Facial soft tissue measuring analysis of normal occlusion using three-dimensional CT imaging (3차원 CT 영상을 이용한 정상교합자의 안면 연조직 계측 분석)

  • Studies for diagnostic analysis using three-dimensional (3D) CT images are recently in progress and needs for 3D craniofacial analysis are increasing in the fields of orthodontics. It is especially essential to analyze the facial soft tissue after orthodontic treatment and orthognathic surgery. In this study 3D CT images of adults with normal occlusion were taken to analyze the facial soft tissue. Norms were obtained from CT images of adults with normal occlusion (12 males, 11 females) using a computer program named V works 4.0 program. 3D coordinate planes were established using soft tissue Nasion as the reference point and a total of 20 reproducible landmarks of facial soft tissue were obtained using the multiple reconstructive sectional images (axial, sagittal and coronal images) of the V works 4.0 program: soft tissue Nasion, Pronasale, Subnasale, Upper lip center, Lower lip center, soft tissue B, soft tissue Pogonion, soft tissue Menton, Endocanthion (Rt/Lt), Alare lateralis (Rt/Lt), Cheilion (Rt/Lt), soft tissue Gonion (Rt/Lt), Tragus (Rt/Lt), and Zygomatic point (Rt/Lt). According to the established landmarks and measuring method, the 3D CT images of adults with normal occlusion were measured and the normal positional measurements and their Net (${\delta}=\sqrt{{X^2}+{Y^2}+{Z^2}}$) values were obtained using V surgery program, In the linear measurement between landmarks, there was a significant difference between males and females except Na' -Sn and En(Rt)-En(Lt). The normal ranges of Na'-Zy, Na'-Ch and Na'-Go' (facial depth) were obtained, which was difficult to measure by two-dimensional (2D) cephalometric analysis and facial photographs. These data may be used as references for 3D diagnosis and treatment planning for patients with malocclusion and dentofacial deformity.

A LONGITUDINAL STUDY ON THREE FACIAL GROWTH PATTERNS IN KOREANS WITH NORMAL OCCLUSION (정상교합자의 3가지 안면골 성장양상에 관한 누년적 연구)

  • The purpose of this study was to compare the difference of the growth aspects in three facial growth patterns. The biennial serial cephalometric radiographs of 33 samples(19males, 14females) with normal occlusion from 8.5 years to 18.5 yews of age were used in this study. The facial growth patterrn was categorized in 3 types(Drop type, Neutral type, Forward type) by the total amounts of the Y-axis which changed from 8.5 years to 18.5 years of age. The growth change of the craniofacial area during 10 years in each growth type was analyzed and was compared among the 3 growth types. The results of this study might be summarized as follows. 1. The samples that were classified by total change of the Y-axis during this study period were distributed to 52% of the neutral type, 27% of the forward type, 21% of the drop type. 2. The anterior growth of the maxilla to the cranial base(N per A) showed larger in the forward type than in other 2 types(p<0.05). 3. The palatal plane to the FH plane showed more anterior-superior inclination in the forward type with age during this study period. 4. The anterior growth of the mandible to the cranial base(N per Pog) appeared large in rank order, of largest the forward type, second the neutral type, and third the drop type(p<0.05). 5. During this study period the mandibular plane(SN/MN,FMA) showed more counterclockwise rotation in the forward type than in the drop type(p<0.05), and this tendency was stronger in males than in females(p<0.05). 6. The growth of the mandibular corpus length(Go-Me) showed smaller in the drop type than in the other 2 types(p<0.05). 7. In the forward type and the neutral type, the anterior growth of the mandible was larger than that of the maxilla(p<0.05). 8. In the craniofacial growth distances and angulations turned out to be somewhat variable, but the vertical proportion had a strong tendency whose original relation was maintained consistently during this study period. 9. Through these analyzed data, the profilograms on each growth type were constructed to evaluate individual growth pattern in the orthodontic diagnosis.

Clinical outcomes of implant supported fixed-hybrid prostheses in the fully edentulous arches (완전무치악 환자에서 고정성 임플란트 하이브리드 수복물의 임상성적)

  • Purpose: The aim of this study was to evaluate clinical outcomes of implant supported fixed-hybrid prostheses (FHP) in the fully edentulous arches. Materials and methods: Patients in this retrospective study were restored with fixed-hybrid prostheses supported by 4 to 6 implants and functioned more than 1 year of loading. Outcome measures were marginal bone change of implant related with sex, anatomical location (maxilla vs. mandible), opposing teeth, loading time of patients, tilting of posterior implant by Mann- Whitney U test and cantilever length of superstructure by regression analysis, and complication rates. Significance level was set P<.05. Results: A total number of 84 implants (16 restorations) placed in 16 patients were observed for 28 months and mean marginal bone loss was $0.53{\pm}0.39mm$. There were no differences of marginal bone loss according to sex, anatomical location (maxilla vs. mandible), opposing teeth, loading time of patients (P>.05), and cantilever length was not significantly related with a marginal bone loss of implant next to cantilever (P>.05). Complication was shown in 11 patients and veneer fracture and dislodging of artificial teeth were most prevalent. Conclusion: Within the limitations of this study, although marginal bone loss of FHP was very little, complication rates were high. Irrespective of tilting of most posterior implants, marginal bone loss of most posterior implants next to cantilever was less than those of the other implants positioned anteriorly. Cantilever length (<17 mm) did not affect a marginal bone loss of most posterior implants.

Position of maxillary central incisor and intercanine width in Korean adults (한국 유치악 성인의 상악 중절치 위치 및 견치간 거리에 관한 연구)

  • Purpose: The aim of this study was to propose the position of maxillary anterior teeth and intercanine width measurements based on the incisive papilla in accordance with the cephalic type and gender of dentate Korean adult with normal teeth alignment. Materials and methods: 42 students with Class I normal occlusion, without crowding or spacing, were selected from the Chonnam National University School of Dentistry. The lateral skull radiographs of the subjects were taken and were classified as different cephalic types, based on their PFH / AFH ratios. 42 casts of their maxilla were prepared and both the distance between the upper central incisors and the middle of the incisor papilla was measured with a vernier caliper (A) and the distance between the maxillary canine cusp tips (B) were measured. Statistical analysis was performed using SPSS version 15 and their significance was investigated. Results: For dolichocephalic group, the mean values for A and B were 8.43 (SD: 0.61) and 36.73 (SD: 2.17), respectively. The mean value for A was 8.51 (SD: 1.27) for the mesocephalic group and 8.76 (SD 1.03) for the brachycephalic group. The mean value for B was 35.91 (SD: 1.86) for the mesocephalic group and 37.34 (SD: 2.23) for the brachycephalic group. For the male group, the mean A value was 8.86 (SD: 1.04) and the mean B value was 37.60 (SD: 0.24). For the female group, the mean A value was 8.41 (SD: 0.93) and the mean B value was 36.18 (SD: 2.01). The difference between male and female group in A values were not statistically significant (P>.05). The B values of the male subjects were greater than those of the female subjects and was statistically significant (P<.05). Conclusion: 42 students with normal dentition and occlusion in korea, the distance from the incisive papilla and the incisal edge of maxillary central incisors had no difference in cephalic type or gender. However, the distance between the cusp tip of both canines had significant difference in gender where the male showed higher values than the female, while having no difference in cephalic types.

A MORPHOMETRIC STUDY OF THE MAXILLARY PRIMARY FIRST MOLARS USING THREE-DIMENSIONAL SCANNER (3차원 스캐너를 이용한 상악 제1유구치 치관의 크기와 형태에 관한 연구)

  • The purpose of this study was to elucidate the size and morphologic characteristics of maxillary primary first molars in Korean children using three-dimensional laser scanner and compare three-dimensional image with preformed stainless steel crown. Scanned three-dimensional images of dental cast taken from 132 children(male 62, female 70) by three-dimensional laser scanner(Breuckmann opto-Top HE100, INUS, Korea) were used. Mesiodistal diameter, buccolingual diameter, occlusogingival height and crown shape of each image were calculated by Rapidform 2004 program(INUS, Korea). The values were statistically compared by independent samples t-test with 95% of significant level. The results were as follows : 1. No significant difference in crown size was found between left and right maxillay primary first molar(p>0.05). 2, Significant difference in mesiodistal diameter, buccolingual diameter, buccal occlusogingival height was found between male and female (p<0.05), and crown size of male was bigger than that of female. 3. Average image of maxillay primary first molar was shaped three-dimensionally and measured. In comparison with 3M stainless steel crown, this image was similar with No.4 or No.5 SS crown in male, No.4 in female. In comparison with ILSUNG SS crown, this image was similar with No.5 in male, No.4 in female. 4 Mesiolingual line angle area, distolingual line angle area and buccogingival ridge were more obvious in average image than 3M stainless steel crown. ILSUNG SS crown was more square and had longer mesiodistal diameter than average 3D image.

Prevalence of anatomical alar band (콧방울띠의 유병율)

  • Purpose: Due to the presence of various muscles around lips, variety of facial expression can be made and changes from aging process such as wrinkles can develop on the facial skin by the action of multiple muscles. In animals, skin and muscles are developed in the entire body. On contrast, they are well developed only in the face and just one is present in the neck and the palm. Alar band was defined as outer wrinkle formed by zygomaticus minor muscle, which is common in Koreans. This study aimed to investigate clinical prevalence of alar band. Materials & Methods: Subjects were chosen from 780 new patients who visited private clinic in Gyeonggi province for orthodontic treatment. Presence of alar band was examined from the smile extraoral photos. Correlation among skeletal form, lip protrusion, gender, and age were evaluated. Results: Prevalence of alar band was higher in women (27.9%) than in men (18.5%) with statistical significance (p<0.05). With respect to age, prevalence of alar band was 19.4% in age 0-9 y, 16.9% in age 10-19 y, 31.2% in age 20-29 y, 39.5% in age 30-39, 56.5% in age 40-49. Prevalence was gradually increased from patients in their 20s to patients in their 40s and statistical significance was found (p<0.001). Concerning SN_NP, prevalence was 26.2% in normodivergent facial type, 22.0% in hyperdivergent facial type, and 32.2% in hypodivergent facial type. Hypodivergent facial group had higher prevalence but statistical significance was not observed. Statistically significant difference was not found regarding upper lip. However, prevalence of the alar band was 26% in patients with normal lower lip, 14.7% in patients with pretruded lower lip, and 33.3% in retruded lower lip. The prevalence was higher in patients with retruded lower lip with statistical significance (p<0.05). Conclusions: 27.8% on previous anatomical study and this study showed 27.8% prevalence of alar band in clinical smile photographs. Clinical photograph study showed that alar band was more prominent in women, older people, and people with retruded lips with statistical significance. This will provide valuable diagnostic information for esthetic consideration.

The efficacy evaluation of $PostPlant^{TM}$ Calcium in dental implant placement (임플란트 식립 환자의 $PostPlant^{TM}$ Calcium 투여 효과에 대한 임상연구)

  • Purpose: The aim of this study was to evaluate the changes in bone density and reduction of time consumed for osteogenesis by $PostPlant^{TM}$ Calcium and to find out efficacy of $PostPlant^{TM}$ Calcium by comparing the group prescribed with $PostPlant^{TM}$ Calcium with the group without $PostPlant^{TM}$ Calcium prescription. Material and methods: The experimental group of 18 patients with 25 dental implant placement and the control group of 7 patients with 9 dental implant placement were randomly selected from the patients who visited prosthetic department of Dankook University Dental Hospital since July, 2006 (IRB Number ; 20060710). The experimental group was instructed to take $PostPlant^{TM}$ Calcium for 6 months after the implant surgery while the control group was instructed not to. Both experimental and control group were assigned for measurement using $Osstell^{TM}$ Mentor and $Periotest^{(R)}$ and radiographic examination was performed using specifically manufactured Aluminum Step Wedge. The results were compared and analyzed. Results: 1. According to the $Osstell^{TM}$ Mentor measurement, both the experimental and control group showed increase in values as time elapses and the experimental group showed significantly higher rate of increase (P < .05). 2. According to the $Periotest^{(R)}$ measurement, both the experimental and control group showed decrease in values as time elapses. In addition, greater decrease can be seen in the experimental group but no statistical significance was found. 3. By examining the radiographic images, both the experimental and control group showed tendency of increase in bone density. In addition, greater increase can be seen in the experimental group but no statistical significance was found. Conclusion: Clinically, taking $PostPlant^{TM}$ Calcium medicine for a long period of time after implant placement is expected for a better prognosis.

THE EFFECT OF DISTAL MOVEMENT OF UPPER MOLAR USING THE PENDULUM APPLIANCE (Pendulum 장치의 상악대구치의 원심이동에 대한 효과)

  • Treatment of class II malocclusions require distalization of maxillary molars into class I relationship. Intraarch distal molar movement techniques have recently assumed an important role in young patients. In this study, the dental and skeletal effects of the pendulum appliance were evaluated by means of cephalometric radiographs. The samples were consisted of 19 patients: 11 females and 8 males, mean age $11.68{\pm}1.52$ years. Measurements were obtained from cephalometric prior to and the day of removal of the pendulum appliance. Treatment changes were analyzed. The following results were obtain. 1. The pendulum appliance produced $2.94{\pm}1.54mm$ distal molar movement with a mean intrusion of $1.17{\pm}0.97mm$, mean period $18.13{\pm}7.95$ weeks. 2. The anchor tooth was $1.34{\pm}1.40mm$ forward movement and $0.48{\pm}0.99mm$ extrusion, and labial tilting of incisors. 3. The angle between palatal plane and mandibular plane increased significantly. 4. There was no significant difference in according to 2nd molar position. 5. Total movement was consisted of 74% distal movement of 1st molar and 26% forward movement of the anchor tooth.

RELATIONSHIP BETWEEN THE DEVELOPMENTAL STAGE AND CHRONOLOGICAL AGE, AND THE CHANGES OF TOOTH POSITION IN RELATION TO THE TOOTH DEVELOPMENT ON MANDIBULAR PERMANENT TEETH (하악 영구치아의 발육과 연령과의 관계 및 치아 발육에 따른 치아의 위치 변화)

  • The purposes of this study were to evaluate the timing of tooth calcification and the change of tooth position with tooth developmental stage on the mandibular teeth. Seven hundred seventy two children(male:446, female:326), 3 to 12 years of age were examined radiographically with panoramic film. Dental development was determined by inspecting radiographs and assigning a rating according to consecutive stages defined by Moorrees, and tooth cusp position and root terminus position were measured from the lower border of mandibular body and calculated the position index to evaluate the movement of tooth with developmental stage. The results were as follows. 1. There were no significant differences between boys and girls in the timing of calcification until crown completion, but timing of calcification tend to be faster in girls than in boys after root initiation stage. 2. In terms of mean age, crown completion of central incisor in boys and girls occurred at the age of 3.71, 4.05 years, at 4.44, 4.60 years for the lateral incisor, at 5.35, 5.11 years for the canine, at 6.62, 6.36 years for the first premolar, at 7.36, 7.17 years of second premolar, at 3.51, 3.69 years of first molar, and at 7.90, 7.64 years for the second molar respectively. Apex 1/2 closed stage of central incisor occurred at the age of 8.70 in boys, 8.18 in girls, at 9.55, 8.99 years for the lateral incisor, at 12.48, 11.60 years for the canine, at 12.30, 12.01 years for the first premolar, at 12.19, 12.26 years of second premolar, at 9.12, 8.87 years of first molar, and at 12.59, 12.45 years for the second molar respectively. 3. There was no noticeable movement of cusp tip until crown completion (Crc), but showed rapid movement toward occlusion plane after root initiation(Ri) and again maintain stable position after root completion stage(Rc). 4. Root terminus position was stable until root 1/4 formation stage(R1/4), followed by rapid movement toward occlusal plane and was stable again after root 3/4 formation stage(R3/4). 5. Developmental stage at the time of alveolar bone penetration by cusp tip varied with each of the permanent teeth. 6. Canine tooth follicle was at the lowest position in the mandibular body during the early stage of calcification, followed by second premolar, first premolar, lateral incisor, second molar, first molar and central incisor in order.

DEVELOPMENT OF ALLOTRANSPLANTED TOOTH GERMS AT VARIOUS DEVELOPMENTAL STAGE INTO THE WHITE RAT'S EXTRACTION SOCKET (흰쥐의 발치와에 이식한 단계별 치아싹의 발육 과정)

  • The purpose of this study is to evaluate at which stage of tooth germ would develop into normal calcification and hence to increase the success rate of transplantation. Therefore, tooth germs on the 15th, 17th embryonic day and the 3rd day of birth were separated for allotransplantation into maxilla of adult rat of 11 weeks. Calcification processes were analyzed radiographically and histopathologically at 4 weeks and 8 weeks after allotransplantation. The results are as follows: 1. Allotransplanted tooth germ at 4 weeks and 8 weeks showed delayed calcification compared to that of normal odontogenesis. 2. At 4 weeks, abnormal calcified tissue, such as odontoma and ankylosis of osteodentin with surrounding alveolar bone were observed. 3. At 8 weeks, allotransplanted tooth germs of the 15th and 17th embryonic day showed calcification and osteodentin surrounded by periodontal ligament. 4. At 8 weeks, allotransplanted tooth germs of the 3rd day of birth showed calcification composed of cementum and osteodentin. In this study, we observed small sized and amorphous calcified tissue from allotropic allotransplantation of tooth germs. Since these calcified tissue were underdeveloped and shaped irregularly, for calcification into normal tooth form, further study needs consideration about the reduction of surgical trauma, developmental stage of transplanted tooth germ, blood supply from recipient site, fixation method in transplanted site and period of transplantation.
