Trend of Unmet Medical Need and Related Factors Using Panel Data (패널 자료를 이용한 미충족 의료의 추세와 관련요인)

  • The purpose of this study was to investigate the current status of unmet medical need using data from the Korea Health Panel study from 2009 to 2013 (excluding 2010), and to analyze the trends of unmet medical need and related factors. The subjects of this study were 11,598 in 2009, 11,035 in 2011, 10,584 in 2012, 10,099 in 2013, and 7,144 people in panel data, and conducted frequency analysis, chi-square test and generalized estimating equation. As a result of the analysis by year, it was found that women, under middle school graduation, medical aid, the lowest household income and low subjective health status experienced more unmet medical need. As a result of analysis using generalized estimating equation, women, under 40 years of age, under elementary school graduation, lowest quartile household income, subjective health status of less than 20 points, and activity restrictions are more likely to experience unmet medical need. Based on these results, we intend to provide basic data for establishing policies on the use of medical services.

Analysis of medical panel binary data using marginalized models (주변화 모형을 이용한 의료 패널 이진 데이터 분석)

  • Longitudinal data are measured repeatedly over time from the same subject, so there is a correlation from the repeated outcomes. Therefore, when analyzing this correlation, both serial correlation and between-subject variation must be considered in longitudinal data analysis. In this paper, we will focus on the marginalized models to estimate the population average effect of covariates among models for analyzing longitudinal binary data. Marginalized models for longitudinal binary data include marginalized random effects models, marginalized transition models, and marginalized transition random effect models, and in this paper, these models are first reviewed, and simulations are conducted using complete data and missing data to compare the performance of the models. When there were missing values in the data, there is a difference in performance depending on the model in which the data was generated. We analyze Korea Health Panel data using marginalized models. The Korean Medical Panel data considers subjective unhealthy responses as response variables as binary variables, compares models with several explanatory variables, and presents the most suitable model.

Gender Differences in Factors Affecting Purchase of Indemnity Private Health Insurance and Impact of Indemnity Private Health Insurance on Healthcare Use: Korea Health Panel Survey Data from 2010 to 2016 (실손형 민간의료보험 가입의 영향요인과 실손형 민간의료보험이 의료이용에 미치는 영향에서 성별 차이: 한국의료패널(2010-2016))

  • This study was aimed to identify factors affecting the purchase of indemnity private health insurance and analyze its effect on healthcare utilization by gender. This study analyzed data of the 2010-2016 Korea Health Panel Survey using panel logit model and panel regression model. Men were less likely to purchase indemnity private health insurance than women. Self-rated health and unmet need for healthcare in women were related to insurance purchases. In men, married people and residents of the metropolitan area were more likely to purchase. The indemnity private health insurance did not affect the women's outpatient expenses per visit, but for men it increased. The indemnity private health insurance significantly increased women's inpatient expenses per admission, but it did not affect men's inpatient expenses. This study confirmed gender differences in the factors affecting the purchase of indemnity health insurance and the impact of indemnity health insurance on healthcare utilization. This suggests that gender differences should be taken into account in private health insurance research and institutional operation.

Medical Costs between Dietary Supplement Users and Non-users Using the Korea Health Panel Data (한국의료패널 자료를 활용한 건강기능식품 섭취에 따른 의료비 지출 비교분석)

  • Background: In recent years, studies have shown conflicting results regarding the benefits of dietary supplements in reducing healthcare expenditures. This study aimed to address this inconsistency by examining the association between supplement consumption and health expenditures using nationally representative data from the Korea Health Panel Survey (2019-2020). Methods: A 1:1 matched case-control dataset was established using propensity score matching technique based on supplement consumption. Then, total annual healthcare expenditures were compared between the two groups. In addition, a multivariate regression analysis (Proc Surveyreg) was performed to determine the association between the supplement consumption and medical costs. Results: The supplement user group spent about 1.72 million Korean won, while the non-user group spent about 1.43 million Korean won on medical services (p=0.0186). The results of multivariate regression showed that the costs were approximately 26.15% higher in the user group than in the non-user group (p=0.0004). Conclusion: Contrary to the previous studies that have shown the benefits of supplement use in reducing healthcare costs, this study showed that those who consistently consumed supplements spent more on medical services. This can be interpreted in the same context as previous studies suggesting that dietary supplement intake is a healthy behavior for managing one's health. However, we caution against drawing firm conclusions due to data limitations. Further analysis using patient-level epidemiologic data is needed.

Analysis of Unmet Healthcare Needs and Risk Factors to Improve the Life Care of Osteoporosis Patients (골다공증 환자의 라이프 케어 증진을 위한 미충족 의료실태와 위험요인 분석)

  • Purpose: This study is a descriptive and secondary analytical study that uses panel data to analysis of unmet healthcare needs and risk factors for improving life care of osteoporosis patients. Methods: The subjects of this study were 941 patients who were diagnosed with osteoporosis using Korea Medical Panel 2015 data(β-version 1.0). Data analysis was performed using Chi-Square and logistic regression using SPSS/win 22.0. Results: The unmet healthcare needs of osteoporosis patients were 22.6%. The factors of unmet healthcare needs were education level and age in Model I of demographic factors, and eating problems, memory problems, activity limitation, and disability in Model II. In Model III, which added socio-psychological factors, eating problems, memory problems, Total family income, and pain/Discomfort were identified. Conclusion: Based on the results of this study, it should be considered in the planning of medical policies to improve the life care of osteoporosis patients, and it is necessary to improve access to medical services and to prevent and mediate realistically to reduce unmet healthcare needs.

Long-and Short-term Relationship between Human Resources Expenditure and Management Performance in South Korea -focusing on Public District General Hospitals- (인적자원관리비용과 경영성과에 대한 장·단기 효과분석)

  • Purposes: 우리나라 공공의료기관들의 경영실적 보고에 따르면 대부분의 공공의료기관들이 만성적자로 인한 경영난 문제를 겪고 있는 실정이다. 이는 공공의료서비스 제공 등이 주요 원인이겠지만 그럼에도 공공의료기관들의 경영효율화 문제는 지속적으로 지적되고 있음은 공공연한 사실이다. 이에 본 연구는 고정비 성격이 강한 인적자원관리비용과 경영성과 간의 관계를 분석하여 공공의료기관의 경영성과 개선을 위한 경영적 함의를 제공하고자 하였다. Methodology: 분석자료는 2014년부터 2019년까지 39개 지역거점 공공병원의 재무자료를 이용하였고, STATA SE 12.0을 사용하여 다중회귀분석과 패널회귀분석을 실시하였다. Findings: 다중회귀분석을 통한 단면연구 결과에서는 인건비가 높을수록 경영성과가 높은 경향을(2014년 의료수익:1.523/ 2015년 의료수익: 2.002, 의료수익의료이익률: 0.002/ 2016년 의료수익: 1.223) 보였으나 패널회귀분석을 통한 종단연구 결과에서는 복리후생비가 높을수록 경영성과가 의미있게 높은 경향을(의료수익: 3.232582/ 의료수익의료이익률: 0.0014502) 보였다. 이는 인건비는 경영성과에 단기적인 효과가 있는 반면 복리후생비는 경영성과에 장기적인 효과가 있음을 반영하는 것이라고 볼 수 있다. Practical Implication: 본 연구는 이러한 결과를 기반으로 장기적인 효과가 있는 복리후생비의 중요성을 제시하고, 공공의료기관의 경영성과에 영향을 미치는 인건비와 복리후생비의 특성 파악을 통하여 효과적인 인적자원관리방법을 위한 경영적 함의를 도출하였다는 점에서 의의가 있겠다.

Trend of Medical Care Utilization in Glaucoma Patients over 5 Years(2008-2012) -Korea Health Panel Data- (녹내장 환자의 의료이용에 대한 5년간(2008-2012)의 변화추이 -한국의료패널 자료를 활용하여-)

  • The purpose of this study is to analyze outpatients of glaucoma aged 20 or older about their five-year use of ophthalmology and investigate trend of prevalence in glaucoma patients. The Korea Health Panel (KHP) data from 2008 to 2012 were analyzed using frequency and chi-square tests. The outpatients of glaucoma have increased significantly from 0.43% in 2008 to 0.47% in 2009, 0.49% in 2010, 0.61% in 2011, and 0.75% in 2012 (p<0.001). The outpatients of glaucoma are in the 20's to 70's, and higher utility rate is shown in those who are older (p<0.001), and the rate of use tends to increase each year in all the age groups (p<0.001). The medical care utilization attributed to glaucoma in Korea tends to increase lately; therefore, it is a crucial task to realize early detection and management according to the characteristics of different age groups.

Factors influencing quality of health care: Based on the Korea health panel data (한국의료패널 자료를 활용한 의료서비스 질 영향 요인)

  • The purpose of this study was to identify factors affecting quality of health care. Methods: The data were derived from the 2011-2013 Korea health panel survey (beta version 1.0). The data were analyzed using SPSS 21.0 with descriptive statistics, ${\chi}^2$-test, and multiple logistic regression analysis. In general characteristics, common factors influencing the quality of health care were age, marital status, education level, and subjective health status. In variables related to health care utilization, unmet healthcare needs, and limitation of dental care utilization were the significant factors affecting quality of health care. The results of this study show that various factors influence quality of health care. These findings can be used to develop strategies to improve health care.

  • The purpose of this study is to prepare basic data necessary for the development of services or systems that can enhance the accessibility of health and medical services or enhance the efficiency of health and medical use of senior citizens by identifying factors for predicting health and medical use behavior of senior citizens in Incheon Metropolitan City through the Korea health panel data(version 1.5). Through the structural equation model established through the SPSS and AMOS, it was confirmed that the predisposing factors, health behaviors and needs factors had significant direct and indirect effects on the use of health care services. Since the imbalance in demand and supply of health and medical services is more severe than in other regions, the results of this study can be used as basic data when checking whether the current health and medical system in Incheon Metropolitan City can operate effectively in an aged society and discussing how to provide health and medical services to the elderly in Incheon.

  • The purpose of this study was to identify the effect of psychological health and physical health on health-related quality of life(HRQoL) in Korean Echo Generation by using Korea Health Panel Data 2013. The Korea Health Panel Data 2013 were collected from February to October 2013 and 2,261 respondents were analyzed. The data were analyzed by Independent t-test, ANOVA and multiple regression using SPSS WIN 24.0 program. he mean score of HRQoL was 0.98. The effect of unmet medical needs, psychological & physical stress, unmet basic needs, anxiety about the future, depression, suicidal ideation, smoking, sleeping time, hearing problem, eating problem, restriction of activity, and self-rated health status) were significant on HRQoL. Health care providers should consider the effect of psychological and physical health when they design program for the improvement of HRQoL for Korean echo generation in community.
