Cathepsin S as a Cancer Therapeutic Target (암 치료 표적으로써 cathepsin S)

  • Cysteine cathepsins are lysosomal enzymes that belong to the papain family and can induce the degradation of damaged proteins through the endo-lysosomal pathway. It is highly upregulated in many cancers by regulating gene amplification and transcriptional, translational, and post-transcriptional modifications. Cathepsin S is part of the cysteine cathepsin family. Many studies have demonstrated that cathepsin S not only plays a specific role in MHC class II antigen presentation but also plays a crucial role in cancers. Cathepsin S is more stable at a neutral pH compared to other cysteine cathepsins, which supports the importance of cathepsin S in disease microenvironments. Therefore, the dysregulation of cathepsin S has participated in a variety of pathological processes, including cancer, arthritis, and cardiovascular disease. Furthermore, a decrease or depletion in the expression of cathepsin S has been implicated in the processes of tumor growth, invasion, metastasis, and angiogenesis. Taken together, cathepsin S has been suggested as an attractive therapeutic target for cancer therapy. In this review, the known involvement of cathepsin S in diseases, particularly with respect to recent work indicating its role in cancer therapy, is examined. An overview of current literature on the inhibitors of cathepsin S as a therapeutic target for cancer is also provided.

  • Osteochondroma is a hyperplasic bony lesion caused by subperiosteal displacement of physeal cartilages, and the most common benign bone tumor. Osteochondromas rarely cause vascular complications, but when encountered, acute ischemia, phlebitis, and arterial puncture are the most common. They are caused by the mass effect or by the sharp end of the tumor. Deep vein thrombosis may be caused by impaired venous blood flow or a hypercoagulable blood state, but sometimes no predisposing cause can be found. We experienced a patient with deep vein thrombosis caused by the mass effect of an asymptomatic osteochondroma, and treated him successfully by excising the osteochondroma and performing a saphenous vein graft after resecting obstructed vein.

  • Pseudoaneurysm of the thoracic aorta is potentially fatal. However, reports of such cases are rare even in large series. We report four cases of thoracic aortic pseudoaneurysm who underwent surgical repair, The causes were considered as infection in two cases (VSD repair, descending thoracic aortic aneurysm resection) and blunt chest trauma by traffic accident in two patients. The pseudoaneurysms developed on ascending aorta suspected as sites of arterial and cardiolplegic needle insertion in one patient. The others were located at descending thoracic aorta immediatly below the left subclavian artery. One patient died of sepsis associated with bile peritonitis and others were followed up from 10 to 18 months with specific morbidity. This study suggest that the incidence of pseudoaneurysm of the thoracic aorta followed by open heart or aorctic surgery can be repaired succesfuly and careful inspection of associated injury is very important in cases of traumatic thoracic pseudoaneurysm.

  • A group of patients with Kawasaki disease (KD) initially present with cervical lymphadenitis or deep neck infection. These unusual KD presentations lead to unnecessary antibiotic therapy or surgical intervention, thereby delaying intravenous immunoglobulin treatment and increasing the risk of coronary artery damage. We present four KD patients whose initial presentations mimicked a retropharyngeal abscess. Nonsuppurative cervical lymphadenitis or suspected neck abscess unresponsive to intravenous antibiotics could signal the possibility of KD.

  • Aorto-cutaneous fistula is a rare complication after performing open heart surgery, but if this develops, it is a fatal condition. So, prompt diagnosis and aggressive surgical treatment is needed. We report here on a patient who had two mechanical double valves placed during heart surgery and she was treated for repeated sternal wound infections for about 5 years. She visited the ER due to abrupt bleeding at the sternal wound. She was diagnosed as having an aorto-cutaneous fistula by performing an aortogram and we then performed cardio-pulmonary bypass surgery. The patient is currently doing well and is under follow up 24 months after the repair.

  • The idiopathic hypereosinophilic syndrome (HES) comprises a heterogeneous group of disorders with unknown pathogenesis characterized by persistent peripheral blood and bone marrow eosinophilia. And the eosinophil infiltrates of multiple organs in HES lead to severe organ dysfunction. The disseminated intravascular coagulation (DIC) is a rare complication of HES. We have experienced a case of HES complicated with DIC and pulmonary thromboembolism. After intravenous injection of methylprednisone, blood eosinophil count was normalized but DIC was persisted. With cortico steroid and cyclosporine therapy, the disease activity was favorably remitted.

  • Kawasaki disease (KD) is a systemic vasculitis that can affect many organ systems. Renal manifestations include pyuria, hematuria, proteinuria, tubulointerstitial nephritis, acute renal failure, hemolytic uremic syndrome, or renal scarring. Although its precise pathogenesis remains unknown, it is considered an autoimmune disease. In the literature, it has been reported that KD may develop in conjunction with urinary tract infections. However, many of these previous studies did not use imaging methods such as renal sonograms, dimercaptosuccinic acid renal scans, and voiding urethrocystograms. We report a case of an 8-month old male infant with high grade vesicoureteral reflux, who developed incomplete KD after recurrent pyelonephritis. Acute pyelonephritis can be an early manifestation of KD. Such cases require the evaluation of urinary tract anomalies according to the guidelines for the management of urinary tract infections.

  • Kawasaki disease (KD) is an acute febrile mucocutaneous lymph node syndrome that commonly presents with stable hemodynamic status during the acute phase. An 8-year-old boy initially presented with severe hypotension and acute kidney injury. He was placed in the intensive care unit and was diagnosed with KD. Observed clinical features were defined as KD shock syndrome. His coronary artery was dilated during the subacute phase. Furthermore, he was given anti-hypertensive medications, owing to hypertension as an unusual complication of KD. We knew the importance of monitoring for blood pressure considering vasculitis as an aspect of the main pathogenesis of KD.

A Systematic Review of Acupuncture Treatment for Thromboangiitis Obliterans (폐색성 혈전 혈관염의 침 치료에 대한 체계적 문헌고찰)

  • Objectives The purpose of this study is to present evidence by analyzing the research trends in acupuncture treatment in the last 10 years for thromboangiitis obliterans. Methods Randomized controlled trials about acupuncture on thromboangiitis obliterans were searched China National Knowledge Infrastructure (CNKI), PubMed, EMBASE, Cochrane Library, Oriental Medicine Advanced Searching Integrated System (OASIS), ScienceON, Koreanstudies Information Service System (KISS) and Research Information Sharing Service (RISS) from January 1, 2011 to November 13, 2021. Only randomized controlled trials were selected and we assessed the risk of bias (RoB) according to the revised Cochrane RoB2 criteria. Results A total of 5 randomized controlled trials (RCTs) were selected in this review and all were conducted in China. A total of 326 participants were in 5 RCTs. SP10, ST36, GB34 was the most commonly used treatment point. There were four types of acupuncture used: manual acupuncture, embedding acupuncture, pharmacopuncture, electroacupuncture. The most commonly used indicator for evaluation was the total efficacy rate, and all five studies were significantly higher. Conclusions All selected studies showed the group treated with acupuncture treatments on thromboangiitis obliterans were statistically more effective than the control group. However, the number of studies is too small and the interpretation of the results is limited due to the inclusion of subjective evaluation. So more objective and systematic studies should be conducted continuously.

Clinical Results of the St. Jude Medical Cardiac Valve (St. Jude 기계판막의 임상성적)

  • St. Jude Medical bileaflet valve was implanted in 42 patients from September, 1988, to July, 199)(mitral valve replacement in 25, aortic valve replacement in 11, multiple valve replacement in 6). Concomitant procedures were performed in 5 patients(11.9%). The early mortality was 4.8%(2 patients) and the late mortality was 9.5%(4 patients). The valve-related causes of late mortality were prosthetic valve endocarditis(threi), unknown(one). There was no instance of structural failure. Follow up was 100% complete, with 179.8 patient-years and a mean follow up of 54 months(from 1.5 to 84 months). The overall actuarial survival rates was 90$\pm$4.7% at 5 years. At 5 years, the acturarial freedom from valve failure was 90$\pm$4.7%, 89.7$\pm$4.5% from thromboembolism and 92.3 $\pm$4.2% from bacterial endocarditis, respectively.
