SNOMED CT를 활용한 한의약 개념 매핑 : 한의임상진료지침에서 도출된 편두통 관련 개념의 의미론적 표현 (Use of SNOMED CT to Represent Traditional Korean Medicine Concepts : A Semantic Characterization of Migraine-Related Concepts from Korean Medicine Clinical Practice Guideline)

  • 목적 : 본 연구는 한의약에서 사용하는 용어가 SNOMED CT로 매핑 가능한지 여부를 조사하고, 한의약 용어를 표현하기 위해 기존 SNOMED CT 온톨로지를 개선할 수 있는 방안을 제안하는 것을 목표로 하였다. 방법 : 선행 연구의 매핑 가이드라인에서 제시된 7단계 과정을 수정하여 활용하였다. 매핑의 목적 및 범위 정의, 용어 추출, 개념 추출, 매핑을 위한 소스 용어 작업, SNOMED CT 개념 검색, 매핑 관계 분류 및 매핑 검증의 과정을 수행하였다. 매핑의 목적은 한의약 임상 아이디어를 표현하는 표준 용어로서 SNOMED CT를 평가하는 것이고, 범위에는 편두통 환자 관리의 평가, 진단, 치료 및 예방을 포함하였다. 결과 : 총 546개의 용어가 추출되었다. 중복된 용어를 제거한 후, 271개의 개념이 SNOMED CT 매핑에 사용되었다. 이중 43.2%는 SNOMED CT 개념과 의미론적으로 동등하게 매핑되었고(117개 개념), 39.1%는 SNOMED CT 개념이 더 포괄적인 의미를 가지도록 매핑되었다(106개 개념). 상대적으로 포괄적인 의미를 가지는 SNOMED CT 개념에 매핑된 한의약 개념 106개 중 19개는 SNOMED CT 후조합을 이용하여 의미론적으로 동등하게 표현이 가능하였다. 나머지 17.7%의 한의약 개념은 SNOMED CT에 매핑할 수 없었다. 결론 : 본 연구는 한의약에서 사용되는 개념을 SNOMED CT에 매핑하여 한의약 용어를 표준화하였다. 연구 결과를 바탕으로, 한의약에서 사용되는 용어를 표준의료용어로 표현하기 위하여 SNOMED CT에 새로운 개념과 속성을 추가하는 것을 제안한다.

일개 광역시민의 지역사회 한의약 건강증진 강좌에 대한 수요 (Needs for Community Health Promotion Courses of the Traditional Korean Medicine among One Metropolitan Citizens)

  • 이 연구는 일개 광역시민 500명을 대상으로 한의약 건강증진 강좌 개설을 위한 기초자료를 마련하고자 수행되었다. 조사 도구로는 대상자의 일반적 특성, 한의약 치료에 대한 경험 및 인식, 한의약 건강증진 강좌에 대한 수요조사 문항으로 구성된 설문지를 이용하였다. 조사 결과 지난 1년간 응답자의 56.8%가 한의약 치료를 이용한 경험이 있었고, 이들 중 74.6%가 한의원을 이용하였다. 질환별로는 근골격계 질환(48.2%), 치료방법으로는 침치료(34.8%)의 이용률이 가장 높았다. 한의약 치료의 효과가 좋다는 응답률은 72.3%였으며, 응답자의 90.9%가 다른 사람에게 한의약 치료를 '추천할 의향이 있다'고 하였다. 응답자의 81.2%가 한의약 건강증진 강좌에 참여할 의사를 보였으며, 강좌 시행에 적합한 장소로는 의료기관(22.5%)을 가장 많이 선택하였다. 강좌 참여 결정에 가장 중요한 사항으로 '강좌의 주제 및 내용'(59.1%)을 수강 희망 주제 질환으로는 순환기질환(23.7%)을 우선으로 선택하였으며, 강좌를 통해 기대하는 사항으로는 '질환에 대한 자가 건강관리 방법'(69.0%)을 가장 많이 고려하였다. 이 연구의 결과는 대상자의 요구를 보다 충족시킬 수 있는 한의약 건강증진 강좌 구성에 도움이 될 것으로 기대된다.

족냉과 수면의 질 : 수면다원검사와 피츠버그 수면의 질 지수를 이용한 탐색적 연구 (Cold Feet and Sleep Quality : An Exploratory Study Using Polysomnography and Pittsburgh Sleep Quality Index)

  • Objectives : This study aimed to investigate the relationship between cold feet and sleep quality using polysomnography (PSG) and Pittsburgh Sleep Quality Index (PSQI). Methods : We divided 11 adults (6 females, 5 males) with Insomnia Severity Index score below 21 into cold feet (CF) and a non-cold feet (NCF) group based on the median feet temperature (Taichong, LR3). PSG and PSQI were administered to assess sleep characteristics and subjective sleep quality. Results : CF group exhibited significantly lower time in bed, sleep period time, and total sleep time compared to NCF group. While there were no significant group differences in sleep latency, wakefulness after sleep onset, or total arousal index, NCF group had significantly lower minimum oxygen saturation and apnea-hypopnea index in REM (rapid eye movement) sleep compared to CF group. Although the PSQI score and the proportion of poor sleepers were both higher in the CF group (7.40 and 80%) compared to the NCF group (5.50 and 50%), these differences did not reach statistical significance. Conclusions : This study showed that foot temperature affects sleep characteristics and suggests the need to utilize PSG in sleep research in Korean medicine.

중년 여성에서 신체의 냉/열감과 수면의 질 및 불면증의 연관성 분석 (Association of Cold/Heat Sensation with Sleep Quality and Insomnia in Middle-aged Women)

  • Objectives: Cold extremities have been suggested to correlate with sleep disturbances. This study aims to explore the relationship between thermal sensations in body, encompassing both cold and heat sensations, with sleep quality and insomnia. Methods: Self-administered questionnaires were utilized to assess thermal sensations in body, sleep quality and symptoms of insomnia in middle-aged women. A multiple logistic regression analysis was performed to ascertain the association between thermal sensations in body and both sleep quality and insomnia symptoms. Results: Among 899 participants, 255 (28.4%) were categorized in the cold sensation group, 95 (10.6%) in the heat sensation group, 70 (7.8%) in the group with both cold and heat sensations, and 479 (53.3%) in the no-sensation group. Pittsburgh Sleep Quality Index and Insomnia Severity Index were notably higher in the group experiencing both sensations when compared to the no-sensation group. After adjustments for covariates, the odds ratios for poor sleep quality, moderate/severe insomnia, and long sleep latency were significantly elevated in the group with both sensations when compared to the no-sensation group. The odds ratios for poor sleep quality in the cold sensation group and for moderate/severe insomnia and low sleep efficiency in the heat sensation group were significantly higher when compared to the no-sensation group. Conclusions: The risk for sleep disturbances varied depending on the presence of thermal sensations in body, with the greatest risk observed for low sleep quality and insomnia in individuals experiencing both cold and heat sensations.

  • 올바른 결핵치료를 계몽하기 위하여 만든 홍보전시판 중의 하나가 문제가 되어 전국의 한의사와 한약종상 1,400여명이 삼일당에 모여 한의약모독성토대회를 열고 결핵협회의 공개사과와 관계책임자의 파면을 요구하며 떼어낸 전시판을 앞세우고 가두시위에 나섰다. 결핵협회의 좀 세련되지 못한 표현과 한의약계의 과잉반응이 빚은 1960년대 초의 시대풍경 한토막.

머신러닝 기반 음성분석을 통한 체질량지수 분류 예측 - 한국 성인을 중심으로 (Application of Machine Learning on Voice Signals to Classify Body Mass Index - Based on Korean Adults in the Korean Medicine Data Center)

  • Objectives The purpose of this study was to check whether the classification of the individual's Body Mass Index (BMI) could be predicted by analyzing the voice data constructed at the Korean medicine data center (KDC) using machine learning. Methods In this study, we proposed a convolutional neural network (CNN)-based BMI classification model. The subjects of this study were Korean adults who had completed voice recording and BMI measurement in 2006-2015 among the data established at the Korean Medicine Data Center. Among them, 2,825 data were used for training to build the model, and 566 data were used to assess the performance of the model. As an input feature of CNN, Mel-frequency cepstral coefficient (MFCC) extracted from vowel utterances was used. A model was constructed to predict a total of four groups according to gender and BMI criteria: overweight male, normal male, overweight female, and normal female. Results & Conclusions Performance evaluation was conducted using F1-score and Accuracy. As a result of the prediction for four groups, The average accuracy was 0.6016, and the average F1-score was 0.5922. Although it showed good performance in gender discrimination, it is judged that performance improvement through follow-up studies is necessary for distinguishing BMI within gender. As research on deep learning is active, performance improvement is expected through future research.

콘볼루션 신경망 기반의 안면영상을 이용한 사상체질 분류 (Sasang Constitution Classification using Convolutional Neural Network on Facial Images)

  • Objectives Sasang constitutional medicine is a traditional Korean medicine that classifies humans into four constitutions in consideration of individual differences in physical, psychological, and physiological characteristics. In this paper, we proposed a method to classify Taeeum person (TE) and Non-Taeeum person (NTE), Soeum person (SE) and Non-Soeum person (NSE), and Soyang person (ST) and Non-Soyang person (NSY) using a convolutional neural network with only facial images. Methods Based on the convolutional neural network VGG16 architecture, transfer learning is carried out on the facial images of 3738 subjects to classify TE and NTE, SE and NSE, and SY and NSY. Data augmentation techniques are used to increase classification performance. Results The classification performance of TE and NTE, SE and NSE, and SY and NSY was 77.24%, 85.17%, and 80.18% by F1 score and 80.02%, 85.96%, and 72.76% by Precision-Recall AUC (Area Under the receiver operating characteristic Curve) respectively. Conclusions It was found that Soeum person had the most heterogeneous facial features as it had the best classification performance compared to the rest of the constitution, followed by Taeeum person and Soyang person. The experimental results showed that there is a possibility to classify constitutions only with facial images. The performance is expected to increase with additional data such as BMI or personality questionnaire.

일반 인구집단의 사상체질에 따른 수면의 질 및 신체활동량과 복부비만과의 연관성 (Association between Sleep Quality, Physical Activity and Abdominal Obesity in the Community-Based Population: Based on Sasang Constitution Types)

  • Background Abdominal obesity, a major public health concern, is related to many health problems. In addition, it is influenced by individual characteristics. We investigated sleep quality and physical activity (PA) as risk factors for abdominal obesity, according to the Sasang constitutional medicine. Methods In this cross-sectional study, we analyzed data from 5,221 community-based participants. Sleep quality and PA were measured using structured questionnaires, and abdominal obesity was classified according to waist circumference. Sasang constitution (SC) was classified as Taeeumin (TE), Soeumin (SE), or Soyangin (SY) type, using an established SC questionnaire. Chi-square test and logistic regression analysis were performed to access the association of sleep quality and PA with abdominal obesity in individuals stratified according to the SC types. Results The percentage of poor sleep quality and inactive PA was the highest in the SE type, and the higher prevalence of abdominal obesity was found in the TE type. After adjusting for variables, inactive PA was associated with abdominal obesity in the TE type (OR=1.694, 95% CI=1.42-2.021), and in the SE type, abdominal obesity was associated with poor sleep quality (OR=1.688, 95% CI=1.091-2.611) and low PA (OR=2.127, 95% CI=1.163-3.89). Moreover, the combination of these two factors were also significantly associated with abdominal obesity in the TE and SE types. Conclusion Abdominal obesity was associated with sleep quality and PA, and these results were different in each SC type. Taking various associated lifestyles and individual characteristics in consideration may contribute to better management of abdominal obesity in clinical practice.

사상체질에 따른 두통 특성 조사 (The Research of Headache Characteristics According to Sasang Constitution)

  • Objectives The study aimed to investigate the prevalence and characteristics of headaches according to Sasang constitution among 5,764 participants residing in Ansung or Ansan in Korea. Methods The Korea Sasang Constitutional Diagnostic Questionnaire was used to classify the constitution, and a self-administered questionnaire was used to collect headache data. The presence of headache, locations and patterns between groups were analyzed using the chi-square test and logistic regression adjusted for age, marital status, education, alcohol consumption, and smoking. Results Results showed that 25.6% of participants had headaches, with a significantly higher prevalence in Soeumin (30.8%) compared to Soyangin (24.9%) and Taeeumin (24.3%). Soeumin had significantly higher odds ratios for headaches than Taeeumin in both crude (1.387) and adjusted (1.319) models. Headache sites showed high frequency in the order of occipital (8.1%), temporal (8.0%), forehead (5.4%), and global regions (5.4%), and Soeumin showed significantly higher odds ratios of forehead (crude: 1.616, adjusted: 1.543) and occipital pain (crude: 1.366 adjusted: 1.379) compared to Taeeumin. No significant differences in headache patterns were observed among the groups. Conclusions This study suggests that Soeumin has a higher prevalence of headaches, particularly in forehead and occipital regions, compared to other Sasang constitutions.
