An Explorator Spatial Analysis of Shigellosis (세균성 이질의 탐색적 공간분석)

  • 세균성 이질은 국내 제1종 법정 전염병으로 분류되어 관리되고 있는 질환으로서 1998년 이후 그 발병 사례가 급속히 증가하고 있다. 본 연구는 1999년 3월 부산시 사상구에서 집단 발병한 세균성 이질을 대상으로 하여, 각 환자들의 발병 시점과 장소의 분포패턴에 대한 지리학적 고찰을 목적으로 한다. 환자분포의 특징적 공간패턴과 그들의 시계열적 확산 양상 등을 탐색하기 위한 방법론은 보건지리학과 지도학 및 공간통계학에 기반을 둔 공간분석기법을 중심으로 설정하였다. 분석자료는 해당 지역의 수치지형도, 지적도, 인구 센서스 자료를 포함한 GIS 데이터베이스로 구축되었다. 인구분포를 감안한 밀도구분도를 바탕으로 개별환자의 위치자료와 동 단위로 집계된 자료를 자료의 형태에 따라 분석기법을 달리하였으며, 환자 발생 밀도, 상대적 위험지수 등을 지도화하여 역학자료의 시각적 통계적 분석을 수행하였다. 환자분포의 공간적 중심위치와 분산의 변화 등 기술적 통계분석과 함께 제1차 공간속성을 커널추정법으로 찾아보았다. 이와 더불어 ‘공간적 의존성’과 관련된 제2차 공간속성은 K-함수와 시뮬레이션을 통해 분석하여 군집성 등이 통계적으로 확인되었다. 본 연구를 통해 역학조사시 GIS의 활용사례가 제시되었으며, 모집단 인구를 고려한 확률지도 작성 기법과 다양한 데이터 가시화 방법, 그리고 시계열별 발생 환자들의 지리적 변이를 분석 하는데 따르는 문제들이 논의되었다.

A Study on the Spacial Environment Satisfaction for Evidence Based Design of Rehabilitative Health-Care Facilities -convergent approach for patient-centered rehabilitative healthcare service (근거중심 재활 공간 디자인을 위한 공간 환경 만족도 연구 -환자 중심 재활의료서비스를 위한 융복합적 접근)

  • The purpose of this study is to investigate patient satisfaction for the spacial environment of the rehabilitative facilities, and by this to examine areas of patients' needs to develop patient-friendly improvement in spacial environment. 308 patients were surveyed at physical therapy centers of 6 hospitals or rehabilitative institutions in Daejeon with patient satisfaction questionnaire. The spatial environment satisfaction was categorized into 7 constructs and the data were statistically analyzed and compared by t-test and ANOVA according to the patient characteristics. As a result, inter-floor transfer was found to be the area with the least satisfaction, especially in the in-patient group. Locating floor level was the second lowest satisfied construct. Comfort was relatively highly satisfied construct overall, but patients with neurological disease had less satisfaction than with musculoskeletal disease. Patients in 20's were less satisfied in wayfinding than 60's. Satisfaction for interior design was lower in subject in 50's than 70's. These findings can be used as evidences to improve spacial environment design for rehabilitative facilities to the better patient satisfaction.

  • Most of the existing studies of hospital ward were concentrated on the bed room and there was no study on the day place and corridor space for the inpatient. According to the existing study, 50% inpatients move here and there freely and they want to have a rest in the day place or corridor of the general hospital. So the purpose of this study is to find out the reason why the inpatients prefer to rest in the day place, to survey the inpatients' behavior there and to stress the importance of exterior environment except the bed room in the public space of hospital.

Measurement of the Spatial Dose Rates During PET/CT Studies (전신 PET/CT 검사에서 공간선량률 측정)

  • In order to evaluate the exposure to the radiologic technologists from patients who had been administrated with radiopharmaceuticals, we measured the spatial dose rates at $5{\sim}300\;cm$ from skin surface of patients using an proportional digital surveymeter, 1.5(PET scan) and 4hr(bone scan) after injection. In results, the exposure to the technologists in each procedure was small, compared with the dose limits of the medical workers. However, the dose-response relationships in cancer and hereditary effects, referred to as the stochastic effects, have been assumed linear and no threshold models ; therefore, the exposure should be minimized. For this purpose, the measurements of spatial dose rate distributions were thought to be useful.

  • The purpose of this study is to suggest a professional consensus on the topics that should be dealt with importantly in the movement program for the recovery of cancer patients in Korea. As a result of calculating the importance of the finalized movement program component, First, the opinions about the environment for the promotion of movement were suggested as pleasant facilities, natural lighting, rest room space, facilities aspect of environment friendly space, education about diseases, participation programs, health feedback, information handouts. Second, the opinions on the purpose of the movement program were suggested to promote prevention and early screening of cancer, education, sharing effective treatment methods of cancer patients, cognitive aspect and psychological support for accurate information transmission, stress relief, and improvement of quality of life. Third, the opinions on the consideration of the movement program suggested stress relieving, cancer patient's needs, correct information delivery, quality of life, and participation of cancer patients. Fourth, the problems of the movement program were presented with different physical characteristics, physical inconvenience, the patient side of passive participation, space narrowness and lack, space aspect of program exclusive space etc.

  • According as the aging procese have been high, last stage patient become more important. Therefore, todays hospice ward of hospital is glandized, specified too. However, its study is few performed in domestic and its architectural planning data is few obtained. Therefore, this study aims to represent the standard of the architectural planning of the hospice ward in hospital. As its method, the present conditions, the interview with staffs, space programs of 4 hospice ward were investigated and analyzed, add to 2 free standing hospice were selected, theirs architectural space programing were studied.

Assessments of Spatial Neglect : Comparison of KF-NAP (Kessler Foundation - Neglect Assessment Process) and Pencil-Paper test (편측 공간무시 환자의 평가: KF-NAP(Kessler Foundation - Neglect Assessment Process)과 지필검사의 비교)

  • Objective : The aim of this study was to identify correlation between KF-NAP and Pencil-Paper test(Line bisection test, Letter cancellation, Albert's test) and to determine the high sensitive assessment tool for spatial neglect. Method : Thirty-two stroke patients who admitted to an inpatient rehabilitation unit at the medical center in Seoul from September, 2014 to September, 2015 were included in our research. They all have spatial neglect (male=19, female=13, the average age 61.8). KF-NAP and Pencil-Paper test were performed within 7 days. The detection of spatial neglect was analyzed by total scores of each assessments. Results : The highest sensitivity assessments of spatial neglect was KF-NAP. The detection of spatial neglect through KF-NAP was 93.8% (Line bisection test 53.1, Letter cancellation 53.1%, Albert's test 37.5%). Line bisection test (r=.430), Letter cancellation(r=.641), Albert's test(r=.398) and KF-NAP scores had significant correlation(p<0.05). Conclusion : KF-NAP is higher sensitive in comparison with Pencil-Paper test for detection of spatial neglect as the assessment tool of spatial neglect during activities daily livings of patients and neglect severity could be assessed with KF-NAP. Therefore, KF-NAP is recommended for assessment of spatial neglect.

The Measurement of Spatial Dose Rate by Gravity Ventilation after Technegas Scanning (Technegas 스캐닝 후 중력환기에 의한 공간선량율 측정)

  • Because examination with technegas produces images through simple diffusion accumulation, the examination room can become contaminated after scan. Therefore, radiation workers and patients awaiting examination will be affected by internal exposure from technegas inhalation. Before and after gravity ventilation, I am trying to find a way to reduce the exposure dose of waiting patients according to a comparative analysis of horizontal spatial dose rates over time. Spatial dose ratio were measured for 10 minutes from various distances and angles around ventilator's location before and after gravity ventilation. Then, mean values, standard deviation and reduction ratio were calculated. The highest reduction rate of gravity ventilation was 95.31% and the highest reduction ratio was 1 to 3 minutes. Therefore, the gravity ventilation could reduce the exposure dose of radiologic technologists, waiting patients, patient guardians and nurses. In conclusion, the reduction of the exposure dose during the technegas ventilation study through gravity ventilation will play a role in optimiging the protection and it is in accordance with the recommended reduction of the medical exposure by ICRP 103.
