The Association between Bone Mineral Density, Bone Turnover Markers, and Nutrient Intake in Pre- and Postmenopausal Women (폐경 전.후 여성의 골밀도 및 골대사 지표에 영향을 미치는 요인)

  • The purpose of this study was to examine the association among bone mineral density (BMD), biochemical bone markers, nutrients, and salt intake in premenopausal and postmenopausal women. We evaluated 431 subjects who visited a health promotion center of a university hospital between January 2008 and July 2009. We excluded those who were taking medications or who had an endocrine disorder affecting osteoporosis. The subjects were divided into premenopausal (n = 283) and postmenopausal (n = 143) women. We evaluated the correlation among BMD of the lumbar spine, femoral neck, and total femoral, as well as biochemical bone markers, hormone, serum profiles, general characteristics, nutrient intakes, and food intake frequencies. From a stepwise multiple regression analysis, lumbar spine BMD was positively correlated with weight (p < 0.001) and negatively correlated with osteocalcin (OC)(p < 0.001), Femoral neck BMD was positively correlated with weight (p < 0.001) and negatively correlated with C-telopeptide (CTx) and alkaline phosphatase (ALP)(p < 0.001, p < 0.05). In premenopausal women, femoral total BMD was positively correlated with BMI (p < 0.001) and negatively correlated with CTx (p < 0.001). In postmenopausal women, lumbar spine BMD was positively correlated with calcium intake (p < 0.01) and negatively correlated with sodium intake (p < 0.01). Femoral neck and femoral total BMD were both positively correlated with weight (p < 0.001), and femoral neck BMD was negatively correlated with age and ALP (p < 0.001, p < 0.05). Femoral total BMD was negatively correlated with age and OC (p < 0.001, p < 0.01). These results suggest that reducing sodium intake may play an important role delaying bone resorption and preventing a decrease in BMD.

Characteristics of the Health Factors in 45~60 Year Old Korean Women related to Menopausal Stages - Based on 2008~2009 Korean National Health and Nutrition Examination Survey - (2008~2009년 국민건강영양조사를 활용한 45~60세 한국여성의 폐경 여부에 따른 건강인자 특성)

  • We analyzed data from the combined 2008~2009 Korean National Health and Nutrition Examination Survey (KNHANES) to compare the health factors related to menopausal stages in 45~60 year old Korean women. In this study, we classified the subjects into a premenopausal group (n = 439) and a postmenopausal group (n = 683). In the postmenopausal group, age was higher (p < 0.001), monthly income (p < 0.01) and education levels (p < 0.001) were significantly lower than in the premenopausal group. Body fat % and waist circumferences were also higher in the postmenopausal group than in the premenopausal group. The serum glucose (p < 0.05), total cholesterol (p < 0.001), LDL-cholesterol (p < 0.001), triglyceride (p < 0.001), GOT (p < 0.001), GPT (p < 0.001) in the postmenopausal group were higher than in the premenopausal group. The postmenopausal group showed a significantly lower quality of life compared to the premenopausal group (p < 0.01). With regard to dietary quality, nutrient adequacy ratio (NAR) of vitamin A, vitamin $B_1$, vitamin $B_2$ and niacin in the postmenopausal group were significantly lower than in the premenopausal group. The levels of glucose, total cholesterol, LDL-cholesterol, and triglyceride showed a significantly positive correlation with age, waist circumferences, body fat % and BMI. The 45~60 year old Korean women in this study showed high levels of obesity and serum lipids. Also, intakes of the vitamins and minerals of the women did not meet the level of Dietary reference intakes for Koreans. Therefore, nutritional risk may be high in the women, especially in postmenopausal women. In order to prevent the health risk, women's health care including the quality of the meal should be considered.

Association of iron status and food intake with blood heavy metal concentrations in Korean adolescent girls and women: Based on the 2010~2011 Korea National Health and Nutrition Examination Survey (한국 여자 청소년과 성인 여성의 혈청 철 영양상태 및 식품 섭취와 혈중 중금속 농도와의 상관성 : 2010~2011 국민건강영양조사 자료를 이용하여)

  • Purpose: This study examined and compared the associations of the iron status and food intake with the blood lead, mercury, and cadmium concentrations among Korean adolescent girls, premenopausal women, and postmenopausal women. Methods: The data from the 2010~2011 Korea National Health and Nutrition Examination Survey (KNHANES) was used. The subjects were classified into three groups: adolescent girls (n = 268), premenopausal women (n = 1,157), and postmenopausal women (n = 446). The iron status was assessed by hemoglobin, hematocrit, serum ferritin, and iron concentrations, as well as the total iron binding capacity (TIBC). The food intake was estimated by a food frequency questionnaire. Results: The blood heavy metal concentrations and poisoning rate in postmenopausal women were higher than in the other groups. The iron status in the adolescent girls and postmenopausal women was higher than that in the premenopausal women. In the adolescent girls, the iron status was inversely associated with the blood cadmium concentration. The dairy food intake was inversely related to the blood lead and cadmium concentrations. In premenopausal women, the iron status was inversely associated with the cadmium concentrations. The fish and shellfish food intakes were positively associated with the mercury concentrations. In postmenopausal women, the iron status was positively associated with the mercury and cadmium concentrations. Fast foods and fried foods were inversely associated with the lead concentration. Conclusion: The premenopausal women showed a lower iron status than the adolescent girls and postmenopausal women. The associations of the iron status with the blood heavy metal concentrations were different among the adolescent girls, premenopausal women, and postmenopausal women. In addition, the relationships of the food intakes with the blood heavy metal concentrations differed among adolescent girls, premenopausal women, and postmenopausal women. Further studies will be needed to confirm these findings.

A Convergence Study of Factors Influencing on Depressive Symptoms in Postmenopausal Middle-Aged Women (폐경 후 중년여성의 우울 증상 영향요인의 융합연구)

  • The purpose of this study was to identify its influencing factors of depressive symptoms in postmenopausal middle-aged women though analyzing secondary data. The data of 647 with 40~65 aged women who participated in the Seventh Korea National Health and Nutrition Examination Survey in 2016 were used. The data were analyzed by complex sample independent t-test and multiple linear regression. In multiple linear regression analysis, economic status, subjective health status, perceived stress, smoking, physical activity, and limitation of active had significant effect on depressive symptoms in postmenopausal middle-aged women. These variables explained 36% of the variance in depressive symptoms. The findings suggest that healthcare providers should be considered to these factors when developing intervention strategies to decrease depressive symptoms in postmenopausal middle-aged women.

The Association of Serum Ferritin and Metabolic Syndrome and Metabolic Syndrome Score in Korean Adults (대한민국 성인에서 대사증후군 및 대사증후군 구성요소의 증가와 혈청 Ferritin의 관련성)

  • This study was conducted to assess the relationship amongst serum ferritin, and metabolic syndrome (MetS) and metabolic syndrome score (MSS) in Korean adults. The data of 16,096 adults (6,840 men as well as 4,916 premenopausal and 4,340 postmenopausal women) aged ${\geq}20years$ in the Korean National Health and Nutrition Examination Survey (KNHANES) between 2010 and 2012 were analyzed. The prevalence rate of MetS was 3,978 (24.7%) (men, 24.6%; premenopausal women, 11.1%; postmenopausal women, 40.3%). The key study results were as follows: First, after the adjustment for relevant variables, the serum ferritin level ($M{\pm}SE$) was significantly higher (p<0.001) in the MetS group (men, $132.25{\pm}1.98ng/mL$; premenopausal women, $39.89{\pm}1.49ng/mL$; postmenopausal women, $73.45{\pm}1.14ng/mL$) than in the non-MetS group (men, $111.08{\pm}1.01ng/mL$; premenopausal women, $32.26{\pm}0.50ng/mL$; postmenopausal women, $63.26{\pm}0.98ng/mL$). Second, the serum ferritin levels increased as MSS increased in all groups (men, premenopausal women, and postmenopausal women) (p<0.001). In conclusion, MetS and MSS increases were positively associated with higher serum ferritin levels.

  • The purpose of this study was to investigate the comparison between metabolic syndrome indicators and nutrient intakes in Korean menopausal women from the Korean National Health and Nutrition Examinations Survey data (2010~2012). Menopausal Status were classified into premature menopause(n=214) and natural menopause(n=2,546). Among the nutrient groups, retinol intake was a significant factor in natural menopausal women according to BMI levels and riboflavin intake was another significant factor in premature menopausal women according to fasting glucose levels. The results suggested that micronutrient including retinol, riboflavin, fiber and calcium were significantly associated with metabolic syndrome risk in Korean postmenopausal women. Further research is required for elucidating the association between nutrient intakes and incidence of metabolic syndrome in postmenopausal women within a large population in prospective studies.

The Difference of Predictor of Bone Mineral Density in Pre and Postmenopausal Women (폐경에 따른 골밀도 예측인자의 차이)

  • We studied the relationship between prediction parameters and bone mineral density for pre-and-post menopausal women. We measured BMI%Fat by BIA, blood pressure and lipid profiles for 483 adult women who are in NPO state. SBP, TC, TG, LDL have significant statistical value in the postmenopause women group and postmenopause woman. The value of postmenopause women of these parameters are lower than premenopause woman. BMD has the most strongest relationship with LBM. The BMD and LDL level of postmenopause women have statistically negative relationship. The results show that for the premenopause cases, weight, BF, and HDL level were the major factors which affect the BMD. For postmenopause cases, however, weight, age, and LDL level turned out to be the most significant factors.

The Relationship Between Bone Mineral Density and Adipose Tissue of Postmenopausal Women (폐경 후 여성의 골밀도 수치와 지방조직과의 연관성)

  • Postmenopausal women are at increased risk for osteoporosis and obesity due to changes in hormones. The relationship between osteoporosis and body weight is known, and its relation with body fat mass is discussed. The purpose of this study was to evaluate the bone mineral density(BMD) changes of epicardial adipose tissue(EAT) and abdominal subcutaneous fat. The subjects of this study were 160 postmenopausal women who underwent BMD and echocardiography. The thickness of the epicardial adipose tissue was measured in three sections and the BMD were meassured according to the diagnostic criteria. The results of this study that age increase the risk of osteoporosis increases, and as the weight and BMI decrease, the risk of osteoporosis increases(p<0.05). The relationship between changes in bone mineral density and adipose tissue in postmenopausal women, increased epicardial adipose tissue was negatively correlated with the bone mineral density(p<0.05). conversely, increased abdominal subcutaneous fat thickness was positively correlated with bone mineral density(p<0.05). In other words, the effect of bone mineral density on the location of adipose tissue was different. If Echocardiography is used to periodically examine changes in the thickness of the epicardial adipose tissue, it may be prevented before proceeding to osteoporosis.

  • This study is designed to investigate the differences of ferritin and hemoglobin levels according to the vitamin D state in 23577 Korean adult subjects by using data from the Korea National Health and Nutrition Examination Survey IV, V. Ferritin and hemoglobin levels are analyzed based on the quintile of vitamin D levels by a generalized linear model. The results of analysis showed that there was a relationship between vitamin D state and hemoglobin levels in females in the reproductive status group (p<0.001). In case of ferritin, there was a relationship with vitamin D state in females in reproductive status group (p<0.001) and males under 65 years old group (p<0.05). Within the male group, high ferritin correlated with low vitamin D state; but in the female group, high ferritin correlated with high vitamin D state. Therefore, it is possible for the ferritin level to be affected by vitamin D state in females in reproductive status group and males under 65 years old group.

폐경 후 여성의 골다공증 예방 프로그램이 골밀도 및 건강신념에 미치는 효과

  • 최근 경제의 발전과 의학의 발달로 인하여 인간의 수명은 점차 연장됨에 따라, 노인인구도 급격히 증가하고 있다. 40세 이상 여성 인구도 8.998천명으로 전체 여성의 38%를 차지하며 이는 매년 증가할 것이다. 그리하여 전체 인구에 대한 폐경 여성의 점유율도 급격히 증가하고 있다. 폐경 이후 $5{\sim}10$년 동안 매년 $2{\sim}4%$씩 골농도가 감소하고 있기 때문에 폐경기 여성을 위협하는 주요 질환 중 하나가 골다공증이다. 하지만 골다공증은 뚜렷한 자각증상이 없어 골 기형 및 골절이 발생하기 전에는 조기발견이 어렵다. 골다공증은 모든 중년 여성의 건강문제이므로 지역사회에서 골절 발생 위험율을 줄일 수 있도록 골다공증 예방을 위한 건강관리가 필요하다. 본 연구의 목적은 지역사회에서 적용할 수 있는 골다공증 예방 프로그램을 개발하여 그 효과를 검정한 후 지역사회 중년 여성 건강증진 프로그램으로 활용코저 함에 있다. 골다공증 예방 프로그램은 운동과 교육으로 구성하였으며, 12주 동안 점핑 동작 위주의 에어로빅과 스텝퍼 운동과 근력강화운동(최대 심박수의 $70%{\sim}80%$, $70{\sim}80$분, 3회/주)을 병행하여 실시하였고, 교육은 운동과 칼슘 섭취의 중요성을 건강 신념 이론에 근거하여 자기 효능감을 증대시키는 교육을 하여 지속적인 운동 실천과 칼슘식이 섭취를 하도록 하였다. 대상자는 서울시 일개 보건소에서 골감소증으로 진단받은 폐경 여성으로 실험군 19명과 대조군 19명이었다. 골다공증 예방 프로그램의 효과는 골밀도, DPD ratio, osteocalcin, 일일 평균 칼슘섭취량, 일일 열량 소모량, 배근력, 최대 산소 섭취량, 제지방량과 건강신념 변수로 측정하였다. 수집된 자료는 PC SPSS 8.0 프로그램을 이용하여 대상자의 일반적 특성은 실수와 백분율로 실험군과 대조군의 동질성 검사는 t-test, $x^2-test$ 및 Fisher exact test를 이용하였고, 두 집단 간 결과변수들의 중재 전후의 차이는 t-test로 분석하였다. 연구 결과는 다음과 같다. 1) $L_{2-4}$ 골밀도는 실험군이 대조군에 비하여 유의하게 높았다. 2) DPD ratio와 osteocalcin은 두 집단간 유의한 차이가 없었다. 3) 배근력은 실험군에서 대조군보다 유의하게 증가하였으나, 제지방량과 최대 산소 섭취량은 유의한 차이가 없었다.
