Ferritin and hemoglobin level of korean according to vitamin D state: The Korean National Health and Nutrition Examination Survey IV, V (2008~2012)

Abstract

This study is designed to investigate the differences of ferritin and hemoglobin levels according to the vitamin D state in 23577 Korean adult subjects by using data from the Korea National Health and Nutrition Examination Survey IV, V. Ferritin and hemoglobin levels are analyzed based on the quintile of vitamin D levels by a generalized linear model. The results of analysis showed that there was a relationship between vitamin D state and hemoglobin levels in females in the reproductive status group (p<0.001). In case of ferritin, there was a relationship with vitamin D state in females in reproductive status group (p<0.001) and males under 65 years old group (p<0.05). Within the male group, high ferritin correlated with low vitamin D state; but in the female group, high ferritin correlated with high vitamin D state. Therefore, it is possible for the ferritin level to be affected by vitamin D state in females in reproductive status group and males under 65 years old group.

본 연구는 제 4기, 제 5기 국민건강영양조사 자료를 이용하여 18세 이상 성인 23577 명을 남성 전체와 여성 전체, 65세 이상 남성과 65세 미만 남성, 폐경 전 여성과 폐경 후 여성으로 나누어 비타민 D의 수준에 따라 헤모글로빈과 페리틴 농도의 변화가 어떤 차이를 보이는지 알아보고자 하였다. 각 조사 대상 군을 비타민 D의 수준에 따라 5분 위수로 나누어 각 군 별 헤모글로빈과 페리틴 농도의 변화를 복합표본설계방법에 따라 일반선형모형을 사용하여 분석하였고 분석 결과 여성 전체 군(p<0.001)과 폐경 전 여성 군(p<0.001)에서 비타민 D 수준에 따라 헤모글로빈 농도의 차이가 통계적으로 유의하였고 폐경 후 여성 군과 남성 전체 및 65세 이상 남성, 65세 미만 남성 군에서는 통계적으로 유의하지 않았다. 페리틴 농도는 여성 전체 군(p<0.001)과 폐경 전 여성 군(p<0.001)과 남성 전체 군(p<0.05), 65세 미만 남성 군(p<0.05))에서 통계적으로 유의한 차이를 나타냈으나 남성에서 비타민 D가 높은 그룹이 페리틴이 낮았던 반면 여성에서는 비타민 D가 높은 그룹이 페리틴 농도가 더 높았고, 폐경 후 여성 군과 65세 이상 군에서 통계적으로 유의하지 않았다. 따라서 비타민 D는 폐경 전 여성에서 헤모글로빈 농도에 영향을 줄 가능성이 있고 폐경 전 여성과 65세 미만 남성의 체내 철 조절에 관여할 가능성이 있다.
