Development of integrative diagnosis methods for the jaundice through statistical analysis (통합의료적 황달진단법개발을 위한 통계적 접근방법)

  • Healthcare approach in Western medicine and Korean Traditional Medicine (KTM) varies from its nature of human understanding and cultural differences. This fundamental difference in their approach of the human pathology has dualised and hindered common medical communication between the two fields of medicines. Within this current difficulty, the integrative medical service is said to become a novel method to provide the patients with the best medical care as their intent is to adapt and combine the advantages stated from the two different fields. This research paper shows the integrative approach of treating jaundice, where the symptoms of dampness and heat on Korean traditional standards are analyzed using statistical methods based on monitoring the blood test results. Therefore, we can explore an approach to diagnose and treat with comprehensive and integrative medicine algorithm.

Operative Treatment of Gastric Carcinoid Tumor Presenting as Multiple Polyps: A Case Report (다발성 용종의 형태로 발현된 위유암종(Gastric Carcinoid Tumor)의 수술적 치료 1예)

  • Gastric carcinoid tumor is a neoplasm that arises from enterochromaffine-like (ECL) cells in the gastric fundus. It is a rare disease that comprises less than 2% of all gastric neoplasms; however its incidence has been recently increasing. We experienced one case of gastric carcinoid tumor that was revealed to be multiple polypoid lesions. A 29-year-old female patient visited a hospital three years ago due to syncope. The blood hemoglobin was measured as 6.0 g/dl. Gastroscopy revealed multiple polypoid lesions with bleeding; therefore endoscopic clipping was performed. The polyps were diagnosed as carcinoid tumor via endoscopic biopsy. She was transferred to our hospital because of persistent iron deficiency anemia that was caused by bleeding at the gastric polyps. Gastroscopy revealed more than twenty various-sized polypoid lesions from the mid-body to the antrum. The blood hemoglobin level was 9.0g/dl. Total gastrectomy was performed under the diagnosis of gastric carcinoid tumor with bleeding. All of the gastric polyps were diagnosed as carcinoid tumors, and any metastasis to the regional lymph nodes was not found. Eighteen months after operation, the blood hemoglobin was increased to 12.8g/dl with no evidence of recurrence. Surgical resection should be considered for treating gastric carcinoid tumor with continuous bleeding.

  • During menopause, elderly woman experience physical and emotional symptoms, including hot flashes, night sweats, weight gain, vaginal dryness, and mood swings. In this study, we investigated the effects of mixed herbal extracts containing Angelica gigas Nakai and Cuscuta chinensis Lam. on menopause symptoms in ovariectomized (OVX) rats. Daily oral treatments of 500 mg/kg and 1,000 mg/kg in OVX rats for 8 weeks significantly reduced weight gain, inhibited hepatic lipid accumulation, and lowered serum lipid levels comparable to the effects induced by estradiol treatments. Mixed herbal extracts (named Jaeummidan) also prevented vaginal dryness as measured by increased vaginal weight. Increased vaginal weight was associated with enlarged epithelium thickness. These data show the preventive actions of Jaeummidan against menopause symptoms and further suggest the possibility of developing functional ingredients in elderly human populations experiencing menopause symptoms.

  • Trigeminal neuralgia is defined as "a sudden, usually unilateral, brief stabbing recurrent pain in the distribution of one or more branches of the fifth cranial nerve" by the International Association for the Study of Pain(IASP). Trigeminal neuralgia is classified as an idiopathic trigeminal neuralgia with no apparent cause and a symptomatic trigeminal neuralgia which is caused by a structural lesion such as brain tumor. Over 80% of the tumors are meningioma, acoustic neuroma, and epidermoid tumors. Symptomatic trigeminal neuralgia can not be excluded even if old-aged patient does not have abnormal neurologic sign and symptom, and good response to pharmacotherapy. Therefore, initial examinations such as MRI or CT are essential to exclude symptomatic trigeminal neuralgia. When compared with CT, MRI, especially gadolinium enhanced MRI, has an increased sensitivity in the detection of intracranial lesions. The most effective medical treatment of trigeminal neuralgia is carbamazepine. The most common side effects of carbamazepine include drowsiness, dizziness, unsteadiness, nausea, anorexia. Hepatotoxicity, bone marrow depression are the most feared side effect of carbamazepine therapy but occurs rarely. It require periodic complete blood cell counts as well as hepatic and renal function tests. It has been recommended that complete blood cell counts is done every 2 weeks for the first 2months and then quaterly thereafter. Oxcarbazepine can be used if neutropenia occurs.

Development of morning bread fortified citrus peels powders and its evaluation of biological activity by human trial (귤피가루를 첨가한 모닝빵 개발 및 인체시험을 통한 생리활성 효과 평가)

  • Purpose: This study was conducted in order to develop morning bread using powdered citrus peels and to examine the effect of its several biological activities for 12 weeks in overweight adults ($$BMI{\geq_-}23$$) living in dormitories of Chonbuk National University. Methods: Control bread was prepared using the same formula except for replacing the wheat flour with 3% citrus peel powder. Subjects were classified according to two groups: Intake group of 3% citrus peel powder fortified morning bread (n = 30) and general morning bread (n = 30). Results: The results of our study showed no significant effects of citrus peel powdered morning bread on anthropometric indices and biochemical characteristics among overweight adults. Despite the slight reduction in total-cholesterol, LDL-cholesterol, GOT, and GPT (p < 0.05), no significant differences in changes of these values were observed between the two groups. Conclusion: The short duration of our intervention might have resulted in the lack of any significant effect. Therefore, conduct of further studies with longer duration is warranted in order that the results from this study can be utilized as a basis for the development of food products with citrus peel powder.

The Effect of Nutrition Education on Weight Control and Diet Quality in Middle-Aged Women (영양교육에 의한 식행동 향상이 중년여성의 체중조절 및 식사의 질에 미치는 영향)

  • We examined the effect of dietary behavior improvement on diet quality, body composition and blood lipid profiles in 68 middle-aged women. Dietary behavior intervention was consisted of counselling in the problem areas of portion control, food selection for low carbohydrate and high fiber food items, and education on the improvement of micronutrient intakes and diet quality. The subjects were divided into two groups; Improved or Not-Improved Group according to the level of changes in dietary behavior scores. After 6 months, body weight, BMI, waist-hip ratio, and visceral fat area were significantly decreased in the Improved Group compared to the Not-Improved Group. Calorie-adjusted protein, fiber, calcium, iron and vitamin C intakes were significantly increased in the Improved Group. The index of nutritional quality (INQ) and mean adequacy ratio (MAR) were increased only in the Improved Group. We observed a significantly increase in HDL-cholesterol and a decrease in total cholesterol, triglyceride and LDL-cholesterol, and an improvement of atherogenic index in the Improved Group. These results showed that dietary behavior intervention improved dietary behavior scores, and those whose dietary behavior score improved showed more improvement in diet quality, body composition, and serum profiles than those whose dietary behavior score did not improve.

Predictors of Blood Transfusion in Hepatoma Embolization (간종양 색전술 환자의 수혈 영향 요인)

  • This study was conducted to investigate the patient and hospital characteristics that affect receiving and non-receiving blood transfusion for hepatoma embolization. We analyzed data describing 757 patients of the Health Insurance Review and Assessment Service's 2011 claims sample data. Chi-squared and logistic regression analysis was performed using STATA 12.0. Logistic regression analysis revealed that anemia (Odds ratio (OR)=9.544, 95% confidence interval (CI)=3.362-27.097), female (OR=2.186, Cl=l.240-3.854), age 65-74 (OR=0.506, CI=0.269-0.952), 1000 over bed (OR=0.053, CI=0.018-0.151), out-department (OR=0.211, CI=0.081-0.551), and 700-999 bed (OR=0.105, CI=0.036-0.304) were significant predictors of blood transfusion in tertiary hospitals. Additionally, anemia (OR=69.681, CI=8.545-568.246) and aged 75 or over (OR=0.112, CI=0.025-0.506) were significant predictors of blood transfusion in general hospitals. This research would expected to cost effective and preliminary data of blood transfusion quality.

Analytical trends in mass spectrometry based metabolomics approaches of neurochemicals for diagnosis of neurodegenerative disorders (퇴행성신경질환의 진단을 위한 신경전달물질 대사체의 질량 분석법 동향)

  • Because neurochemicals are related to homeostasis and cognitive and behavioral functions in human body and because they enable the diagnosis of numerous neurodegenerative disorders, there has been increasing interest in the development of analytical platforms for neurochemical profiling in biological samples. In particular, mass spectrometry (MS)-based analytical methods combined with chromatographic separation have been widely used to profile neurochemicals in metabolic pathways. However, development of delicate sample preparation procedures and highly sensitive instrumental detection is necessary considering the trace levels and chemical instabilities of neurochemicals in biological samples. Therefore, in this review, analytical trends in MS-based metabolomics approaches to neurochemicals in multiple biological samples, such as urine, blood, CSF, and biological tissues, are discussed. This paper is expected to contribute to the development of an analytical platform to discover biomarkers that will aid diagnosis, prognosis, and treatment of neurodegenerative disorders.

Diamond-Blackfan anemia: long-term follow-up of six cases (Diamond-Blackfan 빈혈: 6예 장기간 추적관찰보고)

  • Purpose : Diamond-Blackfan anemia (DBA) is a rare heterogeneous genetic disorder of infancy and early childhood. It is characterized by red cell aplasia, congenital anomalies, and a predisposition to cancer. Corticosteroids and red cell transfusions are the mainstays of therapy. We describe our experience of 6 cases of DBA that were encountered over a period of 16 years. Methods : Medical records of 6 patients diagnosed to have DBA and admitted to the Chonnam National University Hospital between 1992 and 2008 were retrospectively reviewed. Results : Three patients were males. The age at diagnosis ranged from 3 to 18 months (median, 5.5 months). Heart defects were observed in 4, polydactyly in 2, and strabismus in 1 patient. The median number of transfusions was 3 (range, 2-8). All patients responded to initial treatment with steroids and had a hemoglobin level ${\geq}9g/dL$ with a median of 12.5 days (range, 7-22 days). Three patients are currently not receiving steroid therapy. A minimum dose of oral prednisolone (<5 mg per day) was required to maintain hemoglobin ${\geq}9mg/dL$ in 3 cases. Red cell transfusion was infrequently required in 1 patient. In the median follow-up of 14 years, there was no development of malignancy. No significant side effects of steroids were found, except for short stature in 2. Conclusion : The majority of DBA patients achieved complete response and under maintenance therapy with low dose of steroids. Close observation is needed to monitor steroid side effects, cardiac function, and development of malignancy. A nation-wide survey is necessary to further characterize this rare disease in Korean children.

Application of Silver Ion for Clinical Mastitis in Holstein Cows (임상형 유방염을 가진 홀스테인(Holstein) 소에 대한 은 이온의 적용 연구)

  • Mastitis is an inflammation of the mammary gland that develops as a response to pathogenic microorganisms. Somatic cell count (SCC) and bacteriological analysis are the most accepted tools to determine udder health. This study evaluated the effects of intra-mammary gland infusion of silver ion against clinical mastitis in Holstein cows. Silver ion (20 ${\mu}g$/ml) was infused through the intra-mammary gland in quarters having clinical mastitis and milk was collected to determine SCC, and levels of lactoferrin and bovine serum albumin (BSA). Silver ion infusion decreased udder inflammation, firmness and swelling, and reduced clots, BSA, lactoferrin and SCC in milk. However, milk yield and circulating blood cells remained unchanged. The silver ion-mediated reductions of BSA and SCC indicate reduced inflammation and bacterial activity in silver ion-treated mammary glands in Holsteins with mastitis, which may be exploited in a curative strategy.
