  • Care-stress among the dementia caregivers has been an important issue. The purpose of this study is to examine the effects of the dementia elderly's symptoms on the primary caregivers'care -stress. In addition, the moderators, the expert support and the family support, were used to examine the moderating effects between the symptoms and the care-stress. The data was obtained at 10 day-care facilities and services in Seoul, Gyeonggi and Busan province in South Korea. A total of 191 participants were analyzed. They were the spouses or the adult children of the dementia elderly who were diagnosed with dementia within five years. This study was conducted in multiple regression analysis. The main findings are as follows. First of all, the symptoms that the dementia elderly show were significantly associated with the primary caregivers' care-stress. Also, the interactive variable with the expert support was statistically significant. However, it was not significant with the family support. This means that only the expert support from doctors, nurses or social workers decreased the caregivers' care-stress. The implications of this study are 1) the necessities of the education that covers the specific symptoms of the dementia patients', 2) the extension of the supportive policies for caregivers' care-stress, 3) the necessities of more allocation of the dementia specialists in the practical settings and cooperative systems among the dementia specialists in various fields, and 4) the enhancement of the family function among families who have a dementia elderly as a family member.

  • This study aims to obtain information about the ADL and I-ADL functions of people with dementia living in the community and nursing care facilities regarding their dementia stages for the purpose of supporting occupational therapists working in dementia care centers. The information was collected from 100 people with dementia living in the community and 100 people with dementia staying in care facilities from October 1 to November 20, 2015. 15 facilities participated in this study, including nursing homes, day care centers, health centers, geriatric hospitals and metropolitan dementia centers. All of the assessors were trained in the use of the instruments, ADCS-ADL, S-IADL, S-ADL and MMSE-K, by the researcher before conducting any evaluations or interviews. In order to compare the differences in the levels of ADL according to the dementia stage, an analysis was made via the post-hoc test after conducting a one-way ANOVA. An analysis of the residence type and dementia stage was made through the independent t-test. The comparison of the level of ADL between the people with dementia living in the community and those living in nursing homes showed differences in the moderate level of dementia for all items of the I-ADL and ADL (p<.01). The post-hoc test showed that there is a difference among the minimum, mild, and moderate levels. When the differences in ADL according to the residence type were examined, there were no significant differences in any of the stages (p>.05). Considering these results, it may be critical to identify the changes in the I-ADL rather than those in the ADL for the outcome index when conducting future intervention studies on dementia.

  • Detecting probable Alzheimer's disease (AD) at an early stage is crucial in slowing the progression of the disease and initiating drug therapy for more effective symptom management. Therefore, this study aimed to identify linguistic features that allow us to distinguish between patients with AD and normal controls. This paper reports on characteristics of spontaneous speech in subjects in three stages of AD (questionable, mild, moderate) compared with education- and age-matched normal controls. Four components of speech were measured in Korean native speakers with AD and normal aging: speech tempo, hesitation (measured in seconds), rate of articulation errors, and rate of grammatical errors. The results revealed significant differences in most of these speech components among the four groups, including significant differences between normal controls and the questionable AD group in the areas of speech tempo and rate of grammatical errors. Phonological? articulatory ability was preserved in questionable AD, and grammatical ability was preserved in questionable and mild AD. Subjects with moderate AD were severely impaired in grammatical ability. Prospective assessments of spontaneous speech skills using a dialogue and picture-description task are useful in detecting the subtle, spontaneous speech impairments that AD causes even in its early stage.

  • Discourse analysis is useful for examining discourse characteristics of dementia patients and differentiating them from normal adults. However, inconsistent tasks, transcription methods and measures have been utilized, thus analyses were inevitably unsystematic. The purpose of this study was to investigate tasks, transcription methods and measures of discourse studies in dementia. Studies on dementia discourses written in English were collected via Scopus database containing abstracts and citations from 2005 to 2014, and were analyzed in detail. From the results, the most frequently used task was 'storytelling-topic'. It was observed that use of consistent transcription conventions rendered more reliable transcription data. Measures could be categorized into coherence, cohesion, productivity, propositional syntactic lexical analysis, and fluency, and each category belonged to discourse processing levels (i.e., processing of conceptual networks, propositions, and natural language units). In conclusion, future research warrants not only utilization of discourse tasks depending on dementia types and features, but also application of consistent transcription conventions. Additionally, measures based on the discourse processing theory would enable us to systematically interpret discourse studies.
