혈액투석 환자의 삶의 질과 우울, 불안과의 관계 (The Relationship between Quality of Life and Depression, Anxiety in Hemodialysis Patients)

  • 본 연구는 혈액투석 환자의 삶의 질과 우울, 불안과의 관계를 확인하기 위한 서술적 조사 연구이다. S시 소재 N의료원 1개 종합병원 인공신장실에서 혈액투석을 받고 있는 19세 이상, 혈액투석을 시작한지 1개월 이상된 130명의 환자를 대상으로 시작되었으나 중도에 포기하거나 사망, 또는 잘못 기재된 설문지를 제외한 총 112명의 환자를 대상으로 하였다. 연구결과 대상자의 삶의 질 평균점수는 57.77점으로 나타났고, 우울은 12.25점, 불안은 10.52점으로 대상자의 삶의 질은 낮은 편이었고, 우울과 불안은 높은 편이었다. 대상자의 삶의 질, 우울, 불안 간에는 유의한 상관관계가 있었다. 삶의 질은 우울과 음의 상관관계로 나타났고(r=-.782, p<.001), 불안과도 음의 상관관계가 있었다(r=-.719, p<.001). 우울과 불안은 양의 상관관계로 나타났다(r=.873, p<.001). 삶의 질에 미치는 영향 요인에 대한 다중회귀분석을 한 결과 우울이 삶의 질에 영향을 주는 요인으로 확인되었다(β=-.585). 따라서, 본 연구의 결과 혈액 투석 환자들에서 흔히 나타나는 정서적 문제인 우울과 불안 요인 중 특히, 대상자의 삶의 질에 많은 영향을 주는 중요한 변수인 우울의 정도에 집중하여 감소시킬 수 있는 중재 전략과 대상자의 삶의 질 향상을 위한 간호중재 프로그램 개발에 기초자료를 제공하는데 의의가 있다.

  • Purpose: The study was conducted to investigate the correlation between perception of patient safety risk factors, patient safety competency, and safety nursing activities of nurses in hemodialysis units and identify factors affecting patient safety activities. Methods: The participants were 146 nurses from 16 hemodialysis units located in Gyeongsangnam-do. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis using the SPSS, version 24.0. Results: The mean safety nursing activity score was 3.47±0.38. safety nursing activities of the participants were significantly correlated with patient safety competency. The characteristics showing significant differences in safety nursing activities were educational level, hospital type, hospital work experience, number of hemodialysis treatment per day, number of hemodialysis treatment per nurse, educational experience of patient safety, presence of a patient safety incident report registration system, and direct registration of patient safety incident report. The multiple regression analysis revealed that the factors influencing safety nursing activities were patient safety incident report, patient safety competency, and number of daily hemodialysis treatment (<5~7 times/day) per nurse (R2=.34). Conclusion: The results of this study suggest that the safety nursing activities of hemodialysis unit nurses should be intensified. In addition, the registration system of patient safety incident report and nurses' competency on patient safety should be improved, and the number of hemodialysis per nurse should be fewer than 7 times per day.

일차의료기관 혈액투석실 간호사의 간호근무환경, 환자안전문화 및 환자안전간호활동의 관계 (Relationship between Nursing Work Environment, Patient Safety Culture, and Patient Safety Nursing Activities in Hemodialysis Clinics of Primary Care Centers)

  • Purpose: This study aimed to investigate the relationship between nursing work environment, patient safety culture, and patient safety nursing activities in hemodialysis units at primary care centers. Methods: In this cross-sectional descriptive study, 116 nurses working in hemodialysis units at 22 primary care centers were enrolled as participants. They were selected them by the convenience sampling method. Data were collected using structured questionnaires that included questions on general characteristics, nursing work environment, patient safety culture, and patient safety nursing activities. Results: The nursing work environment was relatively good; however, the patient safety culture and patient safety nursing activities were poor. A positive correlation was found between nursing work environment, patient safety culture, and patient safety nursing activities. Factors that affect patient safety culture were rated high in the order of nursing work environment and patient safety nursing activities, and factors affecting patient safety nursing activities were rated high in the order of patient safety culture and age. Conclusion: This study showed that the development of tailored patient safety training for nurses in hemodialysis units working in primary care and administrative support from those institutions are needed. In particular, strategies accounting for nurses' characteristics such as age are required to strengthen patient safety nursing activities.

혈액투석실에서 근무하는 간호사의 특성이 근거기반 혈액투석간호지식과 수행정도에 미치는 영향 (The Effects of Characteristics of Nurses on Knowledge and Nursing Performance Evaluation of Evidence Based Hemodialysis Nursing Practice in Hemodialysis Unit Nurses)

  • Purpose: The purpose of this study was to identify the effects of characteristics of nurses on knowledge and nursing performance of evidence based hemodialysis nursing practice among hemodialysis unit nurses. Methods: The participants were 180 nurses working in hemodialysis unit for more than 6 months in 23 private and general hospitals in Seoul and Gyeonggido. Data were collected from March 30th to April 15t in 2016 and were analyzed using stepwise regression analysis, descriptive statistics, t-test and ANOVA. Results: Nurses' knowledge on evidence based hemodialysis nursing practice was $15.87{\pm}4.52$ out of 23 points. Type of hospitals working in and adherence to evidence based hemodialysis guidelines were significant factors to knowledge and these two factors explained 30.0%. Nurses' nursing performance on evidence-based hemodialysis nursing practice was 4.52 out of 5 points. The performace level was significantly related to total nurisng career and necessity of hemodialysis nursing education and these two factors explained 8.0%. Conclusion: A development of guideline and continuing education is necessary for improving knowledge and performance of evidence based hemodialysis nursing practice.

상대가치를 이용한 혈액투석실 간호수가 산정;중등도 대상자를 중심으로 (Estimation of Nursing Cost for Hemodialysis Using RBRVS(Resource Based Relative Value Scale);Moderately Intensive Patient)

  • Purpose : This study wad aimed to estimate nursing cost for hemodialysis of secondary hospitals using Resource Based Relative Value Scale(RVRBS). Method : The first, calculated nursing workload for the hemodialysis of secondary hospitals. Second, measured each spent time according to 14 nursing behaviors. Third, computed Resource Based Relative Value(RBRV) scores and nursing expenses of hemodialysis nurse. Finally, estimated nursing cost for hemodialysis of secondary hospitals, Result & Conclusion : The mean RBRV scores for each nursing behaviors were from 218.9 to 383.9 point(mean=312.7). The high RBRV socres were 383.9(Complication during hemodialysis), 353.7(Access patient vascular), 345.7 (Patient Teaching). Nursing cost for hemodialysis was estimated 23,234(won).

혈액투석실 간호사의 언어폭력 경험, 직무 스트레스, 소진이 이직의도에 미치는 영향 (Influence of Experiencing Verbal Abuse, Job Stress and Burnout on Nurses' Turnover Intention in Hemodialysis Units)

  • Purpose: The aim of this study was to investigate the relationships between the hemodialysis unit nurses' experience of verbal abuse, job stress, burnout, and turnover intention, and to identify the explained variances for turnover intention. Methods: The research design was a descriptive survey using a probability sampling. The data were measured using self-report questionnaires from 231 hemodialysis unit nurses in seven cities in Korea and were analyzed with SPSS 20.0. Results: The mean scores were $2.45{\pm}0.62$ (out of 5) for experience of verbal abuse, $2.40{\pm}0.26$ (out of 4) for job stress, $2.31{\pm}0.48$ (out of 4) for burnout, and $3.16{\pm}0.05$ (out of 5) for turnover intention. There were significant correlations among the experience of verbal abuse, job stress, burnout, and turnover intention within the hemodialysis unit nurses. Factors influencing turnover intention were 'burnout' (${\beta}=.42$, p<.001) and 'job stress' (${\beta}=.17$, p=.015)which explained 42% of the variance (F=15.98, p<.001). Conclusion: Results suggest that in order to reduce nurses' turnover intention, nurse managers should explore strategies to reduce nurses' burnout and job stress in nurses in hemodialysis units.

혈액투석환자의 사회적 지지와 건강정보이해능력이 치료순응도에 미치는 영향 (Influence of Social Support and Health Literacy on Treatment Adherence in Hemodialysis Patients)

  • 본 연구는 주기적으로 혈액투석을 시행하고 있는 환자를 대상으로 사회적 지지와 건강정보이해능력의 정도를 파악하고 치료순응도에 미치는 영향을 조사하기 위해 시도된 서술적 연구이다. 연구대상자는 지방 Y시에 소재한 2개 종합병원 인공신장실에 등록된 혈액투석 치료를 받은 지 1년이 경과 한, 만 40세 이상 투석환자 140명이며, 자료수집기간은 2019년 11월 1일부터 12월 31까지이다. 수집된 자료는 SPSS/WIN 26.0 프로그램을 이용하여 빈도와 백분율, 평균과 표준편차, t-test, one-way ANOVA, Scheffe test, Pearson의 상관계수, 다중회귀분석을 실시하였다. 연구 결과, 사회적 지지 하위영역인 가족 지지는 친구 지지와 의료인 지지에 비해 치료순응도에 더 유의한 영향을 주는 것으로 나타났다. 건강정보이해능력에 있어 세 가지 하위영역인 기능적, 의사소통, 위기 건강정보이해능력 모두가 치료순응도와 매우 높은 상관관계를 보여 건강정보이해능력이 높을수록 치료순응도가 높은 것으로 나타났다. 사회적 지지와 건강정보이해능력은 치료순응도에 유의한 영향을 미치는 변수로 나타났고 69.6%의 높은 설명력을 보였다. 따라서 혈액투석환자의 치료순응도를 증진시키기 위해서는 사회적 지지를 기반으로 건강정보이해능력을 향상시키는 교육을 제공하는 프로그램 개발이 필요하다.

혈액투석환자의 근감소증 관련요인 (Factors Associated with Sarcopenia among Hemodialysis Patients)

  • Purpose : This descriptive survey aims to identify the prevalence of factors associated with sarcopenia among hemodialysis patients. Methods : The study subjects were 137 patients with chronic kidney failure undergoing hemodialysis in three artificial kidney centers in B and Y cities. Data were collected from August 1 to September 30, 2020, using the SARC-F (Strength, Assistant walking, Rising from a chair, Climbing stairs, Falls) questionnaire, Mini Nutritional Assessment-Short Form (MNA-SF), International Physical Activity Questionnaire (IPAQ-SF), Bioelectrical Impedance Analysis (BIA), and a grip dynamometer. The collected data were analyzed using t-tests, crossover analysis, and logistic regression using the IBM SPSS 23 program. Results : The prevalence of sarcopenia among hemodialysis patients, determined using the SARC-F questionnaire, was 16.1%. The associated factors of sarcopenia among hemodialysis patients were found to be gender (OR=6.44, p =.002), age (OR=1.07, p =.015), nutritional status (OR=10.37, p =.027), and albumin level (OR=0.10, p =.014). These findings are supported by an explanatory power of 46.3% (p =.597). Conclusion : The identified risk factors for sarcopenia in hemodialysis patients were; sex, age, nutritional status, and albumin level. The findings of this study can serve as clinical evidence for the development of an intervention program for preventing and managing sarcopenia in patients undergoing hemodialysis.

혈액투석 환자의 동정맥루 천자 시 에틸클로라이드 스프레이 적용이 통증, 우울 및 치료 지시 불이행에 미치는 효과 (Effects of Ethyl Chloride Spray before Arteriovenous Fistula Puncture on Pain, Depression, and Noncompliance of Hemodialysis)

  • Purpose: This study was conducted to examine the effects of ethyl chloride spray during arteriovenous fistula puncture on pain, depression, and noncompliance in hemodialysis patients. Methods: This study used a randomized controlled trial design on adults with chronic renal failure who received hemodialysis treatment through arteriovenous fistula. Ethyl chloride spray was applied to 20 subjects in the experimental group during arteriovenous fistula puncture, and a placebo spray was applied to 20 subjects in the control group; the intervention was conducted 12 times for 4 weeks. A total of 33 participants were used in the final analysis. The outcomes were measured using the face pain rating scale, Center for Epidemiological Studies Depression Scale (CES-D), and modified United States Renal Data System tools. Results: As a result of the pre-homogeneity test between the two groups, it was confirmed that the sexes were not homogeneous. Therefore, sex was treated as a covariate and analyzed. Puncture pain was significantly reduced in the experimental group (p<.001). However there was no significant difference between the groups in depression and noncompliance. Conclusion: Ethyl chloride spray was convenient and effective pain management intervention for both hemodialysis patients and medical staff.

비디오 유머중재가 혈액투석 중인 환자의 우울과 스트레스반응에 미치는 효과 (Effect of Humorous Video Tape on Depression and Stress Response in Patients Undergoing Hemodialysis)

  • Purpose: The purpose of the study was to identify the effect of humorous video on the depression and stress responses in patients undergoing hemodialysis. Methods: The quasi-experimental study was designed with a nonequivalent control group pre-post test. Research participants were assigned to a treatment group (n=23) and a control group (n=21). Treatment was watching humorous video tapes 30 min each time, three times a week. The depression level and state anxiety level were assessed using a questionnaire. Cortisol was measured by a blood test. The data were analyzed using $x^2$-test, and t-test. Results: Depression and anxiety were decreased significantly in the treatment group. Conclusions: Humorous video tape could be a usefull tool at home and clinical setting as a nursing intervention to decrease depression and anxiety among the patients on hemodialysis.
