The effect of sensorimotor stimulation for ingestion in dysphagic patients who have nonprogressive brain damage - Preliminary Study - (연하장애 환자의 구강인두 기능회복을 위한 감각운동 자극의 효과)

  • 운동학습이론인 감각운동 통합이론을 본 연구의 개념적 틀로하여 한방병원에 입원한 뇌졸중 환자 중 구강인두 기능이 저하된 환자를 대상으로 감각운동자극이 구강인두 기능에 미치는 효과를 조사하기 위하여 1988년 3월초부터 5월 중순까지 예비실험 연구하였다. 감각운동자극을 주기 전에 구강인두기능을 시정하고 대상자 전원에게 저하된 구강인두기능에 따라 감각운동자극을 선택하여 하루 3번 매식후 30분전에 주었으며, 이를 일주일 동안 계속하였다. 일주일 후 다시 구강인두기능을 측정하여 자극을 주기전, 후의 기능정도를 paired t-test로 비교 분석하였으며 다음과 같은 성적을 얻었다. 1. “자극을 주기 전보다 준 후에 머리와 목운동 조절점수가 높을 것이다”라는 제1가설은 지지되었다(t=-2.71, df=5, p=.042). 2. “자극을 주기 전보다 준 후에 씹는 능력이 증가되었으나, 통계적으로는 유의하지 않았다(t=-2.00, df=5, p=.102). 3. “자극을 주기 전보다 준 후에 입술운동의 조절점수는 높을 것이다”라는 제3가설은 지지되었다(t=-4.77, df=5, p=.005). 4. “자극을 주기 전보다 준 후에 혀 운동 조절점수가 높을 것이다”라는 제4가설은 지지되었다(t=-3.61, df=5, p=.015). 5. 자극을 주기 전보다 준 후에 연하능력이 증가되었으나 통계적으로는 유의하지 않았다(t=-2.24, df=5, p=.076). 이상의 결과로 감각운동자극은 연하장애환자의 구강인두기능을 증진시키는데 효과적인 방법이라고 할 수 있다.

  • The tongue, especially its lateral part, is the most common site of oral tumors. Patients who undergo glossectomy for the treatment of tongue cancer may experience difficulty in proper functioning for pronunciation, chewing, swallowing, and oral hygiene maintenance; therefore, a palatal augmentation prosthesis can be used to restore function of the tongue. In this case, an implant overdenture was used in a patient who had residual ridge resorption and obliteration of alveololingual sulcus after undergoing glossectomy for tongue cancer treatment. In addition, a palatal augmentation prosthesis with a metal framework, support, and retention part was fabricated. The palatal vault was reduced, so that even with limited tongue movement, adequate tongue-palate contact could be achieved. After placement of the definitive prostheses, the patient showed improvement in the functions of chewing, swallowing, and pronunciation.

  • Tourette's syndrome is a chronic neuropsychiatric disorder characterized by the presence of vocal and multiple motor tics. Tics are defined as brief, intermittent, repetitive, unpredictable, purposeless, and stereotyped movements or sounds. Some patients experience physical pain from intense and complex tics. In addition, motor tics can result in self-injury which is a common feature of Tourette's syndrome. A 9-year-old boy was referred by the department of neuropsychiatry because of a severe tongue laceration. His parents reported that he had been biting his tongue irregularly for 2 months before referral and suffered from an intense burning sensation. The repeated biting resulted in ulcers on the tongue, which quickly worsened and led to progressive difficulty chewing and swallowing food. We offered to give him a two-piece removable appliance to limit tongue biting; it was made of soft silicone and fitted to both the maxillary and mandibular arches. As we emphasized that the device could help alleviate his pain, he agreed to accept it and adapted well. Just 3 weeks later, his tongue lesions had healed significantly.

  • A problem that affects children's dentitions is the harmful habit which is difficult to treat. Harmful habits for children are such as abnormal swallowing patterns, low/forward tongue rest posture problem, habitual open-lips resting posture, habitual mouth-breathing, excessive digital sucking habit and tongue thrusting. Tongue thrusting habits cause a bit of cranio-facial skeletal changes and a great deal of dental malocclusion such as anterior open bite. Anterior open bite causes masticatory, speech, and esthetic problems in the growing children and difficulties in diagnosis, treatment, and the prediction of its prognosis. The treatments of such abnormal behaviors involve orofacial myofunctional therapy and using of habit breaking appliance. The prognosis is not determined by the presence of severity of oral habit but the skeletal tendency of the patient. Usage of tongue crib resulted in not only the discontinuance of the habit but also improvement in overbite and overbite. This study showed that relatively successful results could be generated by using removable tongue crib and myofunctional therapy in the case of openbite related to tongue thrusting habit.

  • TRAINER for Kids ($T4K^{TM}$, Myofunctional. Research Co, Australia) is a prefabricated myofunctional orthodontic appliance recommended to ClassII division1 malocclusion patients who have bad oral habits such as mouth breathing, tongue thrusting, inappropriate tongue position, thumb sucking and so on. Trainer has a soft texture and a small volume so that those advantages lead to an increase in the agreement rate of young patients of its use. This presentation is to analyze clinical efficacy of Trainer. The analysis is based on a result of regular follow-up on Class II division1 malocclusion patients who has been completely treated by Trainer in the Sanbon Dental Hospital of Wonkwang university. This case report is to present the satisfactory results gained by using Trainer on Class II patients. First, Trainer was applied in Class II malocclusion patients of mixed dentition with expected space insufficient to gain facial improvement. Second, excessive overjet, overbite were improved. Third, main effects are regarded to have been achieved by development of lingual slant of upper jaw, labial slant of lower jaw, and lower part of jaw bone.

Effects of tongue strength training program combined with oral exercises on swallowing disorder, swallowing ability, and tongue pressure in the elderly (입체조를 병행한 혀 근력운동이 노인의 연하장애와 연하력 및 설압에 미치는 효과)

  • Objectives: The purpose of this study is to verify the usefulness and practicality of the program in the daily life of the elderly by performing a three-dimensional tongue muscle exercise for the elderly to identify the dysphagia, swallowing ability, and tongue pressure. Methods: The subjects of the study were 29 elderly people aged 65 or older living in Busan, divided into a oral exercises group and tongue strength training program combined with oral exercises group, and operated 16 times for 8 weeks twice a week, and the pre and post-program evaluation was conducted in the 1st and 8th weeks. For variable selection, 7 general characteristics, 5 oral health-related characteristics, 5 Likert scale for swallowing disorder, repetitive swallowing ability test for swallowing power measurement, and tongue pressure measurement was used. Data analysis was conducted using SPSS window program 25.0 version (IBM), general characteristics and homogeneity tests, oral health-related characteristics and homogeneity tests were chi-square test, swallowing disorder, swallowing ability, and change in tongue pressure were paired t-test, Wilcoxon signed rank test, t-test, Mann-Whitney U test for swallowing disorder and changes in swallowing ability and tongue pressure, and Spearman's correlation for the relationship between swallowing disorder and swallowing ability and tongue pressure. Results: As a result of tongue strength training program combined with oral exercise, both relieve swallowing disorder and improve low eating and tongue pressure than oral exercise, and the difference in improvement effects of both relieve swallowing disorder, swallowing ability, and tongue pressure is higher. Conclusions: Based on the results of this study, it is believed that simple and practical oral muscle function reinforcement products and practical use are needed, and institutional devices to seek oral health promotion programs for the elderly are needed.

Effect of Articulation Abilities on the Articulator Strength Training by IOPI of Spasticity Dysarthric Speech (IOPI를 활용한 조음기관 훈련 프로그램이 경직형 마비말장애의 조음 능력에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of the IOPI articulator strength training program on articulator(tongue and lip) muscle strength, numbers of /l, s, ʨ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy in the alternate motion rates, and sequential motion rate changes in patients with spastic dysarthria. Methods : Three cases of patients with spastic dysarthria living in Jeju, Korea, were included in this study. A single subject design was selected to study changes in articulator(tongue and lip) muscle strength, numbers of /ㄹ, ㅅ, ㅈ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy in the alternate motion rates and sequential motion rates. Results : After the articulator strength training program was conducted on patients with spastic dysarthria, there were positive changes in articulator(tongue and lip) muscle strength, numbers of /ㄹ, ㅅ, ㅈ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy on the alternate motion rates and sequential motion rates. Conclusion : Our findings suggest that IOPI articulator strength training program could be very useful for the most representative childeren with cerebral palsy if conducted in various subtypes of dysarthric patients and linked with articulatory function training with IOPI at home.

Oral-Motor Facilitation Technique (OMFT): Part II-Conceptual Hierarchy and Key Point Technique (구강운동촉진기술: 2 부-개념적 위계 및 핵심 기법)

  • Introduction : OMFT is a therapeutic technique based on sensorimotor, motor control and motor learning, and its major goal is to improve oral motor function. The oral motor conceptual hierarchical development is divided into 5 steps: 1) sensorimotor, 2) movement integration, 3) structural movement, 4) functional oral motor, and 5) comprehensive oral motor. Discussion : The OMFT consists of 3 techniques, 10 categories, and 50 sub-item. 1) Warming up technique: 2 categories, 12 sub-item, warming up by sensory awareness and adaptation, therapy situation adaptation, neck movement; 2) Key point technique: 7 categories, 30 sub-item, oral motor facilitation and increasing chewing skill by direct stroke of oral structures such as the face, lips, cheeks, gum, jaws, and tongue; 3) Application technique: 1 category, 8 sub-item, facilitate food intake and swallowing. Conclusion : The goal of this article is to introduce 3 techniques, 50 sub-item of OMFT, as a comprehensive oral motor therapy method, for application to clients. This article provides information that will help oral motor specialists in treating clients with oral motor problems more effectively and professionally.

The Effects of Program by PNF on the Articulator Function of Stroke Patients (뇌졸중 환자의 조음기관 기능에 PNF를 이용한 프로그램이 미치는 효과)

  • The purpose of this study was to identify an effects of Proprioceptive Neuromuscular Facilitation(PNF) on articulator function of the stroke patients. This study evaluated pre- and post- articulator function, targeting the test group for whom an program by PNF was applied, the contrast group for which an articulator training program was applied, and the control group for which no intermediation was made. Each program were performed total 24 times based on 3 times per 30~50 minutes. In result, MPT, AMR ,SMR of the test group were enhanced. AMR of the contrast group did not show any significant difference in /tə/, /rə/, and /a/. Therefore, Program by PNF is informed to have a effect in advancing the articulator function of the stroke patients, especially it was effective of the tongue movement.
