Purpose: We evaluate the results of subtalar arthroereisis with $Kalix^{(R)}$ implant (Newdeal, Lyon, France) that were performed in painful flatfoot deformity. Materials and Methods: We performed the subtalar arthroereisis on 16 feet of children symptomatic flexible flat feet after more than 6 months of conservative treatment. Average age was 11 years (8-14 years) old. We checked the functional status with AOFAS functional score in pre-operatively and at final follow-up. Radiologically, we took weight bearing anterior to posterior and lateral view of the feet, and measured the talo-$1^{st}$ metatarsal angle, calcaneal pitch angle, cuboid-surface height. Finally, we asked to patient's parents for satisfaction of the surgery. Results: Mean follow up period was 34.1 months. Average AOFAS score improved from preoperatively 71.9 to postoperatively 91.3. Only one patient has subtalar pain. Average lateral $1^{st}$ metatarsal angle reduced from $-18.2^{\circ}$ preoperatively to $-4.6^{\circ}$ at final follow-up. Average anterior to posterior $1^{st}$ metatarsal angle was reduced from $18.9^{\circ}$ preoperatively to $6.5^{\circ}$ at final follow-up. Average calcaneal pitch angle was increased from $8.6^{\circ}$ preoperatively to $12.6^{\circ}$ at final follow-up. Average cuoboid-surface height was improved from 12.1 mm preoperatively to 16.0 mm at final follow-up. All patients had excellent or good satisfaction. Conclusions: Subtalar arthroereisis with $Kalix^{(R)}$ implant is a viable surgical alternative for painful flatfoot deformity of children.
Purpose: A flatfoot that fails to form a longitudinal foot arch is a common lower limb deformity in children. This study evaluated the structural and functional effects of the insole for pediatric flexible flat foot (PFFF). Materials and Methods: Twenty-nine PFFF patients (20 boys and 9 girls, 58 feet) with bilateral symptomatic flatfoot deformities between February 2017 and May 2019 were included in this study. Sixteen patients (32 cases, study group) were treated with a pressured based 3-dimensional printing insole, and 13 patients (26 cases, control group) were followed up regularly without any treatment. Flatfoot was diagnosed by a lateral talo-first metatarsal angle of more than 4° in convex downward and talocalcaneal angles of more than 30° and a calcaneal pitch of less than 20°. The foot pressures, including the midfoot pressure, total foot pressure, and the ratio of the midfoot pressure to the total foot pressure, were evaluated by pedobarography. The clinical scores were assessed using the visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS), and Pediatrics Outcomes Data Collection Instrument (PODCI) scores. Results: The mean age of the study group was 9.16 years, and the mean age of the control group was 7.73 years. The mean follow-up period was 16 months. The change in the lateral talocalcaneal angle was -4.664°±1.239° in the study group and -0.484°±1.513° in the control group. A significant difference in the amount of change of the lateral talocalcaneal angle was observed between the two groups (p=0.034). The midfoot pressures were similar in the two groups. Conclusion: Pressure based customized 3-dimensional printing insole in PFFF may have some effect on the hindfoot bony alignment, but it does not affect the changes in midfoot pressure.

(1) 항원투여전의 형질세포의 rER 는 일반적으로 편평낭상 (flattened sac) 을 정하고, 항원 투여 후 3 일째에는 일반적으로 소포상 (vesic lar) 를 그리고 7 일내지 10 일째에는 공포상 (vacuolar) 를 정하고 그 강내에는 intracisternal granule 이 다수 출현하며 20 일째에는 다시 확장낭상 내지는 편평낭상을 정하게 된다. 이상의 연구성적으로 보아 편평낭상을 정하는 형질세포의 rER에 항원성 자극을 가하면 처음에는 소포상으로 되었다가 공포상으로 변화하며, 항원 성자극이 소퇴하면 다시 편평낭상으로 복귀된다고 생각된다.
Purpose: The purpose of this study was to evaluate the result of open wedge osteotomy at the anterior calcaneus with iliac bone graft (Evans procedure) for symptomatic flexible flatfeet in children. Materials and Methods: A retrospective study was conducted between October 1995 and September 2002. Six cases in 3 patients who had symptomatic flexible flatfeet was included. Follow-up averaged 39.5 months. We evaluated the patients' satisfaction by Mann and Reynolds scorring and compared the radiographic results between preoperative and final radiography. Results: The satisfaction outcomes at the last follow-up were excellent in five and good in one. The mean lateral talo-first metatarsal angle was improved from $-20^{\circ}$ to $-3^{\circ}$. The mean calcaneal inclination angle was improved from $8.5^{\circ}$ to $20.8^{\circ}$. The talo-navicular coverage angle was improved from $47.5^{\circ}$ to $7.5^{\circ}$. In one case, we found the subluxation of calcaneocuboidal jont in postoperative radiography. Conclusion: Open wedge osteotomy at the anterior calcaneus for symptomatic flexible flatfeet in children was considered as one of the effective treatment methods.
The purpose of this study was to the kinetic variables effects from the use of arch support inserts on low-arched people. We selected 10 people for the research and separated them into 2 groups, 5 people for the normal arched group and 5 people for the low arched group. Each group wear shoes which have a 3 step convertible arch support (level 0, level 2, level 5) and we measured their foot pressure and 3D motion analysis data. As a result, we found that the mean pressure at the heel of the low arched group was decreased when using the arch supports. The arch support induced the correct grounding area for the foot and dispersion of foot pressure. 3D motion analysis found that as the height of the arch support was increased, the movement of the Y-axis(inversion-eversion) was increased to relieve the shock to the heel. The arch support insert limited the range of motion(ROM) of the Z-axis(abduction-adduction) of the low arched person's ankle joint and prevented ankle injury caused by the excessive eversion when walking. Low arched people are seen to be easily tired due to the ineffective shock absorption of the knees and abnormal walking motion. In order to improve the problems, a 3 step convertible arch support(level 5) insert would improve the low-arched people's walking ability. In other words, the low arched people should be expected to walk as well as normal arched people when they wear shoes with the arch support insert.
Purpose : The purpose of this study was to identify the muscle activity of leg during stair down in the subjects with flatfoot and normal foot. Methods : The fourteen subjects with the flatfoot group and the fifteen subjects with the normal foot group volunteered for this study. All subjects descended the stairs that the height was 20cm and consisted 25 stairs. The target muscles for recording muscle activity were posterior fiber of gluteus medius, vastus medialis and vastus lateralis oblique, semitendinosus and biceps femoris, tibialis anterior, and medial and lateral fiber of gastrocnemius muscles. The muscle activity was recorded using the wireless EMG system. Results : The flatfoot group had significantly lower the muscle activity of posterior fiber of gluteus medius than the normal foot group in stance phase during stair down(p<.05). The flatfoot group had significantly higher the muscle activity of biceps femoris and tibialis anterior than the normal foot group in swing phase during stair down(p<.05). Conclusions : This study proposed that the subject with flatfoot should train the strengthening exercise for posterior fiber of gluteus medius.
Background: The purpose of this study was to investigate the effect of modified reverse-six taping on the balance and gait performance in Pes Planus. Methods: A total of twelve subjects (4 males, 8 females) in Pes Planus participated in this study. Navicular drop test was used to evaluate Pes Planus. Balance performance(anterior-posterior; A-P, medial-lateral; M-L), overall) was evaluated using the Biodex balance system in two difference condition(no-taping, with reverse-six taping). Gait performance (cadence, velocity) was evaluated using GAITRite System in two difference condition (no-taping, with reverse-six taping). Results: There were significant improvements of A-P and overall in the balance performance after using reverse-six taping (p<.05). there was no significant improvements of M-L balance performance. and, there were no statistical difference of cadence and velocity in the gait performance after using reverse-six taping. Conclusions: This study found that modified reverse-six taping in Pes Planus was improve the balance performance. but dose not affect the gait performance.
We executed the wearing test in order to revise the basic Beosun pattern for five kinds of foot type. And we took sensory test to evaluated that revisions. The result were as follows: 1. The characteristics of slender foot is that the foot circumference III is short as compared with foot length. So Kou of basic Beosun is sharp-pointed Hoimok and beosunmok are broad. For slender foot we took down ou of Beosun and use foot cir-cumference III/2-1.5cm as Hoimok like the part shown in dotted line of Fin 6. 2. The characteristics of broad foot is that the foot circumference III is long as comparison with foot length. So Kou of ba-sic Beosun is stubby Hoimok and Beosunmok are narrow. For broad foot we took up Kou of Beosun put foot circum-ferenceI /2-1.5cm for the width of Apbol and circumfernce III 2/-1.5cm for Hoimok like the part shown in dotted line of Fig. 8. 3. because the arch of flat foot is flat the sole of basic Beosun is not suitable to the flat foot. It developes on wrinkles of Duichuk and sole. For flat foot we took down Hom of the basic Beousun like the part shown in dotted line of Fig. 10. 4 We evaluated the revisions of basic Beousun for foot types were useful in senso-ry test.
Purpose : The purpose of this study was to investigate the changing plantar foot pressure by the backpack load of 0, 10, 15, and 20% of their body weight while level walking in flat foot and so to recommend suitable backpack weight limitations for flat foot subjects. Method : 14 young flat foot subjects($24.29{\pm}2.16yrs$) participated in this study. the subjects were assigned to carry backpack load and there was four level walking modes : (1) unloaded walking(0%), (2) 10% body weight(BW) load, (3) 15% BW load and (4) 20% BW load. Repeated ANOVA was used to compare each region data of foot according to different backpack weight. Results : As backpack load became increased, the contact area of midfoot was significantly increased, and contact area of forefoot and rearfoot were significantly decreased. maximum pressure at each region during walking tended to be greater as the load increased, but a significant difference was found only for the heel medial and lateral regions Conclusion : Based on this data, the weight of backpack could influenced structure and function of the foot in flat foot.
Objectives : The purpose of this study is to find out the effect of Korean Medical treatments combined with Orthotics on patients's center of gravity with flat foot. Methods : We reviewed the medical records of 24 patients diagnosed with flat foot. They had received Korean medical treatments combined with Foot Orthotics. To estimate the efficacy in rebalancing of body, we analyzed the changes in center of gravity. Results : Among the 24 patients, 13 were male and 11 were female. There was no statistically significant change in frontal angle at the time of treatment. But side angle was changed statistically significant(P=0.009). There was no difference according to sex of male and female. Conclusion : In this study, We found that Korean Medical treatments with Foot Orthotics was effective for correction of patients's center of gravity with flat foot.
