Generation of virtual mandibular first molar teeth and accuracy analysis using deep convolutional generative adversarial network (심층 합성곱 생성적 적대 신경망을 활용한 하악 제1대구치 가상 치아 생성 및 정확도 분석)

  • Purpose: This study aimed to generate virtual mandibular left first molar teeth using deep convolutional generative adversarial networks (DCGANs) and analyze their matching accuracy with actual tooth morphology to propose a new paradigm for using medical data. Methods: Occlusal surface images of the mandibular left first molar scanned using a dental model scanner were analyzed using DCGANs. Overall, 100 training sets comprising 50 original and 50 background-removed images were created, thus generating 1,000 virtual teeth. These virtual teeth were classified based on the number of cusps and occlusal surface ratio, and subsequently, were analyzed for consistency by expert dental technicians over three rounds of examination. Statistical analysis was conducted using IBM SPSS Statistics ver. 23.0 (IBM), including intraclass correlation coefficient for intrarater reliability, one-way ANOVA, and Tukey's post-hoc analysis. Results: Virtual mandibular left first molars exhibited high consistency in the occlusal surface ratio but varied in other criteria. Moreover, consistency was the highest in the occlusal buccal lingual criteria at 91.9%, whereas discrepancies were observed most in the occusal buccal cusp criteria at 85.5%. Significant differences were observed among all groups (p<0.05). Conclusion: Based on the classification of the virtually generated left mandibular first molar according to several criteria, DCGANs can generate virtual data highly similar to real data. Thus, subsequent research in the dental field, including the development of improved neural network structures, is necessary.

FACIAL ASYMMETRY OF UNILATERAL CLEFT LIP AND PALATE PATIENTS (편측성 순구개열자의 안면비대칭에 관한 연구)

  • Craniofacial asymmetry was analyzed in 39 patients with complete unilateral cleft lip and palate(UCLP). The samples are devided into three groups nine below 9 years, twenty three from 9 years 1 month to 14 years and seven over 14 years group. Seventeen measurements were obtained from the tracing of PA X-ray cephalometric headfilms to evaluate the asymmetric characteristics and changes accdording to aging in UCLP. The obtained results were as follows. 1. Facial asymmetry in UCLP is variable(1.22-3.47 $mm/^{\circ}$) and the length from midsagitta1 reference line to maxillary 1st molar, to upper central incisor and the length of mandibular ramus showed significant asymmetry 2. Nasal septum and anterior nasal spine were deviated In the cleft side and the lower border of nasal cavity was 1ower in cleft side. 3. The deviation of nasal septum was continued significantly till after 14 years old.

A Radiographic Study on Root Resolution in the Malocclusion Patients before Orthodontic Treatment (부정교합 환자의 교정치료전 치근흡수에 관한 방사선학적 연구)

  • This study was designed to evaluate the frequency and the severity of root resorption of the permanent teeth before orthodontic treatment by means of radiograph in the malocclusion patients. In this study the author analysed the frequency and the severity of root resorption in individual teeth, the relationships of the frequency and the severity of root resorption and age, sex, Angle's classification, overjet, overbite, and maxillary and mandibular incisor inclination, and the relationships of the frequency of root resorption and the characteristics of malocclusion and marked occlusal attrition showed in individual teeth. The results were as follows. 1. All of the persons examined showed some evidence of root resorption in one or more of the permanent teeth, $35.84\%$ of the teeth examined and more frequent in female group than male group(p<0.01). 2. On the susceptibility of the root resorption in individual teeth in this study, the author found the mandibular incisors and the maxillary incisors, in the order named, to be most susceptible in all affected teeth, but maxillary central incisors, maxillary first bicuspids, and maxillary lateral incisors, in the order named, were more susceptible to marked root resolution. 3. The more proclined maxillary Incisors the more affected root resorption in four maxillary incisors and the more proclined mandibular incisors the more affected root resorption in four mandibular incisors. 4. Overbite more affected root resorption than overjet, and the higher tender to openbite the more frequent was root resorption. 5. On the characteristics of malocclusion showed in individual teeth, the openbite teeth combined with crossbite, were most frequent in root resolution.

Effects of vertical head rotation on the posteroanterior cephalometric measurements (정모두부방사선사진 촬영시 두부의 수직회전에 따른 투사오차)

  • This study was performed to find out how much projection errors in the cephalometric measurements were made by vertical head rotation in taking posteroanterior cephalograms. 25 adults without any apparent facial asymmetry or severe sagittal skeletal discrepancy were selected and the posteroanterior cephalograms were taken with the head rotated $5^{\circ},\;10^{\circ}$ superior and inferior each to the reference $position(0^{\circ})$. The 7 height, 5 width and 6 angular measurements were taken at each 5 positions. Through the statistical analysis of all measurements taken at each rotated position, folowing results were obtained.1. The projection errors of height measurements were remarkably target than those of width or angular measure nents. f. Among the height measurements, the farther to the rotation axis the measurements were, the larger the projection errors were. 3. Among the width measurements, mandibular width and mandibular width of mandibular first molars showed significant differences between the values taken at each rotated position, while nasal width, maxillary width and intermolar width of maxillary first molars did not. 4. Among the angular measurements, the angle between horizontal reference line and the line that is connected to crista galli and antegonion or maxillare showed significant differences between the values taken at each rotated Position. The above results suggest that it is needed to the effort to keep constant head position for taking the useful posteroanterior cephalogra, because projection errors are caused by vertical head rotation.

ERUPTION DISTURBANCES OF TEETH IN KOREAN CHILDREN (영구치의 맹출 장애에 관한 조사 연구)

  • Eruption disturbance is an abnormal state of eruption that occurs over a broad chronologic age range. There are many factors that influence eruption such as premature loss of primary teeth, supernumerary tooth, local disease, tongue, lip, mastication muscle and trauma. The purpose of this study was to determine the prevalence, patterns and tooth position that have eruption disturbance in children in Seoul, Korea. The subjects were 8,010 new patients who visited the department of Pediatric dentistry of Seoul National University Dental Hospital in 2004-2005. Clinical and radiographic records of the patients were reviewed by the primary investigator who is a pediatric dentist. Notable eruption disturbances included delayed eruption, early eruption and ectopic eruption and the prevalence of eruption disturbance was 8.79%.

Three-dimensional finite element analysis on stress distribution of the mandibular implant-supported cantilever prostheses depending on the designs (임플란트 지지 하악 캔틸레버 보철물의 디자인에 따른 저작압 분산에 관한 삼차원 유한요소 분석)

  • Statement of problem: The position and length of cantilever influence on the stress distribution of implants, superstructure and bone. In edentulous mandible, implant-supported cantilever prostheses that based 4 or 6 implants between mental foramens has been attempted. Excessive bite force loaded at cantilever prosthesis causes bone resorption and breakage of superstructure prosthesis around posterior implants. To complement the cantilever length of conventional prosthesis, In 1992, (McCartney) introduced "cantilever-rest-implant" and Malo reported "All-on-Four" in 2003. Purpose: Analyze and compare the stress distribution of conventional cantilever prostheses with rest implant and All-on-$Four^{TM}$ implant prostheses. Material and method: The external loads(300 N vertically, 75 N horizontally) are applied to first molar area. The stress value, stress distribution and aspect of stress dispersion are analyzed by three-dimensional finite element analysis program, ANSYS ver. 10.0. Results: 1. The rest implant and "All-on-Four" implant system are superior to conventional cantilever prostheses to reduce stress on the bone and the superstructure around implants. 2. The rest implant was of the greatest advantage to stress distribution on bone, implant and superstructure. 3. With same number of implants, distally tilted implants are preferred to conventional cantilever prostheses for reducing the length of cantilever.

MULTIPLE CONGENITAL MISSING TEETH : CASE REPORT (선천성 다수 영구치 결손 환아의 증례보고)

  • Congenital Missing Teeth(hypodontia, oligodontia) is the developmental absence of one or more teeth. It has been reported as being the most common anomaly of dental development in human, relatively common in the permanent dentition. In a recent review, Vastardis has quoted incidence ranges of $1.6%{\sim}9.6%$ in the permanent dentition. Brook has quoted a prevalence of $3.5%{\sim}6.5%$ in most populations, with severe hypodontia, defined as the absence of six or more teeth, having a prevalence of $0.3{\sim}0.4%$. The most commonly affected teeth are third molars, followed by maxillary lateral incisor, and second premolars. The etiology is unknown, several hypotheses include trauma, nutritional deficiency, infection, metabolic abnormalities, systemic disease and genetic influence. The multiple congenital missing is commonly associated with specific syndrome or severe systemic abnormalities such as cleft lip & palate and Down's syndrome. These cases present that children have multiple congenital missing teeth in the permanent dentition, without any systemic disease. Management of this condition must be considered orthodontic and prosthodontic treatment comprehensively. In these cases, children were treated by space maintainer or orthodontic appliance and follow-up checked.

THE EFFECT OF PRE-OPERATIVE STEROID INJECTION ON THE RELIEF OF COMPLAINT AFTER THIRD MOLAR SURGERY (술 전 스테로이드 투여가 하악 매복 제 3 대구치 발치 후 예상되는 불편감에 미치는 영향)

  • Background : The surgical removal of impacted mandibular third molar can result in considerable pain, swelling, and dysfunction that patient are incapable of work for several days. Factors contributing to post operative swelling, trismus and pain are complex. There is no question but that the procedure of surgically removing an impacted mandibular third molar is inherently a traumatic one and that some sequelae related to the inflammation response are expected. Meticulous surgical technique will minimize the sequelae of inflammation but will not prevent them. In an effort to minimize these sequelae the use of steroid was instituted. Patients and Methods : Present study was to investigate the effect of one preoperative steroid injection in the masseter muscle to the patients(male 9, female 11) who needed prophylactic removal of bilateral, symmetrical, impacted wisdom teeth in the mandible on the complaint like swelling, trismus and pain. through Double-Blind test. Results : 1. After 24 hours investigation, preoperative steroid injection had significantly reduced swelling with 39% and trismus with 57.5%. 2. $7^{th}$ post operative day investigation, reduced swelling and trismus had shown, however, not significant. 3. There wasn't major difference from the group who took preve-ntive steroid in the visual analogue scale, the first analgesic intake time and the pain period. 4. There wasn't any adverse reaction of steroid for 20 patient From the above result, If the patients are not contraindication to steroid and pronounced post operative reaction can be expected the use of steroid to the surgical removal of impacted mandibular third molar is recommended.

Assessment of the fit of zirconia-based prostheses fabricated with two different scan methods (서로 다른 두 가지 스캔법을 이용하여 제작된 지르코니아 보철물의 적합도에 대한 비교)

  • Purpose: This research was conducted to compare the marginal and internal fit of zirconia prostheses fabricated with the model scan method and the intraoral scan method. Materials and methods: In this study, 20 extracted human mandibular first molar was used in the preparation of abutment tooth for the fabrication of zirconia prostheses. In the first group, the model scan method was applied on 10 prepared teeth. In the other group, the intraoral scan method was used on other 10 prepared teeth. Datum of both groups were transmitted to the software system. Afterwards, 20 zirconia prostheses were fabricated using the Ceramill system. Weight technique was used to evaluate the internal gap of the zirconia prostheses. In the Replica technique, marginal gap of the zirconia prostheses were analyzed by optical microscopy. Statistical analysis was based on one-way ANOVA. Results: Model scan group showed lower average weight than intraoral scan group when weight technique was applied, which has significance (P < .05). Also, model scan group showed significantly lower figures in all 5 measurements of replica technique than intraoral scan group (P < .05). Conclusion: Zirconia prostheses of both groups demonstrated clinically acceptable margin and internal fit. However, model scanned zirconia prostheses showed higher marginal and internal fit than intraoral scanned crowns.

ORAL HEALTH STATUS AND SELF-PERCEIVED ORAL HEALTH STATUS OF STUDENTS IN JUVENILE PROTECTION EDUCATION INSTITUTIONS (소년원학교 학생의 구강건강 실태와 구강건강 인식도)

  • The purpose of this study was to investigate the prevalence of dental caries, periodontal status, and self-perceived oral health status of students in juvenile protection education institutions. The subjects of this study were 342 adolescents aged 13-20 years detained in two juvenile protection education institutions located in Gyeonggi Province. The study involved an oral examination to record decayed, missing, filled teeth and index teeth with bleeding, calculus, or periodontal pocket, combined with a questionnaire to investigate self-perceived oral health status. The main findings are as follows. 1. When compared with the result of National oral health survey, the students detained had more decayed teeth, DMFT, and teeth with bleeding, calculus, and shallow pocket than general population of the same age. 2. Female students showed more DMFT, more teeth with healthy periodontium, and fewer teeth with shallow pocket than male students (p<0.05). 3. Female students perceived their health to be poorer than male students (p<0.05). A majority(79.2%) of detainee valued their oral health in their life. 4. A majority(71.5%) of detainee reported their dental anxiety, and more female adolescents showed dental anxiety than male adolescents(p<0.05).
