Effect on the Pain Characteristics of Tension-Type Headache by the Tongue Ridge (설 함요가 긴장성 두통 환자의 통증 양상에 미치는 영향)

  • This study was designed to evaluate the pain characteristics of tension-type headache by the tongue ridge. Patients with tension-type headache visited the Department of Oral Medicine, K University Dental Hospital were recruited to this study. Experimental group(n=65) was composed of tension-type headache with the tongue ridge and control group(n=65) was composed of tension-type headache without the tongue ridge. Evaluation list was pain quality, pain intensity, pain laterality, pain increase by routine physical activity and then it was analyzed statistically. The results were as follows: 1. Pain quality of tension-type headache patient was significantly different by the tongue ridge(p=0.049). 2. Pain intensity of tension-type headache patient was significantly different by the tongue ridge(p=0.010). 3. Pain laterality of tension-type headache patient was not significantly different by the tongue ridge. 4. Pain increase by routine physical activity of tension-type headache patient was not significantly different by the tongue ridge. Therefore, it was considered that the tension-type headache patient was influenced by the tongue ridge in the pain quality and pain intensity.

Effects of Myofascial Release on Nerve Conduction Studies and Pain Scale in Middle-Aged Women (중년여성에서 근막이완요법이 신경전도와 통증에 미치는 영향)

  • The purpose of this study was to examine the effect of myofascical release (MR) on the degree of pain and nerve conduction velocity (NCV) in middle-aged women. Participants were 28 middle-aged women and MR carried out three times (1, 3, 5 day) at intervals of two times. We did survey about changes of pain before the MR and how they changed after the MR. Also measured pressure pain threshold (PPT) and visual analogue scale (VAS) by using the algometer at trapezius muscle. In median nerve, we did motor nerve conduction velocity (MNCV) test and sensory nerve conduction velocity (SNCV) test for measuring incubation period, amplitude and nerve conduction. The most painful time was 18~21 and the most painful part was shoulder. The pain scale, PPT and VAS after the MR had significantly decreased than before the MR. The latency was significantly decreased and the amplitude was significantly increased in the MNCV and the latency was significantly decreased in the SNCV after the MR. Also it was effective in ameliorating pain scale and latency of NCV. Consequently, the MR can be effective in prevent pain scale caused by fatigue in middle-aged women as replacement therapy.

The Effects of Changes In Body Composition Through High Intensity Circuit Training On Spine Curvature And Low Back Pain Among Middle-Aged Men With Abdominal Obesity (고강도서키트트레이닝에 의한 신체조성의 변화가 복부비만 중년남성의 척추만곡도와 요부통증에 미치는 영향)

  • Overweight and abdominal obesity caused by sedentary lifestyle may induce deconditioned and atrophied extensor musculature of the lumbar spine and be a potential risk factor for low back pain (LBP). Therefore, this study was conducted to evaluate the validity of high intensity circuit training (HICT) on weight loss and subsequent alleviation of spine curvature (SC) and visual analog scale (VAS) among middle aged men with abdominal obesity. The training program (1 exercise session for 30 minutes, three times a week for 12 weeks) consisted of 12 different functional exercises based on core strengthening multiple joint circuit training. Portions of the obesity index related to body composition were positively changed, which improved the angles of thoracic kyposis and lumbar lordosis, which appeared to effectively reduce lower back pain. Taken together, HICT specifically designed for LBP effectively decreased obesity related body composition and was superior to other treatments for decreasing aggravation of the spine curvature and LBP caused by abdominal obesity; however, weight loss should be the primary treatment target for LBP patients.

Nerve Growth Factor and Sensory Neuropeptide Levels in Plasma and Saliva of Various Orofacial Pain Patients (다양한 구강안면통증환자의 혈장 및 타액에서의 신경성장인자와 감각성 신경펩티드 농도에 관한 연구)

  • Nerve growth factor (NGF) and sensory neuropeptides are involved in the process of nociception at peripheral nerve fibers and wide spread in central nervous system. The aims of this study were to investigate NGF and sensory neuropeptides (substance P [SP] and calcitonin gene-related peptide [CGRP]) levels in human plasma and saliva, and the associations between these sensory neuropeptides levels and chronic orofacial pain symptoms. NGF, SP, and CGRP levels in plasma and resting whole saliva samples collected from 67 orofacial pain patients (joint pain, dental or periodontal pain, mucosal pain) and 36 pain free control subjects were measured by enzyme immunoassay. The characteristic pain intensity of each subject was measured using the Graded Chronic Pain Scale and the flow rate of resting whole saliva was measured. Joint pain patients group showed significantly higher plasma NGF level compared to each of dental pain patients (p<0.01), mucosal pain patients (p<0.01), and control group (p<0.01). Plasma NGF level of dental pain patients group was significantly higher than that of control group (p<0.01). Saliva SP level of dental pain patients group (p<0.05) and saliva CGRP level of mucosal pain group (p<0.05) were significantly higher than that of control group. Plasma and saliva SP levels of joint pain patients was significantly associated with pain intensity (plasma: standardized coefficient=0.599, p<0.01, saliva: standardized coefficient=0.504, p=0.05). In dental pain patients group, plasma SP (standardized coefficient=0.559, p<0.01), saliva SP (standardized coefficient=0.520, p<0.01) and saliva CGRP (standardized coefficient=0.599, p<0.01) levels were significantly associated with age. In mucosal pain patients group, plasma SP (standardized coefficient=0.495, p<0.05), saliva SP (standardized coefficient=0.500, p<0.05), and saliva CGRP (standardized coefficient=0.717, p<0.01) levels were significantly associated with age. NGF and neuropeptides may play a role in the maintenance of various orofacial pain symptoms. The examination of those levels in plasma and saliva helps understanding the mechanism of orofacial pain, and furthermore, can be applied to the diagnosis and therapy of orofacial pain.

  • Pain can be evaluated by experimental methods and clinical methods, but due to subjective characteristics of pain, clinical methods are generally used. The clinical pain measurement tools are divided into unidimensional and multidimensional assessment tools. The former include Visual Analogue Scale, Verbal Rating Scale, Numerical Rating Scale, Pain Faces Scale, and Poker Chip Tool and the latter include McGill Pain Questionnaire, MMPI, Pain Behavior Scale, Pain disability index, and Pain Rating Scale. Unidimensional pain scales mainly measure the intensity of pain on the basis of the patient's self report and their simple construction and ease of use enable the invesgator to assess acute pain. Multidimensional pain scales are used to evaluate subjective, psychological and behavioral aspects of pain and because of its comprehensive and confidential properties they are applied to chronic pain. Patient's linguistic and cognitive abilities are major factors to restrain accurate assessment of pain. Although behavioral patterns and vital sign are inferior to self-report in the measurement of pain, they can be useful indexes in those situations. When deciding on a pain-assessment tool, the investigator must determine which aspect of pain he or she wishes to evaluate on the characteristics of the group of patients, their backgrounds, and their communication skills. Making the proper choice will facilitate the acquisition of meaningful data and the formulation of valid conclusions.

The Effects of Stretching Exercise to Musculoskeletal pain in the Community-Dwelling Farmers (일개 지역 농부에서 근골격계 통증에 대한 스트레칭 운동의 효과)

  • This study aimed to evaluate the effects of stretching management on musculoskeletal joint pain in community indwelling elderly farmers. The study included 28 residents who lived in a rural community. All participants were assigned randomly to either the stretching group (n=13, $59.67{\pm}4.77$ year-old) or the control group (n=15, $61.44{\pm}10.41$ year-old). Respondents were interviewed by means of a structured questionnaire. Pain severity of 6 body areas caused by symptoms of work-related musculoskeletal disorders and pain severity on day time variations were measured by the visual analogue scale on a self-assessed questionnaire. The stretching group was administered stretching therapy (45 min, 2 sets, warming up and cool down, main exercise; total 19 stretching kinds of subset 5 fields) for 12 times for 4 weeks. There were no significant difference between the two groups in general social and pain characteristics. After 4 weeks of stretching, the stretching group showed significant improvements at almost all joint pain scores except arm/elbow, and day time variation scores of pain compared to both baseline scores, and with control group scores. These results showed that stretching therapy is one of the most useful modalities to manage musculoskeletal pain in community-based elderly farmers.

Evaluation of Temporomandibular Disorders with Tension-Type Headache by Gender (성별에 따른 측두하악장애 환자의 긴장성 두통 양상)

  • This study was designed to evaluate the Temporomandibular Disorders(TMD) with Tension-Type Headache(TTH) by gender. Patients with TMD and/or TTH visited the Department of Oral Medicine, Kyung Hee University Dental Hospital were recruited to this study. Experimental group(n=60) is composed of TMD with TTH and control group(n=111) is composed of TMD without TTH. Evaluation list was pain quality, pain intensity, pain laterality, pain increase by routine physical activity and then it was analyzed statistically. The results were as follows ; 1. In the control group, pain quality was significantly different by gender(p=0.04). But, in the experimental group, pain quality was not significantly different by gender. 2. In the control group, pain intensity was not significantly different by gender. And, in the experimental group, pain intensity was not significantly different by gender. 3. In the control group, pain laterality was not significantly different by gender. And, in the experimental group, pain laterality was not significantly different by gender. 4. In the control group, pain increase by routine physical activity was not significantly different by gender. And, in the experimental group, pain increase by routine physical activity was not significantly different by gender. Therefore, it is considered that not temporomandibular disorder patients with tension-type headache but temporomandibular disorder patients without tension-type headache was influenced by gender in the pain quality.

Pain Management Knowledge, Attitudes, and Performance of Nurses in Long-Term Care Hospitals (요양병원 간호사의 통증관리에 대한 지식, 태도, 수행정도)

  • Purpose: This study investigated long-term care hospital nurses' knowledge and practice of pain management and their attitudes towards the job with an ultimate aim to provide fundamental information for development of a pain education program. Methods: A cross-sectional survey was carried out with 120 nurses from four long-term care hospitals. Nurses' knowledge of and attitudes towards pain management was measured using a tool developed by Watt-Watson. To examine their pain management practice, an instrument was developed based on the pain management guidelines used by the long-term care settings. Results: For pain management knowledge, the participants gave an average of 26.2 (${\pm}13.10$) correct answers out of 40 questions. The most frequently missed question was one about subjectivity of pain, "Patients' physiological and behavioral reactions to pain hint at the presence and intensity of pain" (89.2%), and 56.7% of the nurses believed that increasing tolerance for a narcotic analgesic means addiction. Regarding attitudes towards and practice of pain management, 80.2% of the nurses used placebos to patients who complain about pain often. Conclusion: This study revealed poor level of pain management knowledge of and attitude among long-term care hospital nurses. This might negatively affect their pain management practice. From the educational perspective, nurses should be provided with education on pain management education with emphasis on the nature of pain and misuse of placebo drugs.

Exploratory studies of the music analgesic effect in people with glasses through cold-pressor task (안경 착용 여부에 따른 음악 통증완화효과의 탐색적 연구)

  • The analgesic effects of music in people with glasses on perceived pain through cold-pressor task (CPT) is assessed based on three-sequence, three-period, crossover trial with three treatment conditions(music-listening, news-listening, and no-sound) to each subject. Fifty subjects are divided into three sequence groups by randomization, and CPTs under the pre-assigned treatment conditions at each period are performed. Pain responses after each CPT, subjects' pain tolerance (PT) in time scale and pain intensity (PI) and pain unpleasantness (PU) in visual analog scale (VAS) are measured. After classifying the group by whether or not to wear glasses, which is the phenotype of the myopia gene, pain responses are compared by F-tests and Tukey's multiple comparisons. CPT pain responses in group with glasses during the music intervention are significantly different from responses during the news intervention and the control conditions, respectively. This study investigates the pain responses of music intervention in the group wearing glasses, which can be seen as a phenotype of the nearsighted gene, and this result would play a role in explaining the biopsychosocial model of the pain mechanism.

Percutaneous Radiofrequency Therapy of Benign Bone Tumors in the Femoral Head (대퇴골두 부위에 발생한 양성 골 종양에 대한 경피적 고주파치료 (증례보고))

  • Purpose: To report two cases of bone tumors other than osteoid osteoma in the proximal femur and treated with percutaneous high frequency radioablation method. Cases: We reviewed two cases with intracortical chondroma and enchondroma in the femoral head retrospectively. The patient with intracortical chondroma was a thirty one year old woman and had suffered right hip pain of 1 year duration. The lesion was located in the head of right femur and treated with CT guided percutaneous high frequency radioablation after needle biopsy under general anesthesia. The symptom was gone immediately after the procedure and was discharged postop. 1 day. 15 months has passed without symptom recurrence. Second case having enchondroma, was 56 year old woman complaining of gluteal area pain for 3 months. Radiologic evaluation showed osteolytic lesion with sclerotic rim on the inferior portion of the left femoral head. She received a same therapy with CT guided radiofrequency ablation following needle biopsy. She reported dramatic pain relief after the procedure and was discharged postop. 1 day. No symptom has occurred for 3 months until now. Conclusion: We present 2 cases of bone tumor occurred in the hip joint area other than osteoid osteoma which were treated with CT guided radiofrequency ablation.
