Incidence of Dental Discoloration After Tetracycline Exposure in Korean Children: A Nationwide Population-Based Study

  • Purpose: Tetracycline is not recommended for children under 12 by guideline due to the risk of tooth discoloration. We aimed to assess the incidence of dental discoloration in Korean children prescribed tetracyclines and investigate whether its risk was greater in tetracycline-exposed children than in the general population. Methods: This population-based cohort study using the Health Insurance Review and Assessment service database included children aged 0-12 years exposed to tetracyclines for at least 1 day between January 2008 and December 2020. The primary outcome was the incidence rate of dental discoloration ≥6 months after prescription, and the standardized incidence ratio (SIR) was evaluated as secondary outcome. Results: 56,990 children were included-1,735 and 55,255 aged <8 and 8-12 years, respectively. 61% children were prescribed tetracycline for <14 days with mostly second-generation tetracyclines, doxycycline (61%) and minocycline (35%). The 5- and 10-year cumulative incidence rates of dental discoloration were 4.1% (95% confidence interval [CI], 3.0-5.7%) and 5.7% (95% CI, 4.1% to 7.8%), respectively, in the 0-7 years age group and 0.8% (95% CI, 0.7% to 0.9%) and 1.3 (95% CI, 1.1% to 1.4%), respectively, in the 8-12 years age group. Tetracycline exposure did not increase such risk compared to that in the general population (SIR, 1.08; 95% CI, 0.69 to 1.60). Conclusions: The incidence of dental discoloration was lower than previously suggested. Relieving the age restriction for prescribing tetracyclines may be considered.

  • 치관부 파절이나 치아결손 혹은 치아형태의 이상이나 변색된 치아등으로 보철치료가 요구되는 경우 저작, 연하, 발음등의 회복과 심미적인 회복이 중요한 목적이 되며, 전치부에서는 심미성이, 구치부에서는 기능적인 회복이 더욱 강조된다. 보철치료후 치료목적을 달성하고 건강한 구강내의 상태를 유지하기 위해서는 보철물이 지대치 및 주위조직에 생물학적으로 조화되어 병적상태를 유발시키지 않아야 하며, 이를 위해서는 시술전에 올바른 진단과 적절한 치료계획을 세우고, 지대치 형성, 인상채득 및 임시관 장착, 금속관 및 가공의치 제작, 구강내의 시적(try-in), cementation등 모든 과정을 주위조직과 관련하여 주위 깊게 행하여야 한다.

Regenerative Endodontic Treatment Without Discoloration of Infected Immature Permanent Teeth Using Retro MTA : Two Case Reports (치수 괴사된 미성숙 영구치에서 Retro MTA를 이용한 변색 없는 재생적 근관치료 : 증례 보고)

  • Regenerative endodontic treatment has the potential to heal a necrotic pulp, which can affect root development in immature teeth. However, several drawbacks and unfavorable outcomes are associated with regenerative endodontic treatment, of which the most significant is coronal discoloration due to the presence of minocycline in triple antibiotic paste and mineral trioxide aggregate (MTA). To prevent tooth discoloration following pulp treatment, the modified triple antibiotics (ciprofloxacin, metronidazole, clindamycin) were used as canal disinfectants and Retro MTA, a $ZrO_2$-containing calcium aluminate cement, was used to seal the canal. Following access cavity acquisition, the canal was copiously irrigated with 2.5% sodium hypochlorite. A modified triple antibiotic paste was then applied to the canal. Once the tooth was asymptomatic (after between 3 and 8 weeks), Retro MTA was carefully placed over the blood clot or a collagen plug. Follow-up radiographs revealed normal periodontal ligament space and root development. In two cases, successful regenerative endodontic treatment of the infected immature tooth, without discoloration, was achieved with disinfection using modified triple antibiotics and Retro MTA sealing.

  • Dentinogenesis imperfecta (DI) is a hereditary disorder of dentinal defect. It is generally inherited as a single autosomal dominant trait. DI usually affects both the primary and permanent dentition. Affected teeth have various types of discolorations, rapid destruction of the dentin, and severe attrition. In radiologic view, the affected teeth have bulbous crowns, short roots and narrow or closed pulp chambers. The treatment objective is to prevent additional attrition and recover the vertical dimension of occlusion. The aim of this report was to present the long-term prognosis in 15 years in a pair of siblings. Both the patients had DI with tooth attrition and discoloration. Different treatment procedures were used, depending on the difference in the timing of intervention. The first patient saved most of his teeth. The second patient had all of her teeth extracted. This report could be helpful for early diagnosis and overall treatment of DI.

  • Pulp canal obliteration(PCO) is seen commonly in dental pulp after traumatic tooth injuries and is recognized clinically as early as 3 monthly after injury. Pulp canal obliteration is characterized by deposition of hard tissue within the root canal space and yellow discoloration of the clinical crown. Opinion differs among practitioners as to whether to treat these cases upon early detection of PCO or to observe them until symptoms or radiographic signs of pulpal necrosis are detected. PCO may make root canal treatment necessary because of the development of apical periodotitis or for cosmetic reasons. If carefully executed, root canal treatment in teeth with an PCO is hightly successful and may act as a basis for internal bleaching. During a game, a 12-year-old girl was hit in the face. At that time, she was diagnosis a subluxation of the maxillary right central incisor. At the 24-month recall examination, a root canal of the tooth had been calcified and discolored gradually. We performed endodontic treatment to prevent perfect pulp canal obliteration and internal bleaching.

TOOTH COLOR AND STRUCTURE CHANGES INDUCED BY TETRACYCLINE IN RAT (테트라싸이클린에 의한 백서 치아의 색상 및 구조 변화)

  • 테트라싸이클린에 의한 치아 변색은 심미성을 고려한 임상 치료시 커다란 난제로 남아 있다. 본 연구는 백서 치아에 테트라싸이클린을 주입하여 야기되는 색상과 구조적인 변화도를 측정하여 향후 연구에 기초자료를 제공하기 위해 시행되었다. 24 마리의 백서를 각기 8 마리씩 3 군으로 분류하여 대조군은 정상적인 사육을 식염수군은 TC군과 동량의 식염수를 주사하였으며 TC군은 12 ml에 용해시킨 60 mg의 테트라싸이클린을 복강내로 2 주간 주입하였다. 최종 주입후 1 주간 정상 사육한 다음 cervical dislocation법에 의해 희생시켜 시편을 채득한 후 각 치아를 횡단하여 실물현미경과 FlexCam을 이용하여 컴퓨터에 영상을 저장한 다음 색 변화량을 Adobe Photoshop으로 측정하였으며 구조변화는 주사전자 현미경을 이용하였다. 치아색 변화량의 유의성 검정은 ANOVA와 Scheffe test를 이용하였으며 본 실험 결과는 다음과 같다. 1. 각 군의 $a^*$ 수치는 대조군 -6.12 (1.18), 식염수군 -8.00 (1.33), TC군 -18.56 (2.52)로 나타났으며 $b^*$ 수치는 대조군 1.12 (2.13), 식염수군 1.62 (1.45), TC군 28.67 (5.18)을 보였다. 즉 테트라싸이클린에 의해 $a^*$$b^*$의 수치가 유의성있게 (p<0.01) 초록색과 노란색 방향으로 변화하였다. 2. 테트라싸이클린에 의해 치경부 법랑질 외면이 검게 변색되었으며 울퉁불퉁한 양태로 변화되었다. 그러나 법랑소주의 형태, 방향등은 차이를 보이지 않았다. 3. 각 군간 상아세관의 수, 방향, 크기등이 차이가 없었다. 치수측에서의 관찰결과 대조군과 식염수군에서는 상아세관들의 입구가 크고 명확하게 보인 반면 TC군에서는 입구가 좁고 부분적으로 막힌 양태도 관찰되었다.

Oral rehabilitation of a young adult with amelogenesis imperfect using metal-ceramic restoration: a clinical report (금속 도재 수복물을 이용한 청년기 법랑질 이형성증 환자의 수복증례)

  • Some patients with generalized attrition and teeth discoloration may want their anterior teeth to be treated just for esthetic improvement. Ameologenesis imperfecta, however, should be considered for such patients prior to any treatment with thorough clinical and radiographic examination. If a patient is diagnosed with amelogenesis imperfecta, the treatment on anterior teeth just for esthetic purpose is not advisable. In this case, a young man with amelogenesis imperfecta was treated with metal-ceramic restorations. The patient had generalized attrition, teeth discoloration, crown fracture, and cross-bite on the left teeth. The ultimate objective of this treatment was to enhance esthetics and masticatory function. The cross-bite on the left anterior teeth was treated with restorations, whereas the reverse horizontal overlap was maintained on the posterior. The patient was satisfied with the result esthetically and functionally, and the third month recall examination revealed no pathologic changes associated with the treatment.
