We experienced a patient(53-year-old female) with knee joint effusion, pain and stiffness who had not responded to steroid injection therapy, significantly improved by bee venom acupuncture in clinical symptom. And for a opinion that prohibited bee venom acupuncture on DM because of increasing serum glucose level by increasing serum cortisol level, we also experienced that serum glucose level of DM patient stabilized by bee Venom acupuncture treatments, so we report this case with literature review of articles.

The most of patients and protectors point that the Home Visting Physical Therapy is necessary and compulsive. A Public Health Center and a Welfare Center have to help a home visiter for treatment cure to ask for professional medical services in general hospital. On getting a name lists of patients a treatment of doctors, they must remark the conditions of the patients to keep the mutual relation general hospital. A home visiting physical therapists in the welfare center and public health center need to discuss a main doctors in a same center for revaluation of patients. The system in a general hospital consists of the medical department and the administration with the doctor of hospital as the central figure. A department of home visiting physical therapy has a physical therapy team closely connected with various medical office in hospital. The system in a public health center is composed of the health executive office, health direction medicine office, and the community health office. Department of home visiting physical therapy belongs to community health office. Home visiting physical therapy in a welfare center belongs to home visiting service office. The qualifications of a physical therapist is intended for people who have received clinic experience of three years and regular education. The theory (352 hours) and practice (248 hours) total to 600 hours. They can develop professional skills through these education courses. The frequency of home visiting is proper third a week after talking with a medical attendant about the state of patient. The care time is proper from 30 to 60 minutes.

A 48 year old man, has been suffering from a growing chondrosarcoma of sternum which has deeply invading the anterior mediastinum: He underwent wide resection of the chest wall tumor including a 4 cm free margin of normal tissue on all portions. The tumor as 15 × 16× 10cm in size arising from sternum and include both proximal one third of the clavicle and the 1 st, 2nd, and 3rd coital cartilages. The resected skeletal defect in the anterior wall was very large after wide resection of the'tumor and reconstructed due to paradoxical chest wall movement with sandwich like method of double over lapping Marlex mesh and methylmethacreylate, and steel wires. The soft tissue reconstructive procedure was dont with myocutaneous flap transposition use of pectoralis muscle. But the patient go infected with tuberculosis in the mediastinum two months after the operation. We had removed all of previously inserted prosthetics and performed curettage and drainage. Recently we experienced a case with giant chondrosarcoma of the sternum associated with tuberculous mediastinitis. The patient had an uneventful postoperative course and was discharged with adjuvant treatment such as antituberculous medication for 1 year.

Background : The purpose of this study was find out more effective method for improving muscle strengthening in lower extremities according squatting exercise method among knee joint angle 45, 60, 90 degree and between gym ball behind back and wall. Methods : Participants were 21 university students(males 10, females 11) who didn't have any problem with orthopedic surgery. We divided participants to two groups with gym ball group and wall group. Gym ball group were performance squatting exercise with gym ball behind back. Each group had performance squatting exercise according knee joint angle 45, 60, 90 degree. We collected data from E.M.G of Biceps femoris, Gastrocnemius, Vastus medialis and lateralis, Tibialis anterior in lower extremity according knee joint angle 45, 60, 90 degree of each groups. We analyzed data using by ANOVA and ANCOVA of SPSS PC ver. 12.0 in order to determine more effective method according various squatting exercise. Results : Each groups show significantly difference value of biceps femoris, vastus lateralis and medialis, tibialis anterior, gastrocnemius lateralis and medialis among knee joint angle 45, 60, 90 degree. But there was no significantly difference value between gym ball group and wall group according knee joint angle. Conclusions : We suggested that squatting exercise with gym ball was more effective method improving lower extremity muscle strengthening, and changing knee joint angle was more effective compared than continued knee joint angle.

A 52-year-old man was taken to the emergency room following a motor vehicle accident. An echocardiogram showed moderate to severe tricuspid regurgitation due to rupture of the anterior chordae. An operation to repair the tarumatic tricuspid regurgitation was recommended; however, the patient refused because he was asymptomatic. Two years later, he developed mild generalized edema and dyspnea. The echocardiogram revealed progressive severe tricuspid regurgitation and annular dilatation. We treated the tricuspid regurgitation successfully using artificial chordae and ring annuloplasty.

Bronchiolitis obliterans organizing pneumonia (BOOP) preceding polymyositis is rare. In this report, a 40-year-old patient with fever, chillness, generalized myalgia and progressive exertional dyspnea, had bilateral interstitial infiltrates on chest radiograph. High-Resolution CT showed subpleural and peribronchial distribution of airspace consolidation. Open lung biopsy was consistent with BOOP. Prednisolone therapy led to improvement, but during tapering of prednisolone for 3 months to 30 mg, he complained of weakness of both lower legs. One month later, prednisolone was tapered to 15 mg a day, fever. chillness and generalized myalgia were recurred. He complained of weakness of both arms. The creatine kinase (CK) with MM isoenzyme, lactate dehydrogenase (LDH) and aspartate aminotransferase (AST) were elevated. Anti-Jo1 antibody was positive. Vastus lateralis muscle biopsy was compatible with polymyositis. After injection of methylprednisolone for 1 week, the patient became afebrile, the dyspnea resolved, the pulmonary infiltrates decreased, and the muscle strength improved. The serum CK, LDH, AST levels declined significantly. Patients with idiopathic BOOP should have follow-up for the possible development of connective tissue disorders including polymyositis.

This study is designed to report three cases of postoperative care after hip surgery using conventional Korean Medicine treatments including phototherapy. Three patients undergone hip fracture surgery were treated with acupuncture, herbal medicine, and phototherapy. We used Visual analogue scale (VAS), EuroQol five dimension scale (EQ-5D), and hip disability and osteoarthritis outcome score (HOOS) to evaluate treatment effectiveness. After the treatment, VAS score was improved in all three cases, while EQ-5D and HOOS showed different results by items. EQ-5D (pain/discomfort) and HOOS (pain, symptoms, activities of daily living) were improved in all three cases. EQ-5D (self-care) and HOOS (quality of life) showed improvements in two cases. EQ-5D (mobility, usual activities, anxiety/depression) and HOOS (sports/recreation) were improved in one case. Conventional Korean Medicine treatments including phototherapy can be effectively used to alleviate pain and symptoms after hip fracture surgery.

Background : The purpose of this study was to investigate the effects of taping on the neck rotation angle in the elderly. Methods : The twenty-eight elderly aged over 60 and twenty-seven college students participated in the study. In both groups, neck rotation angles were measured through CROM before and after taping. SPSS 18.0 was used for data analysis. Results : The results indicated that both groups showed significant increases in the neck rotation angles of the affected side after taping. However, there was no significant difference in the rotation angle of both neck regions after taping. The variation of the student group was higher than that of the elderly group after taping. Conclusion : We think that taping can be effective in balancing the neck muscles of both sides and in increasing the rotation angle of the neck.

정상심실중격의 폐동맥협착은 흔한 선천성 심장질환이지만 신생아기에 심한 증세를 보이는 위기적 폐동맥 협착은 드물며 예후도 대단히 나쁘다. 경피적 풍선 판막성형술의 발달과 함께 폐동맥협착의 외과적 치료는 더욱 줄어드는 추세이다. 본 증례는 정상심실중격의 위기적 폐동맥협착증으로 진단받은 생후 2일된 남자 신생아로 심한 청색증과 저산소증을 보여 응급실로 내원하였다. 환아는 산소공급 및 Prostaglandin E$_1$을 투여 후 동맥혈 산소 분압이 19 mmHg에서 54mmHg로 증가하였다. 경피적 풍선 판막성형술을 시도하였으나 유도도관(Guide wire)이 판막의 개구부를 통과하지 못하였고 시술도중 심낭내로 조영제가 고이는 소견을 보여 우심실 천공이 의심되었으므로 응급으로 정상체온의 체외순환하에서 폐동맥 절개후 폐동맥 판막절개술을 시행하였다. 수술 후 중환자실에서의 수술경과는 양호하였고 현재 수술 6개월째 외래 추적관찰중이다.

Obesity often develops to hypothyroidism patients because thyroid hormone is associated with changes in body weight, body temperature, and energy expenditure. Obesity is not only pointed out as the cause of various other diseases, but also can worsen thyroid function, so obesity treatment is essential for hypothyroidism patients. But there is lack of studies about obesity treatment for hypothyroidism patients in Korea. In this study, two obese patients who diagnosed hypothyroidism were treated by Korean medicine. We prescribed herbal medicine (Banhabakchulchunma-tang) for about 2 months. A total of 6.5 kg and 4.5 kg of weight loss were observed respectively, and there were no serious side effects. Further large-scale clinical trials are needed to evaluate the anti-obesity effect and the effect of thyroid function improvement.
