Perianal Fistula: An Overview (항문주위 샛길의 개요)

  • Perianal fistula is a common inflammatory condition in the general population and affects the area around the anal canal. Although most cases are benign, they cause serious morbidity and require surgical treatment due to a high risk of recurrence. MR imaging is a gold standard technique for the evaluation of perianal fistulas and provides accurate information on the anatomy of the anal canal, its relationship to the anal sphincter complex, accurate identification of secondary tracts or abscesses, and reporting associated complications. MR imaging can help monitor treatment effects and determine treatment methods. Crohn's disease-related fistulas often require medical rather than surgical treatment. The radiologist is required to know the anatomy and MR imaging findings of the perianal fistula to present an accurate diagnosis to the clinician.

A Set of Monozygotic Female Twins With Anal Canal Duplication (일란성 쌍둥이에서 발생한 항문관 중복증)

  • Anal canal duplications occurring in a pair of 4 month-old healthy female twins are presented. The openings were located in the posterior midline of the anus since birth without a history of perianal abscess or swelling. Excision of the duplicated anal canals was performed using posterior sagittal approach. Although the anal canal duplication occurs predominantly in female, to our knowledge, this is the first case of anal duplication in a monozygotic female twins reported.

Anal Endosonographic Features of the Normal Anus in Children (소아에서 항문 내초음파 검사상 정상 항문관의 형태학적 소견)

  • Anal endosonography has recently been popularized in adult patients in order to access staging of rectal cancer and other lesions of the anorectum. This study is not familiar to pediatric surgeons. We performed anal endosonography in 30 children without anorectal disease in order to determine the standard morphology of the anorectum. The internal anal sphincter(IAS) was clearly identified as a homogeneous hypoechoic circular band, extending caudally to a level just proximal to the anal verge. The external anal sphincter(EAS) showed mixed echogenicity and different architecture along the anal canal; the EAS was U-shape in the upper canal and it had a circular pattern in the lower canal. In the upper canal of girls, the perineal body and the vagina were found just anterior of the anal canal, which made the U shape of the EAS. In the male, the sphincter tapered anteriorly into two arcs that met in the midline. The perineal body was prominent in the female. Posteriorly, the anococcygeal ligament was represented by a triangular shadow in both sex. The thickness of the IAS was measured in 3 directions, left, right and posterior, at 3 levels, upper, middle and lower areas of the anal canal. The average thickness was 0.86-2.40 mm between 6 and 18 months of age(Group 1), 0.88-3.20 mm between 19 and 36(Group 2), 1.07-2.20 mm between 37 and 54(Group 3) and 1.18-2.42 mm more than 54-month-old(Group 4). The thickness was correlated with the age of the children only in the right upper(p=0.008) and the left middle portion(p=0.015). We were able to obtain standard morphologic features of the normal anal canal in children with anal endosonography. We believe that this technique is a safe and an effective procedure to evaluate anorectal lesions in children.

Diagnostic Performance of Rectal CT for Staging Rectal Cancer: Comparison with Rectal MRI and Histopathology (직장암 병기결정에서 직장 CT의 진단능: 직장 MRI 및 병리결과와의 비교분석)

  • Purpose To compare the diagnostic performance of rectal CT with that of high-resolution rectal MRI and histopathology in assessing rectal cancer. Materials and Methods Sixty-seven patients with rectal cancer who underwent rectal CT with rectal distension using sonographic gel and high-resolution MRI were enrolled in this study. The distance from the anal verge/anorectal junction, distance to the mesorectal fascia (MRF), extramural depth (EMD), extramesorectal lymph node (LN) involvement, extramural venous invasion (EMVI), and T/N stages in rectal CT/MRI were analyzed by two gastrointestinal radiologists. The CT findings of 20 patients who underwent radical surgery without concurrent chemoradiotherapy were compared using histopathology. Interclass correlations and kappa statistics were used. Results The distance from the anal verge/anorectal junction showed an excellent intraclass correlation between CT and MRI for both reviewers. For EMD, the distance to the MRF, presence of LNs, extramesorectal LN metastasis, EMVI, T stage, and intermodality kappa or weighted kappa values between CT and MRI showed excellent agreement. Among the 20 patients who underwent radical surgery, T staging, circumferential resection margin involvement, EMVI, and LN metastasis on rectal CT showed acceptable concordance rates with histopathology. Conclusion Dedicated rectal CT may be on par with rectal MRI in providing critical information to patients with rectal cancer.

First Record of the Oilfish, Ruvettus pretiosus (Perciformes: Gempylidae) from Korea (한국산 갈치꼬치과 어류 1미기록종, Ruvettus pretiosus)

  • Two specimens of Ruvettus pretiosus (227.0 and 238.3 mm in standard length), belonging to the family Gempylidae, was firstly collected by using a gill net from the eastern coastal waters of Jejudo Island, Korea in November 2013. This species is characterized by having the following morphological traits: XV, 17~19+2 dorsal fin rays; II, 15~16+2 anal fin rays; belly keeled between pelvic fin and anus; body covered with cycloid scales, interspaced with rows of sharp bony tubercles. R. pretiosus can be easily distinguished from three Korean Snake mackerels (Thyrsitoides marleyi, Rexea prometheoides and Gempylus serpens) by having belly keeled between pelvic fin and anus, and cycloid scales covered the body and interspaced with rows of sharp bony tubercles. Based on morphological and molecular approaches, the specimens were identified as R. pretiosus. We propose new Korean names, "Gi-reum-chi-sok" and "Gi-reum-chi" for the genus and species, respectively.

Structure on the Ventral Process and the Ovipositor of a Korean Oily Bitterling, Acheilognathus koreensis (Pisces, Cyprinidae), in Relation to Spawning Season (산란시기의 칼납자루 Acheilognathus koreensis (잉어과)의 배측융기와 산란관의 구조)

  • During spawning season of female in a Korean oily bitterling, Acheilognathus koreensis, the ventral region near the base of the pectoral fin becomes to be protruded outward of body and enlarged. This ventral process consists of both organs as rectum (vent) and inner ovipositor. The rectum consists of mucosa, lamina propria-submucosa, muscularis, and squamous epithelial layer (peritoneum=serosa) surrounding them. The mucosa contains numerous mucous cells meaning acid mucopolysaccharides in nature. The inner ovipositor is similar to that of the rectum, but the mucosa have no mucous cell, unlike that of the rectum. Whereas, the outer ovipositor has a straight and long tube which are not connected with the ventral process any more. The outer ovipositor was similar to the structure of the inner ovipositor in the ventral process. However, the outer ovipositor has no muscularis, and consists of three layers: mucosa, lamina propria-submucoa, and squamous epithelia. The outer ovipositor without the muscularis seems serves as a tube that eggs discharged from the outer ovipositor allow to send inside mussel, unlike that of the inner ovipositor performing rhythmic contractions of the layers of the muscularies for propelling to the matured oocytes to the outer ovipositor.

Analysis of Neonatal Gastrointestinal Diseases in a Neonatal Intensive Care Unit for 3 Years Neonatal GI Diseases in a NICU for 3 Years (3년간 단일기관 신생아실의 위장관 질환의 분석)

  • Purpose: To report our experience of gastrointestinal operations performed in neonates including low birth weight infants and to evaluate their clinical characteristics. Methods: We retrospectively reviewed the medical records of patients who underwent neonatal gastrointestinal surgery or had necrotizing enterocolitis (NEC) or inguinal hernia from January 2008 to December 2010 at Pusan National University School of Medicine. Results: The main disease was anorectal malformation and male patients were dominant. Twenty four patients (19.2%) had one or more associated anomalies including hydronephrosis and congenital heart disease. Eighteen patients (43.9%) of anorectal malformation had other anomalies. Seventy six percent of NEC cases were very low birth weight infants. Concerning mean days of full enteral feeding after operation, NEC patients needed 30.8 days, which was the longest period. Overall mortality of operation (except NEC and inguinal hernia operation) was 1.6%. The mortality of NEC with surgical treatment was 18.8%. The direct bilirubin in the operation group was significant higher than in the non-operation group in NEC patients. Conclusion: The morbidity and mortality after neonatal gastrointestinal surgery were excellent. The direct bilirubin in the operation group was significant higher than in the non-operation group in NEC.

Study of Radiological Approach to Treat a Chronic Constipation (만성변비환자의 방사선학적 접근방법에 관한 고찰)

  • I. Introduction : The constipation is one of the common gastrointestinal symptoms seen in the clinical practice. When people come to the clinic complaining of the constipation, it is generally one of the cases of infrequent, painful or difficult evacuation as well as the hardened feces and unsatisfactory evacuation sense. Since the constipation is heavily influenced by dietary habit as well as the social and medical environment, the diagnostic radiology is useful to establish the objective and standardized definition in consideration of those various factors before diagnosis and treatment of the constipation patient. This paper describes the study of such diagnosis. II. Main Subject : Testing of CTT (colon transit time) is key study of the colon performance. CTT is very helpful in classifying the pathologic physiological types and defining the treatment plan for the chronic constipation. The study methods include using the radipaque marker, multiple marker technique and scintigraphic measurement. The defecography is the functional radiologic examination a that provides not only the anatomical information of anorectal but also performance of the pelvic floor and rectal change during evacuation. Study of dynamic movement of the anorectal during evacuation is helpful for diagnosis and treatment planning as well as follow-up testing for the constipation patient. One of the issues essential for the case history is the thorough observation of whether the patient shows the psychic psychological symptoms such as the behavior disorder or emotional disturbance. In that case, the decision must be made whether or which type of medication is needed for such psychiatric problem. III. Conclusion : The main causes of the constipation are insufficient intake of fiber or liquid. The key objective of such tests is to check etiology of the constipation. In general, the radiological examination does not provde the colon or anorectal performance information. It is envisaged that this study will provide the information to decide the testing and treatment plans and predict the prognosis of the patient by classifying the pathologic physiological types.

Korean Medical Treatment’s Report about a Spontaneous Pregnancy with Low Anti-Müllerian Hormone (AMH) Level and Thin Endometrium Infertility after Anorectal Cancer Surgery (항문암 수술 후 혈중 항뮬러관 호르몬 저하 및 자궁내막이 얇은 난임 환자의 한약치료 자연임신 1례)

  • Objectives: This paper is to report a spontaneous pregnancy with low AMH level and thin endometrium infertility after Korean medical treatments.Methods: A 38-year old patient who had low AMH level, thin endometrium, sleep disturbance and anxiety happened after anorectal cancer surgery was treated by korean medicine such as herb medications per day for 6 months. And we observed the effects of treatments by improvement of symptoms checking endometrium ultrasonography.Results: After treatments, Symptoms of anxiety and sleep disturbance were improved. The number of ovarian follicle and endometrium thickness were upgraded during artificial pregnancy trials. Finally the patient was a spontaneous pregnancy after failures of one intrauterine insemination and one in vitro fertilization trial.Conclusions: This case shows that the korean medicine has effective option for a patient with low AMH level and thin endometrium infertility. Further ultrasonography would be valid measurements to check the effectiveness of oriental treatments as well as symptoms.

Intestinal obstruction caused by a duplication cyst of the cecum in a neonate (신생아에서 맹장의 장 중복낭종에 의해 발생한 장 폐쇄 1예)

  • Duplication cysts are rare congenital malformations, that may be detected anywhere along the alimentary tract, and they may communicate with the intestinal tract. Cystic duplication of the cecum is especially rare. About 80% of these cases are detected in the first 2 years of life as a result of an acute intestinal obstruction, which manifests as vomiting, recurrent abdominal pain, recurrent gastrointestinal bleeding and constipation. We report a case of intestinal obstruction secondary to a duplication cyst of the cecum in a neonate. The patient underwent surgery and was diagnosed subsequently, and is presently healthy.
