Impact of Respiratory Motion on Breast Cancer Intensity-modulated Radiation Therapy (유방암 세기조절방사선치료에서의 호흡운동 영향)

  • In this study, we evaluate the effect of respiration on the dose distribution in patient target volume (PTV) during intensity-modulated radiation therapy (IMRT) and research methods to reduce this impact. The dose distributions, homogeneity index (HI), coverage index (CVI), and conformity index of the PTV, which is calculated from the dose-volume histogram (DVH), are compared between the maximum intensity projection (MIP) image-based plan and other images at respiration phases of 30%, 60% and 90%. In addition, the reducing effect of complication caused by patient respiration is estimated in the case of a bolus and the expended PTV on the skin. The HI is increased by approximately twice, and the CVI is relatively decreased without the bolus at other respiration phases. With the bolus and expended PTV, the change in the dose distribution of the PTV is relatively small with patient respiration. Therefore, the usage of the bolus and expended PTV can be considered as one of the methods to improve the accuracy of IMRT in the treatment of breast cancer patients with respiratory motion.

  • Purpose: To develop the respiration simulating phantom with thermocouple for evaluating 4D radiotherapy such as gated radiotherapy breathing control radiotherapy and dynamic tumor tracking radiotherapy. Materials and Methods: The respiration monitoring mask(ReMM) with thermocouple was developed to monitor the patient's irregular respiration. The signal from ReMM controls the simulating phantom as organ motion of patients in real-time. The organ and the phantom motion were compared with its respiratory curves to evaluate the simulating phantom. ReMM was used to measure patients' respiration, and the movement of simulating phantom was measured by using $RPM^{(R)}$. The fluoroscope was used to monitor the patient's diaphragm motion. relative to the organ motion, respectively. The standard deviation of discrepancy between the respiratory curve and the organ motion was 8.52% of motion range. Conclusion: Patients felt comfortable with ReMM. The relationship between the signal from ReMM and the organ motion shows strong correlation. The phantom simulates the organ motion in real-time according to the respiratory signal from the ReMM. It is expected that the simulating phantom with ReMM could be used to verify the 4D radiotherapy.

Evaluation of the Accuracy for Respiratory-gated RapidArc (RapidArc를 이용한 호흡연동 회전세기조절방사선치료 할 때 전달선량의 정확성 평가)

  • The position of the internal organs can change continually and periodically inside the body due to the respiration. To reduce the respiration induced uncertainty of dose localization, one can use a respiratory gated radiotherapy where a radiation beam is exposed during the specific time of period. The main disadvantage of this method is that it usually requests a long treatment time, the massive effort during the treatment and the limitation of the patient selection. In this sense, the combination of the real-time position management (RPM) system and the volumetric intensity modulated radiotherapy (RapidArc) is promising since it provides a short treatment time compared with the conventional respiratory gated treatments. In this study, we evaluated the accuracy of the respiratory gated RapidArc treatment. Total sic patient cases were used for this study and each case was planned by RapidArc technique using varian ECLIPSE v8.6 planning machine. For the Quality Assurance (QA), a MatriXX detector and I'mRT software were used. The results show that more than 97% of area gives the gamma value less than one with 3% dose and 3 mm distance to agreement condition, which indicates the measured dose is well matched with the treatment plan's dose distribution for the gated RapidArc treatment cases.

  • Oxygen supply is one of the most basic things in human body. Breathing is controlled by the lungs' stationary function and the respiratory center which is in the mesulla oblongata. Nothing but, the external breathing that air movement between the lungs and atmosphere and the internal breathing that cellular air movement between the hemoglobin and a single cell. The adult's number of times of the respirations is about 15∼20 per 1 minute, but it depends ages, exercise, temperature, disease, etc. The important thing in detecting the respiration is that doing it in object person's resting time. Detecting the respiration have to be done without attracting any attention of object person. In present using method is detecting the pulse with catching an object person's wrist and observing the object person's movement. In this paper, we propose the mobile respiration detection diagnostic system using ultrasound sensing method that does not be influenced by the inertia error and the pressure error.

Aerobic Capacity and Ventilatory Response During Incremental Exercise in Elite High School Cyclist (점진부하 운동에서 중고교 엘리트 사이클 선수들의 유산소능력과 폐환기 반응)

  • This study was designed to examine the aerobic capacity and ventilatory response during an incremental exercise in elite high school cyclists. Twelve boys ($17{\pm}1\;yr$, $175{\pm}5\;cm$, $70{\pm}9\;kg$) participated in anthropometric measurements, incremental exercise testing, and pulmonary function tests. During incremental exercise testing using a cycle ergometer, their maximal oxygen uptake ($VO_2max$), maximal power output, ventilation, ventilatory equivalents for oxygen ($V_E/VO_2$) and carbon dioxide ($V_E/VCO_2$), respiratory rate, and tidal volume were measured. Time variables such as inspiratory time (Ti), expiratory time (Te), breathing time (Tb), and inspiratory duty cycle (Ti/Tb), as well as inspiratory flow rate ($V_T$/Ti) were assessed. Pulmonary function of vital capacity (FVC), forced expiratory volume in one second ($FEV_1$), $FEV_1$/FVC, and peak expiratory flow were evaluated. Their $VO_2max$, maximal heart rate, and Wmax were $57.5{\pm}3.9\;ml{\cdot}kg^{-1}{\cdot}min^{-1}$, $194.1{\pm}8.6\;beat{\cdot}min^{-1}$, and 452 W, respectively. $VO_2max$ was not related to any anthropometric parameters. Most ventilatory variables progressively increased with exercise intensity. As intensity increased, Ti, Tb, Tb decreased while Ti/Tb was maintained. Below an intensity of 250 W, height, weight, body mass index, and body surface were highly correlated with $V_T$/Ti and Ti/Tb (p<0.05). Collectively, $VO_2max$ appeared to be lower than adult cyclists, suggesting a different pattern of ventilatory control as age advances. Morphological characteristics were not related to $VO_2max$ in the population. Time variables of ventilatory response seemed to be related only at an exercise intensity level of less than 250 W. $V_T$/Ti may be related to exercise endurance capacity, but Ti/Tb was similar to adult cyclists.

  • Respiration sating radiotherapy technique developed In consideration of the movement of body surface and Internal organs during respiration, is categorized into the method of analyzing the respiratory volume for data processing and that of keeping track of fiducial landmark or dermatologic markers based on radiography. However, since these methods require high-priced equipments for treatment and are used for the specific radiotherapy. Therefore, we should develop new essential method whilst ruling out the possible problems. This study alms to obtain body surface motion by using the couch based computer-controlled motion phantom (CBMP) and US sensor, and to develop respiration gating techniques that can adjust patients' beds by using opposite values of the data obtained. The CBMP made to measure body surface motion is composed of a BS II microprocessor, sensor, host computer and stopping motor etc. And the program to control and operate It was developed. After the CBMP was adjusted by entering random movement data, and the phantom movements were acquired using the sensors, the two data were compared and analyzed. And then, after the movements by respiration were acquired by using a rabbit, the real-time respiration gating techniques were drawn by operating the phantom with the opposite values of the data. The result of analysing the acquisition-correction delay time for the data value shows that the data value coincided within 1% and that the acquistition-correction delay time was obtained real-time $(2.34{\times}10^{-4}sec)$. And the movement was the maximum movement was 6 mm In Z direction, In which the respiratory cycle was 2.9 seconds. This study successfully confirms the clinical application possibility of respiration gating techniques by using a CBWP and sensor.

마스크를 이용한 고지적응 훈련방법의 생리학적 타당성 검증을 위한 연구

  • 고지대에서 개최되는 국제운동경기의 경우에 선수들의 고지대 적응이 경기 력에 큰 영향을 미치는 경우가 많다. 고지대에서의 경기는 선수들에게 현기 증을 일으키고 호흡, 순환계의 곤란을 유발시키며, 피로감을 증대시킨다. 국 내에서는 1983년 멕시코 세계 청소년 축구대회 당시 고지적응훈련의 일환으 로, 국내에서 마스크를 착용하고 연습한 사례가 있었다. 이 훈련방법은 정신 력 강화라는 심리적 측면에서 효과가 있을 수 있으나, 일반적으로 고지에서 2-3주간을 지속적으로 체류한 후에 순응이 일어난다는 사실과, 순응된 기능 이 저지대로 돌아올 경우 단시간내에 회복된다는 점을 감안할 때, 연습 시에 는 마스크를 착용하는 훈련방법이 동정맥산소교차의 감소로 저산소증을 유 발하는 고지상황에서의 호흡문제를 해결하는 데는 유효하지 않은 것으로 판 단되었다. 따라서 효과적이고 경제적인 고지적응 훈련은 인간기능에 관련된 변수들의 연관관계를 체계적으로 파악하고, 이 변수들을 효과적으로 조절하 여야만 가능할 것으로 평가되었다.

Effects of Some Metabolic Inhibitors on Phototactic Movement in Cyanobacterium Synechosystis sp. PCC 6803 PTX (람세균 Synechocystis sp. PCC 6803 PTX의 주광성 운동에 미치는 몇가지 대사 억제제의 효과)

  • For understanding physiological nature of phototaxis in Synechocystis sp. PCC 6803 PTX(S. 6803 PTX), we examined the effects of some metabolic inhibitors and cation ionophore on the phototactic movement. In the presence of DCMU, which blocks the photosynthetic electron transport just after photosystem II acceptor, there was no inhibitory effect on the phototaxis up to $100\;\mu\textrm{M}$. Instead, the respiratory electron chain inhibitor such as sodium azide dramatically impaired the phototaxis in S. 6803 PTX. These observations indicate that the phototaxis is linked not to photo-phosphorylation, but to respiratory phosphorylation. When the cells were treated with un couplers such as CCCP or DNP, which dissipate the electrochemical gradient of proton($\Delta\mu_{H}+$) across the cytoplasmic membrane, these chemicals did not affect phototaxis. In contrast, when cells were treated with DCCD or NBD which deprive cells of A TP but leave $\Delta\mu_{H}+$ intact across the membrane, the phototactic movement was severly reduced. These results imply that ATP production, not proton motive force, is involved in the phototactic movement in this organism as a driving motive force. The application of specific calcium ionophore A23187 strongly impaired positive phototaxis. Calcium fluxes should be engaged in the sensory trans-duction of phototactic orientation. Finally, when ethionine was supplimented to culture media, the photomovement of this organism was inhibited. This implies that methylation/demethylation mechanism controls the process of phototaxis in S. 6803 PTX like chemotaxis in E. coli and Salmonella typhimurium.murium.

The Study of Functional Independence and Bone Mineral Density in Athletes With Spinal Cord Injury (척수 손상을 가진 운동선수와 비운동선수의 일상생활 기능과 골밀도 비교연구)

  • 목적 : 본 연구는 척수손상환자들 중 신체활동 정도가 급격하게 차이 나는 운동 선수군과 비선수군의 일상생활동작과 골밀도를 비교 분석하는데 그 목적이 있다. 방법 : 본 연구를 위해 20명(선수 10명, 비선수 10명)의 SCI 환자가 실험에 참가하였다. 골밀도 측정을 위해 이중에너지 방사선골밀도 측정기(Lunar Prodigy, GE Healthcare. England)를 이용하여 종골부위(calcaneus)의 골밀도를 측정하였다. 척수손상환자의 일상 생활 기능을 측정하기 위해 자조관리(self care), 호흡과 괄약근 조절 (respiration and sphincter management task), 이동(transfer)의 세 영역으로 나누어진 SCIM II(Spinal Cord Injury Measurement II)을 이용하였다. 척수손상환자들 중 운동 선수군과 비운동 선수군의 일상생활 동작과 골밀도를 비교 하기 위해 SPSS 14.0 통계 프로그램의 independent t-test를 이용하여 통계분석을 실시하였고 일상생활 동작과 골밀도의 상관관계는 Pearson correlation을 실시하였다. 유의수준은 ${\alpha}$=.05로 통계처리 하였다. 결과 : 검사 결과 선수군이 비선수군보다 통계적으로 유의하게 큰 SCIM 점수와 T-score를 보여주었다. 결론 : 일상생활동작을 측정하기 위하여 SCIM II(SpinalCordIndependenceMeasureII) 척도를 사용하였는데 선수군이 비선수군에 비해 유의하게 높은 SCIM II 총점을 나타내어 운동을 통한 훈련이 척수손상환자의 기능적 활동을 향상시킬 수 있다고 사료된다. 그리고 골밀도 측정 결과 선수군이 비선수군에 비해 통계적으로 유의하게 높은 골밀도를 나타내었다. 이 역시 강도 높은 훈련이 척수손상환자에게 나타날 수 있는 신체구성을 강화시켜줄 수 있을 것으로 사료된다. 운동을 통한 재활 중에서 그 강도가 높을수록 신체의 구성적, 기능적 측면 뿐 아니라 신경의 가소성 측면에서도 증가를 기대해 볼 수 있을 것으로 판단된다.

Studies of Exercise-Induced Allergy Anaphylaxis Mechanisms and the Effects of Vitamin C and Catalase Supplementation in Exercise-Induced Allergy Anaphylaxis Models (운동 유발성 알레르기 질환분석 및 비타민 C와 catalase 투여 효과 분석)

  • Exercise-induced anaphylaxis (EIA) is defined as the onset of allergic symptoms during, or immediately after, exercise, the clinical signs being various degrees of urticaria, angioedema, respiratory and gastrointestinal signs, and even anaphylactic shock. Food-dependent exercise-induced anaphylaxis (FDEIA) is a specific variant of exercise-induced anaphylaxis that requires both vigorous physical activity and the ingestion of specific foods within the preceding several hours. To describe the physiopathologic mechanism, etiologic factors, and clinical manifestations, we evaluated the supplementation of vitamin C and catalase on spleen index, proliferation assay, ROS, and ASAS in sensitized and exercise trained mice. The results were as follows: Spleen index showed the highest level in the ST12 group compared to other groups; this level increased in a time dependent manner and in significant amounts. In proliferation assay of Med and OVA, the ST12 group showed the highest level compared to other groups; this level also increased in a time dependent manner. On the other hand, spleen ROS did not show a statistically significant difference, and peritoneal ROS showed the highest level in the ST12 group. ASAS showed the highest level in the ST12 compared to the S; this was also in a time dependent manner and in significant amounts. From the results, we chose the ST9 and ST12 groups to evaluate allergy anaphylaxis with supplementation of Vitamin C and catalase. In both the ST9 and ST12 groups, peritoneal ROS and ASAS were lower in vitamin C treatment group than in the catalase treatment group. This was a statistically significant difference. From the results, allergy anaphylaxis showed a higher level in the long trained group than in the short trained group. Also, treatment with vitamin C was more effective in lowering allergy anaphylaxis than catalase treatment.
