A study on the knowledge and awareness of care workers on the geriatric oral health (요양보호사의 노인 구강보건에 대한 지식과 인식도 연구)

  • The study aims to examine the knowledge and awareness of care workers on oral health knowledge at the geriatric care facilities in order to improve and develop oral health services. For the research method, the study carried out a questionnaire survey to 163 care workers working at the geriatric care facilities (10) in D Metropolitan City from May 1~31, 2015 and then the collected data were statistically analyzed. As a result, the oral health knowledge of care workers was 20.09 points out of total 26 points (77.2%). The highest knowledge was 'proper toothbrushing method' with 93.0% and the lowest one was 'knowledge on the use of oral supplementary goods' with 58.1%. Those with experience of oral health education had higher demand on oral health education (P<0.001), and those with higher experience of oral health education had significantly higher oral health knowledge (P<0.001), demand (P<0.01) and awareness (P<0.05). As for factors influencing care workers' oral health awareness, if they had higher oral health knowledge points (P<0.01), oral health education demand (P<0.01), they had higher oral health awareness level. Accordingly, it is required to provide efficient, sustainable and practical oral health education of care workers considering oral characteristics of the long-term hospitalized patients at the care hospital. And, the role of dental hygienist as the person in charge of oral health education is necessary. Therefore, institutional support from the government is required to assign professional personnel.

Health Status of Vulnerable Preschool Children and Their Mothers' Health Management (취약계층 학령전기 아동의 건강상태와 어머니의 아동 건강관리실태)

  • Purpose: The purpose of this study is to find out preschool children's health status and their mothers' health management in the vulnerable classes. Methods: The assessment tool was developed, taken into consideration existing studies, materials produced by the customized visiting health care system, and review of visiting nurses of health centers and related experts. Data were collected January to February 2010 from 259 mothers by visiting nurses, and analyzed using SAS program for descriptive statistics. Results: Body weight less than 3 percentile was found for 5.0% children and over 97 percentile for 7.7%. Atopy was found in 17.8% children, no hand-washing after toileting and before meal in 30.9% and 36.7% respectively, no breakfast in 15.8%, and irregular meal in 32.0%. Sex education was made by 45.7% mothers, regular dental check by 56.6%, and hearing and eyesight test by 61.1% and 66.8% respectively. Home environment for upbringing is 34.3 in the scale of 41, and accident prevention 17.5 in the scale of 22. Conclusion: It is necessary to make an intervention on children's weight, personal sanitation and meal time in the vulnerable classes. Mothers need to be educated for appropriate health care, and home environments to improve upbringing and accident prevention.

Client Services Evaluation in Dentistry Sector (치과의료기관 방문시 불만처리서비스에 대한 의료소비자의 평가)

  • The purpose of this study was to examine how clients who visited dental institutions perceived client services, what sorts of client services were provided to them and to what extent they were satisfied with them. It's ultimately meant to seek ways to enhance and enlarge client services. A survey was conducted on 379 residents who used dental institutions in Seoul and Gyeonggi provinces, and the findings of this study were as follows: 1. 64.6 percent of the people investigated weren't aware of client services, and 33.0 percent viewed them as part of medical services. 77.5 percent had grievances about dental institutions they'd ever used. 2. The biggest complaint was that the dental treatments they received weren't covered by the dental insurance. As for how to solve their grievances, the largest group of them told the employees of the dental institutions about their complaints or didn't use them again. When asked whether the dental institutions took any steps to get rid of their grievances, the greatest group replied they had no idea. and the second largest group answered they took no measure. 3. By age and educational level, those who were in their 60s and up and stopped at elementary school were best cognizant of client services. Among client service variables, they were most satisfied with how the employees handled their complaints, and as to overall satisfaction, their willingness to revisit ranked highest. 4. Concerning connections among client service awareness, service variables and overall satisfaction level, their awareness of client services had a positive correlational relationship with every service variable and satisfaction level. Among the service variables, prompt client services and employee attitude were positively correlated to overall satisfaction level, but service procedure, facilities and information services exercised little impact on that. 5. As for what factors affected their content with client services, their client service awareness was identified as one of the important factors to influence their use of dental institutions, the outcome of their visit and their willingness to revisit. The above-mentioned findings suggested that dental institutions should strengthen publicity activities to inform people of client services, and encourage them to express their grievances. In addition, they should take an immediate action to remove their complaints, and try to get a successful feedback to offer higher-quality medical services and customer-oriented services.

MANAGEMENT OF THE IMPACTED TOOTH ASSOCIATED WITH DENTIGEROUS CYST IN AUTISTIC YOUNG PATIENTS (자폐증 환자에서 함치성 낭종과 연관된 매복치의 처치)

  • Autism is a developmental, neuropsychiatric disorder that begins in early childhood. A patient with autism seen in the dental office frequently may have many complications. Therefore, it may be needed to consider modified or alternative therapy for dental care of autistic patients. This is the case of a 16-year old boy who have autism. He came to the department of the pediatric dentistry, Yonsei University Dental Hospital, for evaluation and treatment of dentigerous cyst associated with impacted teeth(#33,34,35). Under daily hospitalization and general anesthesia, the cyst was enucleated with surgical extraction of #34 and autotransplantation of #33,35. And during the periodic dental followup, apexification of #33,35 was performed for periapical lesion and root maturity. At 2 year 6 months follow- up, now, bony healing was completed and there are some complications like external resorption of #33 and space loss of #34 area. Generally, the marsupialization has been widely recommended for treatment of dentigerous cyst. However, in this case, there is a little possibility of spontaneous eruption after marsupialization considering of patient's age, location and angulation of the impacted tooth, root maturity. And there is necessity to choose the treatment that has low recurrence risk and needs short-term follow-up for autism. Above all, poor oral hygiene and lack of cooperation for decompression treatment is a matter of primary consideration. Consequently, enucleation of the cyst was chosen for the final treatment plan in this case. It is important to consider the conditions that affect the eruption of a dentigerous cyst-associated tooth to predict the successful eruption and special health care needs of the patient when the treatment plan is settled.

A study on the oral health state and oral health care of dental clinic patients (치과 내원환자들의 구강건강수준과 구강건강관리 행태에 관한 연구)

  • The purpose of this study was to examine the oral health status and oral health care of adults using dental clinics and the relationship between their oral health state and oral health care. The subjects in this study were 219 patients who visited dental clinics in Gyeonggi province between February and April 2008. A dental checkup and survey were implemented, and SPSS 12.0 program was utilized to analyze the collected data. The findings of the study were as follows: 1. The DT, MT and FT rates of the respondents who used oral hygiene supplies respectively stood at 30.7, 24.6 and 24.6 percent, which were all low. The gaps between them and the others were statistically significant. The DMFT index of the users of oral hygiene supplies was 12.3, which was high, but the difference between them and the others was not statistically significant. 2. The DT rate of those who got a dental checkup on a regular basis stood at 20.8 percent, which was low. The gap between them and the others was statistically significant. Both the MT and FT rates of the former numbered 28.8 percent, which was low, but the gap between them and the others was statistically insignificant. The DMFT index of the respondents who got a regular dental checkup was 12.8, which was high, and the gap between them and the others was statistically significant. 3. The DT and FT rates of respondents who had ever had their teeth scaled respectively stood at 24.6 and 27.1 percent, which were both low, and the differences between them and the others were statistically significant. The MT rate of the former was 27.1 percent, which was low, but the gap between them and the others was statistically insignificant. The DMFT index of those who had ever had their teeth scaled was 12.2, which was high, and the gap between them and the others was statistically significant. The above mentioned findings suggested that there was a very close relationship between oral health care and oral health status. Therefore people in general should be encouraged to pay more attention to their own oral health care, and they should be well informed about how to take care of the oral cavity to promote their oral health.

Effects of Professional Oral Care for Long-term Patients in Nursing Facilities on the Streptococcus mutans Population in the Intraoral Region (전문가구강관리가 요양병원 장기입원환자들의 구강 내 Streptococcus mutans 수에 미치는 영향)

  • In this study, professional oral care was provided and 3 times a day twice a week by a dental hygienist for 3 months from July 24 to October 25, 2011, to 43 long-term patients in a single nursing facility located at the city of D, and the number of mutans strepococci colonies within the saliva and the level of salivation were then analyzed after 3 samplings with an interval of 1 month to compare the intraoral conditions among the subjects and examine the effects of professional oral care on the Streptococcus mutans population in the intraoral region of long-term patients in a nursing facility. The level of salivation was elevated from 5.8 ml to 6.4 ml after one month and to 7.5mL after two months. The population of bacteria decreased from $6{\times}10^8CFU/ml$ to $3{\times}105CFU/ml$. In terms of the change in the bacterial population in accordance with systemic diseases, the patients with hypertension and diabetes showed a meaningful decrease in the population. As more dental charge and prosthesis are present, the number of bacteria decreased significantly. On account of the close relationship between a professional oral care and the population of Streptococcus mutans at the intraoral region of long-term patients in nursing facilities, various dental health programs should be researched and developed to consider the characteristics of long-term patients in nursing facilities and manage them continuously but effectively.

A Study on the Primary Factors in the Selection of Dental Medical Organizations (치과의료기관을 선택하는 요인에 관한 조사연구)

  • This study was conducted to find out basic information on better customer satisfaction and service by examining and analyzing in what routes patients mostly have selected their dental medical organizations and how they have reacted to their dissatisfaction. The subject of study included the patients who have come to dental clinics and dental hospitals to be treated in Cheonan City, Chungnam and the researcher of this study used a survey and analyzed its gathered data using an SPSS(Statistical Package for the Social Science) WIN 11.5 Program. As analysis methods, frequency and percentage were calculated to figure out the general characteristics of its subject. In addition, a ${\chi}^2$(Chi-square) test was performed to figure out the factors that patients have selected their dental medical organizations and the following results were obtained. 1. The largest number of patients selected their dental medical organizations by hearsay and more female and married patients selected them by hearsay than other patients, and there were significant differences(p<.05). 2. The dental patients put the priority on treatment techniques in their visiting hospitals and the patients who were more than 40 years old and married patients put more emphasis on treatment techniques in their visiting hospitals than other patients and there were significant differences(p<.05). 3. The said dental patients were not satisfied with the kindness of medical staffs in their visiting medical organizations; meanwhile, the patients who were aged below 30 and had an academic background of more than university degree and were unmarried were less satisfied with the kindness of medical staffs in their visiting medical organizations than other patients and there were significant differences(p<.05). 4. In terms of improvement methods or complaints, the largest number of them said that they just ignored and endured; meanwhile, the patients who were aged below 30 and had an education of more than university degree and were unmarried were more patient of improvement methods or complaints than other patients and there were significant differences(p<.05). 5. There were more patients who have introduced their visiting medical organizations to their neighboring people than the patients who have not done it; in the meantime, the patients who were aged over 40 and were married have introduced their visiting medical organizations to their neighboring people more than other patients(p<.05).

Clinical Evaluation of Passive Eruption Using Occlusal Reduction on Periodontium (교합면 삭제(occlusal reduction)를 이용한 수동적 정출 (passive eruption)이 치주조직에 미치는 영향)

  • The relationship between occlusion and periodontal health has been extensively studied. However, reports on the effects of passive eruption using occlusal reduction has not been sufficient. The purpose of the present randomized clinical trial was to assess the influence of passive eruption using occlusal reduction on the clinical periodontal parameters consisting of attachment level, pocket depth, tooth mobility, width of keratinized gingiva and osseous defect. The study was performed on 40 teeth of 16 subjects who have been treated for the moderate periodontitis at the Department of Periodontology, Pusan National University Hospital. At the baseline examination, after hygienic-phase and after 6 month from passive eruption using occlusal reduction, clinical parameters were monitored and radiographs were taken. The 20 teeth in the test group received passive eruption using occlusal reduction while the 20 control teeth did not receive any occlusal reduction. The results were as follows; 1) Degree of inflammation of periodontium was improved by initial therapy 2) Teeth received passive eruption using occlusal reduction demonstrated significantly greater reduction in pocket depth, tooth mobility and amount of bone loss, and increase in the width of keratinized gingiva, but no significant changes in the attachment level compared to the control teeth 3) There was significantly greater reduction in pocket depth, mobility, amount of bone loss and attachment level in the test teeth after initial hygienic phase when compared with baseline data. Taken together, these results suggest that the passive eruption using occlusal reduction would be helpful to improve periodontal health.

DENTAL TREATMENT UNDER GENERAL ANESTHESIA IN A PATIENT WITH LOWE SYNDROME : A CASE REPORT (로웨 증후군 환아의 치아우식 치료 증례 보고)

  • Lowe syndrome is X-linked gene disorder, characterized by cataracts, renal dysfunction and brain abnormalities. Oral healthcare of young patients with Lowe syndrome could be easily neglected due to the uncooperative behavior or other systemic condition of the child. We are presenting a case of successful treatment under general anesthesia with uncooperative child with Lowe syndrome. A 3-year old boy with Lowe syndrome visited Seoul National University Dental Hospital for multiple caries. He had been suffering from congenital cataract, medullary nephrocalcinosis and hypotonia. Multiple caries were observed from clinical and radiographic examination. Concerning behavior management problem and possibility of aspiration due to hypotonia, dental treatment under general anesthesia was planned. Left upper primary first molar was extracted to prevent further infection. Other teeth were treated with Stainless steel crown or composite resin restoration based upon the extent and the severity of dental caries. Under general anesthesia, dental procedure was carried out successfully and safely. Considering uncooperative behavior and other medical conditions of the patient, general anesthesia could be effective. Home oral care and periodic visit to dental clinic should be emphasized to the caregiver of patient with Lowe syndrome, considering the susceptibility of dental caries and other oral manifestation.

Investigation about the Actual Prevention of Infection and Vaccination against B-type Hepatitis among Dental Workers in Daegu (대구지역 치과 의료기관 종사자의 B형간염 예방접종 및 감염관리 실태)

  • This study was conducted on 185 workers at 73 dental clinics and university hospitals in Daegu to investigate the actual prevention of infection and vaccination against B-type hepatitis among dental workers. 1. According to the result of medical examination, only 35 out of 144 (24%) dental clinic workers had periodic health examination while 7 out of 9 (78%) university hospital workers did. 2. In a survey on vaccination against B-type hepatitis, 52 workers aged 29 or younger (96.3%), 38 dental hygienists (51.9%) and 44 dental workers at dental clinics (81.5%) have not had any vaccination against B-type hepatitis. The rest appeared to have had vaccination or be aware that they had antibody against B-type hepatitis without having to have any vaccination. 3. According to the result of a survey on the existence of antibody by job, 42 (56%) of dental hygienists, 15 (20%) of assistant nurses, 12 (16%) of medical assistants and 6 (8%) of dental technicians did not know whether or not they had antibody. This suggests that all types of dental workers except dental hygienists have low awareness of whether or not they have antibody. 4. In a survey on the relation between general characteristics of subjects and the sterilization of dental equipment, alcohol disinfection of high speed handpiece and low speed handpiece was most common among dental workers aged 29 or younger, and all of those aged over 40 used autoclave. By position, alcohol disinfection was used most commonly for high-speed handpiece. antiseptic solution deposition for disposable suction lips, and autoclaving for impression. By workplace, dental workers at university hospitals used autoc1aving most frequently for high/low speed handpiece while those at dental hospitals and dental clinics used alcohol disinfection most frequently and even some respondents replied that they did not disinfect. For metal cups, workers at dental clinics and dental hospitals did not use any sterilizing method while those at university hospitals used autoclaving. For disposable suction tips, workers at dental clinics used antiseptic solution deposition and those at dental hospitals used alcohol disinfection but some respondents replied that they did not disinfect. For metal suctions and impression trays, autoclaving was most common in all workplaces but some dental clinics replied that they did not disinfect impression trays. According to work experience, alcohol disinfection was most common for high/low speed handpiece. For disposable suction tips, dental workers with 3 years' or shorter work experience, those with 3~6 years' experience and those with 9~12 years' experience used antiseptic solution deposition most commonly, and many of those with 6~9 years replied that they did not disinfect. The results of this study stated above suggest that systematic education is necessary for all dental workers for enhancing th eir awareness of B-type hepatitis and the prevention of infection. Moreover, dental workers are required to make efforts to prevent infection with B-type hepatitis voluntarily and actively.
