Purpose: The Planning of High-Dose-Rate (HDR) brachytherapy treatments are becoming individualized and more dependent on the treatment planning system. Therefore, computer software has been developed to perform independent point dose calculations with the integration of an isodose distribution curve display into the patient anatomy images. Meterials and Methods: As primary input data, the program takes patients'planning data including the source dwell positions, dwell times and the doses at reference points, computed by an HDR treatment planning system (TPS). Dosimetric calculations were peformed in a $10\times12\times10\;Cm^3$ grid space using the Interstitial Collaborative Working Group (ICWG) formalism and an anisotropy table for the HDR Iridium-192 source. The computed doses at the reference points were automatically compared with the relevant results of the TPS. The MR and simulation film images were then imported and the isodose distributions on the axial, sagittal and coronal planes intersecting the point selected by a user were superimposed on the imported images and then displayed. The accuracy of the software was tested in three benchmark plans peformed by Gamma-Med 12i TPS (MDS Nordion, Germany). Nine patients'plans generated by Plato (Nucletron Corporation, The Netherlands) were verified by the developed software. Results: The absolute doses computed by the developed software agreed with the commercial TPS results within an accuracy of $2.8\%$ in the benchmark plans. The isodose distribution plots showed excellent agreements with the exception of the tip legion of the source's longitudinal axis where a slight deviation was observed. In clinical plans, the secondary dose calculations had, on average, about a $3.4\%$ deviation from the TPS plans. Conclusion: The accurate validation of complicate treatment plans is possible with the developed software and the qualify of the HDR treatment plan can be improved with the isodose display integrated into the patient anatomy information.
This study is therefore aimed at measuring the surface dose rate and the spatial dose rate in and outside the radionuclide facility in order to ensure safety of the patients, radiation workers and family care-givers in their use of such equipment and to provide a basic framework for further research on radiation protection. The study was conducted at 4 restrooms in and outside the radionuclide facility of a general hospital in Incheon between May 1 and July 31, 2014. During the study period, the spatial contamination dose rate and the surface contamination dose rate before and after radiation use were measured at the 4 places-thyroid therapy room, PET center, gamma camera room, and outpatient department. According to the restroom use survey by hospitals, restrooms in the radionuclide facility were used not only by patients but also by family care-givers and some of radiation workers. The highest cumulative spatial radiation dose rate was 8.86 mSv/hr at camera room restroom, followed by 7.31 mSv/hr at radioactive iodine therapy room restroom, 2.29 mSv/hr at PET center restroom, and 0.26 mSv/hr at outpatient department restroom, respectively. The surface radiation dose rate measured before and after radiation use was the highest at toilets, which are in direct contact with patient's excretion, followed by the center and the entrance of restrooms. Unsealed radioactive sources used in nuclear medicine are relatively safe due to short half lives and low energy. A patient who received those radioactive sources, however, may become a mobile radioactive source and contaminate areas the patient contacts-camera room, sedation room, and restroom-through secretion and excretion. Therefore, patients administered radionuclides should be advised to drink sufficient amounts of water to efficiently minimize radiation exposure to others by reducing the biological half-life, and members of the public-family care-givers, pregnant women, and children-be as far away from the patients until the dose remains below the permitted dose limit.
I made inquires about mammographic equipments and circumstances of mammography rooms in the 64 medical facilities in areas of Seoul and Kyong Gi Do. Moreover I had experments about exposure dose with patients and radiologic technologists. so there is the data indicated follows. 1. There are inclined to improve in quality and function of mammographic equipments, it has been proven that s/f system exchanged to DR system. 2. It is certain that the number of examinations are becoming increasingly significant. 3. The Space Scattered Dose of mammography rooms are much more larger than portable equipments. 4. I worry about the affection of expose dose about Space Scattered Dose of mammography room. 5. There is need of study how to cope with the situation about increasing exposure dose of radiologic technologists in small space and numeruous number of examinations.
This interest in radiation exposure makes increasing doctor's awareness and knowledge of radiation dose in patients during X-ray test important in reducing patient's uneasiness. However, very few facilities are equipped with measurement instruments. Therefore, an intensive study to find out patient dose using computational method has been initiated. This study used special features of the bit system and NDD-M and directly measured the output dose of diagnostic X-ray instruments used in Korea to create tables. Two different methods were found to be adequate when applied to cases when X-ray outputs were both known and unknown, and comparative experiments with real measurement doses were carried out. Presented methods were found to provide more accurate results compared to the bit system and NDD-M. Therefore, patient dose during clinical trials were found to be more easily acceptable to medical personnel in the radiation field in terms of radiation exposure and reduction of medical X ray dose.
Low energy x-rays that occur in the low tube voltage radiography of general radiography are absorbed strongly in the body and do not aid image quality enhancement. This study maintains titer in general radiography while using tube current that are proportional to density and the tube voltage 15% principle according to density to reduce patient exposure doses, and area doses and entrance surface doses were measured to compare patient exposure doses. In hand, knee, abdomen, and skull radiography, kVp was increased to 115% and mAs was decreased to 50% and kVp was decreased to 85% while mAs was increased to 200% and area doses and entrance surface doses were measured to compare relative doses. Also, 5 places in each image were set, density was measured, and Kruskal wallis H test was conducted to observe significance probabilities between groups. To fix density, kVp was increased to 115% and mAs was decreased to 50% and after measurements of mean area doses and entrance surface doses were made by each part, each decreased to 58.68% and 59.85% when standard doses were set to 100%, and each increased to 147.28% and 159.9% when kVp was decreased to 85% and mAs was increased to 200%. Comparisons of density changes showed that hand, knee, abdomen, and skull radiography all displayed significance probabilities>0.05, showing no changes in concentration. Radiography that increases kVp and lowers mAs through reasonable calculations within ranges that don't affect resolution and contrast seems to be a simple way to decrease patient exposure doses.
In order to reduce the irresistible radiation exposure of patients who perform periodic examinations using a CT among various scan parameters a method to reduce patient dose was investigated through changes in the tube voltage close to X-ray penetrating power. As a result of the experiment 100 kVp was applied instead of 120 kVp which is commonly used in clinical practice and CTDI decreased by about 41% during scan. In addition the degree of change in image quality was measured as 1046.1±3.7 HU for CT value and 71.4±7.9 for Pixel value and statistical analysis showed no significant difference (0.05

Purpose: Opposing portal irradiation with helmet field shape that has been given to a patient with brain metastasis can cause excess dose in patient's scalp, resulting in hair loss. For this reason, this study is to quantitatively analyze scalp dose for effective prevention of hair loss by comparing opposing portal irradiation with scalp-shielding shape and tomotherapy designed to protect patient's scalp with conventional radiation therapy. Materials and Methods: Scalp dose was measured by using three therapies (HELMET, MLC, TOMO) after five thermo-luminescence dosimeters were positioned along center line of frontal lobe by using RANDO Phantom. Scalp dose and change in dose distribution were compared and analyzed with DVH after radiation therapy plan was made by using Radiation Treatment Planning System (Pinnacle3, Philips Medical System, USA) and 6 MV X-ray (Clinac 6EX, VARIAN, USA). Results: When surface dose of scalp by using thermo-luminescence dosimeters was measured, it was revealed that scalp dose decreased by average 87.44% at each point in MLC technique and that scalp dose decreased by average 88.03% at each point in TOMO compared with HELMET field therapy. In addition, when percentage of volume (V95%, V100%, V105% of prescribed dose) was calculated by using Dose Volume Histogram (DVH) in order to evaluate the existence or nonexistence of hotspot in scalp as to three therapies (HELMET, MLC, TOMO), it was revealed that MLC technique and TOMO plan had good dose coverage and did not have hot spot. Conclusion: Reducing hair loss of a patient who receives whole brain radiotherapy treatment can make a contribution to improve life quality of the patient. It is expected that making good use of opposing portal irradiation with scalp-shielding shape and tomotherapy to protect scalp of a patient based on this study will reduce hair loss of a patient.

목적 : 폐암 환자 세기변조방사선치료 과정을 소개하고, 방사선치료계획의 최적화를 위한 빔 수와 방향, 가상장기 설정 (virtual organ delineation, VOD) 및 선량 제한 인자들의 이용에 대해 평가함으로써 폐, 심장 등에 조사되는 선량을 최소화하는데 사용하는 세기변조방사선치료 (intensity modulated radiotherapy, IMRT) 기술의 유용성을 평가하고자한다. 대상 및 방법 : 종양이 종격동을 침범하여 상대적으로 장기움직임에 의한 오차가 적은 폐암환자 5 명을 대상으로 하였다. 환자고정장치는 상반신을 편안하게 유지함과 동시에 팔의 위치를 고정시킴으로써 기대할 수 있는 환자고정효과와 벨트를 이용하여 환자 상복부를 압박해줌으로써 호흡운동에 의한 장기 움직임을 감소시킬 수 있는 형태로 고안하였다. 치료계획시 빔 수와 방향은 5,7,9 문 (from 200 to 160, equispaced field, arbitrary field), 4 문 (anterior, posterior, bilateral posterior oblique field) 과 비등방 7, 9 문 (non-equispaced field, arbitrary field) 등을 사용하였다. 선량제한 ($V_{20}V_{25}$)은 문헌에 기초하여 설정하였으며, 가상장기를 적절히 사용하여 최적화된 치료계획 결과를 얻었다. 방사선치료계획 평가는 선량-체적간 히스토그람 (DVH), 등선량곡선 및 선량통계 등을 이용하여 수행하였다. 특히 가상장기 설정 전, 후의 결과 값을 분석함으로써 그 유용성을 확인하였다. 결과 : 9문 등방-IMRT와 7문 비등방-IMRT 방법이 치료계획용적의 선량균질성 (PTV dose homogeneity), 평균 폐선량 (mean lung dose) 및 $V_{20}V_{25}$ 모두에서 20% 이내의 좋은 결과를 얻을 수 있었고, 가상 장기를 설정함으로써 같은 결과를 가져옴을 알 수 있었다. 또한 폐암 세기변조방사선치료 프로토콜을 작성하여 임상에 사용함으로써 치료과정 중 발생할 수 있는 오류를 보완할 수 있음을 알 수 있었다. 결론 : 폐암 세기변조방사선치료 시 사용할 수 있는 프로토콜을 작성하였고, 적절한 가상 장기 및 조사계획 설정으로 치료계획의 최적화를 얻을 수 있음을 알 수 있었다.

목적 : 현재 국내에서 사용되고 있는 방사선치료계획 장비는 거의 모든 방사선종양학과에서 외국 회사의 제품을 사용하고 있는 실정이다. 본 연구는 국내 기술로 개발된 3 차원 조형 방사선치료계획 장비인 Core Plan의 선량계산 알고리즘의 정확성 및 방사선 치료계획 장비로서의 유용성에 관해 평가하고 본 장비의 특징에 대해 간략히 소개하고자 한다. 재료 및 방법 : 본 연구는 2002 년 11 월에 가톨릭대학교 성모병원 방사선종양학과와 서울씨앤제이의 연구계약에 의해 시행되었다. 본 장비에 대한 평가는 방사선분포 및 계산상의 정확성과 임상적용시의 유용성의 관점에서 시행되었다. 본 장비에서 이용된 광자선 선량계산은 Clarkson-Cunningham 모델이며 전자선 선량계산은 2.5D Hogstrom 알고리즘이다. 방사선분포 및 계산상의 정확성 평가를 위하여 방사선치료 장비는 본 병원이 보유한 Clinac 2100CD (Varian, USA)를 이용하였고 폴리스티렌 팬텀과 필름 및 이온 전리함을 이용하여 방사선분포 및 계산상의 정확성을 평가하였다. 방사선분포의 평가 방법은 필름을 이용한 방사선분포의 중심부단면 선량분포와 CorePlan 에서 재현된 방사선분포의 동일면 선량분포를 비교하였다. 임상적용은 2003 년 3 월부터 7 월까지 방사선치료를 받은 50 명의 환자를 대상으로 분석하였다. 본 시험에 적용된 환자는 본 병원이 보유한 3 차원 방사선치료계획 장비인 Prowess 3D (SSGI Inc., USA)를 이용하여 실제 치료된 환자들이며 이 환자를 대상으로 CorePlan에서 동일하게 재현하여 비교하였다. 결과 및 결론 : 방사선분포 및 계산상의 정확성 평가에서는 실제 측정된 결과와 CorePlan에서 재현된 결과가 모두 $\pm$3% 이내로 평가되었다. 50 명의 환자를 대상으로 시행한 임상시험 결과 Prowess 3D에서 나타낸 결과와 비교하여 두경부에서는 1.678%, 흉부 1.578%, 복부 1.271%의 선량값의 오차를 보였다. 본 연구를 진행하는 과정에서 많은 부분의 프로그램 수정이 있었으며 실제임상에 필요한 부분에 대한 추가 및 보완이 이루어졌다. 앞으로 진행될 과정은 실제임상의 사용에 필요한 부분의 계속적인 업그레이드와 전자선에 대한 방사선분포 및 계산정확성 평가, 임상적용에 있어서 Prowess3D 뿐만 아니라 다양한 방사선 치료계획 장비와의 비교를 할 예정이다.
The medical diagnostic examination using ionizing radiation has improved the patients' life and brought revolution in medical examination along with the mechanical development. However, the development of medical imaging systems has also been the reason to increase the patients' exposure for ionizing radiation. ICRP recommends that each country adopts diagnostic reference levels depending on regional and national situations. The Korea Food & Drug Administration suggested the dosimetry measurement guideline for patients in 2007. Nonetheless, in reality, it is hard to know the skin dose of the patients when applying a x-ray since there is no radiation dosimeter in most of clinical situation. Therefore, this study sets a program based on the bit system to figure out easily the skin dose of a patients using MS Excel program in the PC setting. The results showed 10% better outcome.
