A Study on Behavior Health Care Competency between Psychiatric Ward Nurse and General ward Nurse (정신과병동과 일반병동 간호사의 행동건강간호역량에 관한 비교 연구)

  • This study is a cross sectional, descriptive research that utilized the Korean version of the BHCC (Behavior Health Care Competency) measurement tools to compare and contrast the BHCC level between nurses stationed in psychiatric and general ward departments. The research subjects were selected from 6 hospitals that have at least 300 beds located in Seoul city and Gyeonggi Province. There were a total of 190 nurses, consisting of 90 nurses from the psychiatric ward department and 100 nurses from the general ward department. The comparison demonstrated that nurses from psychiatric ward department showed a higher BHCC compared to general department nurses in most items. In the case of psychiatric ward nurses, compared to general ward nurses, the assessment was 5.29 times higher, the intervention was 6.06 times higher and the proper use of resources was 2.63 times higher. On the other hand, the treatment recommendation had no influence. Accordingly, education and training for general ward nurses should be improved and hospital administrators should pay more attention in conducting the BHCC education for the general ward nurses and on fostering education experts to develop related programs.

A Case Study of Group Music Psychotherapy for Improving Adolescents' Awareness of the Motivation Behind Their Excessive Online Gaming (게임 과몰입 청소년의 게임 이용동기 인식을 위한 그룹 음악심리치료 사례)

  • This case study aimed to investigate changes in adolescents' awareness of the motivation behind their excessive internet gaming after group music psychotherapy. Five middle school students participated in six 60-minute group sessions over 4 weeks. The group music psychotherapy program was developed to help participants understand their implicit reasons for excessive internet gaming. Verbal responses were obtained from pre-program and post-program interviews and original lyrics created by participants during program were collected for analysis. Obtained data were analyzed in terms of emotional and cognitive aspects in relation to the participants' awareness of the motivation behind their excessive gaming. The results showed that song writing as creative musical process allowed participants to be aware of positive and negative influence of gaming on themselves and express their motivation to change their perception on gaming behavior. Also, the participants reported that the use of musical product for their daily lives could be an effective coping strategy to manage their gaming behaviors and an alternative for gaming. The findings from this study support that group music psychotherapy can be an effective approach to promote awareness of adolescents' motives for excessive internet gaming and to help adolescents better manage their gaming behaviors.

A Study on the Stages of Exercise Behaviors in Patients with Rheumatoid Arthritis (류마티스 관절염 환자의 운동행동 변화단계에 관한 연구)

  • This study examined factors affecting the stages of exercise behavior among patients with rheumatoid arthritis. The analysis was based on data collected from 238 patients who enrolled at the medical center of D university. The participants showed 5 stages of exercise behavior: pre-contemplation 23.9%, contemplation 23.0%, preparation 33.2%, action 5.0%, and maintenance 29.0%. There were statistically significant differences in perceived barriers of exercise, exercise self-efficacy, and perceived health status according to the stage of exercise behavior of the subjects. It was found that among the barriers to exercise, physical and cognitive psychological factors significantly influenced the stages of exercise behavior. Also, exercise self-efficacy significantly affected the stages of exercise behavior. Thus, for rheumatoid arthritis patients to practice and maintain proper exercise behavior, the nursing staff should continue to assess and reduce the barriers to exercise. Exercise programs should enhance self-efficacy to achieve long-term exercise behavior.

Factors related to the short-term and long-term intentions of healthy eating among Chinese adults living in Shanghai and parts of Anhui Province of China using the theory of planned behavior (계획적 행동이론 기반 상하이 및 안후이성 거주 중국 성인의 건강한 식행동의 장단기 의도와 관련된 요인)

  • Purpose: This study investigated the relationship between 3 major constructs of the theory of planned behavior (TPB), i.e., attitude, subjective norms and perceived behavioral control (PBC) and past experience of healthy eating and intentions of healthy eating in the short-term and long-term in adults living in Shanghai and parts of Anhui Province, China. Methods: The online study questionnaire for this cross-sectional study was based on previously validated items. A total of 408 Chinese adults (aged 18-64 years) residing in Shanghai and parts of Anhui Province, China were included to examine relationships between 3 major constructs of TPB and past experience of healthy eating, and short-term and long-term intentions of healthy eating. Multiple linear regression model adjusted for age and body mass index (BMI) was employed to test relationships. Results: Only PBC among 3 major constructs of TPB was significantly related to short-term (p < 0.001) and long-term (p = 0.002) intention of healthy eating after adjustment for age and BMI. Past experience of healthy eating was more significantly related to long-term intention (p < 0.001) compared to short-term (p = 0.020) intention of healthy eating. The short-term and long-term intention models explained 70.5% and 48.8% of the variance, respectively. Conclusion: PBC is a potential determinant of both short-term and long-term behavioral intention of healthy eating regardless of past experience of healthy eating in adults residing in Shanghai and parts of Anhui Province, China. Our results indicate that programs promoting healthy eating to Chinese adults incorporate PCB to perform healthy eating under his or her control.

Non-Pharmacological Interventions for Behavioral and Psychological Symptoms of Neurocognitive Disorder (신경인지장애의 정신행동증상에 대한 비약물학적 개입)

  • Patients with neurocognitive disorder show behavioral psychological symptoms such as agitation, aggression, depression, and wandering, as well as cognitive decline, which puts a considerable burden on patients and their families. For the treatment of behavioral psychological symptoms, patient-centered, non-pharmacological treatment should be used as a first line approach. This paper describes non-pharmacological interventions to manage and treat behavioral psychological symptoms in patients with neurocognitive disorder. In order to control behavioral psychological symptoms such as agitation, depression, apathy, insomnia, and wandering, it is important to identify and evaluate factors such as environmental changes and drugs, and then solve such problems. Non-pharmacological interventions include reassurance, encourage, distraction, and environmental change. It is necessary to understand behavior from a patient's point of view and to approach the patient's needs and abilities appropriately. Reminiscence therapy, music therapy, aroma therapy, multisensory stimulation therapy, exercise therapy, light therapy, massage therapy, cognitive intervention therapy, and pet therapy are used as non-pharmacological interventions, and these approaches are known to improve symptoms such as depression, apathy, agitation, aggression, anxiety, wandering, and insomnia. However, the quality of the evidence base for non-pharmacological approaches is generally lower than for pharmacological treatments. Therefore, more extensive and accurate effectiveness verification studies are needed in the future.

Dental Hygienist-Led Dental Hygiene Process of Care for Self-Support Program Participants in Gangneung (강릉시 자활근로사업 참여자 대상 치위생 과정 사례보고)

  • This study aimed to provide basic data for establishing the clinical basis for dental hygienist-led dental hygiene process of care by identifying multiple risk factors for self-support program participants in Gangneung city; we also compared oral health status and behavioral changes through customized oral health care. Four dental hygienists who were evaluated for degree of conformity provided dental hygiene process of care to eight self-support program participants who were selected as having an oral health risk among people in the self-support center. The clinical indicators measured during dental hygiene assessment and evaluation and behavioral changes due to dental hygiene intervention were compared and analyzed. With respect to clinical indicators, at the time of probe, the retention rate of patients with gingival bleeding decreased from 61.4% to 14.7% after intervention (p=0.004). Furthermore, the retention rate of patients with a periodontal pocket >4 mm decreased from 15.6% to 5.8% (p=0.001). The average modified O'Leary index of the patients improved from 23 to 40 (p=0.002). Previously, all eight subjects used the vertical or horizontal method of brushing; after dental hygiene care interventions regarding method and frequency of toothbrushing, use of oral care products, and individual interventions, they started using the rolling or Bass method of toothbrushing. Four of eight subjects reported using interdental toothbrushes after intervention. As a result of applying the change model to the transtheoretical behavior change of the subject, the result of strengthening the health behavior was confirmed. For promotion of oral health by the prevention-centered incremental oral health care system, dental hygienist-led dental hygiene management and maintenance is essential. It is thought that continuous research, such as for feasibility evaluation, cost benefit analysis, and preparation of legal systems, is needed to establish and activate dental hygiene management.

Occupation-based Occupational Therapy for an Youth With Sensory Integrative Dysfunction - A Single Case Study (감각통합기능장애를 가진 청소년의 작업수행에 초점을 맞춘 작업치료 사례)

  • Introduction : Sensory Integration(SI) theory, science it is developed by an occupational therapist A. Jean Ayres, is the one of the most popular frames of reference used in occupational therapy(OT) intervention. It has been proved as a scientific theory especially in neuroscience discipline through abundant research and practice. Occupational therapists apply the SI therapy with strong clinical reasoning to improve adaptive behaviors of their clients and try to link the adaptive behaviors with occupational performance in the clients' everyday life. One of the manners regarding clinical reasoning is Top-down approach. In occupational therapy discipline, Top-down approach is well-reflected within two evaluation tools; Canadian Occupational Performance Measure(COPM) and Assessment of Motor and Process Skills(AMPS) and two models of practice; Canadian Model of Occupational Performance(CMOP) and Occupational therapy intervention process model(OTIPM). Objective : The purpose of this paper demonstrates how SI therapy can be employed within OTIPM and how the OT process (evaluation-intervention-outcome) can be structuralized based on the Top-down approach. This single-case study recognizes the impact of a SI therapy for a male adolescent on his occupational performance. Intervention Examined : "P" was 16 years old male adolescent with no diagnosis and junior of the high school when he was referred. P was always with mouth opened, showed difficulties in gathering things need to be prepared and managing and paying money for shopping, and his colleges dislike getting close to him because he can't was his body well. AMPS was administrated in initial evaluation and reevaluation of P's occupation performance, Bruininks-Oserestky Test of Motor Proficiency-2(BOT-2) was carried out to assess motor functions and perception skills related in sensory integration, and occupational therapist performed clinical observation in order to complement the evaluation quantitatively and quantitatively. Based on the evaluation, it is concluded that the SI therapy is primary means to improve P's occupational performance, and three therapeutic approaches were constructed; restorative, acquired and compensatory approach. P showed improved motor and process skills in occupational performance after undergone the occupational therapy. Conclusions : The sensory integration therapy was practical enough to build the bridge between the occupational performance(Top) and the underlying component problems (Bottom). The OTIPM was helpful to identify meaningful occupation for P and P's family within P's contexts, and the AMPS was valuable to analyze and clarify the cause of difficulties in the chosen occupational performance.

Effects of Cognitive Behavioral Therapy on Attention Deficit Hyperactivity Disorder among School-aged Children in Korea: A Meta-Analysis (한국 학령기 ADHD 아동을 위한 인지행동중재의 효과 연구: 메타분석)

  • Purpose: This study was a meta-analysis designed to identify effects of Cognitive Behavioral Therapy (CBT) interventions in alleviating main symptoms of Attention Deficit Hyperactivity Disorder (ADHD) among school-aged children in Korea. Methods: Examination of several databases including Research Information Sharing Service, Korean Studies Information Service System, Data Base Periodical Information Academic and hand-searched article references, resulted in identification of 1,298 studies done between 2000 and 2013 of which 21 met the inclusion criteria. Comprehensive Meta-Analysis version 2.0 was used to analyze effect sizes, explore possible causes of heterogeneity, and check publication bias with a funnel plot and its trim-and-fill analysis. Results: Overall effect size of CBT intervention was large (g=1.08) along with each outcome of self-control (g=1.26), lack of attention (g=1.02), social skills (g=0.92), and hyperactivity (g=0.92). For heterogeneity, moderator analysis was performed, but no significant differences were found between the RCT (Randomized Controlled Trials) group and the NRCT (Non RCT) group. Also, meta-regression was performed using sample size, number of sessions, and length of session as predictors, but no statistically significant moderators were found. Finally, a funnel plot along with trim-and-fill analysis was produced to check for publication bias, but no significant bias was detected. Conclusion: Based on these findings, there is clear evidence that CBT intervention has significant positive effects on the main symptoms of school-aged children suffering ADHD. Further research is needed to target diverse age groups with ADHD along with more RCT studies to improve the effectiveness of the CBT intervention.

The Mediating Effects of Health Promotion Behavior in the Relationship between Organizational Culture and Job Satisfaction of Hospital managers (병원 관리자의 조직문화인식이 직무만족에 미치는 영향과 건강증진행위의 매개효과)

  • The purpose of the study was to identify the mediating effects of Health Promotion Behavior in the relationship between Organizational Culture and Job Satisfaction of Hospital managers. To analyzed by frequency, t-test, ANOVA, correlation and a hierarchical multiple regression analysis as suggested by Baron and Kenny(1986) using SPSS/WIN 21.0. There was a positive relationship between organizational culture and job satisfaction. There was also a positive relationship between health promotion behaviors and job satisfaction of hospital managers. There was a partial mediating effect of Health Promotion Behavior between Organizational Culture and Job Satisfaction. The result need to develop programs that effectively promote the Health Promotion Behavior of Hospital managers to increase Organizational Culture and maximize Job Satisfaction.

A Comparative Study on Total Payment of Mental and Behavioral Disorders Patients Admitted through the Emergency Room: Focusing on Main & Sub Sick (응급실 경유 정신 및 행동장애 입원환자들의 총진료비 비교 : 주상병과 부상병 환자 중심으로)

  • The purpose of this study was to investigate the determinants of total payment for mental and behavioral disorders patients admitted through the emergency room. Study data was selected from the Korean National Health Insurance sample data in 2009. This data was consisted of 753 inpatients who are 331 inpatients with only main sick(F31-F39) and 422 inpatients with main sick codes(F31-F39) and sub sick(F00-F99) admitted through the emergency room. SPSS v.18 was used for the statistical analysis such as descriptive analysis, t-test, ANOVA, and multiple regression analysis. In multiple regression analysis, significant variables affecting total payment of main sick patients were gender(p<.01), treatment result(p<.001), path of the emergency room(p<.001), and length of stay(p<.001). Also, main sick patients with sub sick were gender(p<.01), age(p<.001), treatment result(p<.001), path of the emergency room(p<.001), and length of stay(p<.001). These findings implied that it is necessary to build short, middle, and longterm program and system for high risk mental and behavioral disorders groups.
