경견부 근막통증증후군에 대한 침치료 효과와 압통역치의 변화 (Effects of Acupuncture on Upper Back Myofascial Pain and Pain Pressure Threshold)

  • Objective : This study was performed to confirm the effects of acupuncture on myofascial pain syndrome(MPS) through the change of visual analogue scale(VAS) and pain pressure threshold(PPT) and the usefulness of pressure algometer on the evaluation of pain. Methods : We perfomed this study with 20 outpatients complaining of upper back pain. Before acupuncture therapy(AT), immediately after AT and 2-3 days after AT, we respectively checked visual analogue scale(VAS) and pain pressure threshold(PPT) through pressure algometer, with patients seated and relaxed. The PPT was checked at major trigger point of upper trapezius, levator scapulae, supraspinatus, infraspinatus, rhomboideus minor. and the patients were needled at the same points and maintained for 15 minutes. Results : VAS of immediately after AT was mild higher than that of before AT, but not significantly different. and VAS of 2-3 days after AT was significantly lower than before AT and immediately after AT. PPT of immediately after AT was lower than before PT, but not significantly different. PPT of 2-3 days after AT was significantly higher than that of before AT and immediately after AT. Also PPT was significantly correlated with VAS. Conclusion : PPT of omen was signicantly lower than that of men. and there was no significant difference by age. PPT was increased according to pain duration. Effectiveness of acupuncture on myofascial pain syndrome through PPT and VAS is showed at 2-3 days after AT rather than immediatly after AT. and pressure algometer is useful for the evaluation of Acupuncture therapy on myofascial pain syndrome.

말초성 안면신경마비에 대한 평형침 병행치료 효과의 대조군 연구 (A Controlled Trial on the Effect of Complex Oriental Medical Treatment with or without Pyung-Hyung Acupuncture on the Treatment of Peripheral Facial Palsy)

  • Objectives : The aim of this study was to observe the effect of Pyung-Hyung acupuncture on peripheral facial palsy. Methods : We investigated 44 cases of patients with peripheral facial paralysis, and divided patients into two groups. We treated one group by complex oriental medical treatment with Pyung-Hyung acupuncture, and the other group by complex oriental medical treatment without Pyung-Hyung acupuncture. To evaluate the effectiveness of treatment applied for two groups, we used Yanagihara's unweighed grading system(Y system) and gross grading system of House-Brackmann(HB score) at baseline and final. Results : 1. The final Y-system in both the Pyung-Hyung acupuncture group and the control group significantly increased compared to the value at baseline. 2. At final, Pyung-Hyung acupuncture group showed significantly increase on Y-system compared with control group. Conclusions : Pyung-Hyung acupuncture can be used for relieving symptoms related with peripheral facial paralysis.

마미증후군 환자의 침, 봉약침, 한약 복합치료 1례 (A Case of Cauda Equina Syndrome Cared with Acupuncture, Sweet Bee Venom Pharmacopuncture, Herbal Medicine Combined Treatment)

  • Objectives : This study was done to determine the effect of Korean medical treatment to patient diagnosed as cauda equina syndrome(CES) suffering from both lower limb dysesthesia, saddle dysesthesia, buttock dysesthesia, dysuria, dyschezia and gait disturbance. Methods : A patient with CES was treated with acupuncture, sweet bee venom pharmacopuncture, herbal medicine from February 2 to April 9 at Department of Acupuncture & Moxibustion, Daejeon Korean Hospital of Daejeon University. Patient received acupuncture treatment($CV_1$, $CV_3$, $CV_4$, $CV_6$, $ST_{36}$, $BL_{31}$, $BL_{32}$, $BL_{33}$, $BL_{34}$) 12 times a week, bee-venom pharmacopuncture($BL_{23}$, $BL_{28}$) was conducted twice a week, herbal medicine(Samilsingihwan decoction) was applied to patient thrice a day for nine weeks. The following symptoms were observed saddle, and lower limb dysesthesia, dysuria, dyschezia and gait disturbance. Results : After treatment, dysesthesia, dysuria, dyschezia and gaiting developed good outcome. But anal reflex remained still. Conclusions : Acupuncture, bee-venom pharmacopuncture, herbal medicine combined treatment might be effective to patients with CES.

마라톤 후 발생한 스포츠 손상환자 199례의 응급 침 치료에 대한 증례분석 (A Descriptive Analysis of 199 Marathon Participants with Acute Sport Injuries Treated by Acupuncture in the Prehospital Setting)

  • Objectives : This study aimed to perform descriptive analysis on demographic characteristics and symptom distribution of 199 marathon participants in Pusan, Korea. Methods : Brief medical charts of 199 marathon participants in 11 marathon rallies who had visited the emergency medical support team of Korean Medicine Hospital, Pusan National University were collected. Participants' demographic and clinical characteristics were descriptively analyzed. Results : More than two third of participants who have visited the support team was male(71.9 %) and their mean age was 44.0(years). The main intervention was manual acupuncture with vigorous stimulation techniques. Most of treated symptoms were lower extremity pain(77.0 %). Knee pain was the most frequently recorded symptoms, following the ankle pain. No information on the safety of acupuncture treatments were reported in the records. Conclusions : Symptom distribution of marathon participants who have visited the emergency medical support team of Korean Medicine Hospital was similar to previous literatures of acute injuries of marathon athletes and runners. The role of acupuncture for emergency support care of acute injuries in marathon participants should be further explored in well-designed clinical studies.

봉약침으로 치료한 요추간판탈출증 환자의 임상적 평가 (Clinical Evaluation of Herniation of nucleus purposus patients treated by Bee venom therapy)

  • Objective : The purposes of this study are to evaluate the efficacy of Bee Venom therapy(BV) on HNP(Herniation of Nucleus Purposus) of Lumbar spine by use of Visual Analog Scale(VAS), Pain Rating Scale(PRS) and Digital Infrared Thermographic Imaging(DITI), and to investigate their correlation. Methods : We researched 20 patients who were diagnosed by MRI as having a HNP, and treated them by Oriental medical therapy(including BV) for 4 weeks. The evaluation was peformed twice(admission day and after treatment for 4 weeks), and we compared the results. Results : 1. VAS, PRS and ${\Delta}t$(by DITI) were decreased after BV for 4 weeks significantly(p<0.01). 2. There was significant correlation between VAS and PRS(p<0.05). 3. There was significant correlation between PRS and ${\Delta}t$(p<0.05). 4. There was no significant correlation between VAS and ${\Delta}t$. Conclusions : BV improved HNP subjectively and objectively, and correlation was found between VAS and PRS and between PRS and ${\Delta}t$. Further study is needed for investigating their correlation.

근골격계 통증 환자에서 침 치료 후 심박수 변이도(Heart rate variability) 변화에 관한 연구 (Study on the Change of the Autonomic Nervous System by HRV Test in Musculoskeletal Pain Patients Treated with Acupuncture)

  • Objectives : This study was designed to evaluate the changes of autonomic nervous system by heart rate variability(HRV) test on the musculoskeletal pain patients treated with acupuncture on the Back-su points and the unfixed points, and untreated healthy adults. Methods : This clinical study have been carried out with 64 cases of musculoskeletal pain patients and 26 healthy adults. 35 patients were treated with acupunctureon on the Back-su points(Group I) and 26 patients were treated on the Unfixed points(GroupII), and two groups were tested by HRV at pre and post acupuncture treatment. 26 healthy adults(GroupIII) were tested twice by HRV without any stimulation. Results : In Group I, VAS was decresased $5.83{\pm}1.62$ to $5.17{\pm}1.89$ significantly(p<0.05) after acupuncture treatment, and LF/HF ratio was decreased significantly(p<0.05) as compared with healthy control Group. In GroupII, SDNN & RMSSD were increased singnificantly(p<0.05) as compared with other Groups. in Group I & II, MHRT was decreased significantly(p<0.05) as compared with healthy control Group. Conclusions : These results suggest that acupuncture treatment on the Back-su points was able to reduce the pain, suppress the sympathetic activities and enhance the parasympathetic activities in musculoskeletal patients.

군진의학에서의 침치료 활용을 위한 연구동향 고찰 (A Study on Research for Utilization Acupuncture Treatment in Military Medicine)

  • Objectives Military medicine is the study of research which includes health, sanitation, clinic, and quarantine in military personnel. This study is focused on research the acupuncture treatment of military medicine. Methods The study on acupuncture research for Military Medicine was referred from via KoreaMed, OASIS, The Journal of Korean Military Medical Association, KMbase, KAMJE, NDSL, KISS, KISTI, PubMed, MEDLINE, EMBASE, and The Cochran library. As a result, 18 research papers were found and they were analyzed according to the year of publishment, the titles of journals, the nation of research center, the types of study, the purpose of study, the outcome of measurement, the type of acupuncture, and the assessment of acupuncture effect. And then assessed the risk of bias about RCT, NRCT, prospective cohort studies, and before and after studies. Results The number of the published research papers on acupuncture was increased since 2013. The studies were mainly published in the USA and eight out of 18 were RCT. The purpose of study is mostly for musculoskeletal and psychological disease and medical decision for acupuncture. The type of the acupuncture used is not only traditional acupuncture but also the auricular acupuncture. Even though the research results about the effect of acupuncture were excellent and positive, some of studies had a high risk of bias. Conclusions The studies state about acupuncture treatment as mostly positive and effective, however, further clinical studies are required to obtain objective clinical evidence of acupuncture treatment. Furthermore, future trials would be needed to find out effective acupuncture treatments on military duty.

교통사고로 인한 편타 손상의 침 치료에 대한 임상연구의 국내외 동향 (A Review of Clinical Studies about Acupuncture Therapy for Whiplash Associated Disorder)

  • Objectives This study aimed to research current trends of acupuncture treatment of whiplash associated disorder (WAD). Methods Clinical studies about acupuncture therapy on WAD were searched in 7 databases. The included studies were classified according to their publication dates, methods, interventions, outcome measures. Interventions were analyzed by accompanied intervention, acupoint, acupuncture retaining time and treatment period. Results 14 articles were included. The number of studies on acupuncture for WAD shows increasing tendency since 1990. The included studies consist of 6 randomized controlled trials (RCTs), 2 non-randomized controlled trials (nRCTs), 2 cohort studies, 1 before and after study, 1 case study and 2 case reports. Most studies used visual analogue scale (VAS) as primary outcome. Conclusions This review demonstrates that further clinical studies need to be carried out under the formal clinical practice guidelines of acupuncture therapy for WAD in order to support clinical treatment objectively.

강직성 척추염의 침 치료에 대한 최근 연구동향 분석: China National Knowledge Infrastructure 검색을 중심으로 (Acupuncture Treatment for Ankylosing Spondylitis Using China National Knowledge Infrastructure: A Literature Review)

  • Objectives This study was performed to research the trends of Chinese acupuncture treatment for ankylosing spondylitis (AS). Methods We searched clinical studies about Chinese acupuncture treatment for AS through China National Knowledge Infrastructure. We analyzed the characteristics of selected studies according to research design, sample size, publication year, treatment methods, evaluation criteria and adverse events. Results 16 studies published from 2015 to 2020 were selected. They consisted of 11 randomized controlled trials and 5 case reports. In these studies, several types of acupuncture treatments were performed and especially there were many treatments that applied thermal stimulation of needles. The most commonly used evaluation criteria was effective rate. Conclusions This study showed that acupuncture treatment could be effective for AS. It suggested that various studies should be conducted to provide reliable evidence about acupuncture treatment for AS in Korea.

국내 교통사고 후유증의 침치료에 대한 체계적 문헌고찰 (Acupuncture for Rehabilitation in Patients with Traffic Accident in South Korea: a Systematic Review)

  • Objectives : This study aims to evaluate current clinical evidence of acupuncture treatment for rehabilitation in patients with traffic accident in South Korea. Methods : Seven Korean databases were searched for prospective clinical trials for acupuncture on rehabilitation in patients with traffic accident from their inception to June, 2009. Only studies conducted in Korean language were searched. Risk of bias in included randomized controlled trials were assessed by Cochrane Handbook procedure. Results : Fifteen clinical trials were included among 31 studies searched. Eight were observational studies, five were non-randomizedcontrolled trials, and two were randomized controlled trials. In all of included studies, acupuncture were conducted with other concomitant treatment. Included studies dealt with such conditions as neck pain, low back pain tinnitus after traffic accident, post-traumatic stress, oculomotor nerve palsy, diplopia and insomnia. All of included studies reported favorable effects of acupuncture group compared to baseline or control group. All of included studies lacked the occurrence of adverse events. High risk of bias were observed in two randomized controlled trials. Conclusions : There is no evidence that acupuncture is effective for rehabilitation of traffic accident. All of included studies lacked appropriate methodological qualities and internal validity. Future welldesigned clinical trials that evaluate the effects and safety of acupuncture treatment for rehabilitation in patients with traffic accident is needed.
