전방 탈구인 경우 첫탈구이거나 생애에 몇번만 탈구를 경험하고 일상생활에 별 지장이 없다면 보존적으로 치료하는 것이 바람직하다. 다만 early surgey가 요구되는 경우는, 첫탈구가 teen aged이면 관절경적 수술을, 골편이 큰 Bankart병변이 있는 첫탈구는 수술하는 것이 좋다. 전방 불안정성인 경우, collision sports는 개방술로, non-athletes이거나 throwing 또는 contact sports는 관절경술로 시행해 주는 것이 바람직하다. 우리가 명심하여야 할 것은 전방 불안정성을 다루는데 환자의 입장에서 서야 한다는 것이다. 의사의 능력 정도나 욕망보다도 환자의 상태, 활동 정도와 환자의 요구에 맞추어 환자를 치료하여야 한다.

A traumatic dislocation of the sternoclavicular joint is a rare injury, and among them, anterior dislocation is more common than a posterior dislocation. Posterior dislocation is a potential risk by compressing the mediastinal structures, but an anterior dislocation has not been considered a risk. Traumatic sternoclavicular joint anterior dislocation associated with anterior angulation of a sternal fracture can develop mediastinal compression and have a risk in the same way as a posterior dislocation. This case report is about a traumatic sternoclavicular joint anterior dislocation with a sternal fracture accompanied by mediastinal compression that was treated surgically using a plate and showed relatively good clinical results. This rare case is reported along with a review of the relevant literature.

Volar rotatory dislocation of the proximal interphalangeal joint(PIP) of the finger is rare. We report a female judo player who had volar rotatory dislocation of the PIP joint of the middle finger. She had dislocation of PIP joint total 4 times. At operation, the central tendon was identified as being distension, with the ulnar collateral ligament ruptured. The ruptured ulnar collateral ligament was interposed within the joint. The ruptured ulnar collateral ligament was repaired and extensor expansion was repaired. At last follow-up, she didn't have recurrent dislocation during judo. Accurate diagnosis, early intervention and progressive rehabilitation of this injury are very important as other dislocations.

It is rare that the complete avulsion, intrusive luxation, and extrusive luxation occurred by trauma at the same time. In July, 2003, 10-year and 2 month-old-girl was referred to Department of Pediatric dentistry, Chonnam National University Hospital due to complete avulsion of upper right lateral incisor, intrusive luxation of upper right cental incisor and extrusive luxation of upper left central incisor. Traumatized teeth were surgically repositioned and fixated with resin-wire splint at the same day. Endodontic treatment of avulsed tooth was performed 2 weeks after trauma. A radiograph was taken 3 months after trauma, which demonstrated root resorption of both upper central incisors. External root resorption was arrested by the root canal therapy with calcium hydroxide. It showed good results for 1 year and 4 month but further follow-up is needed to check root resorption and ankylosis.

외상성 전방탈구의 경우 일차성 탈구이거나 일상생활에 큰 불편을 호소하지 않는 불안정성이 있을 경우 및 비외상성 다방향 불안정성의 경우 보존적 치료가 바람직하며, 10대 및 20대의 재탈구로 인한 불안정성이 유발될 경우 관절경적 혹은 개방성 Bankart 술식을 통하여 관절순의 해부학적 정복 및 술 후 잘 조절된 재활치료를 시행할 경우, 좋은 결과를 얻을 수 있다. 치료 방법의 선택 시 가장 중요한 요소는 환자의 원하는 결과를 만족 시켜줄 수 있는 치료 방법을 선택하는 것이며, 이는 불안정성의 원인, 정도, 탈구의 횟수, 환자의 활동도 및 재활 의지등을 종합적으로 감안하여 결정되어야 한다.

Dislocation of the temporomandibular joint (TMJ) occurs when the mandibular condylar head is displaced completely out of the glenoid fossa and cannot be reduced by the patient. The occurrence of bilateral anterior dislocation is the most common. Dislocations can be classified into three types in terms of duration and frequency of dislocation, i.e., acute, chronic and recurrent. There are various treatment modalities for dislocation from conservative try to surgical intervention. The selection for the appropriate modality mainly depends on the types of dislocation as previously stated. The authors report three cases of dislocation with different treatment modalities according to the duration of dislocation. In particular, we tried prosthetic approach instead of surgical intervention in the patient with chronic dislocation.

견관절 전방 탈구의 치료 방법을 결정하는데 있어서 중요한 점은 탈구를 유발한 병태 생리에 대한 충분한 숙지와 함께 적절한 환자의 선택과 치료 결과에 대한 환자의 기대치를 파악하여 그에 맞는 치료를 선택하는 것이 중요하다. 탈구의 횟수에 있어서 첫 탈구이거나 아주 적은 횟수의 탈구로 환자가 일상 생활에 어려움을 호소하지 않는다면 보존적 치료를 시행해야 할 것이며, 수술적 치료의 경우에 환자의 병변 상태나 그 원인 인자에 대한 평가가 정확히 이루어진 후 시행되어야 한다. 환자의 기대치는 일반인의 경우 수상 이전의 활동도의 회복보다는 안정성이 중요할 것이며, 운동 선수인 경우에는 안정성이 회복 되었더라도 운동에 다시 복귀하지 못한다면 만족도는 매우 떨어질 것이다. 이렇듯 의사의 능력이나 욕망 보다는 환자의 활동 정도와 견관절의 병적 상태, 환자의 요구에 맞는 치료방법을 선택해야 할 것이다.

Unstable pelvis ring trauma can accompany urologic injuries such as bladder, ureter, or urethral injuries. In very rare cases, testicular dislocation can accompany a pelvic ring injury. Testicular dislocation causes pain, but can be regarded as the pain caused by pelvic ring injury. If not detected early, testicular dislocation may result in gangrenous testes. The authors report a case of pelvic ring injury with testicular dislocation.

A 7-year-old, intact female Golden Retriever weighing 38 kg was referred for left coxofemoral joint luxation. On physical examination, pain and crepitus were noted atthe left hip joint during joint extension. Radiological examination revealed coxofemoral joint luxation and mild degenerative bone changes in the left hip joint. We performed minimally invasive arthroscopy-assisted reduction with TightRope$^{(R)}$ as a treatment option; however, the left coxofemoral joint was reluxated after one month postoperatively. We performed cementless total hip replacement (THR) of the luxated left coxofemoral joint. At tenmonths after surgery, the dog could sit, stand, walk normally and jump comfortably without lameness. The thigh girth measurement of the operated limb was 108.6% of the contralateral limb. Based on the present case, THR can be a successful treatment for coxofemoral joint luxation in dogs with failed coxofemoral joint reduction.

Hill-Sachs lesion is the most common lesion in shoulder dislocation patient and it represented by postero-lateral bony defect of humerus. The lesion could be found in patients who dislocate shoulder joint first time, and it is reported that Hill-Sachs lesion proportionally increases as dislocation does. The location and presence of Hill-Sachs lesion play significant roles in shoulder instability. Authors experienced a double Hill-Sachs lesion in a recurrent anterior shoulder dislocation patient, and thereby report the case with a review of the literature.
