean age at operation was 35 years and mean period of follow up was 34 months. Clinically Lysholm knee socre, posterior drawer test and step off sign were evaluated. Instrumented posterior laxity test with $Telos^{\circledR}$ device(Telos stress device; Austin & Associates, Inc., Polston, US) was performed. Results : Lysholm knee score was improved from 56.9 points preoperatively to 94.6 points at fellow up. Posterior drawer test showed grade II instability in 1 case, grade III in 18 and Grade IV in 6 preoperatively and improved to grade I In 12, grade II in 10 and grade III in 3 at follow up. Step off signs were changed from -10mm in 6 cases, -5mm in 18 and flush in 1 preoperatively to -5mm in 3, flush in 10 and +5mm in 12 at follow up. Side to side difference of instrumented posterior laxity test was improved front 13.6mm preoperatively to 4.3mm at follow up. Conclusions : PCL reconstruction by tibial inlay technique is considered to be a good treatment method to restore posterior stability of the knee, which could result in satisfactory clinical and radiological results at more than 2 years' follow up.

The Reliability of the Conventional Tomographic Interpretation for the Patients with Temporomandibular Disorders (턱관절장애환자의 일반단층촬영 판독에 대한 신뢰도)

  • 본 연구는 측두하악관절장애의 증상이 있는 악관절의 통상적인 방사선 단층촬영술에 대한 해석과 관절강 측정에 대한 조사자내, 조사자간 신뢰도를 조사하였다. 신뢰도 검사는 조사자들 간에 판독의 일관성이 있는지와 일정 시간이 지난 후 반복 측정 시 판독의 정확성이 있는지를 알아보기 위하여 시행하였다. 조사자내 상관계수는 과두 형태에 대해서는 각각 0.812와 0.619, 골변화 형태에 대해서는 모두1.00, 골변화에 대해서는 0.846과 0.991, 전방관절강에 대해서는 0.919와 0.966, 상방관절강에 대해서는 0.864와 0.955, 그리고 후방관절강에 대해서는 0.718과 0.672였다. 통상적인 방사선 단층촬영술이 신뢰도가 있음을 보여주는 이 연구의 결과로 보아, 측두하악 관절을 평가하는데 있어서 통상적인 방사선 단층촬영술은 높은 진단학적 정확성과 조사자간 일치성을 가진다고 할 수 있다.

Calcific Tendinitis in Posterosuperior Labrum of Shoulder - Case Report - (견관절의 후상방 관절와 순에서 발생한 석회화 건염 - 증례보고 1례-)

  • Most of the caicific tendinitis have been reported to be found on the rotator cuff, in particularly on supraspinatus. We reported a case of calcific tendinitis on the posterosuperior glenoid labrum. The location of the lesion was diagnosed accurately by the MRI and easily removed the lesion with arthroscopic surgery and got satisfactory results.
