Evaluations on the Characteristics of Pressure Drop f3r the Design of Intravascular Artificial Lung Assist Device (혈관 내 폐 보조장치 설계를 위한 압력손실 특성 평가)

  • In this study, we try to formularize simultaneous equations to make a prediction about pressure drop for designing intravascular artificial lung assist device. Designing parameters to predict the effect of pressure drop and designed modules under various conditions were studied through an experimental modeling before inserting the artificial lung assist device into as venous. We measured pressure drop in various number of hollow fiber membranes, when the inside diameter of shell is fixed in 3 cm, and tried to develope the prediction equations by curve fitting based on the correlation between the experimental pressure drop and the device frontal area or packing density. The results showed that pressure drop increased with 2nd order functional formula as the liquid flow rate, the frontal area, and the packing density increased. Also, we can estimate the pressure drop as a function of the frontal area or packing density. The pressure drop obtained from the experiment was similar to that from the equation, confirming the usefulness of the equation.

  • In this paper a newly designed oxygenator module was used to perform the experiments for pressure drop and oxygen transport in order to evaluate the efficiency of the artificial lung. Also, distilled water. sodium sulfite solutions used in this experiment were evaluated for the performance of other artificial lungs. Substituted bloods have many advantages over whole blood in studying pressure drop and oxygen uptake. They are relatively inexpensive, and require fewer variables to be controlled. Furthermore, deoxygenation is not necessary when those solutions are used. In addition to these advantages. assays and interpretation of the experimental results are relatively easy. In the case of using the sodium sulfite solution having the same oxygen partial pressure as whole blood. the oxygen transfer rate of the sodium sulfite solution was basically same as that of whole blood. It was concluded in evaluating the function of artificial lungs that the sodium sulfite solution was suited for measuring oxygen transfer rate. In our module, artificial blood was flowed into the outside of hollow fiber membrane. The artificial lung created in this experiment showed that pressure drop was reduced to $1/3\~1/6$ of that compared to the inside-flow case.

The evaluation of borage oil to use in pet foods (반려견 건강증진을 위한 보라지유 활용 가능성 평가)

  • The purpose of this study was to investigate the safety and anti-inflammatory effects of borage oil on dogs. Twelve dogs were fed on a commercial diet of 1% or 2% borage oil alone for twelve weeks. To assess safety, the changes in body weight, blood cells, and immune-related cytokines were analyzed. The results showed that there was no significant difference in body weight, complete blood count (CBC), and immunomodulatory cytokines between the dogs fed with diets without or with borage oil. Also, there was no change in transepidermal water loss (TEWL). However, the amount of lactate dehydrogenase (LDH) was reduced significantly in the dogs fed on a borage oil diet. In summary, the addition of borage oil to pet food did not result in any significant health issues. Moreover, borage oil could contribute to a reduction in cell damage in aged dogs although it did not decrease TEWL. Therefore, borage oil could be safe for use in pet foods.

  • An accurate analysis method for the determination of lead in whole blood by ICP/MS was developed. Whole blood samples were decomposed in microwave digestion system without any contamination and loss of lead. The 96 samples were analyzed by ICP/MS using mass$^{208}$ isotope of lead. Lead concentrations of human whole blood were ranged of $2.50{\sim}22.8{\mu}g/dL$. The accuracy of this analysis method was verified by analyzing of NIST SRM 955a series(lead in blood).

  • 본 연구에서는 심장의 세포 변화에서부터 혈류 순환의 시스템 변화까지 일련의 과정을 시뮬레이션 할 수 있는 통합모델을 개발하였다. 본 통합 모델을 이용하여 대동맥의 탄성도 변화 따른 Pulse Wave Velocity를 추정하였으며 심근의 수축 Mechanics의 변화를 시뮬레이션 하였다. 심장은 단순한 구 형상으로 모델링 되었다. 특히 동맥순환의 특성인 Wave propagation 과 Wave deflection의 현상을 모델링하기 위해 기존 모델에서 사용된 동맥계 순환 모델을 수정하였다. 즉 기존의 동맥 모델을 1차원의 운동방정식과 연속방정식을 기반으로 하는 Distributed arterial model로 대체하였다. Distributed arterial model은 혈액의 점성에 의한 에너지 손실, 혈관의 점탄성 효과 그리고 분지 되는 혈관에서의 에너지 손실을 포함하는 정교한 동맥 순환 모델이다. 정교한 동맥계 순환 모델의 동맥 탄성도 값을 조절함으로써 탄성도 변화에 대한 PWV를 계산 할 수 있었다. 이러한 수치적 방법을 사용하여 노화에 따른 동맥벽 탄성도의 저하가 심근세포의 Cross-bridge 동역학에 미치는 영향을 시뮬레이션 하였다.

  • We measured the flow at peripheral blood vessel of finger dorsum surface using noninvasive LDF(Laser Doppler Flowmeter). We investigated the flow properties between the normal and the diabetic with the Amplitude Spectral Densities of FFT analysis. We classified the diabetic as four groups of negative neuropathy, positive neuropathy, hyperthyroid and hypothyroid. And tested the neuropathic and thyroid effects of peripheral blood flow. As the results, the diabetic, the positive neuropathic and the hypothyroid respectively showed the abnormal blood flow properties on the contrary of the normal flow of negative neuropathic and hyperthyroid.

Blood Loss Prediction of Rats in Hemorrhagic Shock Using a Linear Regression Model (출혈성 쇼크를 일으킨 흰쥐에서 선형회귀 분석모델을 이용한 출혈량 추정)

  • Hemorrhagic shock is a common cause of death in the emergency department. The purpose of this study was to investigate the relationship between blood loss as a percent of the total estimated blood volume (% blood loss) and changes in several physiological parameters. The other goal was to achieve an accurate prediction of percent blood loss for hemorrhagic shock in rats using a linear regression model. We allocated 60 Sprague-Dawley rats into four groups: 0ml, 2ml, 2.5ml, 3 mL/100 g during 15 min. We analyzed the heart rate, systolic and diastolic blood pressure, respiration rate, and body temperature in relation to the percent blood loss. We generated a linear regression model predicting the percent blood loss using a randomly chosen 360 data set and the R-square value of the model was 0.80. Root mean square error of the tested 360 data set using the linear regression was 5.7%. Even though the linear regression model is not directly applicable to clinical situation, our method of predicting % blood loss could be helpful in determining the necessary fluid volume for resuscitation in the future.

Factors Influencing on Early Patency Rate of Autogenous Arteriovenous Fistula for Hemodialysis (혈액투석을 위한 자가 동정맥루 수술에 있어서 조기개존율에 영향을 미치는 요인)

  • It is very important for hemodialysis in patients with end stage renal disease to obtain vascular access that resists repeated punctures and maintains adequate blood flow. This study was designed to indentify factors that may influence early patency rate of autogenous arteriovenous fistula. Material and Method: 49 cases in 47 patients who underwent radiocephalic fistula formation in our hospital from June 2002 through May 2003 were reviewed and analyzed. Result: The early patency rate was 79.6%. Age, sex, hypertension, and diabetes mellitus were not significant factors for patency. Body mass index and duration of hypertension and diabetes did not influence the early results either. Cephalic vein diameter measured preoperatively and blood flow at radio-cephalic fistula were significantly positive correlative factors. Groups with the vein diameter less than 2.7mm, or with the blood flow less than 100 mL/min had significantly lower early patency rate than the other groups. Conclusion: To improve early patency rate of radiocephalic fistula, large sized cephalic vein should be selected and if the intraoperative flow at radiocephalic fistula is less than 100 mL/min, another arteriovenous fistula formation should be considered.

  • 최근 우리나라는 anaplasmosis 유행 지역으로부터 사슴을 비롯한 반추수를 수입하고 있다. 이처럼 리켓치아성 질환을 비롯한 원충성 질병의 국내 유입 기회가 높아지고 있다. 그러나 사슴에서의 anaplasmosis에 대한 연구보고 예는 접할 수 없었다. 1993년 전라북도의 한 사슴 목장에서는 호주 등지로부터 수입한 250 여두의 사슴에서 빈혈을 수반한 심한 쇄약증세를 나타내는 20마리 사슴의 혈액도말 표본을 Giemsa stain 과 acridine orange stain 방법으로 진단하였던 바, 이 중 8 마리에서 Anaplasma spp가 관찰되었기에 anaplasmosis에 의한 경제적 손실을 최소화하는데 일익을 도모하고자 사슴에서의 anaplasmosis 발병을 보고하는 바이다.

The Effects of Autologous Blood Pleurodesis in the Pneumothorax with Persistent Air Leak (지속성 기흉에서 자가혈액을 이용한 흉막유착술의 효과)

  • Background : In patients with severe chronic lung diseases even a small pneumothorax can result in life-threatening respiratory distress. It is important to treat the attack by chest tube drainage until the lung expands. Pneumothorax with a persistent air leak that does not resolve under prolonged tube thoracostomy suction is usually treated by open operation to excise or oversew a bulla or cluster of blebs to stop the air leak. Pleurodesis by the instillation of chemical agents is used for the patient who has persistent air leak and is not good candidate for surgical treatment. When the primary trial of pleurodesis with common agent fails, it is uncertain which agent should be used f or stopping the air leak by pleurodesis. It is well known that inappropriate drainage of hemothorax results in severe pleural adhesion and thickening. Based on this idea, some reports described a successful treatment with autologous blood instillation for pneumothorax patients with or without residual pleural space. We tried pleurodesis with autologous bood for pneumothorax with persistent air leak and then we evaluated the efficacy and safety. Methods : Fifteen patients who had persistent air leak in the pneumothorax complicated from the severe chronic lung disease were enrolled. They were not good candidates for surgical treatment and doxycycline pleurodesis failed to stop up their air leaks. We used a mixture of autologous blood and 50% dextrose for pleurodesis. Effect and complications were assessed by clinical out∞me, chest radiography and pulmonary function tests. Results : The mean duration of air leak was 18.4${\pm}$6.16 days before ABP (autologous blood and dextrose pleurodesis) and $5.2{\pm}1.68$ days after ABP. The mean severity of pain was $2.3{\pm}0.70$ for DP(doxycycline pleurodesis) and $1.7{\pm}0.59$ for ABDP (p<0.05). There was no other complication except mild fever. Pleural adhesion grade was a mean of $0.6{\pm}0.63$. The mean dyspnea scale was $1.7{\pm}0.46$ before pneumothrax and $2.0{\pm}0.59$ after ABDP (p>0.05). The mean $FEV_1$ was $1.47{\pm}1.01$ before pneumothorax and $1.44{\pm}1.00$ after ABDP (p>0.05). Except in 1 patient, 14 patients had no recurrent pneumothorax. Conclusion : Autologous blood pleurodesis (ABP) was successful for treatment of persistent air leak in the pneumothorax. It was easy and inexpensive and involved less pain than doxycycline pleurodesis. It did not cause complications and severe pleural adhesion. We report that ABP can be considered as a useful treatment for persistent air leak in the pneumothorax complicated from the severe chronic lung disease.
