Gamrosu was originally conceptualized from Jeho-tang, a selected thirst quencher of the kings in Chosun Dynasty and Saeng-Maek-san, a qi-vigorating summer beverage recommended by Dongeuibogam. It is a modified fasting therapy beverage which is manufactured from the single herbal medicines composed of those two prescriptions. This study was conducted on 6 obese patients with hypertension. A modified fasting therapy with Gamrosu was practised on them for 10 consecutive days. After the therapy, their average blood pressures were descended from 148/89 mmHg to 119/79 mmHg. The modified fasting therapy with Gamrosu is supposed to be more effective than general diet program or dietary sodium reduction on controlling hypertension. And, Gamrosu improves anti-hypertensive effect by reducing the side effects, such as fatigue, electrolyte imbalance, heartburn, nausea, and headache, during the modified fasting therapy.

Nuts are cholesterol free with high poly-unsaturated fatty acids(PUFA) and have lower intakes of sodium than non-consumers, and thus they can decrease blood pressure. Hypertension is a common primary diagnosis in Korea. Because of extending exposure to 24-hour raised blood pressure and heart rate hypertension is likely to be caused by prolonged short sleep durations. This study examined the relationship between nut consumption and hypertension according to sleep duration among Korean adult. Based on data from the 2010-2016 Korea National Health and Nutrition Examination Survey(KNHANES), the final analytic sample(n=25,359) was used for current analysis. The dietary intake was assessed through a 24-hour recall method. Associations of nut consumption with sleep duration and hypertension were determined using multiple logistic regression with odds ratio(95% CI). All the analyses were carried out in SAS version 9.4, and the significance level was set at p<0.05. With increasing nut intake, the prevalence of hypertension significantly decreased(p for trend=0.02). After controlling for sleep duration, the nut consumption showed significant association with the prevalence of hypertension when sleep duration was 6 to 6.9 h per day(p for trend=0.03) or 7 to 7.9 h per day(p for trend=0.03). In conclusion, dietary total nut intake was found to be significantly associated with the prevalence of hypertension.

The possible inolvement of central opiate system in the control of cardiovascular function and in the antihypertensive action of clonidine has been examined in unanesthetized rats with shamoperated or 2-kidney, 1-clip (2K1C) renal hypertension. In both groups of rats, intraventricular clonidine $(3-30\;{\mu}g/kg)$ produced hypotension and bradycardia. Hypotensive action of clonidine was more potent in the hypertensive rats than in the normotensive sham-operated rats. Yohimbine $(30\;{\mu}g/kg,\;i.v.t.)$ inhibited the hypotension and bradycardia produced by clonidine. Naloxone ($50\;{\mu}g/kg$, i.v.t.) inhibited the action of clonidine in 2K1C hypertensive rats but not influenced in the sham-operated rats. Intraventricular morphine $(10-100\;{\mu}g/kg)$ also reduced rats. Intraventricular morphine $(10-100\;{\mu}g/kg)$ also reduced blood pressure and heart rate in both groups of rats. But these effects were not affected by yohimbine, but antagonized by naloxone ($50\;{\mu}g/kg$, i.v.t.). Chronic treatment of 2K1C rats with clonidine ($3{\times}20\;{\mu}g/kg$, p.o.,) for 14 days from 1 day after 2K1C operation) suppressed the development of hypertension and maintained the blood pressure in normal level and this errect of clonidine was abolished by naloxone (2 mg/kg, i. p.). In the 2K1C hypertensive rats, immunoreactive ${\beta}-endorphin$ content was significantly decreased, but maximum binding (Bmax) of $(^3H)-naloxone$ was significantly increased in brain of 2K1C hypertensive rats. However, Kd value was not changed. These results suggest that the opioidergic component might be involved in the antihypertensive action of clonidine only in hypertensive and that central opiate system might play important roles in pathophysiology of development and maintenance of hypertension.

Purpose: To evaluate whether or not sodium restriction had its own beneficial effect and increased the efficiency of the anti-hypertensive drugs on the progression of renal failure. Methods: We studied using the excision remnant kidney model. Treatment groups were as follows: 5/6 nephrectomy and a 0.49% (normal-high) sodium diet (NN); 5/6 nephrectomy and a 0.25% (normal-low) sodium diet (LN); 5/6 nephrectomy, a 0.49% sodium diet and enalapril (NNE); 5/6 nephrectomy, a 0.49% sodium diet and nicardipine (NNN); 5/6 nephrectomy, a 0.25% sodium diet and enalapril (LNE); 5/6 nephrectomy, a 0.25% sodium diet and nicardipine (LNN). Both diets were isocaloric and had the same content of protein, phosphorus and calcium. Proteinuria, remnant kidney weight, mesangial expansion scores, and glomerular volume were assessed. Results: Blood pressure tended to be lower in LN compared to NN (P<0.05). NN developed progressive hypertension. LNE, LU, NNE, and NNN reduced blood pressure. LNE, LNN, NNE, NNN, and LN had significantly less proteinuria than NN at 16 weeks (P<0.05). At 24 weeks, LN developed proteinuria (82 mg/day), which were lessened in LNE (54 mg/day) and not lessened in LNN (76 mg/day). Mesangial expansion scores were significantly less in LN rats compared to those in NN rats. Glomerular volumes at 24 weeks in LN rats were significantly less compared to those at 16 weeks in NN rats. Mesangial expansion scores and glomerular volumes at 4, weeks, 12 weeks, and 24 weeks were not different among LN, LNE, and LNN groups. Conclusion: Dietary salt restriction lessens renal damage, at least in part, by inhibiting compensatory renal growth and reducing blood pressure. Enalapril was particularly successful in reducing proteinuria and glomerular injury when combined with dietary salt restriction.

Purpose : Syncope appears to be common. However, the mechanism of syncope is not clear. Increased vagal activity and withdrawal of sympathetic stimulation cause hypotension, bradycardia and finally loss of consciousness. The purpose of this study was to evaluate changes of cerebral blood flow velocity, blood pressure, and heart rate during tilt test in children with vasovagal syncope. Methods : Sixty four children with a past history of syncope were evaluated. The stand up test was performed for 15 minutes after a rest at supine position for 10 minutes, followed by an $80^{\circ}$ tilt test lasting 45 minutes. If presyncope(lightheadedness, nausea, blurred vision, or sweating) or syncope occurred, the study was discontinued. 12-lead electrocardiography, echocardiography, and electroencephalography were performed. Transcranial Doppler study was performed at the middle cerebral artery with 2 MHz continuous Doppler probe in 10 children with positive tilt test. Systolic, diastolic, mean cerebral blood flow velocity, integral, and pulsatility index were measured with blood pressure, heart rate, and $O_2$ saturation. Results : The positive rate of tilt test was 31.3%(20/64). Systolic, diastolic, and mean cerebral blood flow velocity decreased significantly in absence of hypotension or bradycardia during presyncope. Time velocity integral of cerebral artery also decreased significantly. Conclusion : Decreased cerebral blood flow velocity can predict the presyncope manifestation. Impairment of autoregulation of cerebral blood flow might play an important role in the pathophysiology of vasovagal syncope.
This study investigated the effect of endurance exercises on blood pressure, angiogenesis, and the vascular endothelial growth factor (VEGF) expression in the skeletal muscle of spontaneously hypertensive rats (SHR). Five week old SHRs and Wistar-Kyoto rats (WKY) were randomly divided into 3 groups: Wistar-Kyoto rats (WKY, n=9), SHR control (SHR-C, n=9), and SHR endurance exercise training (SHR-E, n=9). Endurance exercise training was performed on a treadmill (12-20 m/min, 0% grade, 60 min/day, 5 days/week, 16 weeks). Systolic blood pressure was monitored with the tail-cuff method. The expression of VEGF protein and capillary density were identified using western blotting and H&E staining in the soleus muscle, respectively. Systolic blood pressure was reduced by endurance exercise in SHR ($p$ <0.05). The capillary density of skeletal muscles in SHR-C was lower than in WKY ($p$ <0.05), but it was recovered by endurance exercise training (SHR-E) compared to SHR-C ($p$ <0.05), and VEGF protein was also increased by endurance exercise training compared to SHR-C ($p$ <0.05). These data suggest that the enhancement of capillary density via an increase of VEGF expression in skeletal muscles by endurance exercise training could be an important factor to inhibit blood pressure elevation in SHR.
Extracorporeal membrane oxygenation(ECMO) provides stable oxygenation to prevent elevation of pulmonary vascular resistance and bypasses a significant part of cardiac output to the pulmonary vascular bed to reduce pulmonary perfusion pressure. In addition, ECMO prevents right heart failure and low cardiac output by means of ventricular assist and reduction in volume load to right ventricle. As a result, ECMO can be used for the treatment of pulmonary hypertensive crisis after surgery for congenital heart disease, especially when it is refractory to conventional measures. We report a case of postoperative pulmonary hypertensive crisis, developed in a 37-year-old male with patent ductus arteriosus with secondary pulmonary hypertension, which was successfully managed including ECMO.

This study examined the effects of an extract of Dendropanax morbifera on blood pressure, Angiotensin II, Angiotensin Converting Enzyme, Aldosterone, and lipid levels of spontaneously hypertensive rats. The groups were as follows: Control group, Hypertension control group, Water extract treated group, Ethanol extract treated group, n- hexane fraction treated group, Ethyl acetate fraction treated group, n- butanol fraction treated group, and Water fraction treated group. The blood pressure, and Angiotensin II, and Angiotensin Converting Enzyme, and Aldosterone levels were lower in the Ethyl acetate fraction treated group than in the hypertension control group. The change in blood pressure was lower in the Water extract treated group, Ethyl acetate fraction treated group, Ethanol extract treated group, n- hexane fraction treated group, and n- butanol fraction treated group than the hypertension control group. The concentration of Angiotensin II was lower in the Ethyl acetate fraction treated group, Ethanol extract treated group, n- hexane fraction treated group, and n- butanol fraction treated group than the hypertension control group(p<0.05). The level of Angiotensin Converting Enzyme was lower in the Ethyl acetate fraction treated group, Ethanol extract treated group, n- hexane fraction treated group, and n- butanol fraction treated group than the hypertension control group. The concentration of Aldosterone was lower in the Ethyl acetate fraction treated group, and n- butanol fraction treated group than the hypertension control group(p<0.05). In addition, the concentration lower in the Water extract treated group, Ethanol extract treated group, n- hexane fraction treated group than the hypertension control group. Overall, the effect of the Dendropanax morbifera extracts might be useful as anti-hypertensive, and functional food agents.
