Surgical Treatment of Pulmonary Aspergillosis (폐국균증의 외과적 치료)

  • Between July 1988 and August 1991, 8 cases of pulmonary resection for pulmonary asp-ergilloma had been performed in the department of thoracic surgery, Korea Cancer Center Hospital. The patients were consisted of 4 males and 4 females and were evenly distributed from fourth decade to sixth decade. Hemoptysis was the most common chief compla int. In chest roentgenogram, patch infiltration was noted in 4 cases[50%] and intracavi-tary fungus ball was noted in only 2 cases[25%]. A. fumigatus was identified pre-operatively in 2 cases by bronchoscopic washing and in 1 case by culture of pleural effusion, Eight pulmonary resections were done by 5 lobectomies, 1 segmentectomy, 1 wedge resection and 1 pneumonectomy, Postoperative pathologic findings showed that 6 cases [75%] were combined with bronchiectasis, 1 case with tuberculosis and 1 case with pneumonia. We experinced 1 case of postoperative pulmonary edema but there was no mortality case.

Surgical Treatment of Pulmonary Aspergillosis (폐국균증의 외과적 치료)

  • From January, 1980, to August, 1989, 23 patients underwent thoracotomy for treatment of pulmonary aspergillosis on the Department of Thoracic and Cardiovascular Surgery, School of Medicine, Keimyung University. There were 12 male and 11 female patients ranging from 23 years to 61 years old[mean age, 40.7 years]. The main clinical symptoms were hemoptysis[91.3%], cough[65.2%], chest pain[34.5%]. Sixteen patients[69.6%] had simple aspergilloma and 7[30.4%] had complex aspergilloma. The most common indication for operation was a hemoptysis, indeterminate mass, chronic cough, or obstructed bronchus. Anatomical location of lesion was mainly located upper lobe [82.6%] and most of cases were managed by lobectomy. Postoperative pathologic findings showed that 13 case[56.5%] were combined with tuberculosis, two were combined with bronchiectasis and two were combined with lung tumor, but 6 cases were not combined with other disease. Early complications occurred in 33.5% of patients with simple aspergilloma and in 85.7% of patients with complex aspergilloma. But there was no hospital death.

A Case of Endobronchial Aspergilloma Presented by Solitary Pulmonary Nodule (고립성 폐 결절 형태로 나타난 기관지 내 국균종 1예)

  • A 53 year-old woman visited to our hospital due to increased hemoptysis for 4 days. The chest X-ray showed solitary pulmonary nodule on right upper lobe and computed tomography of chest demonstrated mass on posterior segmental bronchus of right upper lobe. Bronchoscopic examination revealed that this segmental bronchus was completely obstructed by a yellow and brownish mud like mass, which was identified as an aspergilloma by pathologic examination. Patient had undergone bilobectomy because of persistent hemoptysis. Tracheobronchial apergillosis in an immunocompetent person is very rare disease. then, we report this case with review of the Korean literature.

Leiomyoma of the Bronchus -A Case Report- (기관지에 발생한 평활근종 - 수술치험 1례 보고 -)

  • Leiomyoma of the bronchus is a very rare, benign tumor of the lung. We herein report a case of endobronchial leiomyoma which was treated by sleeve bronchoplasty without resection of the lung and without any complications. A 35-year-old man was admitted to our hospital complaining of stridor. Bronchoscopy revealed a round, hypervascular and smooth tumor nearly obstructing the left main bronchus at 1.5 cm distal portion from the carina. Biopsy was not performed for fear of massive hemorrhage. A sleeve resection of left main bronchus including the tumor and end-to-end anastomosis were performed. The histologic diagnosis was leiomyoma. The early diagnosis and appropriate surgical treatment to preserve the pulmonary function are important points of consideration.

Analysis of Surgical Risk Factors in Pulmonary (폐국균종의 수술위험인자 분석)

  • Background: The purpose of this study is to analyze the types of complications, the incidences of complications, and preoperative and postoperative risk factors affecting the incidence of the complication. Material and Method: Between August 1990 and August 1997 in Asan Medical Center, 42 patients(24 men and 18 women) underwent surgical resection for pulmonary aspergilloma. The mean age was 46.6${\pm}$11.5 years(range 29 to 69 years). Hemoptysis(90%) was the most common presentation. Pulmonary tuberculosis was the most common predisposing cause(81%). The associated diseases were bronchiectasis(n=11), active puolmonary tuberculosis(n=9), diabetes mellitus(n=8), lung carcinoid(n=1), and acute myeloblastic leukemia(n=1). Lobectomy was done in 32 cases(76%), segmentectomy or wedge resection in 4, pneumonectomy in 2, and lobectomy combined with segmentectomy in 4. Result: Operative mortality was 2%. The most common postoperative complication was persistent air leakage(n=6). The variables such as age, sex, pulmonary function test, amount and duration of hemoptysis, associated diseases(diabetes mellitus, active pulmonary tuberculosis), mode of preoperative management(steroid, antifungal agent, bronchial arterial embolization), and modes of operative procedures were statistically insignificant. The radiologic extent of infiltration to normal lung parenchyme was statistically significant(p=0.04). Conclusion: We conclude that the extent of the infiltration to normal lung parenchyme in preoperative radiologic studies should be carefully evaluated to reduce the postoperative complications in surgery for pulmonary aspergilloma.

Clinical Study of the Treatment of Chronic Empyema with Open Window Thoracostomy: 10 Years Experience (개방식 배농술을 이용한 만성 농흉 치료의 임상적 고찰 - 10년 경험 -)

  • Background: The curative treatment of choice for empyema is decortication of the pleura. The risks of this treatment however are increased for the patient with reduced pulmonary function, complicated calcification or septic shock. In the past, open window thoracostomy was a final stage treatment for chronic empyema. Relatively safe treatment of empyema could be achieved in difficult cases with a closure of the open window after open drainage and use of a myocutaneous flap (one stage or staged). Material and Method: A retrospective study of the cause, progression and final outcome of empyema patients who received open window thoracostomy was performed. 21 patients were followed from 1995 to 2004 in the department of Thoracic and Cardiovascular Surgery in the College of Medicine, Pusan National University. Result: The average age of the patients was $57.5{\pm}15.5$ years (range $25{\sim}78$ years), of whom 16 (76.2%) were men and five (23.8%) were women. Pulmonary function test results showed an average FEV1 of $1.58{\pm}0.49 L$. The type of empyema was tuberculous empyema in 13 cases (61.9%), aspergillosis in three cases (14.3%), parapneumonic empyema in three cases (14.3%) and post-resectional empyema in two cases (10%). Bronchopulmonary fistula was seen in 14 cases. Eight cases were complicated by severe calcification of the pleura. For the four cases of bronchopulmonary fistula, the patients' serratus anterior muscle was covered in their first operation. The average number of ribs resected was $4{\pm}1$. Closure of the open window thoracostomy was performed in 12 cases. The average time to closure after open drainage was $10.22{\pm}3.11$ months and the average defect of the empyemal cavity before the final operation was $330{\pm}110 cc$. Among the 12 cases, there were two cases of spontaneous closure. In two cases closure was only achieved by using the reserved skin fold during the first surgery. Of the remaining eight cases, in seven we used the myocutaneous flap (four cases of lattisimus dorsi muscle and three cases of pectoralis major muscle), and in one case we used soft tissue. As regards complications of the closure, tissue necrosis occurred in one case, which led to failed closure, and there was one case of abdominal hernia in the rectus abdominis muscle flap. One patient died within 30 days of the surgery and one patient died of metastatic cancer. Conclusion: A staged operation with a final closure using open window thoracostomy, which consists of open drainage, transposition of the muscle and a myocutaneous flap, can be a safe and effective option for the chronic empyema patient who is difficult to cure with traditional surgical methods.

The Clinical Effects of Surgical Treatment for Hemoptysis due to Inflammatory Lung Disease (염증성 폐질환에 의한 객혈에 대한 수술적 치료 효과)

  • Background: Many studies have demonstrated the various therapeutic options for treating hemoptysis caused by inflammatory lung disease. However, there is debate over the surgical management of the ongoing hemoptysis. Therefore, we evaluated the clinical results of pulmonary resection that was done due to hemoptysis in patients with concomitant inflammatory lung disease. Material and Method: We performed a retrospective analysis of 75 patients who received pulmonary resection for hemoptysis and concomitant inflammatory lung disease between 2001 and 2007. The mean age was $52.1{\pm}12.5$ years old, and the male; female ratio was 52:23. Result: The underlying disease was aspergilloma in 30 patients (40%), pulmonary tuberculosis in 20 patients, bronchiectasis in 18 patients and other causes in 7 patients. The surgical treatment included lobectomy in 55 patients, bilobectony in 2 patients, pneumonectomy in 17 patients and wedge resection in 1 patient. There were 3 early deaths, and the causes of death were pneumonia in 1 patient and BPF in 2 patients. The early mortality was statistically higher for such risk factors as a preoperative Hgb level <10 g/dL, COPD and an emergency operation. Conclusion: In conclusion, pulmonary resection for treating hemoptysis showed the acceptable range of mortality and it was an effective method for the management of hemoptysis in patients with inflammatory lung disease. However, relatively high rates of mortality and morbidity were noted for an emergency operation, and so meticulous care is needed in this situation.
