A COMPARISON OF DECISIONS FOR PRIMARY ANTERIOR TEETH BETWEEN PEDIATRIC DENTISTS AND GENERAL DENTISTS (유전치 우식에 대한 치과의사들의 치료 선택 현황 조사)

  • Children usually have varying degree of caries in primary anterior teeth, and treatment planning for each case prescribed by each dentist can also be varied. This survey was conducted to compare the preferred treatment method and restorative materials between general dentists and pediatric dentists in regard to the treatment of primary incisors. The questionnaires, composed of 18 questions were sent to 45 general dentists and 50 pediatric dentists. Among which 30 and 31 questionnaires were retrieved respectively. The collected data were analyzed by rate and the results were as follows: 1. For the teeth with initial caries without cavitation, general dentists showed the tendency to prefer restorative treatment(30%) or observation without any treatment(42%), whereas pediatric dentists prefer preventive treatment(76%). 2. The primary factor in choosing restorative materials by both groups was its manipulativeness. 3. For anterior esthetic restoration, general dentists seldom use the full-coverage restoration(13%) but resin restoration(75%), whereas pediatric dentists frequently used full-coverage crow(64%). 4. In the treatment of dentinal caries, pediatric dentist did not perform the treatment lesser than 2.0 years before the exfoliation (compared to 1.2 years of general dentist). 5. In the treatment of 1 year children, both pediatric and general dentists tend to select preventive procedure as first choice of treatment(84%, 52%). When treating primary incisor caries, it is shown that pediatric dentists are more interested in restorative/preventive treatment than general dentists are.

  • 치경부 병소는 부적절한 칫솔질, 화학적 침식 및 차아의 굴곡 등의 여러 원인에 의해 크게 cervical abrasion, erosion 및 abfraction의 형태로 나타난다. 최근 노령 환자층의 증가와 치주 치료에 의한 구강내 치아 잔존율의 증가는 치경부 병소의 증가를 가져왔다. 이러한 치경부 병소는 지각과민증, 우식 및 심미적 이유 등으로 수복을 시행하며 구치부위에 작은 우식이 없는 치경부 병소가 있는 경우 환자에게서 지각과민증를 유발하지 않는 경우라면 수복을 시행하지 않을 수도 있지만, 많은 예에서 더 이상의 치아 및 치주 조직의 손상을 억제하고 치수의 보호를 위해서 수복을 시행한다. 특히 전치부의 경우에는 병소가 작고 우식에 이완되지 않았다 할지라도 그 심미적 측면 때문에 수복을 시행하는 예가 증가하고 있다.(중략)

Influence of Lifestyle-Related Diseases on the Oral Health of the Rural Elderly in Korea (일부 농촌지역 60세 이상 노인의 생활습관병이 구강건강에 미치는 영향)

  • Objectives: The purpose of this study was to analyze the influence of four lifestyle-related diseases, diabetes, hypertension, obesity and hypercholesterolemia, on oral health in the rural elderly in Korea. Methods: We enrolled 399 subjects over 60 years of age over a six year period, year 2000-2006/ year 2001-2007. All subjects received a routine health examination as part of a program conducted by the National Health Insurance Corporation at Seongju-gun Public Health Center in Gyeongsangbuk-do Province, South Korea. All subjects were surveyed and examined to determine their general and oral health statuses. Results: Our results suggest that the duration of lifestyle-related diseases has a significant influence on oral health. The following factors were all significant in the results of analyses: duration of diabetes, tooth decay, which teeth had decayed, missing and filled teeth(DMFT), duration of hypertension, and duration of obesity and hypercholesterolemia(p<0.05). Our results also suggest that the number of lifestyle related diseases has a significant influence on oral health. Subjects with more than two diseases had significantly greater numbers of missing teeth and greater numbers of DMFT(p<0.01). The results of simple regression analysis indicate that patients exhibiting longer durations of diabetes also exhibit more tooth decay, and that patients exhibiting longer durations of hypertension and obesity are characterized by greater numbers of missing teeth. The longer the duration of any of the four lifestyle-related diseases we considered, the more DMFT we observed. Multiple regression analyses also demonstrated that longer duration of lifestyle-related disease was associated with greater numbers of missing teeth. As the number of lifestyle-related diseases increased, DMFT also increased. Conclusions: In summary, lifestyle-related diseases such as diabetes, hypertension, obesity and hypercholesterolemia have significant influences on oral health in the elderly. In this context, the prevention and management of lifestyle-related disease is critical for the maintenance and promotion of oral health.

ORAL HEALTH STATUS AND SELF-PERCEIVED ORAL HEALTH STATUS OF STUDENTS IN JUVENILE PROTECTION EDUCATION INSTITUTIONS (소년원학교 학생의 구강건강 실태와 구강건강 인식도)

  • The purpose of this study was to investigate the prevalence of dental caries, periodontal status, and self-perceived oral health status of students in juvenile protection education institutions. The subjects of this study were 342 adolescents aged 13-20 years detained in two juvenile protection education institutions located in Gyeonggi Province. The study involved an oral examination to record decayed, missing, filled teeth and index teeth with bleeding, calculus, or periodontal pocket, combined with a questionnaire to investigate self-perceived oral health status. The main findings are as follows. 1. When compared with the result of National oral health survey, the students detained had more decayed teeth, DMFT, and teeth with bleeding, calculus, and shallow pocket than general population of the same age. 2. Female students showed more DMFT, more teeth with healthy periodontium, and fewer teeth with shallow pocket than male students (p<0.05). 3. Female students perceived their health to be poorer than male students (p<0.05). A majority(79.2%) of detainee valued their oral health in their life. 4. A majority(71.5%) of detainee reported their dental anxiety, and more female adolescents showed dental anxiety than male adolescents(p<0.05).

The Result of Oral Health Care on Primary School Children Using Dental Caries Activity Test (일부 초등학생의 치아우식활성검사를 이용한 계속구강건강관리 효과)

  • The purpose of this study was to evaluate the effectiveness of oral health care including some of dental caries activity test on dental clinics of regular visiting on primary school children. The dental surveys and initial caries control and some of dental caries activity test (Alban's, Lactobacillius, S'mutans, un-stimulation saliva test)were performed by trained dental hygienists with dentist from on 6 to 13 years old patients of S dental hospital in Gwangju Metropolitan Cities. The sample size of 39 children (18 in oral health care group, 21 in control group ) and accepted to their parents. The results of caries activity test score(Alban's and Lactobacillius test)were reduced 1.4 to 1.6 times in oral health care group(OHCG). Oral Hygiene Simplified Index of OHCG was reduced 1.6 times during six months and 1 year. DMFT index of OHCG was reduced 1.4 times more than Control Group(CG) after 1 year. DT index of OHCG was reduced 12 times more than Control Group(CG) after 1 year. Dental health capacity of the first perment molar of OHCG was reduced 1.02 times more than CG after 1 year. From the above results, long life related incremental oral health care system has reinforced to primary school children in dental clinic with oral health professional teams.

  • 치아 스케일링은 치아에 단단하게 결착된 치석을 제거하는 치과 시술로서 치아 우식 및 잇몸염증을 예방하는 중요한 시술이다. 특히 요즘에는 치료시간의 단축을 통한 효율성 증대의 목적으로 전기적 에너지를 미세한 진동에너지로 바꾸는 원리를 활용한 초음파 스케일링 기법이 많이 행해지고 있다. 하지만 치아 및 치석 확보에 따른 어려움으로 인해 스케일링 시술을 충분히 훈련하기란 쉽지 않다. 본 논문에서는 사용자가 가상현실을 통해 시각 및 촉각 피드백을 받으며 초음파 스케일링 시술을 훈련할 수 있는 치아 스케일링 시뮬레이션을 위한 알고리즘을 제안한다. 치아, 치석 및 잇몸의 볼륨모델과 스케이러 팁을 구성하는 각 부문의 관통깊이를 이용한 햅틱 랜더링 기법을 적용하여 스케일러의 모양에 따른 햅틱 피드백을 생성하였다. 그리고 치아의 손상을 줄이기 위해 스케일러의 팁 부문이 치아 표면에 되도록 평형을 이루어야 한다는 점에 입각하여, 치석을 구성하는 복셀들의 치아 디스턴스필드 값 비교를 통해 치석과 치아 사이의 접착면을 추출하고 스케일러의 팁 부분과 충돌하는 추출된 집착면의 각도를 고려한 스케일링 알고리즘을 구현하였다. 또한 수동 스케일링과는 달리 초음파 스케일링은 초음파의 진동에너지에 의해 점진적으로 치아와 치석 사이의 결속력이 감소된다는 점에 착안하여 치아와 치석의 접착면을 구성하는 지점 사이의 거리에 따른 결속력 감쇠 모델을 고안하였다.

  • A person with neurological disorder is defined as a person with limitation of ambulation or basic life activity due to the cerebral palsy or traumatic brain damage, stroke. Recently, the DMFT of the children with neurological disorders is similar or lower than the DMFT of the children with no disability. But, this article is about the severe-early childhood caries in children with grade 1 neurological disorder. It is supposed that the primary motor impairment and secondary physical weakness, frequent hospitalization, sugar- rich food and drugs, lack of oral care, delayed dental visit made high caries susceptibility. In case of treatment of the children with severe neurological disorders, dentists should make a thorough caries preventive plan based on individualized caries risk assessment. Also parents and medical doctors should recognize the importance of oral care and do the early dental visit and home care.

  • CATCH22 syndrome or DiGeorge syndrome is a medical acronym of cardiac defects, abnormal facial appearances, thymic hypoplasia, cleft palate, and hypocalcemia. Patients with CATCH22 syndrome are susceptible to infection due to an absent or hypoplastic thymus and often have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with CATCH22 syndrome under general anesthesia. A 4-year-old, 14.8 kg boy with CATCH22 syndrome visited Seoul National University Dental Hospital for dental check up. The patient had multiple caries requiring dental treatment. He experienced the corrective heart surgery due to Tetralogy of Fallot a few years ago. General anesthesia was planned because his heart rate and vital sign had shown unstable during the previous conscious sedation procedure. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with CATCH22 syndrome could be performed with the help of general anesthesia and careful monitoring.

  • Early childhood caries prevalence has increased significantly in children ages 2~5 years. ECC disproportionately affects lower socioeconomic and minority groups, is a predictor for future decay, but is preventable and manageable. Caries risk assessment systematically derives a patient's caries risk and is important during an infant oral health visit beginning at age 1. Information obtained through a risk assessment can guide a disease management care path tailored to an individual's age and risk to effectively treat and manage one's caries disease process.

THE PREVALENCE OF MOLAR INCISOR HYPOMINERALIZATION AND STATUS OF FIRST MOLARS IN PRIMARY SCHOOL CHILDREN (초등학생의 Molar Incisor Hypomineralization 유병률과 제1대구치 치아우식증 및 수복상태 조사)

  • Molar Incisor Hypomineralization(MIH) have recently gained intensive research interests and various clinical trials. Most prevalence, etiology, treatment studies in MIH were carried out in the European countries, and data from the Korean were seldom. This study aimed to investigate the prevalence of MIH and the status of dental caries and treatment on the first permanent molars in primary school children. For this study, 1,344 primary school students in Busan and Ulsan cities were examined directly the permanent incisors and first molars. The results of the survey were as follows: 81 MIH cases were identified among 1,344 cases. The prevalence of MIH in this group of children was 6.0%. Children with MIH showed a significantly higher DMFT value for permanent teeth than children without MIH. The mean number of decayed, missed, and filled in 1st permanent molars(DMFT index) was 1.17. The rate of children with restoration on 1st permanent molar was 13.6% and the sequence of restoration materials was as follows : composite resin, amalgam, gold inlay.
