The effect of copper alloy scaler tip on the surface roughness of dental implant and restorative materials (구리 합금 초음파 스케일러 팁이 치과 임플란트 및 수복 재료 표면에 미치는 영향)

  • Purpose: This study is designed to investigate the various impacts of different types of scaler tips such as cooper alloy base tip and the others on the surface roughness of teeth and implant by the method which is currently in clinical use. Materials and methods: Four different types of disc shaped porcelain, titanium, zirconia, and Type III gold alloy dental materials sized 15 mm diameter, 1.5 mm thickness were used for the experiment. Plastic hand curette (Group PS), cooper alloy new tip (Group IS), and stainless steel tip (Group SS) were used as testing appliances. A total of 64 specimens were used for this study; Four specimens for each material and appliance group. Surface roughness was formed with 15 degree angle in ultrasonic scaler tip and with 45 degree angle in hand curette of instrument tip and the specimen surface with 5 mm long, one horizontal-reciprocating motion per second for 30 seconds by 40 g force. To survey the surface roughness of each specimen, a field emission scanning electron microscope, an atomic force microscope, and a surface profiler were used. (Ra, ${\mu}m$). Results: According to SEM, most increased surface roughness was observed in SS group while IS groups had minimal roughness change. Measurement by atomic force microscope presented that the surface roughness of SS group was significantly greater than those of PS, IS and control groups in the type III gold alloy group (P<.05). IS group showed lesser surface roughness changes compared to SS group in porcelain and gold alloy group (P<.05). According to surface profiler, surface roughness of SS group showed greater than those of PS, IS and control groups and IS group showed lesser than those of SS group in all specimen groups. Type III gold alloy group had large changes on surface roughness than those of porcelain, titanium, zirconia (P<.05). Conclusion: The result of this study showed that newly developed copper alloy scaler tip can cause minimal roughness impacts on the surface of implant and dental materials; therefore this may be a useful alternative for prophylaxis of implant and restored teeth.

Scanning electron microscopic study on characteristics of tooth surface when using gracey curet and ultrasonic scaler (Gracey curet과 Ultrasonic scaler 사용 시의 치면 특성의 주사현미경적 연구)

  • Objectives: The purpose of this study is to assess the characteristics of tooth surface after using Gracy curet and Ultrasonic scaler Methods: In this study, 12 teeth extracted were used. 12 specimens were divided into three groups with the same numbers, which were classified into Control group, Gracy curet use group, and Ultrasonic scaler use group, and after performing instrument operation, we measured the roughness and the loss degree of tooth surface by using SEM. Results: In groups using Gracy curet and Ultrasonic scaler, the roughness and the loss of tooth surface increased significantly(p<0.05). In the roughness of groups using Gracy curet and Ultrasonic scaler, Ultrasonic scaler group was higher in crown, but Gracy curet group was higher in root. As a result of observation through SEM, the roughness and the loss degree increased in order of Control group, Ultrasonic scaler use group, and Gracy curet use group. Conclusions: Taken together above results, both hand instrument and ultrasound equipment create roughness and loss in crown and in root, and hand instruments makes rougher than ultrasonic instruments in root, so it is thought to require thorough and accurate technical application not to damage tooth surface when removing plaque.

Comparing the efficiency of periodontal instrument sharpening using aluminum oxide stones with different levels of roughness (다양한 거칠기의 알루미늄 옥사이드(Al2O3) 연마석을 이용한 치주기구 날 세우기의 효율성 비교)

  • Purpose: This study examined the efficiency and wear of periodontal instrument sharpening using aluminum oxide stones with different levels of roughness. Materials and Methods: Thirty new No. 9-10 Gracey curets were used in this study. All curets had become dull after scaling and root planing. After similar blunting, the instruments were divided randomly into three groups (240, 600, 800 grit) containing 10 curets each. The stones were applied correctly to the lateral surface of each curet to maintain the $70-80^{\circ}$ angle. After resharpening, sharpness of the curets was examined by an optical microscope. After 20, 40, 60, and 80 strokes, the wear was measured at 1 mm and 2 mm from the tip of the cutting edge using a digital caliper. The data was analyzed statistically using analysis of variance (ANOVA) with repeated measures, 2-way ANOVA, and a Fisher's exact test. Results: The degree of sharpness increased significantly (P < 0.001) as the number of sharpening strokes grew for all stones. A comparison of the degree of sharpness on the same number of strokes showed that the 240 grit group significantly excelled the other groups on 5 and 10 strokes, respectively (P < 0.001). The mean wear showed no statistically significant difference among the groups (P > 0.05). Conclusion: The efficiency of Gracey curet resharpening was enhanced with more coarse stones, though we should consider the wear of the instrument during resharpening.

Evaluation of Wear of Periodontal Curet's Lateral Surface in Working-end (수종 치주큐렛의 작업측 마모도의 비교연구)

  • The purpose of this study was to evaluate the degree of wear of periodontal curet's cutting edges made by three different manufacturers. In the first case of the experiment, this study was done with each new curett in the following three experiment. Twelve new double-ended No. 11/12 Gracy curettes from three different manufacturers (A, B and C) was randomly selected from our stock at first test. They were weared by wear tester with fixed pressure and limited distance. This study measured the width of worn curettes'cutting edge. The results were as follows ; 1)In 50 times experiment, this study discoverd that each manufacturer's curette was appeared with different degree of wear (p<0.01) 2)In 100 times experiment, this study result similarly to the 50 times experiment(p<0.01). 3)In 150 times experiment, the result of this study were alike as the X50 experiment and X100 experiment. In the second case of the experiment, the study was done with a curett of a manufacturer in the following three experiment. Two double-ended No. 11/12 Gracy curette from three different manufactures (A, B and C) was randomly selected from our stock at second test. 1)In 50 times experiment, this study discovered that each manufacturer's curette was appeared with different degree of wear (p<0.05). 2)In 100 times experiment, the result was same, compared with the 50 times experiment. 3)In 150 times experiment, this study also discovered that the result was same with the result of the upper two cases. In conclusion, this study discovered that the two cases of experimental were shown with a same result.

Electrocautery as Adjuvant Treatment of Giant Cell Tumor (거대세포종의 보조 요법으로의 전기소작술)

  • Purpose: This study was designed to investigate the clinical and oncological results of giant cell tumor, treated with electrocautery as an adjuvant. Materials and Methods: We evaluated 47 giant cell tumors treated in both institution from 1989 to 2004, retrospectively. We treated all 47 cases with intralesional curettage and speed burring and followed electrocautery as an local adjuvant. Results: The mean follow up periods was 44 months and ranged from 12 to 180 months. The local recurrence occurred in 8 patients (17%) and one case lung metastasis was found. There was one case complication (septic knee) associated with electrocautery as an adjuvant. Conclusions: We thought electrocautery was feasible adjuvant treatment method of giant cell tumor, because it is very convenient technique, and the local recurrence rate was similar to any other methods.

The Effect of a Piezoelectric Ultrasonic Scaler with Curette Tip on Casting Gold Removal in Vitro (큐렛팁을 장착한 압전방식 초음파치석제거기의 작업조건에 따른 치과주조용 합금의 삭제에 관한 연구)

  • Periodontal debridement is most important procedure in periodontal treatment, because periodontal disease is the biofilm infection. The use of ultrasonic instrument has many clinical advantages compared to classical hand instrument. The introduction of newly developed ultrasonic scaler tips made the use of ultrasonic scaler popular. However the study of tooth substance removal according to the working parameters of ultrasonic scaler with newly developed tips is not sufficient. The purpose of this study is to evaluate the effects of working parameters of piezoelectric ultrasonic scaler with curette tip on casting gold removal. The working parameters was standardized by the sledge device which controls lateral force(0.5 N, 1.0 N, 2.0 N) and power setting was adjusted 2, 4, 8 in P mode and S mode and instrumentation time was 5 seconds. The defect depth and width were measured with profile meter and defect surface was examined by SME. The depth of defect was significantly large in S mode( $39.58{\pm}19.35{\mu}m$) compared to P mode( $8.37{\pm}6.98{\mu}m$). There was significant decrease of depth of defect between 1.0N($32.87{\pm}27.18{\mu}m$) and 2.0N( $14.86{\pm}15.04{\mu}m$). The area of defect was also significantly large in S mode($4482.42{\pm}3551.71{\mu}m^2$) compared to P mode( $922.06{\pm}960.32{\mu}m^2$). There was significant decrease of area of defect between 1.0N($3889.12{\pm}3936.00{\mu}m$) and 2.0N( $974.66{\pm}986.01{\mu}m$). The change of mode did not effect on the width of the defect. The change of power setting did not effect on the depth, width, and area of defect. In spite of limitation of this study it could be concluded that the use of piezoelectric ultrasonic scaler with curette tip on S mode could make significant tooth substance loss.
