Purpose: This study aimed to investigate factors affecting frailty by age groups among vulnerable elders in Korea. Methods: In this secondary analysis, data were collected from records for 22,868 elders registered in the Visiting Health Management program of Public Health Centers in 2012. Health behaviors, clinically diagnosed disease, frailty, depression and cognitive condition were assessed. Data were analyzed using stepwise regression to determine the associated factors of frailty by age group. Results: Alcohol consumption, physical activity, number of diseases, DM, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors significantly associated with frailty among the elders aged 65~74 (F=135.66, p <.001). Alcohol consumption, physical activity, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors associated with frailty in the elders aged 75~84 (F=245.40, p <.001). Physical activity, CVA, arthritis, depression and cognitive condition were factors associated with frailty in the elders over 85 years of age (F=96.48, p <.001). Conclusion: The findings show that frailty of elders and associated factors were different by age group, and common factors affecting frailty were physical activity, CVA, arthritis, depression and cognitive condition. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to age group.

The purpose of this study was to identify the effects of a visiting exercise program for physical activity promotion on physical (grip strength, static balance) and psychological (depression, perceived health status) function of frail elderly patients receiving home visiting service care. Subjects included a total of 49 patients; 24 in the control group and 25 in the experimental group. The visiting exercise program was carried out 16 times for 8 weeks. The collected data were analyzed by ${\chi}^2-test$ and t-test using SPSS/WIN 21.0 program. There was a significant improvement in the left grip strength (t=2.19, p<.000), right grip strength (t=5.07, p=.032), static balance (t=3.98, p=.050), and depression (t=-8.83, p=.009) in the experimental group compared with the control group. The visiting exercise program for physical activity promotion was effective on physical and psychological functions for frail elderly patients receiving home visiting service care. Further research is needed in a longitudinal study incorporating various centers to examine the maintenance of such effects.
Although the proportion of frail elderly among the elderly population has been rapidly increasing, there is little preparation to provide housing alternatives for them in Korean society. Long-term care facility a housing alternative for frail elderly that enables them to receive social and medical care services that enhance the quality of their life. This research was conducted to study the opinions of middle-aged Koreans about long-term care facilities for the frail elderly. This study provides useful suggestions for the development of user-paid long-term care facilities in the near future. A survey questionnaire was administered to 600 adults in their 50s to collect data. The sample was stratified according to region, gender, and housing structure type. The results showed that most of the respondents were aware of long-term care facilities for the frail elderly; but they were relatively unwilling to live in these facilities, as compared to other housing alternatives. However, a larger number of respondents said that as they became very frail, they would be willing to live in these facilities. The respondents expressed a high level of need for user-paid long-term care facilities as an alternative to hospitalization. Also, they wanted to have a variety of long-term care facilities that are designed for different levels of disability. The education level of respondents and their spouses, average monthly income, subjective economic level, and religion were major variables that differentiated significantly the opinions about long-term care facilities for the frail elderly.
Purpose: The purpose of this study was to identify the effect of subjective xerostomia and salivary pH in salivary glands stimulated by laughter therapy in frail elderly women. Methods: The research used a one-group pretest-posttest design. Data were collected from July 2, 2015 to September 30, 2015. A sample of 41 frail female elderly patients was recruited at A Nursing Home in K City, Korea. We measured xerostomia and salivary pH using a questionnaire and BCP test paper (pH 5.6~7.2). Laughter therapy was given once a week for four weeks (3 items). Data were analyzed through descriptive statistics, independent t-test, ANOVA, paired t-test and Cronbach's using the SPSS 18.0 program. Results: There were significant differences in salivary pH according to oral health (t=-2.06, p<.05). There were significant differences in xerostomia (t=4.41, p<.001) and salivary pH (t=-7.94, p<.001) after salivary glands stimulated by laughter therapy. Conclusion: Salivary glands stimulated by laughter therapy improved xerostomia and salivary pH of the frail elderly. Therefore, salivary glands stimulated by laughter therapy may be useful in promoting and maintaining oral health among the frail elderly in the rapidly increasing population of old people.
Purpose: This study was performed to evaluate effects of a stim-up matt walking exercise program on balance and gait of the frail elderly. Methods: A total of 37 elderly people recruited from S city were randomly assigned to the experimental group (n=22) and control group (n=15). The stim-up matt walking exercise program was offered twice a week for 8 weeks. Data were analyzed by SPSS 21.0. Results: The dynamic balance ability Timed Up and Go test of the experimental group was significantly faster than that of the control group (t=21.72, p<.001). The static balance ability open-eye standing test (t=44.15, p<.001) and close-eye standing test (t=9.01, p=.005) also showed increase in effects of the experimental group. In the walking ability, gait cycle (t=2.48, p=.018), cadence (t=-2.21, p=.034) and gait speed (t=-2.78, p=.009), positive effects were on. However, no statistically significant differences were found in stride length and double support. At the ankle joint range left ankle plantar flexion (t=3.92, p<.001) and left ankle dorsal flexion (t=4.51, p<.001) were higher in the experimental group than in the control group, and also right ankle plantar flexion (t=2.79, p=.008) and right ankle dorsal flexion (t=2.92, p=.006) increased in the experimental group. Conclusion: The significance of this study is that the stim-up matt walking exercise program for the frail elderly proves to be useful for improving balance and walking.

This study was performed to investigate the factors which impact on the health-related quality of life of young-old(65-69 yr), old-old(70-79 yr), and oldest-old(80 yr or above) women in vulnerable elderly received home care service from public health centers in B city. The data were collected from 383 elderly women using structured questionnaires from September to November, 2010. Multiple regression with the SPSS WIN 18.0 program were used to analyze the data. There were statistically significant differences among young-old, old-old, and oldest-old women regarding the health-related quality of life, life satisfaction, cognitive function, frail condition. The models including life satisfaction, frail condition, cognitive function, perceived health status, number of chronic diseases were explained variance of the health-related quality of life elderly women differently like 42.8% of young-old, 28.9% of old-old, and 31.5% of oldest-old. Finally, frail condition and life satisfaction were predictors in explaining the level of health-related quality of life among vulnerable old women regardless of age. Based on the findings of the study, health promotion programs should be developed to improve health-related quality of life of vulnerable aged women according to age differences.
Purpose: The purpose of this study is to provide a disease management, nutrition education, and group exercise program for three months to the moderately frail elderly whose physical functions have deteriorated, and to investigate its effects in order to develop an intervention program. Methods: As a quasi-experiment, this study was conducted based on non-equivalence studies designed as a similar experiment. The milk intake group and calcium intake group participated in the disease education, individual nutrition education, and group exercise program for three months, and the control group was visited once in the three months by a nurse who provided disease education, nutrition education, and oral instruction of exercise, and asked them to exercise on their own every day. For the data analysis, ${\chi}^2$-test, ANOVA and $Scheff{\acute{e}}$ test were used. Results: After three months of intervention, there was a significant difference in the frailty level (p=.029) and bone density (p=.001) between the groups. Conclusion: The comprehensive intervention program had an effect on the bone density and the frailty level of the socially-vulnerable and moderately frail elderly, suggesting that the program can be used as a nursing intervention to prevent functional deterioration and damage of the moderately frail elderly.
Purpose: The electronic frailty index (eFI), which is derived from electronic health records, has been recommended as screening tool for frailty due to its accessibility and ease of use. The objective of this systematic review was to identify the prevalence of frailty assessed by the eFI and its influence on health outcomes in older adults with chronic diseases. Methods: We searched PubMed, Embase, Web of Science, CINAHL, SCOPUS, Cochrane, Google search, and nursing journals in Korean from January 2016 to December 2022. Results: Twelve studies were analyzed. The eFI score, based on routine clinical data, was associated with adverse health outcomes. The most frequent outcome studied was mortality, and the eFI was associated with increased mortality in nine studies. Other outcomes studied included hospitalization, length of stay, readmission, and institutionalization in relation to hospital care usage, and cardiovascular events, stroke, GI bleeding, falls, and instrumental activities of daily life as health conditions. Conclusion: Early identification of frailty in older adults with chronic diseases can decrease the burden of disease and adverse health outcomes. The eFI has a good discriminative capacity to identify frail older adults with chronic diseases.

This study is for the elderly with mental and physical dysfunction, developing resistance exercise program using tube elasticity, and analysing the effects of this movement on the improvement of gait ability, balance ability and muscle strength of the elderly. After a 12-week program for 58 participants of the daycare center, dependent t-test was performed. As a result, the total participants showed average improvement in gait, balancing and hand grip strength. This means a lot in terms of improving one's ability of ADL(Activities of daily living) and reducing the risk of falling. Therefore, if the program expands and activates the target as an entire elderly, it is expected to be a preventative way to prevent healthy elderly people from moving to the frailty who need treatment and to delay the deterioration of the frailty with disabilities.
Purpose: The purpose of this study was to review and identify the meaning and components of the concept, Frailty. Method: We conducted literature review of studies that concluded the word of 'frail' or 'frailty between 1980 and 2008, and used MEDLINE, CINAHL database to select the articles. Results: Frailty is defined as a concept with multidomains, which are physical, cognitive, psychological, social. Critical characteristics of Frailty include multidominal deficiency, combined accumulation, diminished ability to keep up the independence of daily living, states beyond one's reserve capacity, dynamic relativity, proximity to adverse health outcome, aggregated symptoms. Frailty is caused by decreased physical activity, loss of sensory function, Chronic symptoms or signs, relationship with Caregiver, social isolation. Moreover, Frail elderly is at risk of falls and institutionalization. Conclusion: Frailty is very useful concept, because it has the potential to identify the elderly population at risk of adverse health outcomes. Based on this results, the appropriate tool for screening Korean Frail elderly and Nursing intervention for them needs to be developed.
