DRUG INDUCED GINGIVAL HYPERPLASIA IN TAKAYASU'S ARTERITIS : DENTAL CONSIDERATION (Takayasu's Arteritis 환자에서 약에 의해 유발된 치은 비대)

  • Takayasu's arteritis(TA) is commonly known as Aortitis Syndrome, Pulseless disease. Cardiac involvement due to vascular occlusion or stenosis is common in TA, expressed in forms of heart failure, aorta hypertension, alteration of artery or cardiac muscle. In this case, a 9 year old boy was referred to our dental clinic by his cardiology doctor for delayed eruption and gingival hyperplasia on upper incisor. The patient was diagnosed as TA with history of taking Amlodipine, a calcium channel blocker as hypertension medication. He was diagnosed as drug induced gingival hyperplasia. Under taking preventive antibiotic, gingivectomy was done. In case dental treatment of TA patient, dentist should be aware of two possible problems. First is the antibiotic prophylaxis due to the high risk of endocarditis. Second is the possibility of drug induced gingival hyperplasia.

  • Incidence and mortality rate of maxillofacial infection is relatively low in the era of antibiotics. Despite the use of antibiotics, delayed treatment, underlying systemic diseases, drug-resistant microorganisms may result in life-threatening situations. The deep neck infection developed from odontogenic infection may result in sepsis, mediastinitis, aspiration pneumonia, asphyxia. Sepsis is the most dangerous complication which can quickly result in a number of lethal situations. The treatment of sepsis includes awareness of such complication, use of sensitive antibiotics, removal of infection source, and hemodynamic, respiratory and metabolic support. We experienced a patient who died of sepsis, which developed from odontogenic infection. The initial diagnosis was a buccal space cellulitis. However, in spite of medical and surgical treatment, this progressed to Ludwig's angina and then deep neck infection and finally sepsis. On the 10th hospital day, the patient died of multiorgan failure caused by sepsis.

The Effect of Azithromycin on the Cyclosporin-Ainduced Gingival Fibroblast Overgrowth (Azithromycin이 Cyclosporin-A에 의한 치은섬유아세포 과증식에 미치는 영향에 대한 in vitro 연구)

  • Cyclosporin-A(CsA)는 장기와 조직 이식에 따른 거부반응을 조절하기 위해 사용되는 면역억제제로, 이식의학의 발달과 더불어 사용량이 증가하고 있다. CsA의 부작용중의 하나인 치은과증식은 30-50%의 빈도로 발발하고 있다. 최근 macrolide 계열의 항생제인 azithromycin을 이용하여 이런 부작용을 억제시킨다는 임상 보고가 있어서, 이를 실험적으로 확인하고자 하였다. 이를 위해 CsA를 투여한 적이 없는 환자에서 정상 치은조직을 채취, 치은섬유아세포를 배양하였다. 우선 CsA에 대한 치은섬유아세포의 반응을 보기 위해 다양한 농도($10^{-8}-10^{-10}$g/ml)로 처치하여, 세포 증식량과 교원질 합성량을 MTT assay와 Sirol Collagen Assay를 이용하여 측정하였다. 이에 반응을 보인 조건과 세포를 대상으로 다양한 농도($10^{-8}-10^{-10}$g/ml)의 azithromycin을 CsA와 동시 처치하여 아래와 같은 결과를 얻었다. 1. CsA는 일부 치은섬유모세포의 증식을 증가시켰다. 그러나 Collagen 합성능에는 변화를 주지 않았다. 2. Azithromycin은 정상 치은섬유아세포의 증식능에 영향을 미치지 않았다. 3. Azithromycin은 CsA 에 반응을 보인 세포의 증식을 감소시켰으며, 이는 정상 수준과 유사하였다. 이상의 결과에서 azithromycin이 CsA에 의한 치은과증식 치료에 유익하다고 사료된다.

  • Acute lobar nephronia is a focal of acute bacterial nephritis, affecting one or more of the renal lobules. Nephronia is being diagnosed more frequently in accordance with the advancing imaging techniques of urinary tract. We report a case of acute nephronia in a 4 month old boy who presented persisting urinary tract infection after intial antibiotic treatment. Ultrasonography and computerized tomography showed round wedge-shaped, non-enhancing mass in right kidney. Gallium scan also revealed the lesion in the right kidney. This lesion resolved completely on one month follow up examinations. (J. Korean Soc Pediatr Nephrol 5 : 73- 7, 2001)

  • Retropharyngeal abscess and descending necrotizing mediastinitis is a potentially life-threatening condition that rarely develops following trauma to the oropharynx in children. We describe a case of a 17-month-old girl with a retropharyngeal abscess that extended to the posterior mediastinum, producing an acute descending necrotizing mediastinitis and pyopneumothorax. Culture of blood and pleural pus yielded Streptococcus pyogenes. The patient underwent repeated drainage and debridement, was treated with antibiotics and recovered. This report aims to review the retropharyngeal abscess with descending necrotizing mediastinitis in children and to highlight the fact that minor pharyngeal trauma, although not significant at first, should be observed with suspicion for serious potential complications.

  • An 1.28 kg, male, thirteen years of age, red-eared slider (Trachemys scripta elegans) was presented with one month history of anorexia, decreased mobility, and swelling and erythema of forelimbs. Hyperuricemia and tophaceous gout such as osteolysis at the digit, carpal and metacarpal bones with radiopaque densities around the lesion were detected. Allopurinol (20 mg/kg, PO, once a day) and u/d (Hill's diet) were selected for treatment and other antibiotics or anti-inflammatory drugs were not administered. One month after initial presentation, clinical signs and radiographic findings were improved. According to the medical response, the turtle was presumptively diagnosed as articular gout and allopurinol revealed effective response to the articular gout in turtles.

  • The purpose of the present study was to screen the effective of lactic acid bacteria (LAB), as probiotics which are able to protect bacterial fish diseases and investigate their characteristics. Twenty strains of lactic acid bacteria were isolated from fish intestine. fermented fish foods and kimchis. These bacteria were screened for antagonistic activity against fish pathogenic bacteria. Seven tested LAB strains were able to inhibit the fish pathogenic bacteria, including Vibrio anguillarum, Edwardsiella tarda and Streptococcus sp. Of the probiotic candidates, BK19 strain which from fermented pollack viscera indicated the largest inhibition activity. This particular probiotic bacteria was identified and named as Lactobacillus sakei BK19. In the scanning electron microscope observation, L. sakei BK19 supernatant treated V.anguillarum cell wall had been destroyed incubate after 3 hr.

  • Lemierre syndrome is caused by an acute oropharyngeal infection with secondary septic thrombophlebitis of the internal jugular vein and frequent metastatic infections. The usual etiologic agent is Fusobacterium necrophorum. Lemierre syndrome was a common disease with a high mortality rate in the pre-antibiotic era. Since the advent of antibiotics and their widespread use for the treatment of pharyngeal infections, there has been a substantial decrease in the incidence of this malady and it has become a "forgotten disease". Prompt diagnosis and antibiotic therapy for lemierre syndrome is essential to avoid morbidity and mortality. We describe here a case of Lemierre syndrome with multiple septic pulmonary emboli.

  • A 42-year-old male was admitted because of an anterior neck injury due to accidental firing of a nail gun. On chest X-ray, the nail was stuck in the anterior neck, migrated to the chest, and then to the abdomen. Only the trachea was damaged, leaving no injury in the esophagus. The nail in the intestine was removed by colonoscopy. The patient showed complete recovery without complications after fasting and conservative treatment. We report this case with a literature review.

Antimicrobial Activity of Oleanolic Acid for Foodborne Bacteria (식중독 세균에 대한 Oleanolic Acid의 항균활성)

  • Oleanolic acid and its derivatives are pentacyclic triterpene acids, which are produced in many plants and herbs. These are considered safe and thus, oleanolic acid is now used for cosmetic and pharmaceutical industry. Oleanolic acid affects peptidoglycan in cell wall of bacteria. Hence, the antimicrobial activity of oleanolic acid is not very obvious to Gram-negative bacteria such as Escherichia coli, Yersinia enterocolitica, Shigella flexneri, and Shigella sonnei because the peptidoglycan is covered with outer membrane. However, oleanolic acid derivatives showed improved antimicrobial activity to Gram-negative bacteria. For Gram-positive bacteria such as Staphylococcus aureus and Listeria monocytogenes, oleanolic acid was very effective on reducing the cell counts of the pathogens. In addition, the cytotoxicity of oleanolic acid for human cell lines was minimal. Therefore, oleanolic acid should be considered as an antimicrobial food additive and a therapeutic agent to control foodborne pathogens.
