Symptomatic and Clinical Profiles Across Motoric Subtypes in Delirium (섬망의 운동성 아형에 따른 증상과 임상적 특징)

  • Objectives : To investigate clinical and symptomatic differences among motoric subtypes of delirium. Methods : A total of 256 patients referred to psychiatric consultation services for delirium due to general medical condition were assessed retrospectively. Motoric subtypes were determined according to Lipowski's criteria for hyperactive, hypoactive and mixed subtypes. All patients were evaluated according to Delirium Rating Scale-Revised-98(DRS-98-R) by trained psychiatrists to obtain symptomatic profiles of delirium. Results : Hyperactive subtype were 50.8%(n=130), mixed 46.1%(n=118) and hypoactive 3.1%(n=8). Hyperactive patients were younger than mixed subtype($69.62{\pm}13.976$ vs. $73.97{\pm}11.569$, p=0.022) and received antipsychotics to manage symptoms of delirium more frequently(83.8% vs. 57.6%, p<0.001). Hyperactive patients had higher DRS-R-98 scores on both noncognitive($7.14{\pm}3.543$ for hyperactive, $5.62{\pm}3.279$ for mixed subtype) and cognitive subscales($10.00{\pm}3.574$ for hyperactive, $6.38{\pm}2.875$ for hypoactive, $7.43{\pm}3.771$ for mixed subtype, p<0.001). Conclusions : We demonstrated that clinical and symptomatic profiles were different across motoric subtypes in delirium. Diagnostic and therapeutic approach should be made differently according to motoric subtypes of delirium and special attention is needed not to underestimate or delay treatment in specific motoric subtype of delirium.

Movement Disorders that Psychiatrists Should Know (정신과의사가 알아야할 운동장애)

  • The movement disorders in psychiatry have been neglected, though it is an important psychiatric dimension to exert unfavorable influence on patients'quality of life. The etiologies of movement disorders in psychiatry can be classified as primary neurological disorders, psychiatric comorbidities of neurological disorders, manifestations of primary psychiatric disorders, drug-induced movement disorders and psychogenic movement disorders. For the rapid and proper treatment for movement symptoms and signs easily observed from psychiatric patients, psychiatrists' ability toward precise disgnosis and differential diagnosis of movement disorders should be preceded.

Lower Motor Weakness and Complex Regional Pain Syndrome of Lower Limb in the Patient of Frontotemporal Dementia: A Case Report (이마관자엽 치매 환자에서 나타난 하지 근력 저하와 복합부위 통증증후군에 대한 증례 보고)

  • Frontotemporal dementia, the second most common cause of early onset dementia, is a neurodegenerative clinical syndrome characterized by progressive deficits in behavior, executive function and language. Although motor symptoms in frontotemporal dementia are represented by motor neuron disease, parkinsonism and progressive supranuclear palsy syndrome, there have been no reports of motor weakness caused by the direct involvement of central motor nervous systems in frontotemporal dementia. Moreover, no association between clinical dementia groups and complex regional pain syndrome has been reported. We diagnosed a rare case with motor weakness and complex regional pain syndrome of lower limbs due to central nervous system lesion in a patient with frontotemporal dementia by magnetic resonance imaging, electrodiagnostic study and three phase bone scan. Following steroid therapy for complex regional pain syndrome, pain was improved. Functional improvement was noted after rehabilitation therapy, including functional electrical stimulation, muscle strengthening exercise and gait training during hospitalization. This case report suggests that rehabilitation therapy for motor weakness in frontotemporal dementia could be effective for improving overall function.

  • The purpose of this study was to determine the efficient way of stretching technique from three methods, and of stretching duration and frequency. Sretching is known to improve musculotendinous flexibility and joint suppleness, and employed in many warm-up routines during physical activities and athletic performances. The benefits and the rationale of stretching have been reviewed. Many authors agree that, for the best results, static stretching has to be held for approximately 30 sec. in a trial, three trials in a session, at least three times a week for four weeks. Further clinical studies are required for elucidating more informations on this subject.

  • 대동작 운동 수행능력 측정도구(GMPM)는 뇌성마비 아동의 움직임을 질적인 면에서 평가하기 위해 개발된 도구이다. 이 연구의 목적은 대동작 운동 수행능력 측정도구의 측정자간 신뢰도를 알아보는 것이다. 뇌성마비 아동 10명(평균 5.6세, 범위 4~8세)에게 GMPM 평가를 실시하였다. 평가 과정을 비디오로 녹화하여 각 속성 항목별로 3명의 평가자간의 급간내 상관계수로 일치도를 보았다. 전반적으로 측정자간 신뢰도는 '불량~보통'범주에 속했다. 이 연구의 결과는 충분한 교육을 받지 않고 평가하면 그 결과를 신뢰하기 어렵다는 것을 말해준다. 향후 임상에서 GMPM을 이용하여 평가할 때 측정자간 신뢰도에 어떤 변화가 있는지 알아보는 연구가 필요하다.

Acupuncture Therapy for Pyometra in Small Animal (소동물의 자궁축증에 대한 침술요법의 개발)

  • 개에서 정상 발정주기 때와 자궁 축농증에 이환 되었을 경우 이에 대한 전침 자극의 효과를 알아보기 위하여 본 실험을 실시하였다. 정상 자궁 운동은 발정기 때에 최대이었으며 발정정지기 때에 운동성이 감소되거나 결여되었다. 발정정지기 때에 전침자윽을 하거나 Prostagrandin $F_2{\alpha}$를 투여하였을 경우에는 자궁의 운동성이 크게 증가하였다. 자궁경관이 열린 자궁축농증에 이환된 두 마리에서 전침자극을 3~4일간 실시하였다. 자궁의 직경은 감소되었고 질 분비물은 증가한 후 이후 감소하였다. 환축은 회복되는 경향을 보였으나 축주의 요구에 의하여 난소자궁절제술을 시행하였다. 본 실험의 결과를 통하여 전침자극은 자궁 축농증에 이환된 환축에서 축주가 번식용으로 계속 기르기를 원한다거나 또는 난소자궁절제술을 환축에 적용하는데 있어서 수술적인 위험이 높을 때에 적용 가능한 새로운 치료적 대안이 될 수 있음을 알 수 있었다.

Online Survey on Clinical Application of Constraint-Induced Movement Therapy in Children with Hemiplegic Cerebral Palsy in Korea (편마비 뇌성마비 환아에서 강제유도운동치료의 국내 임상적용에 대한 설문조사)

  • Objective : The aim of this study was to evaluate the current knowledge regarding constraint-induced movement therapy (CIMT) and its application in clinical practice by physiatrists and therapists in pediatric rehabilitation area in Korea. Methods : Online survey via E-mails was sent to a total of 510 members (204 physiatrists and 306 therapists) of the Korean Society of Pediatric Rehabilitation and Developmental Medicine (KSPRDM). Results : The response rate was 35.1% (179 of 510). A total of 179 questionnaires was completed by 39 physiatrists, 89 physiotherapists, 48 occupational therapists, and 3 speech therapists. 45.8% of responders had worked over 6 years in the pediatric rehabilitation setting and a total of 58.1% (n=104) of the sample had used CIMT. The main limitations of clinically applying CIMT included limited staff and inappropriate clinical setting (35.1%, n=61), lack of understanding (19.5%, n=34), and developmental issues of function on the unaffected side (13.8%, n=24). The cooperation of patients (77.6%, n=76), cognitive/behavioral factors (42.9%, n=42), and cooperation of caregivers (25.5%, n=25) were the 3 major concerns that could be limitations with CIMT. Conclusions : Although considerable evidence supports the use of CIMT, many of physiatrist and therapists do not apply this method in practice. The improvement of limitations is necessary for wide use of CIMT in clinical practice in Korea.

A Preliminary Study on Effects of Abdominal Bracing Exercise on Respiratory Function of Normal Adults (복부 브레이싱 운동이 20대 성인의 호흡기능에 미치는 영향에 관한 예비연구)

  • Lumbar stabilization exercise is a popular exercise method in recent clinicians, as it strengthens muscles around the spine and abdominal muscles, but there is a lack of research on the effects of pulmonary function and respiratory pressure. So, this study examined whether abdominal bracing exercise, a typical lumbar stabilization exercise, was effective in pulmonary function and respiratory pressure. In this study, a total of 20's 40 subjects were recruited and randomly divided into an abdominal bracing exercise group(n=20) and control group(n=20). Abdominal exercise group underwent 25 minutes of exercise. Pulmonary function and respiratory pressure values were measured to analyze respiratory function. As a results of the experimental group, there were significant improvements in FVC, FEV1, PEF and MEP(p<.05) and there was a significant difference in the comparison between groups, except the MIP. These results suggest that abdominal bracing exercise can be presented as effective exercises to improve respiratory function.

  • Backgrounds: It was thought that sport was related with mentality. Compared to psychology, psychiatry has so far paid rather little attention to athletes. In this overview, I attempted to introduce the ideas of various observers of sports with the view of clinical psychiatry and summarized basic principle role of psychiatrist. Results and Discussions : To understand the athletic frame of mind, it is helpful to observe athletes in their psychologic development and when we do so we can quickly see trends that are not apparent by observing mature athletes alone. Psychobiology of sport would discuss some of the endogenous neurochemicals(hormone, neurotransmitter) that may be part and parcel of the athlete's psychiatric status. The psychological problems that frequently interfere with training and competition may be divided into five categories 1) choke, 2) slump, 3) injury, 4) interpersonal problem, 5) training dilemma. Psychiatric problem also arise in athletes like general people. various method for psychiatric patients like psychotherapy, group therapy, family systems perspective, and pharmacotherapy could been applied to sports psychiatry. Conclusions Although the contribution of sports psychiatry to sport is clinical one, psychiatrist's interest in role and conflict of athlete would enhance the molding of humanistic athletes who enjoy their life and sports.

A Study on Rehabilitation Treatment Using Radiofrequency Treatment (고주파 통증치료기를 이용한 재활 치료에 대한 연구)

  • When Radiofrequency energy is applied to the human body, the vibration width is very short. Therefore, the electrolyte burn generated when using the direct current does not occur. Ion molecules, polarized molecules, etc., vibrate more than 40,000 times per second, converting them into frictional heat to generate deep heat. The blood flow of capillaries increases 4-5 times more than at rest, increasing the supply of oxygen, nutrients, antibodies, and white blood cells. In addition, the electrochemical reaction does not occur because the vibration width and the pulsation period are very short. It is a physical factor treatment method that does not stimulate the sensory nerves and motor nerves. In this study, an isotonic exercise is performed in a young normal adult using a Radiofrequency pain treatment device. The purpose of this study is to integrate rehabilitation therapy by measuring electromyography data during isotonic exercise and confirming the effect on changes in motor neuron response. The EMG data generated when isotonic exercise of the forearm biceps muscle and the EMG data measured after the use of a Radiofrequency pain treatment device after exercise were RMS, respectively, and verified through t-test. It was confirmed that there was a significant difference in both men and women because the t-value was smaller than the significance level p (<.05).
