Effects of health behaviors on the periodontal health status of Korea young man: The 6th Korean National Health and Nutrition Examination Survey (한국 청년의 치주건강에 대한 건강행태의 영향- 제6기 국민건강영양조사)

  • Objectives: The aim of this study was to confirm health behaviors that affect periodontal health status. Based on these results, we also intended to provide information for the prevention of gingivitis prior to periodontitis and establish strategies for maintaining healthy periodontal tissues. Methods: Data for this descriptive study were extracted from the Korea National Health and Nutrition Examination Survey 2013-2015. A total of 2,710 adults, aged 19-39 years were included in this study. Complex logistic regression analyses were performed to confirm relationships between health behaviors and periodontal health, after adjusting for socio-demographics, oral and systemic health status. Results: Dental clinic visits (odd ratio [OR] = 1.761, 95% confidence interval [CI]; 1.391-2.229), use of floss (OR = 1.504, 95% CI; 1.213-1.865), and non-smoking (OR = 1.269, 95% CI; 1.040-1.549) were found to support periodontal health. Conclusions: Dental visits for continued professional oral health care, self-oral care including interdental flossing, and a non-smoking habit, are necessary for periodontal health maintenance. Additionally, the results suggest that physical activities such as regular walking can help. These methods should be considered as the primary preventative care strategies for minimizing the occurrence of gingival inflammation.

The effects of dietary protein intake and quality on periodontal disease in Korean adults (한국 성인의 단백질 섭취량과 식생활의 질이 치주질환에 미치는 영향)

  • Objectives: This study aimed to examine the effects of dietary protein intake and quality on periodontal disease in Korean adults. Methods: The data used for analysis were obtained from the 7th Korean National Health and Nutrition Examination Survey (2016-2018). Data were analyzed using chi-square and t-test. Additionally, multiple logistic regression analysis was performed to assess the association between dietary protein intake and quality and periodontal disease. Statistical significance level was set at <0.05. Results: Multiple logistic regression analysis of dietary protein intake and periodontal disease in the model adjusted for socioeconomic factors showed that were significantly related to the Q1 (odds ratio [OR]: 1.18, 95% confidence interval [CI]: 1.01-1.39). However, this correlation was not significant in the model in which all variables were corrected. Moreover, analysis of the dietary protein quality and periodontal disease in model 4, which was adjusted for socioeconomic variables, showed that were significantly related to the low score (OR: 1.13, 95% CI: 1.00-1.27). Conclusions: The results showed a significant association between periodontal disease and poor intake and quality of dietary protein in the Korean adult population.

Changes in the number of P. gingivalis bacteria before and after professional periodontal prevention care in periodontal disease patients (전문가 치주 예방관리 전·후 치주질환자의 P. gingivalis 세균 수 변화)

  • Background: This study aimed to investigate the changes of the amount of P. gingivalis in saliva by professional preventive treatment of periodontal disease by dental hygienists. Methods: A total of 109 subjects participated in this study with informed consent. The control group (38 subjects) performed oral hygiene management individually. The periodontal treatment group (35 subjects) underwent root planning once every 3 weeks for a total of 4 times. The professional periodontal prevention group (36 subjects) underwent interdental cleaning and professional tooth brushing once every 3 weeks for a total of 4 times. Paired T test and analysis of variance were performed to compare the difference among the groups in the amount of P. gingivalis. Results: The copies of P. gingivalis in the professional periodontal prevention group decreased from 773.62±1,198.09 to 241.40±430.40 after treatment significantly. The control group decreased from 525.22±582.54 to 244.29±385.88 after treatment. The periodontal treatment group showed insignificant change of P. gingivalis. Conclusions: This study showed the professional periodontal prevention was more effective than periodontal treatment in decrease of P. gingivalis.

The change of PHP index and copy numbers of periodontal bacteria in patients with professional periodontal prevention management (PHP INDEX에 따른 전문가 치주예방관리 환자의 치주질환 원인균 copy 수 변화)

  • Background: By regulating the factors that contribute to oral diseases in a healthy way, oral health can be maintained and prevented. Methods: The general characteristics, PHP index, oral health behavior, and clinical periodontal index of each group were calculated by frequency analysis, and a cross-analysis (χ2 test) was conducted to assess the homogeneity of the general characteristics, oral health behavior, and clinical periodontal index of the study subjects. An Oral bacteriological examination was performed by gargling with saliva. Results: The expert periodontal prevention group showed a decrease in the copy number of periodontal disease causative bacteria, and A. actinomycetemcomitans, P. gingivalis, T. forsythus, andT. denticolashowed a significant difference before and after treatment (p=0.021). In the periodontal treatment group, A. actinomycetemcomitans, P. gingivalis, T. forsythus, andT. denticolaall showed a decrease in copy number, but there was no significant difference. Conclusions: This study showed professional periodontal prevention management had some effect on periodontal bacterial reduction.

Reliability evaluation of periodontal disease oral health information on YouTube (동영상 공유 플랫폼에서 치주질환 구강건강정보의 신뢰도 평가)

  • Objectives: The study aimed to assess the form, reliability, and quality of information related to periodontal diseases shared on YouTube. Methods: On october 23, 2023, we conducted searches on YouTube using four keywords related to periodontal diseases. The searches retrieved a total of 394 videos from the first five pages for each keyword. Of these, 151 were included in the analysis. Videos were categorized based on the source and usefulness of the information, and their reliability and quality were assessed. Results: Analysis of the information sources revealed that medical websites or TV channels (45.7%) were the most prevalent, whereas commercial websites (2.0%) were the least prevalent. Videos uploaded by physician or hospitals (3.08±0.48) and medical websites or TV channels (3.01±0.94) demonstrated higher reliability. Videos categorized as having good and excellent quality were predominantly from physician or hospital sources (88.2%). Useful information related to periodontal diseases was available in 85.4% of videos, with physician or hospital sources contributing the highest proportion of useful information (52.7%). Conclusions: Videos from physician or hospital sources provide high-quality useful and reliable information. To ensure the provision of more useful and accurate information, the involvement and interest of experts are deemed essential.

Pontic site development with an implant submergence technique for unaesthetic implant in the anterior maxilla (상악 전치부 임플란트의 비심미성 개선을 위한 임플란트 침수(submergence)를 동반한 치조제 증대술)

  • Improving implant esthetics is very difficult, especially in cases where unaesthetic problems are related to implants in the maxillary anterior dentition. A 69-year old male patient was referred by a prosthodontist for periodic pus discharge and an unaesthetic implant prosthesis (maxillary right lateral incisor). The implant was placed too deeply and showed soft tissue volume deficiency and a long clinical crown. After a clinical and radiographic examination, implant submergence and alveolar ridge augmentation were performed to enhance the aesthetics instead of an explantation. The treatment plan was as follows: extraction the adjacent teeth with tooth mobility, secondary caries, and poor prognosis; placement an additional dental implant with hard and soft tissue grafting; fabrication a fixed bridge using implant abutments. A fixed esthetic prosthesis using implants was fabricated, and the patient was satisfied with the prosthesis. A ridge augmentation with implant submergence may be an alternative for solving the problems of unaesthetic implant restorations in the esthetic zone.

3D analysis of soft tissue around implant after flap folding suture (Flap folding suture를 활용한 판막의 고정에 따른 임플란트 주변 연조직 3차원 부피 변화 관찰)

  • Purpose: The various suture techniques can be utilized in order to maximize the keratinized tissue healing around dental implants. The aim of this study is to compare the soft tissue healing pattern between two different suture techniques after implant placement. Materials and Methods: 15 patients with 18 implants were enrolled in this study. Simple implant placement without any additional bone graft was performed. Two different suture techniques were used to tug in the mobilized flap near the healing abutment after paramarginal flap design. Digital intraoral scan was performed at baseline, post-operation, stitch out, and 3 months after operation. The scan data were aligned using multiple points such as cusp, fossa of adjacent teeth, and/or healing abutment. After subtracting scan data at baseline with other time-point results, closed space indicating volume increment of peri-implant mucosa was selected. The volume of the close space was measured in mm3. The volume between two suture techniques at three time-points was compared using nonparametric rank-based analysis. Results: Healing was uneventful in both groups. Both suture technique groups showed increased soft tissue volume immediately after surgery. The amount of volume increment significantly decreased after 3 months (P < 0.001). Flap folding suture group showed higher median of volume increment than interrupted suture group after 3 months without any statistical significance (P > 0.05). Conclusion: After paramarginal flap reflection, the raised flaps stabilized by flap folding suture showed relatively higher volume maintenance after 3-month healing period. However, further studies are warranted.

Discussion of Neurologic Factor Influencing on Chewing Ability of Implant (임플란트 저작능에 영향을 주는 신경학적 원인에 대한 고찰)

  • Many researches have been published about the numerous factors related to the chewing ability of implant prosthesis. Most respective studies have concluded that the chewing ability of implant prosthesis is mostly fine compared to other type of prosthesis. However, some patients are not satisfied with their chewing ability of implant prosthesis. Therefore the neurologic factor, one of the factors related to dissatisfaction of chewing ability was reviewed in this study to understand the mechanism of action of mastication. Data was searched using the keywords; 'implant chewing ability, masticatory ability' in Pubmed database and reviewed. Definitions of chewing ability, factors of chewing ability are reviewed and the neurologic factor, one of the factors influencing on chewing ability, is reviewed. Mechanoreceptor of Periodontal ligament(PDL) is providing the mastication information to brainstem. Due to the absence of mechanoreceptions of PDL in implant, masticatory ability is decreased especially when chewing hard food. Masticatory muscles and mechanoreceptor in TMD may compensate the lack of mechanoreceptor of PDL in implants. Furthermore sensitivity of nerve fiber around peri-implant tissues may support the mechanoreception and sensory reaction in the implant mastication. However, further studies should be conducted to prove the relationships between neurologic factors and mastication.

Change in Periodontal Status by Professional Toothbrushing, and Patients' Satisfaction (전문가잇솔질에 의한 치주상태 변화와 환자만족도)

  • The purpose of this study was to examine the effects of the toothpick method, one of professional toothbrushing methods, on the prevention of periodontal diseases and the satisfaction level of patients with that. The subjects in this study were 33 patients who included 16 men and 17 women. After the toothpick method was applied to the selected patients from January 9 to February 28, 2008, the collected data were analyzed with SPSS (Statistical Package for the Social Science) 12.0 program. The findings of the study were as follows: First, the patients investigated showed a 0.71 reduction in PI than in the past. The index of bleeding upon probing stood at 5.12, which dropped from 8.09 in the beginning. Second, the patients gave a mean of 4.20 to the professional toothbrushing out of possible five points, which showed that they expressed satisfaction with that. Third, as for the relationship between general characteristics and satisfaction level, the women were more satisfied with that ($4.25{\pm}0.33$). By age group, those who were in their 60s and up found that more satisfactory ($4.31{\pm}0.30$). Fourth, as to changes in oral symptoms by gender, the largest number of the men and women considered their gums to become healthier after the professional toothbrushing was applied. The above-mentioned findings suggested that dental institutions should be equipped with well-educated oral health care personnels who are responsible for the periodontal health of patients, and the development of oral health promotion programs is urgently required as well.

Evaluation of Periodontal Status according to Sociodemographic and Health Behavior Characteristics (인구사회학적 특성 및 건강행위 특성에 따른 치주상태 평가)

  • This study was carried out in order to offer basic data available for being helpful to promoting periodontal health, by analyzing effects in socio-demographical and health-behavior characteristics on index of evaluating periodontal status. The study conducted a survey for the 151 adults aged 30 to 59 from October. 1, 2006 to March. 30, in 2007. The collected data were analyzed with t-test and one-way of variance using SPSS 12.0. Then, the following result were obtain. 1. sociodemographic and health-behavior characteristics is not related to O'Leary plaque index, Loe & Silness gingival index. 2. The number of sextants possessing periodontal pocket was higher in men(3.34) than women(2.43), also statistically significant difference in the occupational fields. Smoker(3.38) was higher than non-smoker(2.72), and it was higher over 3 times a week than almost no drinking as for frequence alcohol intake. 3. The amount of loss for inter-dental alveolar crest was higher in men(71.79) than women(42.24), was highest of the 'professional' in the occupational fields, and smoker(67.23) was higher than non-smoker(48.22). 4. There was significantly difference in the number of missing tooth between age and frequence of alcohol intake. These results that occupational fields, smoking and alcohol intake were related to the number of sextants possessing periodontal pocket and amount of loss for inter-dental alveolar crest.
