Background: The aim of this study was to determine the effectiveness of self-efficacy promoting pulmonary rehabilitation in patients with chronic obstructive pulmonary disease (COPD). Methods: thirty six patients, with clinically stable COPD were randomly assigned: 18 to a rehabilitation group and another 18 as a control group, The subjects participated in a the self-efficacy promoting pulmonary rehabilitation program for 8 weeks. This program consisted of education, breathing retraining, exercise training, relaxation and counseling. The control group received education only. The outcome variables were self-efficacy, dyspnea, exercise endurance, pulmonary function, and quality of life. Dyspnea was measured using the modified Borg scale. Exercise endurance was measured by the six minute walking distance. The quality of life was measured by the quality of life index for pulmonary disease patients. Results: In the rehabilitation group after performing the self-efficacy promoting pulmonary rehabilitation program, the self-efficacy score, exercise endurance, and quality of life score were higher than the control group (p=0.007, p=0.038, and p=0.039, respectively). and the exertional dyspnea score was significantly lower than controls(p=0.045). However, the dyspnea score and FEV1 were similar after performing the self-efficacy promoting pulmonary rehabilitation program. Conclusion: The self-efficacy promoting pulmonary rehabilitation program is effective to in improve self-efficacy, exertional dyspnea, exercise endurance and quality of life in patients with COPD.

The purpose of this research was to investigate the effects of respiratory rehabilitation using a wind instrument for patients suffering from spinal cord injuries. From January 15, 2018 to April 15, 2018, we conducted ten 1-hour sessions of a harmonica program with eight patients with spinal cord injuries with average age of 37 years who could not perform abdominal breathing by themselves. We measured and compared patients' breathing capacity before and after the 10 sessions. Designed particularly for patients with spinal cord injuries resulting in a limited range of neck movement, the study used a 10-hole diatonic harmonica whose length was relatively short. For those patients who had difficulty using their hands, a harmonica holder was provided. Participants were trained to play simple tunes. They were guided to use abdominal breathing to make sounds, with emphasis on those parts requiring long and strong breathing. The results showed that for all eight patients both their breathing volume and their inspiratory volume increased following participation in the harmonica program. Also, the program had psychological benefits (e.g., more life satisfaction and less sadness) and additional physical benefits (e.g., less dizziness due to low blood pressure and better phlegm spitting). This study offers a unique way to help patients with spinal cord injuries to improve their breathing capacity, which may also be associated with a greater quality of life.
Purpose: The purpose of this study was to examine the effects of an inpatient pulmonary rehabilitation program on dyspnea, exercise capacity, and health related quality of life in inpatients with chronic lung disease. Method: This quasi experimental study was designed with a nonequivalent control group pre-post test time series. Twenty three patients were assigned to the experimental group and nineteen to the control group. The inpatient pulmonary rehabilitation program was composed of upper and lower extremity exercise, breathing retraining, inspiratory muscle training, education, relaxation and telephone contacts. This program consisted of 4 sessions with inpatients and 4 weeks at home after discharge. The control group was given a home based pulmonary rehabilitation program at the time of discharge. The outcomes were measured by the Borg score, 6MWD and the Chronic Respiratory Disease Questionnaire(CRQ). Results: There was a statistically significant difference in dyspnea between the experimental group and control group, but not among time sequence, or interaction between groups and time sequence. Also significant improvements in exercise capacity and health related quality of life were found only in the experimental group. Conclusions: An Inpatient pulmonary rehabilitation program may be a useful intervention to reduce dyspnea, and increase exercise capacity and health related quality of life for chronic lung disease patients.
The purpose of this study is to investigate the influence on respiration, oral motor and articulation using wind-instrument centered music therapy for patients with cervical cord injury who need continuous train of respiratory muscle. Three patients with cervical cord injury who needed continuous training of respiratory muscle were selected and post data was analyzed. The harmonica using both exhalation and inspiration was selected as an wind-instrument. Each session was taken 30 ~ 40 minutes, once a week from total 6 weeks with small group. Material about every session's work was provided for individual to maintain practice after program. Oral motor and articulation test was done to assess reflecting features of maximal expiratory flow and wind-instrument of factor related breath. Maximal expiratory flow has increased by average 25ml more, articulation has increased by 3.16 points more and the movement of oral motor has increased 11.67 points more than pre-test from the analyzation. In the comparison of the details, the increase of oral motor function was confirmed from scores on the jaw and tongue except for the lips. Based on the results of this study, this study suggests that wind-instrument centered music therapy will be a practical and effective intervention for respiratory rehabilitation in patients with cervical cord injury.
The purpose of this study was to develop a stuttering therapy device to aid in stutter therapy. The research method used for this study was as follows: First, the stuttering therapy device based on analysis of the prolonged speech method used at home and abroad was designed to achieve the goal of research. Second, the stuttering therapy device was to be developed to maintain a vocalization state, to use bio-feedback visualization, to have enough inspiration, to use Korean language in this device, and to use transfer and maintenance training in daily life. Third, the stuttering therapy device effectiveness was to be verified through use in clinical cases. The results of subjects receiving speech therapy and using the breathing control prolonged speech device and SI(stuttering Interview) evaluation programs for 3 months were as follows: For subject A, the stuttered word rate was reduced from 3.20 SW/M to 0.5 SW/M. For subject B, the stuttered word rate was reduced from 1.90 SW/M to 0.75 SW/M. For subject C, the stuttered word rate was reduced from 3.37 SW/M to 0.34 SW/M. For Subject D, the stuttered word rate was reduced from 0.51 SW/M to 0 SW/M. Follow-up evaluations verified the effectiveness of how the stuttering therapy device can reduce subjects' SW/M.
It is known that a pulmonary rehabilitation program improves dyspnea and exercise tolerence in patients with chronic obstructive pulmonary disease. However, it is also known that although it does not improve pulmonary function. This study was performed to evaluate the effect of a 4 week pulmonary rehabilitation on pulmonary function, gas exchange, and exercise tolerance in patients with chronic obstructive pulmonary disease. The pulmonary rehabilitation programs included breathing exercises, such as pursed-lip breathing and diaphragmatic breathing, upper-limb exercises, and inspiratory muscle training. These activities were performed for 4 weeks in twenty one patients with chronic obstructive pulmonary disease. Pre and post-rehabilitation pulmonary function and exercise capacities were compared after the 4 week period. Results are as follows: 1) Before the rehabilitation, the predicted value of FVC and FEV1 of the patients were 70.3$\pm$16.7% and 41.1$\pm$11.9% respectively. These pulmonary functions did not change after pulmonary rehabilitation. 2) Aloility of walking a 6 minute distance (325.29$\pm$122.24 vs 363.03$\pm$120.01 p=.01) and dyspnea (p=.00) were significantly improved after rehabilitation. Thus showing that pulmonary rehabilitation for 4 weeks can improve exercise performance and dyspnea in patients with chronic obstructive pulmonary disease.
Purpose: This study is aimed to analyze the trend of research on pulmonary rehabilitation of patients with COPD (Chronic Obstructive Pulmonary Disease), and to suggest a direction for future research of pulmonary rehabilitation of COPD. Method: A total of 39 studies published from 1991 to 2002 were examined according to the year of publication, type of journal, characteristics of patients, rehabilitation program, research design, and instruments and research findings. Results: 1) The number of studies related to pulmonary rehabilitation of COPD patients has rapidly increased since 1996's (87.18%) 2) There were 10 studies published in the chest. There were 6 studies published in domestic journals and 33 studies published in international journals. 3) The samples in the studies were mostly outpatients whose lung function was moderate. 4) The total period of the pulmonary rehabilitation program shown in the literature was 5 to 12 weeks. The programs included exercise interventions, educational interventions and psychosocial interventions. 5) The outcomes of the pulmonary rehabilitation were measured with lung function, 6-minute walk test, dyspnea, and chronic respiratory disease questionnaire. Research findings were found to be effective on 6-minute walk test, dyspnea, and chronic respiratory disease questionnaire but not on lung function. Conclusion: Based on these findings, pulmonary rehabilitation should be further emphasized, and standardized protocols and community based pulmonary rehabilitation programs need to be provided. More qualitative research should also be conducted in the future to describe the experiences of the pulmonary rehabilitation of patients with COPD.
Purpose: The purpose of this study is to exam the effects of a short-term pulmonary program on lung function, exercise tolerance, and quality of life in chronic lung patients. Method: Randomized controlled pre-post test design was used. The outcome measures were forced expiratory volume in one second (FEV1, % predicted), 6 min walking distance (6MWD), Borg score after 6MWD, and Chronic Respiratory Disease Questionnaire (CRDQ). Experimental group performed the 4-week home-based pulmonary rehabilitation program composed of inspiratory muscle training, upper and lower extremity exercise, relaxation, and telephone visit. Patients in control group were only given education about self-management strategies. Thirty four patients with moderate-to-severe respiratory impairment were recruited, and 28 patients (19 in experiments, 15 in control) completed the study. Result: Significant improvements in lung function, exercise tolerance, and health related quality of life were found only in the experiment group. Conclusion: This study yielded evidence for the potential and beneficial effects of home-based pulmonary rehabilitation program in patients with moderate to severe chronic lung disease. The program could be adequately utilized for improvement of health related quality of life in chronic lung patients.
Purpose: The purpose of this study was to analyze the trend of research on the use of home-based pulmonary rehabilitation. Method: Using Medline and CINAHL search engine, experiemental research, titled as 'home-based pulmonary rehabilitation' and 'chronic lung disease', statistics were collected. The total 29 experimental studies published between 1981 to 2000 were selected and analyzed according to the ATS guidelines. Result: 1) The study samples in research were mostly of men, whose lung function was moderate to severe. 2) The total period of the intervention program varied from 5 to 12 week. The program included excercise intervention and educational intervention. Psychosocial intervention was minimal. 3) The outcome of the home-based pulmonary rehabilitation had been measured mostly by physical aspects. Quality of life and cost were less frequently measured. 4) In terms of effectiveness of the home based pulmonary rehabilitation program, physiologic outcome, such as lung function and hematologic markers, there was more 'no effect' than 'positive effect', whereas there was more 'positive effect' in decreasing dyspnea, improving excercise capacity, and improving quality of life. Conclusion: Based on these findings, future research on home-based pulmonary rehabilitation should be emphasized and provide standardized protocol, including psychosocial intervention, and analyses on cost and quality of life.

The Many patients who have lumbar instability(LI) could make a different trunk movement pattern by reduction of their respiratory function and altered breathing pattern. This study was conducted to investigate the change in the thoracoabdominal usage rate(TAUR) on three circumference lines(axillary, xiphoid junction, 10th rib) during the resting and forced breathing (RB and FB) after respiratory pattern correction(RPC) exercises in patients with LI. 15 patients in the experimental group performed RPC exercises, and 15 patients in the control group conducted lumbar stabilization exercises. Before the intervention, both groups had a significant difference in the usage rate among the three thoracoabdominal lines during the RB and FB(p<.05). After the intervention, the experimental group was seen no significant difference in the usage rate among the three lines during the RB(p>.05) and exhibited significant differences in the usage rate between the two thoracoabdominal lines during the FB(p<.05). The RPC exercises might improve the trunk movement patterns by restoring the respiratory patterns. We suggest that the RPC can apply in the re-education and reinforcement process at the reha-program for LI patients.
