The Effects of Fowler's Position Change on Back Pain and Discomfort of the Patients Following Percutaneous Coronary Intervention (경피적 관상동맥중재술 후 반좌위 체위변경이 환자의 요통과 불편감에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of position change from supine to Fowler's on back pain and discomfort in patients who remained on bed rest after undergoing PCIs with a vascular closure device. Methods: Data was collected from 35 inpatients who were hospitalized in coronary-care unit to perform PCIs with a vascular closure device at S hospital in Seoul from December, 2006 to May, 2007. Back pain, discomfort, presence and grade of bleeding and hematoma from femoral arterial puncture site and blood pressure/pulse rate were measured prior to, 10 minutes, 1 hour and 2 hours after position change. Data was analyzed with descriptive statistics, $x^2$ tests and t-tests using SPSS/WIN 12.0 for Windows program. The level of significance (${\alpha}$) was set at 0.05 for this study. Results: We found that there was no significant difference in back pain and discomfort in 10 minutes after position change between the two groups. However, the experimental group reported significantly less back pain and discomfort than the control group in 1 hour (p<.01, respectively) and 2 hour (p<.01, respectively) after position change. There was no significant difference in the presence and grade of bleeding and hematoma at the puncture site, blood pressure, and pulse rate in 10 minutes, 1hour and 2hours after position change between the two groups. Conclusion: Fowler's position change after PCIs in, therefore, safe and effective method of reducing back pain and physical discomfort without causing additional bleeding and changes in blood pressure and pulse rate.

A Study on Perceived Discomfort in Patients Following Cardiac Catheterization (심도자 검사 환자의 불편감에 관한 연구)

  • Cardiac cathererization has become a routine diagnostic procedure indicated for evaluation of a wide varity of cardiac conditions. Patients are admitted to the coronary care unit after cardiac catheterization. These conscious patients report feeling uncomfortable in the CCU, but no studies have been done on the effects of bed rest, sand bags on the femoral puncture site and restricted mobility for 4 to 12 hours or longer postprocedure. The main objective of this thesis is to provide basic data to nursing on interventions which de-crease the uncomfortableness experienced by patients in the CCU following cardiac catheterization. In this phenomenological study, the various discomforts felt by the patients were collected and classified. The study subjects were a convenience sample of 29 patients who were admitted to the CCU of a general hospital in Inchon following cardiac catheterization. They were conscious, so they were able to communicate without difficulty. The data were collected over an U days period from July 21, to October 14, 1994. The subjects were interviewed using unstructured open questions and the interviews were tape recorded with the patient's permission The data were analyzed using the Van Kaams phenomenological method. Reliability and validity were exammed by two professor of nursing science, one head nurse, one staff nurse and one cardiologist. The results of the study are summarized as follows ; 1. The 129 descriptive statements by the postcardiac catheterization patients of discomfort were organized into 19 themes. 2. The 19 themes were divided into 3 categories ; physical, psychological, and environmental aspects. 3. The problems concerning the physical aspect were the discomfort of restriction of movement, dysuria, medical devices, pain in the puncture site, symptom is related to the procedure of cardiac catheterization, headache and dizziness, leg painand tingling sensation, and chest pain. The problems concerning the psychological aspect were regret resulting from dependency, economic burden, dissatisfaction with medical personnel, dissatisfaction with medical service system, anxiety about the result of the procedure, concern about the prognosis, loneliness, and concern over treatment procedure. The problems concerning the environmental aspect were influence from neighboring patients, noise, and maladaptation to environmental change. The necessity for holistic care which satisfies physical, psychological, and environmental need must be emphasized in order to solve these discomforts.

  • The prosthesis of the implant installed in inappropriate positions presents aesthetic and functional problems. If the implants are placed in the wrong position, re-implantation is often limited. There are surgical and non-surgical methods for resolving complications without re-implantation. The surgical costs, healing time, discomfort and unpredictability make this choice unpopular. On the other hand, a gingival mask has the advantage of solving complications quickly and simply. The patient was a 80-year-old male with palatally installed implant in maxillary anterior region and dissatisfied with his unesthetic philtrum and food impaction between the upper lip and the prosthesis. It was difficult to predict the prognosis of surgical operation, and the patient wanted treatment economically and physically burdenless because of his age and financial situation. Thus, the gingival mask was planned and the results were satisfactory.

A study on the effect of massage and exercise on discomfort in patients following cardiac catheterization (맛사지와 운동이 심도자검사 환자의 불편감에 미치는 영향에 관한 연구)

  • Cardiac catheterization has become a routine diagnostic procedure indicated for evaluation of a wide variety of cardiac conditions. Patients are admitted to the coronary care unit after cardiac catheterization. These conscious patients used to report discomfort in the CCU, but no studies have been done to reduce discomfort induced by bed rest, sand bag on the femoral puncture site and restricted mobility for about 20 hours postprocedure. The main objective of thesis is to provide basic data to nursing on interventions which decrease discomfort perceived by patients in the CCU following cardiac catheterization. The subjects were 79 patients following cardiac catheterization who were admitted to the CCU of a general hospital in Inchon. These patients were divided into a massage-applied group, an exercise-applied group and an contrast group. Questionare was prepared by Lee(1995), and tested for content reliability by item analysis : Cron bach's ${\alpha}$ for the instrument measuring discomfort was. 63. The data were collected from January 25 to May 15, 1996. Analysis of data was done by paired t-test, ANOVA ,SNK test, Chi-square test. The results of the study are summarized as follows : 1. There were no significant differences among a massage-applied group, an exercise-applied group and an contrast group according to general information except religion and admission period. 2. There were found no significant differences among the three groups in the results of pretest (The score of the first : 55.5, the second : 54.3 and the last : 51.6), 3. In the degree of discomfort according to general characteristics were found some significant differences for marital status. But there were no significant differences for sex, age, occupation, number of family, religion, educational status and admission period. 4. There was effect on reduced discomfort with massage or exercise following cardiac catheterization.

Discomfort and Bleeding in Relation to Bedrest Time after Bone Marrow Examination among Hemato-oncology Patients (혈액종양 환자의 골수검사 후 침상안정 시간에 따른 불편감 및 출혈)

  • Purpose: The purpose of this study was to investigate hemato-oncology patients' discomfort and bleeding in relation to the bedrest time after bone marrow examination. Methods: A descriptive correlational study was conducted. The data were collected using self- report questionnaire from total of 131 patients who underwent bone marrow examination from January 2017 to September 2017. Data were analyzed with descriptive statistics, Wilcoxon Signed-rank test, McNemar's test and logistic regression. Results: The level of discomfort after 4 hours of bedrest was significantly higher when compared to 2 hours of bedrest(p<.001). The occurrence of bleeding after 2 hours of bedrest was significantly higher than 4 hours of bedrest(p<.001), however the degree of bleeding was slight. No bleeding occurred in 84% of the patients after 2 hours of bedrest. Conclusion: The results of this study demonstrated that shortening the bed rest time after bone marrow examination was helpful in improving the patient's well-being. Bedrest time could be shortened according to the site of bone marrow examination and patient's condition.

  • Purpose: Patients usually suffer from pain and discomfort after oral surgery and their consumption of food is severely affected. Accordingly, the patients' quality of life reduce significantly. The aim of this study was to analyze patient satisfaction on the nutritional supplement drinks following periodontal surgery. Materials and Methods: Total 90 patients who underwent periodontal surgery were recruited and commercially available nutritional drinks were provided. Group I received two bottles per each meal for 2 days, Group II had one bottle per each meal for 3 days, and Group III had no drinks. The survey for the patient-reported outcome measures (PROMs) were given at the next visit for the removal of sutures. Results: The result of PROMs showed the nutrition drink was easy to intake, appeared to help the healing and can be a satisfying supplement for the meals. Conclusion: Nutritional drinks appeared to help the healing after periodontal surgeries and supported the food intake. Also, it was helpful for the patients in nutritional supply and psychological stability.

Efficacy of Aloclair$^{TM}$ for Treatment of Intraoral Ulcer Caused by Orthodontic Appliance (교정장치에 의해 유발된 구내궤양 치료에 대한 Polyvinylpyrrolidone-sodium hyaluronate (Aloclair$^{TM}$)의 임상적 효과)

  • This experimental study was undertaken to evaluate the efficacy of polyvinylpyrrolidone-sodium hyaluronate(Aloclair$^{TM}$) in decreasing pain in orthodontic procedures. A total of 60 patients who were use Aloclair$^{TM}$ ordered to were included. These patients were categorized by 3 cases; 1. Patients who were bonded with their first brackets. 2. Patients who were in pain with additional appliance such as anchorage devices. 3. Patients who got ulcer caused by appliances. We collected information about pain and assessed it using visual analogue scale. According to questionnaire, it took 3.5 min. for pain to subside and pair relief durated for 2.7 hours. Most patients in the study reported rapid pain relief and found Aloclair$^{TM}$ easy and pleasant to use than wax. Accordingly Aloclair$^{TM}$ provided raid relief of resting and provoked ulcer pain.

A Study on the Influence of Business performance on the Adoption Factors of Information Systems in Information and Communications Industries (정보시스템 도입 요인이 기업 성과에 미치는 영향에 관한 연구 - 정보통신산업 중심으로-)

  • We, in this study, tried to examine the influence of business environment, organization structure and information technology characteristics used on the performance of the information systems in Information and Communications Industries. The result shows that frequency of system usage and users' satisfaction are positively affected by the factors of business environment, organizational structure and information-technology. The user satisfaction of the system is affected by the frequency of system usage. Both financial performance and non-financial performance are affected by the frequency of the systems usage and user satisfaction of the system. With path analysis, we verified the overall model goodness-of-fit verified for the research model. The result shows that organization structureand technological factors are positively related to the frequency of usage and user satisfaction of the system, while the environmental factors are not. The frequency of system usage is not linked to both financial and non-financial performance. The user satisfaction of the system does not affected to the financial performance. In conclusion, the usage frequency and user satisfaction of the system are the moderating variables affecting business performances. These two variables are affected by the environment, organization structure and information technology. The Path analysis also supports the relationships among these variables.

Effect of Jang-Gu Program and Self-help Management Program on Depression, Stress, Pain and Body Discomfort in Women with Osteoarthritis (장구치기와 관절염 자조관리교육이 골관절염 여성 환자의 우울, 스트레스, 통증, 신체불편감에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of music therapy (Jang-Gu) on depression, stress, pain and body discomfort in women with osteoarthritis. Methods: The study is designed using one group pretest-posttest experimental design. 31 arthritis patients, the subjects of the group participated in the music therapy program. The program was provided for 12 weeks (two times per week). The questionnaires were used to measure the outcome variables before and after the program. Data were analyzed with the SPSS 17.0 using descriptive statistics, paired t-test. Results: Depression, Stress, Pain did not decrease. But body discomfort decreased (p=0.001). Conclusion: Therefore this program was effective in body discomfort and can be a community based self-help management program for arthritis patient.

Evaluation of sleep quality and stress response in patients requiring dental prosthetic treatment (치과보철치료를 요하는 환자에서의 수면질과 스트레스 반응 평가)

  • Purpose: The purpose of this study is to investigate the stress response and sleep state of the new patients who visit the hospital for prosthodontic treatment due to tooth loss to make reference to further treatment direction. Materials and methods: Examinations, consultations, and surveys were conducted on 236 patients (94 males, 142 females, and 24-86 years old), who newly registered at the department of prosthodontics at Pusan national university hospital, for 2 years from 2018. The number of remaining teeth, edentulous arch, possibility of mastication, and future prosthetic treatment directions were recorded. The visual analogue scale (VAS), stress response inventory (SRI) and Insomnia severity index (ISI) were used as tools to measure patient's subjective discomfort, stress response, and sleep status. Results: Among the entire prosthodontic patient group, the average value of 'fatigue' was significantly higher among the 7 items of SRI, and the value of 'aggression' was significantly lower (P<.001). There are a significant difference between the edentulous and full dentition in the oral discomfort value measured by VAS (P=.004). In cases where less than 20 teeth remain, mastication is impossible, and patients who are planning treatment with removable prostheses, VAS value and 'depression' and 'fatigue' values in SRI were higher than those in the reverse case (P<.05). The patients showed severe level in VAS value have high value of 'tension', 'somatization', 'depression', 'fatigue'and 'frustration'in SRI than non-serious patients (P<.05). Clinical insomnia occurred in 11.4% of all patients, and women had significantly lower sleep quality than men (P=.044). Patients with insomnia showed significantly higher scores on the VAS value and all 7 SRI items than those of normal sleepers (P<.05). Conclusion: The new prosthetics had high 'fatigue' value among the 7 items of SRI. The oral condition, number of residual teeth and mastication function and clinical insomnia affected oral discomfort and the stress response.
