Effects of Silsosangami-extract on Endotoxin-induced Experimental Thrombosis in Rats (실소산가미 및 그 구성약물이 세포내독소로 인한 혈전증 흰쥐에 미치는 영향)

  • 목적: 한의학에서 어혈증으로 야기되는 여러 가지 증상의 개선에 사용되는 실소산가미의 범발성혈관내응고증 및 혈전증에 미치는 영향을 연구하는 것이다. 방법 : 동물은 Wistar-King atrain Rats(150-200g)를 사용하였고, 혈전증은 세포내독소로 유발하였다. 측정은 실소산가미와 각 구성약물들에 대한 혈소판, 섬유소원, 프로트롬빈시간, 섬유소섬유소원분해산물에 미치는 영향을 연구하였다. 결과: 항혈전의 특성에 관한 것은 실소산가미와 구성약물중 포황, 오령지, 적작약, 도인 그리고 울금에서 억제특성이 나타났으며, 또한 실소산가미와 구성약물은 정상쥐에서 범발성혈관내응고증에서 혈소판과 섬유소원의 감소가 억제되었고, 섬유소분해산물의 증가가 억제 되었다. 실험관 실험에서 실소산가미와 구성약물은 트롬빈에 의해 섬유소원에서 섬유소로 전환이 억제되었으며, 플라스미노겐 또는 플라스민의 활성을 억제하였다. 결론: 실소산가미가 세포내독소로 유발된 혈전증에 대한 억제작용을 보이므로, 혈전증으로 야기되는 심혈관계질환등의 치료 및 예방에 응용가능성이 있을 것으로 사료된다.

In-vitro Anti-thrombosis Activity of R4-Nuruk Made from Rhizopus oryzae KSD-815 (Rhizopus oryzae KSD-815를 이용하여 제조된 R4 누룩의 항혈전 활성)

  • Nuruk has been used as fermentation starter in the alcohol industry for some time in Korea. Various bioactivities, such as antiproliferative and anti-obesity, of R4 nuruk made from Rhizopus oryzae KSD-815 have been previously reported. In this study, the hot water and ethanol extract of R4 nuruk and their subsequent organic solvent fractions were prepared, and their antithrombosis activities were evaluated. The ethylacetate fraction showed strong anti-coagulation activity, and the ethylacetate and butanol fraction from hot water extract demonstrated platelet aggregation inhibitory activity, without hemolysis against human RBC. Our results suggest that R4 nuruk has the potential to act as a new anti-thrombosis agent.

Anti-coagulation and Anti-platelet Aggregation Activity of the Mature Fruit of Sorbus commixta (성숙 마가자의 혈액 응고저해 및 혈소판 응집저해 활성)

  • The mature fruit of Sorbus commixta is known as magaja in Korea and is consumed in the form of tea and wine. In addition, it has been used to treat hypertension and articular neuralgia in folk medicine. In this study, the ethanol extract of magaja and its subsequent organic solvent fractions were prepared, and their in vitro anti-coagulation, and platelet aggregation inhibitory activities were evaluated. Among the fractions, the ethylacetate fraction demonstrated significant inhibition against thrombin, prothrombin, blood coagulation factors and platelet aggregation, without hemolysis activity up to 0.5 mg/ml. Our results suggest that the ethylacetate fraction of magaja has the potential to be used as a new anti-coagulation agent.

Effects of Preoperative Continuous Use of Aspirin and Plavix in off-pump Coronary Artery Bypass Surgery (아스피린과 플라빅스의 수술 전 지속적 사용이 무심폐기 하 관상동맥우회술에 미치는 영향)

  • Background: The benefits of preoperative use of aspirin and plavix in coronary patients have been well documented. Due to their bleeding tendency, there have been many discussions about when to stop the antiplatelet agent before operation. We evaluated the effects of preoperative continuous use of aspirin and plavix in OPCAB patients. Material and Method: 123 patients underwent OPCAB from March, 2004 to Feb., 2005. We divided them into two groups; those who had continuous administration of aspirin and plavix during the preoperative period (n=45, 36.6$\%$) and those who discontinued them at least one day before the operation (n=78, 63.4$\%$). We then compared the platelet count, hemoglobin/hematocrit level, graft patency, postoperative bleeding and related complications, and operation time between the two groups. The patients were also divided into long-term users ($\geq$ 1 month) and short-term users (< 1 month), with the aforementioned factors equally compared. Result: There was no statistical difference between the two groups regarding postoperative bleeding, related complications, graft patency, operation time and mortality. Continuous users showed significantly low platelet levels on immediate post operation (p=0.02), postoperative day (POD) $\sharp$1 (p=0.002) and POD $\sharp$2 (p=0.021), respectively. But there was no difference on POD $\sharp$7. Long-term users showed statistically significant difference in pre- and postoperative platelet count, but none in postoperative bleeding and related complications. Conclusion: Continuous use of aspirin and plavix did not increase postoperative bleeding or related complications. Also graft patency and mortality had no statistical differences in continuous users. We think that there is no need to stop aspirin and plavix before OPCAB.

Anti-thrombosis and anti-oxidative activity of the root of Arctium lappa L. (우엉 뿌리의 항혈전 및 항산화 활성)

  • To investigate anti-thrombosis and anti-oxidation activities of the root of Arctium lappa L (RALL), which has been used as foodstuff and oriental medicine in Korea, the ethanol extract and its subsequent organic solvent fractions of the RALL were prepared. The yield of ethanol extraction was 10.94%, and the content of total polyphenol and total sugar of ethanol extract were 5.01 and 694.53 mg/g, respectively. The fraction yields of n-hexane, ethylacetate (EA), butanol and water residue were 1.62, 0.42, 5.98 and 85.38%, respectively. In anticoagulation activity assay, the ethanol extract of RALL did not show significant changes in thrombin time (TT), prothrombin time (PT) and activated partial thromboplastin time (aPTT), whereas the EA fractions showed 13 folds extended TT, PT, and aPTT respectively. Interestingly, the water residue showed strong activation effect against blood clotting factors with shortened aPTT, which might provide the evidence of coagulation agent of RALL in folk remedy. In anti-platelet aggregation assay, the activity of the ethanol extract and its fractions were comparable to that of aspirin. Especially the EA fraction showed 2-folds higher inhibitory activity than aspirin. In anti-oxidation activity assay, the EA fraction also showed strong in DPPH, ABTS and nitrite scavenging activity, and reducing power activity. The extract and fractions of RALL have ignorable hemolytic activity against human RBC up to 0.5 mg/mL concentration. Our results suggest that the EA fraction of RALL have potentials as safe and novel anti-thrombosis agent.

Antithrombotic Activity of Hermetia illucens (Black Soldier Fly) (동애등에의 항혈전 활성)

  • Insect industry has been focused as production of food, animal feed, pollinator and for environmental remediation. Hermetia illicens, called as black soldier fly (BSF) is famous as nutritive feed. In this study, to evaluate the antithrombotic activities of BSF, the larvae (instar 2~6), pupae, residue after adult emergence [RAAE] and adult of Hermetia illicens [black soldier fly, BSF] were collected and their ethanol extracts were prepared. Growth of BSF larvae was very rapid and the weight of larvae was increased to 25-folds during 10 days cultivation. The ethanol extraction ratios showed from 1.0% (pupae) to 18.5%(adults). The highest total polyphenol, total flavonoid, and total sugar contents were observed in RAEE (17.2 mg/g), pupae (3.4 mg/g), and instar 6 (37.6 mg/g), suggesting that metabolic changes occur during the life cycle of the BSF. Anti-coagulation assay showed that extracts of RAEE, instar 6 and pupa of BSF significantly inhibited thrombin, prothrombin, and blood coagulation factors. Furthermore, the extracts of RAEE, instar 3 and adult of BSF showed a strong platelet aggregation inhibitory activity. Our results suggest that pupae and RAEE of BSF have potential as antithrombotic agents. This is the first study to provide evidence of the antithrombotic activity of the BSF and bioactivity alterations during its life cycle.

Anti-microbial, Anti-oxidant, and Anti-thrombosis Activities of the Lees of Bokbunja Wine (Rubus coreanus Miquel) (복분자주 주박의 항균, 항산화 및 항혈전 활성)

  • The immature fruit of Rubus coreanus Miquel (bokbunja in Korean) is mainly consumed as a fruit wine due to its sour taste and low sugar content. The lees (LBW) remaining after the production of bokbunja wine are discarded as they have no specific usage. The aim of this study was to develop high-value-added biomaterials for functional foods and beauty/health products by investigating the anti-microbial, anti-oxidant, and anti-thrombosis activities of LBW using ethanol and hot water extracts and their subsequent organic solvent fractions. The ethyl acetate (EA) fraction of LBW extracts has a high polyphenol content (413–459 mg/g), and showed strong anti-microbial activity against gram-positive bacteria. The EA fraction also showed excellent radical-scavenging activity against DPPH anion, ABTS cation, and nitrite, with strong reducing power. The polyphenol-enriched EA fraction strongly inhibited thrombin, prothrombin, and blood coagulation factors. The butanol fraction showed a specific inhibition of coagulation factors, as measured in activated partial thromboplastin time assay, which is linked to intrinsic blood coagulation. The butanol fraction also showed strong inhibition of platelet aggregation, at levels comparable to aspirin. The residue of the hot-water extract, which is produced by sequential solvent fractionation of the LBW extract, showed superior inhibition against platelet aggregation when compared to aspirin. Our results suggest that the LBW, which are currently discarded, are a promising source of novel functional foods and beauty/health products.

Evaluation of the Anti-thrombosis Activities of the Aerial Parts of Sageretia thea (상동나무 지상부의 항혈전 활성)

  • Recently, thrombotic diseases have become rapidly more prevalent due to Westernized lifestyles and high-fat diets. In this study, the anti-thrombosis activities of the aerial parts of Sageretia thea were evaluated using ethanol extracts of the leaf (ST-L), branch (ST-B), and fruit (ST-F), and their anti-coagulation, platelet aggregation inhibition, and hemolytic toxicity were assessed. In comparison to the ST-F extract, the ST-B exhibited 6.7 times more polyphenol content, and the ST-L had 2.7 times more total flavonoid content. The ST-L and ST-B extracts showed stronger inhibitions of thrombin, prothrombin, and blood coagulation factors than aspirin, berry extracts, or commercial oriental herbs. Furthermore, ST-L and ST-B showed superior platelet aggregation-inhibitory activities than aspirin. The ST-F extract demonstrated only minor anti-thrombosis effects, and none of the extracts showed hemolysis against red blood cells up to 1 mg/ml. Phenolic acid and flavonoid analysis of the ST-L and ST-B extracts showed abundant rutin, isoquercitrin, and astragalin as the major active compounds. Further research on the anti-thrombotic activity of isoquercitrin, a rare flavonoid from quercetin, is necessary. This is the first report of isoquercitrin in Sageretia thea, and our results suggest that ST-L and ST-B extracts could therefore developed as anti-thrombosis agents.

A Case of Cytomegalovirus Infection in a Neonate with Osteopetrosis (골화석증에 동반된 거대세포바이러스 감염의 1례)

  • Infantile osteopetrosis is a rare congenital disorder caused by abnormal bone resorption. Patients with osteopetrosis can have severe anemia, thrombocytopenia, hepatosplenomegaly, rickets, visual impairment, and deafness. Cytomegalovirus also can cause a congenital infection with anemia, thrombocytopenia, hepatosplenomegaly, and calcifications in the brain. We report a 38-day-old infant with severe hepatosplenomegaly, thrombocytopenia, hypocalcemia, and growth failure. Real time polymerase chain reaction detected cytomegalovirus in the plasma. Skeletal radiography revealed generalized bone sclerosis. He was diagnosed with osteopetrosis along with cytomegalovirus infection. Only the test for mutation of the CLCN7 gene, representing the most common and heterogeneous form of osteopetrosis, was available, and the result was negative. With supportive care and antiviral treatment, severe thrombocytopenia due to the cytomegalovirus infection almost normalized despite the possible immunosuppression caused by osteopetrosis. We present the first report of an infant who suffered from osteopetrosis and CMV infection which was successfully treated by long term antiviral agent therapy.

Anti-platelet Effects of Isoscopoletin through Regulation of PI3K/Akt and MAPK on Collagen-induced Human Platelets (콜라겐-유도의 사람 혈소판에서 PI3K/Akt 및 MAPK 조절을 통한 Isoscopoletin의 항혈소판 효과)

  • When blood vessels are damaged, a rapid hemostatic reaction occurs to minimize blood loss and maintain normal circulation. Platelet activation and aggregation is essential in this process. However, excessive platelet aggregation or abnormal platelet aggregation may be the cause of cardiovascular disease, such as thrombosis, stroke and atherosclerosis. Therefore, it is important to prevent and treat cardiovascular disease by finding substances that can regulate platelet activation and suppress aggregation reactions. Isoscopoletin, which is mainly found in the roots of plants Artemisia or Scopolia, has been reported to have potential pharmacological effects on anticancer and Alzheimer's disease, but its role and mechanisms for platelet aggregation and thrombus formation are unknown. This study confirmed the effect of isoscopoletin on major regulation of collageninduced human platelet aggregation, TXA2 production and intracellular granular secretion (ATP and serotonin release). In addition, the effects of isoscopoletin on phosphorylation of phosphorylated proteins PI3K/Akt and MAPK involved in signal transduction in platelet aggregation was studied. As a result, isoscopoletin significantly inhibited the phosphorylation of PI3K/Akt and MAPK, significantly inhibiting platelet aggregation through TXA2 production and intracellular granular secretion (ATP and serotonin release). Therefore, we suggest that isoscopoletin is an anti-platelet substance that regulates phosphorylation of phosphorus proteins such as PI3K/Akt and MAPK and is valuable as a preventive and therapeutic agent for platelet-derived cardiovascular disease.
