A Design of Clinical Information Exchange Framework for Performance Improvement based on Lazy Response Model (지연 응답 모델에 기반한 성능 개선 진료정보 교류 프레임워크의 설계)

  • Recently medical service environment, the clinical information exchange which contribute to medical safety, promotion of service quality and patient's convenience, efficiency of medical procedures and medical management is essential medical service model. But, practical exchange of clinical information which variation of information level, absence of standardization system, build of heterogeneous information systems is difficult in each medical institute. In this paper, We analyzed the related technical standardizations and the models of clinical information exchange. So, we designed the clinical information exchange system based on the ideal lazy response model which is aimed at vitalizations the exchange of clinical information under domestic law environment. In case of exchange the clinical information, we separate CDA document flow from metadata flow. As a experimental result we acquired 24% improved performance compared with existed system based on the lazy response model.

  • This study will be used as basic data to hold clinical training effective by researching and analyzing frequency, satisfaction and stress for the clinical practice of dental hygiene course students. The survey was undergone from Feb. 1, 2014 to Mar. 31 for 219 dental hygiene course juniors who had experienced clinical training in 3 year colleges located in Daejeon and Chungcheong Province. From the study, general and university hospital holds the highest satisfaction in clinical practice and dental offices in acquiring skills. Also, a significant difference is shown between the frequency and satisfaction according to clinical training sheets. The highest degree of stress is shown in dental offices for 'toothbrushing training and dental health education' and in general and university hospital for 'patient counselling'. Therefore, consideration should be made to establish the direction of education, standard criteria and evaluation method for student clinical training to raise its effectiveness.

The Standardization of the Korean Version of Brief Edinburgh Depression Scale as a Screening Tool for Depression in Cancer Patients (암 환자에서 우울증의 선별도구로서 단축형 에딘버러 우울증 척도의 한국 표준화)

  • Objectives : Depression is a common psychiatric disorder in cancer patients. The Brief Edinburgh Depression Scale(BEDS), which is an abbreviated version of the Edinburgh Depression Scale, may serve as a useful tool in screening for the depression in patients with the medical illnesses. This report investigated the reliability and validity of the Korean Version of the BEDS(K-BEDS) for the depression in cancer patients. Methods : One-hundred cancer patients were enrolled in this study. All subjects completed the K-BEDS, the Hospital Anxiety Depression Scale(HADS), and the Karnofsky Performance Status Scale(KPSS). Reliability, validity and Receiver Operating Characteristic(ROC) curve analysis measures were assessed. Results : The K-BEDS showed good internal consistency(Cronbach ${\alpha}$=0.77) and test-retest reliability(0.94, p<0.001). All item-total correlations were above 0.3. Also, it revealed moderate correlation with the depression subscale of the HADS(r=0.617), but no correlation with the KPSS. Exploratory factor analysis produced only one factor, accounting for 47.1% of the total variance. The most valid cutoff value to screen for depression was a total score of 5 on the K-BEDS, which showed sensitivity of 62.5% and specificity of 86.4% with a positive predictive value of 4.60 and a negative predictive value of 0.43.

A study about the medical communication proficiency of Korean traditional medical students using standardized patients of Infertility (표준화 난임환자를 활용한 한의대생의 진료 및 의사소통 수준연구)

  • Objective : The purpose of this study is to assess the communication and medical interview level of the students in the college of korean traditional medicine using Standardized Patients(SP). Method : 35 students of 5th grade in the college who are doing their clinical practice class participated and conducted medical interview on standardized infertile female patient. SP and professor who observed the interview evaluated the students' communication skill and medical interview level using the assessment sheet composed of 23 questions. Results : 1. Satisfaction of SP was 1.66 of 3. In the communication section, students conducted well in appropriate voice and no inappropriate interruption but they didn't show good practice in considerate physical examination, understandable explanation relatively. 2. The scores that were acquired by students in informing the time required, obtaining agreement at initiating and closing, family history taking, checking mental status and physical examination 2 were low in the medical interview section. 3. There were some differences in the communication and medical interview scores by the students' sex (p<.01), age(p<.05) and medical camp experience(p<.05). But we couldn't find any evidence about the relation between character type and the communication and medical interview scores(p>.62). Conclusion : On the whole, students who participated this study had difficulties in building relationship with patient and conducting medical interview. They need to improve their ability in those fields by appropriate education before their graduation.

Implementation and Design of WISD(Web Interface System based DICOM) for Efficient Sharing of Medical Information between Clinics (의료기관간 효과적인 의료정보 공유를 위한 WISD의 설계 및 구현)

  • For efficient compatible system between medical clinics, the medical information has to be built on a standardized protocol such as a HL7 for text data and a DICOM for image data. But it is difficult to exchange information between medical clinics because the systems and softwares are different and also a structure of data and a type of code. Therefore we analyze a structure of DICOM file and design an integrated database for effective information sharing and exchange. The WISD system suggested in this paper separate the DICOM file transmitted by medical clinics to text data and image data and store it in the integrated DB(database) by standardized protocol respectively. It is very efficient that each medical clinic can search and exchange information by web browser using the suggested system. The WISD system can not only search and control of image data and patient information through integrated database and internet, but share medical information without extra charge like construction of new system.

A Study about the Medical Communication Proficiency of Korean Traditional Medical Students Using Standardized Patients of Menopausal Disorder (표준화 갱년기장애환자를 활용한 한의대생의 진료 및 의사소통 수준연구)

  • Objectives: The purpose of this study is to analyze clinical performance examination (CPX) ability using standardized patient (SP) in Korean traditional medical school. Methods: Standardized middle-aged female patients were used for clinical examination of 53 medical students, grade 6. The SP and professors assessed their communication skills and level of medical interviews. Results: 1. There is no statistically significant difference between SP score, communication skill score, medical skill score and inter-test score at the extracurricular activities level. 2. There is no statistically significant difference between SP score, communication skill score, medical skill score and inter-test score at number of medical service. 3. There is no statistically significant difference between SP score, communication skill score, medical skill score and inter-test score at personality type. 4. There is a statistically significant positive correlation between SP score, communication skill score and medical skill score. Conclusions: A variety of educational programs and practices should be conducted to improve the level of medical care and communication of Korean traditional medicine students.

  • Owing to the recent advances in biological knowledge on stem cells, many efforts are being made to apply them to clinical practice. Although mesenchymal stem cells were first found in bone marrow aspirates, they are understood to be multipotent stromal cells that can be extracted from a variety of tissues, such as adipose, dermal, skeletal muscle, and umbilical-cord tissues. The osteogenicity of mesenchymal stem cells has been verified through various experiments and animal studies. Some successful bone regenerations have also been reported in difficult clinical situations, such as large bone defects, osteonecrosis, and nonunion. On the other hand, there are no standardized indications or application methods for each clinical situation, and convincing evidence of its efficacy and safety is still lacking. Bone regeneration therapies using mesenchymal stem cells are likely to expand further in the future, but there are some issues that need to be addressed in order for them be recognized as standard treatments.

Analysis of the causes of high-risk intravenous medication errors recognized by hospital nurses (병원간호사가 인식한 고위험 정맥주사 투약오류 원인 분석)

  • This study was attempted to identify the perception and experience of hospital nurses on medication errors of high-risk intravenous drugs, and to identify the causes of medication errors and ways to improve them. The subjects of the study were nurses with work experience related to high-risk intravenous administration working at a university hospital located in D City, and data were collected between May 16 ~ 30, 2021. As a result of the study, six key factors were identified as the key factors in the safety of high-risk intravenous injections: the lack of a protocol for the administration of major drugs in each ward, the lack of training in the operation of the injection machine, the lack of standardized procedures for administering high-risk intravenous injections, the lack of individualized medication training for nurses, the lack or lack of the hospital's own drug list, and the lack of identification of drugs packaged in similar containers. At the nursing practice level, it is proposed to apply a high-risk intravenous medication safety program and conduct a future study to identify safety outcome indicators.

Postoperative Clinical Courses According to the Length of Preoperative Drug Therapy in Pulmonary Tuberculosis (폐결핵 환자의 수술전 항결핵제 투여기간에 따른 수술후 임상경과)

  • Background : Though surgery plays an important role in the management of patients with Mycobacterium tuberculosis infection, there is little information regarding the timing of resection. We tried to find out the ideal timing of operation. Method: A retrospective review was performed in 69 patients underwent pulmonary resection for pulmonary tuberculosis between January 1993 and December 1997. They were categorized into various groups according to the length of preoperative specific drug therapy. The rates of treatment failure, realpse and complication in each group were compared statistically by $x^2$-test. Results: Eighty one point two percent were men and 18.8 % women with a median age of 33 years(range, 16 to 63 years). The mean number of resistant drugs was 3.l(range, 0 to 9). Patients were treated preoperatively with multidrug regimens, which mean number of preoperative specific drugs was 4.6, in an effort to reduce the mycobacterial burden with the mean length of preoperative drug therapy, 5.0 months. Postoperative treatment was conducted for a mean period of 13.0 months with a mean number of postoperative specific drugs, 4.4. Postoperative treatment failures were confirmed in 8 among 69 patients(11.6%). 2 of these 8 patients were showed up in the preoperative 3 to 4 months medication group and each of the rest was occurred in the preoperative 2 to 3, 5 to 6, 6 to 7, 12 to 13, 17 to 18 months, less than one month medication group, respectively. 59 of 69 patients were available for evaluation of the relapse rate with the mean duration of the postoperative follow-up, 19.8 months. In 4 patients bacterial relapse was confirmed(6.8%). Each of these 4 was in the preoperative 1 to 2, 2 to 3, 3 to 4, 5 to 6 months medication group. Categorized into various groups according to the length of preoperative specific therapy, there were no statistical significances of the treatment failure rate, relapse rate and complication rate in the groups. There were seven treatment failures of 28 who were AFB culture positive until the time of operation(25%, p<0.01). Categorized the preoperative AFB culture positive group into various groups according to the length of preoperative drug therapy, there were no statistical significances, either. Conclusion: We believe that operation plays an important ancillary role in the treatment of pulmonary tuberculosis. Our results indicate that the timing of resection according to the length of preoperative drug therapy may not cause trouble.
