MICROLEAKAGE OF 7TH GENERATION BONDING SYSTEM (7세대 결합제의 미세누출에 관한 연구)

  • Dentin bonding system have recently been developed in an effort to simplify and shorten bonding procedure. The intent of this study was to evaluate microleakage of newer generation of dentin bonding system comparing with other generations. Forty extracted human premolar teeth were randomly assigned to four groups for bonding : the 4th generation bonding system (Scotchbond Multi-Purpose). the 5th generation bonding system (Clearfil SE Bond), 6th generation bonding system (AQ bond) and 7th generation bonding $system(i-bond)^{TM}$ Class V cavities were prepared in buccal and lingual surface. The cavities were restored with composite resin (Z100) after the application of dentin bonding systems according to the instructions of the manufacturer Samples were thermocycled and immersed in methylene blue dye(2%). Each sample was sectioned with Isomet and evaluated for microleakage using stereomicroscopy. The mean value of the microleakage in 7th generation bonding system was greater than those of 4, 5, 6th generation dentin bonding system. Statistically, the mean values of microleakage between 4th and 7th generation bonding system were significantly different from each other (p<0.05).

  • Ectopic eruption and impaction of canines is a frequently encountered clinical problems. The incidence of impaction ranges between 1-3%. The cause of canine impaction can be the result of localized factors or can be a polygenic multifactorial inheritance and associated with other dental anomalies. The general dentist and pediatric dentist should know how to properly diagnose and manage potential disturbances in the eruption of maxillary canine. Diagnosis of impacted canine at age 8-10 years can significantly reduce serious ramifications, including surgical exposure and orthodontic traction as well as root resorption of the lateral incisors. Extraction of primary canine would be one of the method to prevent the impaction. The surgical procedure should be designed to minimize the destruction of periodontal tissue of impacted canine. Closed eruption technique is thought to be optimal method of surgical exposure compared with other methods. An overview of the incidence, sequela as well as the surgical periodontal, and orthodontic consideration in the management of impacted canine was presented.

  • 임상에서 점차 Mineral Trioxide Aggregate (MTA)에 대한 우수성이 소개되면서, apical matrix로의 사용은 주목할 만하다 할 수 있다. 본 연구의 목적은 개방형근관에서 MTA가 apical matrix로 사용될 때의 치근단 밀폐효과를 알아 보고, 근관충전의 시기와 matrix의 두께가 치근단 밀폐에 미치는 영향을 치근단 미세누출의 측면과 matrix탈락빈도의 측면에서 관찰하고자 하는 것이다. 개방형 근관을 재현하고자 45개의 발거된 단근치에 #90크기로 근단공을 형성하였고, 투명레진등을 이용하여 치근단 병소를 갖는 치조골을 재현한 후, 4개의 실험 군과 1개의 대조 군으로 분류하였다. A군: 2mm두께의 MTA matrix 형성후, 열연화된 Gutta-percha와 AH26 sealer를 이용하여 즉시 근관충전. B군: 2mm 두께의 MTA matrix 형성후, A군과 같은 방법으로 24시간 후 근관충전. C군: 4mm 두께의 MTA matrix 형성후, A군과 같은 방법으로 즉시 근관충전. D군: 4mm 두께의 MTA matrix 형성후, A군과 같은 방법으로 24시간 후 근관충전. 대조군: matrix를 사용하지 않고 열연화된 Gutta-percha와 AH26 sealer를 이용하여 근관충전하였다. Matrix의 탈락이 있는 경우 기록하고 완전 수세 후 다시 시행하였다. 색소침투정도의 평가를 위해 methylene blue에 치아를 48시간 침수시킨 후 협설로 양분하였다. 각 시편들을 고배율 현미경 관찰 후 비디오촬영 하였고, digitalized image analysing program을 이용하여 두명의 관찰자가 누출의 정도를 평가한 후, One-way ANOVA로 통계적 유의성을 검증하여 다음과 같은 결과를 얻었다. 1. Matrix를 사용한 실험 군이 matrix를 사용하지 않은 대조 군에 비해 유의성 있게 낮은 누출을 보였다(p<0.05). 그러나 근관충전시기와 matrix의 두께를 달리한 실험군 내에서는 누출에 유의성 있는 차이가 나타나지 않았다. 2. 모든 실험군에서 gutta-percha를 이용한 근관충전의 시기에 matrix탈락 및 gutta-percha의 압출은 발생하지 않았다.

EFFECT OF PRE-HEATING ON SOME PHYSICAL PROPERTIES OF COMPOSITE RESIN (중합 전 열처리가 복합레진의 일부 물성에 미치는 영향)

  • The purpose of this study was to evaluate the effect of pre-heating on some physical properties of composite resin. Eighty extracted, noncarious human molars were used in the present study. Four different temperatures of composite resin were used: $4^{\circ}C$, $17^{\circ}C$, $48^{\circ}C$, and $56^{\circ}C$. The $4^{\circ}C$ and $17^{\circ}C$ values represented the refrigerator storage temperature and room temperature respectively. For $48^{\circ}C$ and $56^{\circ}C$, composite resin was heated to the temperatures. As physical properties of composite resin, shear bond strength, microhardness, and degree of conversion were measured. The data for each group were subjected to one-way ANOVAs followed by the Tukey's HSD test at 95% confidence level. Both in enamel and dentin, among composite resin of $4^{\circ}C$, $17^{\circ}C$ $;48^{\circ}C$, and $56^{\circ}C$, the pre-heated composite resin up to $56^{\circ}C$ revealed the highest shear bond strength, and pre-heated composite resin to the higher temperature revealed higher shear bond strength. Microhardness value was also higher with composite resin of higher temperature. Degree of conversion was also higher with composite resin of the higher temperature. In this study, it seems that pre-heating composite resin up to the higher temperature may show higher shear bond strength, higher microhardness value, and higher degree of conversion. Therefore, when using composite resin in the clinic, preheating the composite resin could be recommended to have enhanced physical properties of it.

The influence of combining composite resins with different elastic modulus on the stress distribution of class V restoration: A three-dimensional finite element study (탄성계수가 다른 복합레진의 혼합수복이 5급 수복물의 응력분포에 미치는 영향에 관한 3차원 유한요소법적 연구)

  • This study was to investigate the influence of combining composite resins with different elastic modulus, and occlusal loading condition on the stress distribution of restored notch-shaped non-carious cervical lesion using 3D finite element (FE) analysis. The extracted maxillary second premolar was scanned serially with Micro-CT. The 3D images were processed by 3D-DOCTOR. ANSYS was used to mesh and analyze 3D FE model. A notch-shaped cavity was modeled and filled with hybrid, flowable resin or a combination of both. After restoration, a static load of 500N was applied in a point-load condition at buccal cusp and palatal cusp. The stress data were analyzed using analysis of principal stress. Results showed that combining method such that apex was restored by material with high elastic modulus and the occlusal and cervical cavosurface margin by small amount of material with low elastic modulus was the most profitable method in the view of tensile stress that was considered as the dominant factor jeopardizing the restoration durability and promoting the lesion progression.

  • The induction of the IL-8 and MCP-1 by the stimulation of Substance P and TNF-${\alpha}$ (IL-8 agonist) and the specificity for SP using Spantide (SP antagonist) in the dental pulp tissues was measured quantitatively. In addition, the secretion of the IL-8 in the human dental pulp tissue 36 hrs after the stimulation of SP was observed after the stimulation of SP qualitatively. According to this study the results were as follows: 1. There was the significant IL-8 induction at 36 h after SP (10$^{-4}$M) stimulation of the pulp tissue comparing with the unstimulated dental pulp tissues (p < 0.05) . IL-8 irnmunostaining was weakly detected along the periphery of the pulp tissue after Mock stimulation and IL-8 immunostaining was detected around the fibroblast in the pulp tissue 36h After SP (10$^{-4}$M) stimulation, 2. The secretion of MCP-1 from the dental pulp tissues comparing with Mock stimulation was induced at 36 hrs after TNF-$\alpha$ (40 ng/ml) stimulation, but no induction with SP(10$^{-4}$M) TNF-${\alpha}$ (40 ng/ml) did not induce the IL-8 secretion from the pulp tissue, weak IL-8 imrnunostaining was detected along the periphery of the pulp tissue 3. Spantide (10$^{-5}$M) inhibited IL-8 induction from the pulp tissues 36 h after SP (10$^{-4}$M) stimulation These results suggest that SP significantly induces IL-8 recruiting neutrophils in localized human dental pulp tissue MCP-1 appears to be less involved in the early establishment of pulpal inflammation in response to irritation such as mechanical insult of dentin. SP may have positive relation with the inflammation of the human dental pulp tissues.

THE CHANCE OF ADAPTABILITY CHANCE IN ADHESIVE SYSTEMS TO DENTIN SUBSTRTE ACCORDING TO STORAGE TIME (상아질 접착 후 저장기간에 따른 접착제의 접착력 변화)

  • This study compared the microtensile bond strength (${\mu}$TBS) and microscopic change of two 2-step and two 1-step self-etching adhesives to dentin according to storage times in distilled water. Occlusal dentin was exposed in 48 human molars. They were divided to four groups by different adhesives: SE Bond group (Clearfil SE Bond), AdheSE group (AdheSE). Adper group (Adper Prompt L-Pop), and Xeno group (Xeno III) . Each group was stored in 37$^{\circ}C$ distilled water for 1, 15, and 30 days. Resin-bonded specimens were sectioned into beams and subjected to ${\mu}$TBS testing with a crosshead speed of 1 mm/minute. For SEM observation, one specimen was selected and sectioned in each group after each stroage time. Resin-dentin interface was observed under FE-SEM. In all storage times, mean ${\mu}$TBS of SE group was significantly higher than those of other groups (p < 0.05). There was no significant difference between mean ${\mu}$TBS of SE group and AdheSE group among all storage times, but significant difference between 1- and 30-day storage in mean y${\mu}$TBS of Adper group and Xeno group (p > 0.05). For 1-and 15-day storage, all groups showed the close adaptation between resin-dentin interfaces. For 30-day storage, resin-dentin interfaces showed wide gap in Adper group and separate pattern in Xeno III group.

MARGINAL ADAPTATION OF STAINLESS STEEL IN POSTERIOR PRIMARY TOOTH (유구치 기성 금관의 변연 적합도에 관한 연구)

  • Stainless steel crowns are invaluable restorative material for the treatment of badly broken down primary teeth in pediatric dentistry. But it is difficult to fit margin because selection of size is not easy and they are not adjusted for Korean children. The purpose of this study was to examine and analyze the marginal adaptation of stainless steel crown of posterior primary tooth. Marginal surface was taken by Fine Pix S602 digital camera and measurements of crown were recorded at 20 points that were randomly selected for marginal gap evaluation by Kappa image base program. 1. Mean marginal gap were large upper 2nd primary molar, lower 1st primary molar, lower 2nd primary molar, upper 1st primary molar in order(p<0.05). 2. Mean marginal surface dimension ratio was more than 20% irrespective of tooth. 3. Largest amount of marginal gap was shown at mesial surface in upper 1st, 2nd primary molar and distolingual surface in lower 1st primary molar, buccal surface in lower 2nd primary molar.

Effects of one or two applications of all-in-one adhesive on microtensile bond strength to unground enamel (Unground enamel에 대한 all-in-one adhesive의 1회 또는 2회 적용이 미세인장 결합강도에 미치는 영향)

  • The purposes of this study were to compare the effects of one or two applications of all-in-one adhesives on microtensile bond strengths (${\mu}$TBS) to unground enamel and to investigate the morphological changes in enamel surfaces treated with these adhesives using a scanning electron microscopy (SEM). Twenty-five noncarious, unrestored human mandibular molars were used. The unground enamel surfaces were cleansed with pumice. The following adhesives were applied to lingual, mid-coronal surfaces according to manufacture's directions; Clearfil SE bond in SE group, Adper Prompt L-Pop$^{TM}$1 coat in LP1 group, 2 coats in LP2 group, Xeno$^{\R}$III1 coat in XN1 group, and 2 coats in XN2 group. After application of the adhesives, a hybrid light-activated resin composite was built up on the unground enamel. Each tooth was sectioned to make a cross-sectional area of approximately 1.0 mm$^2$ for each stick. The microtensile bond strength was determined. Each specimen was observed under SEM to examine the morphological changes. Data were analyzed with one-way ANOVA. The results of this study were as follows; 1. The microtensile bond strength values were; SE (19.77 ${\pm}$ 2.44 MPa), LP1 (13.88 ${\pm}$ 3.67 MPa), LP2 (14.50 ${\pm}$ 2.52 MPa), XN1 (14.42 ${\pm}$ 2.51 MPa) and XN2 (15.28 ${\pm}$ 2.79 MPa). SE was significantly higher than the other groups in bond strength (p < 0.05). All groups except SE were not significantly different in bond strength (p < 0.05). 2. All groups were characterized as shallow and irregular etching patterns.
