THE MANAGEMENT OF REGIONAL ODONTODYSPLASIA OF PERMANENT TEETH IN CHILDREN (어린이 영구치에 나타난 국소적 치아 이형성증의 관리)

  • Regional odontodysplasia is a relatively rare developmental anomaly of dental hard tissue with characteristic clinical, radiographic and histologic features. It requires a continuous and multidisciplinary approaches, and the aim of treatment for these patients should include aiding mastication, improving aesthetics, maintaining normal vertical dimension and space, allowing normal jaw growth and eruptional management of affected teeth. This report describes three cases of regional odontodysplasia with 2-5 years of follow-up. Conservative treatment is chosen to preserve the affected teeth as long as possible, and periodic radiographic and clinical examination was done. During this time, all teeth except one showed progressive development. An interesting finding observed in our cases was that each tooth even in the same person showed different degree of tooth development and eruption rate. Thus, we colcluded that the treatment plan for regional odontodysplasia should be conservative and individualized and based on the assessment of each tooth.

The evaluation of rotational movements of maxillary posterior teeth using three dimensional images in cases of extraction of maxillary first premolar (3차원 영상을 이용한 상악 소구치 발치공간 폐쇄 후 상악 구치부 회전양상의 평가)

  • Cephalometric superimposition is unable to evaluate tooth movement along the occlusal plane and the errors of photographing and superimposition of the occlusogram is also inevitable. The purpose of the present study was to evaluate the rotational movements of the maxillary posterior teeth following space closure after extraction of maxillary first premolars. using 3D dental images. In 19 adult females. analysis of rotational movements of the maxillary posterior teeth was performed between the initial and final 3D dental images superimposed on the maxillary hard palate. The results showed a diversity of rotational movements of posterior teeth. The causes of various rotational movements may be different rotation of molars in each case at the beginning of treatment. individual characters of the dental arch and tooth morphology. The results of the study indicate that accurate evaluation of rotational movements of posterior teeth following space closure after extraction of premolars using 3D dental images can be obtained through the subdivision of tooth movement during the initial aligning, space closure and finishing stages.

Reestablishing the occlusal plane in full mouth rehabilitation patient, using Shilla system (전악수복환자에서 Shilla system을 이용한 교합평면 재구성 증례)

  • Occlusal plane is a sagittal expression of dental arch form, and it composes the shape of occlusion, which is one of the most important elements of Maxillo-oral system. In this case, vertical, horizontal coordinates of bionic-median-sagittal plane was produced in articulator, and to achieve relation of left and right position of upper, lower teeth and deficits in alveola, Shilla system was used to reconstruct occlusal plane. In this case, a 41 year-old male patient visited for fracture of 10 unit metal-ceramic fixed partial denture of upper anterior teeth and for overall treatment. Clinical, radiographical, model examination was held, full mouth rehabilitation was achieved by placing dental implant. Maxillo-oral relation was recorded using Gothic arch Tracer complex and were mounted. And for the next step, we estimated original occlusal plane using Shilla system. After analysis we produced diagnosis wax pattern. On the basis of this, radiography stent was manufactured and dental implant was placed, and temporary prosthesis was made by using diagnosis wax pattern. Cross mounting and anterior guiding table were performed in order to reproduce temporary restoration morphology and bite pattern, followed by final restoration made of all ceramic crown with zirconia coping. As stated above, appropriately esthetic and functional results can be seen in using Shilla system in diagnosis and treatment procedure of full mouth rehabilitation patient.

MANDIBULAR DIFFERENTIAL PREMOLAR EXTRACTI0N IN GROWING PATIENTS (성장기 환자에서 하악의 차등적 소구치 발치)

  • The extraction lot orthodontic treatment can be adopted for aligning crowded dentition, improving facial esthetics and solving a skeletal discrepancy as alternative for a surgical option. Mandibular second premolar extraction was often selected as treatment plan when there we very little or no space shortage in lower arch or limited retraction of the lower incisors was required. The primary object of this study was evaluate a pretreatment condition and examine the amount of tooth movement ior a mandibular second premolar extraction in growing patients. Pretreatment and posttreatment lateral cephalograms of 14 cases that had their four first premolar extracted (4/4 group), 15 cases with upper first and lower second premolar extraction (4/5 group) were selected. Structural method superimposition was conducted to evaluate a difference of dental change between 4/4 and 4/5 group. The results were as follows, 1. Pretreatment factor for 4/4 extraction or 4/5 extraction choice included maxillary incisor axis to occlusal plane, Class II molar relationship, IMPA and interincisal angle. 2. The amount of molar anterior movement in 4/5 group was greater than that of 4/4 group(p<0.05). 3. There was no significant difference between 4/4 group and 4/5 group in aspects of maxillary tooth movement(p<0.05).

TREATMENT OF HEAVY MANDIBULAR BUCCAL FRENUM USING APICALLY POSITIONED PARTIAL-THICKNESS FLAP IN CHILD (치아 교환기 어린이에서 근단변위부분층 판막술을 이용한 하악 거대 협소대의 처치)

  • The mandibular buccal frenum becomes a problem when its attachment is too close to the marginal gingival. Heavy buccal frenum mucogingivally results in insufficient attached gingiva, inadequate vestibular depth and also difficulty in eruption of mandibular premolar. The aim of this study is to demonstrate the effectiveness of apically positioned partial-thickness flap as adequate method in order to remove heavy buccal frenum and expose impacted teeth. The author presents the case of a 12-year-old boy. He had problems that was prolonged eruption of #34, #35 due to high pull buccal frenum. After space regaining for normal eruption of #34, #35, we performed apically positioned partial-thickness flap on #34, 35 area. As a results, we confirmed that eruption of #34, #35 has been processed normally And vestibular depth, position of buccal frenum and width of attached gingival was within a normal range. Decrease in muscle pull, adequate width of attached gingiva and increased vestibular depth can be expected from this treatment.

Evaluation of Impacted Maxillary Canine Position Using Panoramic Radiographs and Cone-beam Computed Tomography (파노라마 방사선사진과 CBCT를 이용한 매복 상악 견치의 위치 평가)

  • The purpose of this study was to compare the mesio-distal and vertical position of impacted maxillary canines on panoramic radiographs with the labio-palatal position of impacted canines and root resorption of adjacent teeth on CBCT (Cone-Beam Computed Tomography). A total of 54 patients with 69 impacted maxillary canines were included in this study. On panoramic radiographs, the mesio-distal (Sector I - V), and vertical position, and angulation of impacted maxillary canines were evaluated. Labio-palatal position and root resorption of adjacent teeth were evaluated on CBCT. Labial impaction of canines was most frequent in panoramic sector I. Impaction within the arch was most frequent in sector I and II. Palatal impaction was most frequent in sector III, IV, and V. Mesially positioned canines on panoramic radiographs tended to be palatally impacted. Most of the root resorption of adjacent teeth occurred in sector III, IV, and V (p < 0.05). Canines with low angulation on panoramic radiographs tended to be palatally impacted and resorb the roots of adjacent teeth. The vertical position of impacted canines on panoramic radiographs was not statistically significant with respect to labio-palatal position and root resorption of adjacent teeth on CBCT. Analysis of the mesio-distal position and angulation of impacted maxillary canines on panoramic radiographs can be used to predict the labio-palatal position of impacted canines and root resorption of adjacent teeth.

Treatment of the cleft lip and palate patient with few remaining posterior teeth using hybrid telescopic crown denture (치주질환에 이환된 소수 잔존치를 가지는 구순구개열 환자에서의 상악 혼성 이중관 의치를 이용한 수복증례)

  • For individuals with cleft lip and palate, the removable partial dentures (RPDs) have been an important treatment option. Some modifications from conventional prosthetic treatment may be necessary to achieve satisfactory functional and esthetic results in cleft patients. In case of cleft palate patient with periodontally compromised and only posterior few remaining teeth, removable partial prosthesis connected to telescopic crown can be the alternative treatment option. When connected to the RPD, telescopic crowns increase the prosthetic stability and retention, optimize favorable force transmission to the long dental axis, and improve esthetics. And the cross arch stabilization of double crown denture helps to stabilize both divided maxillary ridges in CLP patient. This case present one adult CLP patient using an RPD connected with hybrid telescopic crowns with friction pins to improve not only retention and stability but also aesthetics.

A STUDY OF DENIAL ARCH FORM IN NORMAL OCCLUSION (정상교합의 치열궁형태에 관한 연구)

  • This study was to investigate the fitness of the dental arch line to the parabola, and to estimate correlation between the parabola and some ratios of the dental arch measurements. The sample was consisted of the 64 plaster casts showing normal occlusion which was taken from males and females of Korea, aging from 15 to 18 years. The photos of occlusal surface of the plaster casts were taken, and 38 landmarks on the film were selected. The 3 dental arch lines on each dental arch were imaged. One is the dental arch line passing the points of the most buccal surfaces of the teeth, another is that passing the buccal cusp tips of the posterior teeth and the incisal edges, and the other is that passing the midpoints of the teeth. The landmarks on the film were digitized, and measurements and statistics were performed by the IBM computer. The results were as follows; 1. The fitness of the dental arch to the parabola was very good. The fitness of thor upper dental arch was above $91\%$, and that of the lower dental arch was $93\%$. 2. The dental arch line passing the points of the most buccal surfaces of teeth was best fit to the parabola, the buccal cusp tip arch line and the midpoint arch line in order. 3. Correlation between the ratio of oblique molar dental height to molar width and the parabola was very high. 4. The ratio of oblique molar dental height to molar width was devided into 4 groups by the quartiles, and mean parabolic equations and curves were calculated and drawn.

A STUDY ON SOFT TISSUE FACIAL PROFILE CHANCES IN ORTHODONTIC TREATMENT WITH FOUR PREMOLAR EXTRACTIONS (4개 소구치 발치를 통한 교정치료시 나타나는 안모 연조직 변화에 관한 연구)

  • The purpose of this study was to evaluate the differences of soft tissue profile changes between the growing patients and the adult who had passed the growth peak, in orthodontic treatment with four premolar extractions. The results which was taken by correlating the soft tissue changes with hard tissue changes, lip thickness, molar relationship and arch length discrepancy in both groups was like the followings. 1. Significant hard tissue changes were decrease of VIs, VIi, UlPP, LlMP, HIi and increase of HPog'in adults and decrease of VIs, VIi and increase of VA, VPog'and all the vertical measurements in adolescents. 2. Significant soft tissue changes were decrease of VLs, VLi, and VILS in adults and increase of VSn, VSLS, VLs, VPog' and almost all vertical measurements in adolescents, and ${\Delta}LsE,\;{\Denta}LiE$ in adults and ${\Delta}LsE,\;{\Denta}LiE,\;{\Delta}LiSP\;and\;{\Delta}Mang$ was also significant 3. Correlation coefficient between ${\Delta}VIs\;and\;{\Delta}VLs$ was the greatest in adults and the next was ${\Delta}Ii/{\Delta}Li,\;{\Delta}Ii/{\Delta}ILS,\;{\Delta}ID/{\Delta}Li\;and\;{\Delta}ID/{\Delta}ILS$. In contrast all the vertical and horizontal measurements of hard and soft tissue in adolescents showed statistically significant corerrlation. 4. There were differences in correlation between soft tissue changes and incisor inclination and retraction at both groups, but the lower lip, nasolabial angle and mentolabial angle were commonly less influenced by the hard tissue changes in both groups. 5. The thinner the upper lip was, the more the ${\Delta}LsSP$ was in both groups, and the thinner the lower lip was, the more the ${\Delta}LiE\;and\;{\Denta}LiSP$ was in adolescents. 6. Molar relationship didn't influence the soft tissue profile changes. 7. Arch length discrepancy didn't influence the soft tissue profile changes.

THE SECOND MESIOBUCCAL CANAL OF UPPER PRIMARY MOLAR : CASE REPORT (제 2근심협측근관이 존재하는 상악유구치의 치험례)

  • In a primary teeth, dental caries is rapidly advanced the pulp disease, because the primary teeth have the thinner and the weaker enamel layer and the wider pulp chamber than relatively the permanent teeth. And the pulps of primary teeth are exposed during caries removal or even they are exposed by unexpected movement of the children or by trauma. For successive pulp treatment in primary teeth, it is necessary to understand completely about multiple canal morphology, variation of root canal anatomy and specific problem related to root formation and resorption of primary teeth. In upper primary molar, canal configuration of mesial root has the most variation same as upper molar. If not canal treatment is completely, most of all endodontic treatment should be failed. In a clinical case report, upper primary molars existed persistent pain or bleeding during treatment were founded the second MB canal and were performed the endodontic treatment of theirs. As a result, the upper primary molars have no symptom and good prognosis. In the examination of extracted upper primary molar, we found that 8 of 35 teeth(22.8%) in the upper primary first molars and 22 of 33 teeth(66.6%) in the upper primary second molar had the second mesiobuccal canal. It has revealed the high prevalence of two canals in mesiobuccal roots of upper primary molars. The frequency of occurrence of the second mesiobuccal canal must be taken into consideration when endodontic treatment is planned and as a possible cause of otherwise un explained failure.
