FINITE ELEMENT ANALYSIS OF MANDIBULAR STRESSES AND DENTURE MOVEMENTS INDUCED BY VARIOUS DENTURE BASE MATERIALS (의치상 재료에 따른 하악응력 및 의치의 변위에 관한 유한요소법적 분석)

  • 의치상 재료 종류에 따라 의치, 점막 및 하악골에 발생되는 변위 및 응력을 연구하기 위하여 컴퓨터를 이용한 수치적 해석인 2차원 유한 요소법을 이용하였다. 2차원 유한 요소 모형으로선 표준 크기의 하악골 및 의치를 고려하여 231개의 사변형 요소 및 268개의 절점으로 분할한 후 각 구성 성분의 물리적 성질인 탄성률 및 프와송비를 대입시켰다. 사용된 의치로서는 일반 합성수지의치, 2mm후경의 탄성재를 의치상 하부에 이장한 합성수지의치, 2mm후경의 탄성재를 치아와 의치상 중앙에 삽입한 합성수지의치 및 0.5mm후경의 금속상의치의 4종류였으며, 하중시에 하악의 고정 부위로선 생체와 동일 조건을 부여하기 위하여 교근, 내측익돌근, 측두근등의 하악 폐구근이 부착되는 하악각 부위 및 하악 근돌기 부위의 16절점을 고정점으로 하였다. 하중 조건으로선 하악 제1대구치의 일점에 10kg의 수직 집중하중, 하악 중절치의 일점에 7kg의 수직 집중하중 및 하악 제 1소구치로 부터 하악 제2대구치까지의 교합면에 10kg의 수직 분산하중을 부여하여 분석한 결과는 다음과 같다. 1. 하중이 의치 교합면위의 가해진 부위에 따라 다양한 의치 회전 및 강하 현상을 보였으며, 탄성재를 이장 및 삽입한 합성 수지 의치의 변위가 일반 합성수지의치 및 금속상 의치의 변위보다 더 컸다. 2. 주응력을 고려할때 점막 부위에는 주로 압축 응력이 작용하였으며 치조제 부위는 압축응력과 인장 응력이 함께 작용하였다. 3. 탄성재를 삽입한 합성수지의치에 최고 등가 응력이 집중되었으며 그 다음은 탄성재를 이장한 합성수지의치, 일반 합성수지의치의 순이였으며 금속상의 경우는 금속을 따라서 높은 등가 응력이 넓게 분산되었다. 4. 의치상 종류에 관계없이 동일 하중 조건하에선 점막에 나타나는 등가 응력의 크기 및 분산양태는 유사하였다. 5. 하악골에서 등가 응력은 의치지지 부위에만 국한되지 않고 넓게 분산 되었으며 의치상 종류 및 하중 조건에 관계없이 치조제 후방 및 하악연의 후방 부위에 특히 높은 등가응력이 집중되었다. 6. 하악 중절치의 일점에 수직 하중을 가한 경우가 다른 하중 조건에 비하여 지지점과의 거리차이로 인하여 하악골에 가장 높은 등가 응력을 유발하였다. 7. 의치상 재료에 따른 하악 골에 발생되는 응력의 크기 및 분산에는 큰 차이가 없으나 금속상의 경우가 교합압을 분산하는데는 효과적이었다.

  • When the patient with class III malocclusion needs extensive oral rehabilitation due to multiple missing teeth, accurate diagnosis, and careful analysis, such as the patient's occlusal relationship, facial changes, and evaluation of the temporomandibular joint are essential. Orthognathic surgery is often performed for aesthetic improvement, depending on the patient's chief complaint. If it is not possible due to certain circumstances, partial aesthetic improvement can be achieved through minimal elevation of the vertical dimension. As this patient may have unexpected issues, such as temporomandibular joint disorder, oral habits like bruxism, and masticatory muscle tension, it was determined whether the patient could adjust to a reversible temporary removable partial denture. After this, the maxillary implant-supported fixed prostheses and the mandibular fixed prostheses were used to achieve stable posterior support and to partially improve the maxillary anterior esthetics. The patient was satisfied with the results both aesthetically and functionally. The prognosis is expected to be good if regular check-ups are conducted.

Stiffness and Elasticity of the Masticatory and Facial Expression Muscles in Patients with the Masticatory Muscle Pain (저작근통 환자에서 저작근 및 안면표정근의 경도와 탄성도 평가)

  • This study aimed to assess stiffness and elasticity of the masticatory muscle in the patients with the masticatory muscle pain using a tactile sensor and to investigate whether the masticatory muscle pain affects the facial expression muscles. From those who visited Department of Oral Medicine in Dankook University Dental Hospital, 27 patients presenting with unilateral muscle pain and tenderness in the masseter muscle (Ms) were selected (mean age: $36.4{\pm}13.8$ years). Exclusion criterion was those who also had temporomandibular joint (TMJ) disorders or any neurological pain. Muscle stiffness and elasticity for the muscles of mastication and facial expression was investigated with the tactile sensor (Venustron, Axiom Co., JAPAN) and the muscles measured were the Ms, anterior temporal muscle (Ta), frontalis (Fr), inferior orbicularis oculi (Ooci), zygomaticus major (Zm), superior and inferior orbicularis oris (Oors, Oori) and mentalis (Mn). t-tests was used to compare side difference in muscle stiffness and elasticity. Side differences were also compared between diagnostic groups (local muscle soreness (LMS) vs myofascial pain syndrome (MPS) and between acute (< 6M) and chronic ($\geq$ 6M) groups. This study showed that Ms and Zm at affected side exhibited significantly increased stiffness and decreased elasticity as compared to the unaffected side.(p<0.05) There was no significant difference between local muscle soreness and myofascial pain syndrome groups and between acute and chronic groups. The results of this study suggests that masticatory muscle pain in Ms can affect muscle stiffness and elasticity not only for Ms but also for Zm, the facial expression muscle.

Effects of the Changes of Mandibular Position on the Muscle Activity in Masseter and Anterior Temporalis and on the Bite Force (하악위의 변화가 교근과 전측두근의 근활성 및 교합력에 끼치는 영향)

  • The author studied masticatory muscle activity and bite force in normal persons without Temporomandibular Disorders(TMD) signs and symptoms, The number of subjects was 15, and the age of them was from 22 to 25 years. Electromyography was used to record the muscle activity in tapping and clenching movement with or without occlusal splint. 3 splints were made from 3 different mandibular position, that if, centric occlusion position, Rocabado's mandibular rest position, Dawson's centric relation position. The thickness of splint was 3.0-3.5㎜ at molar region. The muscle examined were Masseter and Anterior Temporalis attached with surface electrodes and the device used to measure the EMG level was Bioelectric processor Model EM2. After recording the EMG, the author measured the bite force level in clenching movement with bite force meter Model MPM-3000 in the dame position used in the EMG experiment. The obtained results were as follow : 1. With occlusal splints insetion, the amount of decreased muscle activity in Anterior Temporalis was more than those in Masseter. 2. In the three maxillomandibular relationships with occlusal splints, Masseter showed slightly increased level of muscle in centric occlusion but Ant. Temporalis showed decreased level of muscle activity reversely in that position. 3. Muscle activities between Rocabado's rest position and Dawson's centric relation position were generally similar whatever the muscles or the movements the author examined. 4. Bite force in clenching movement increased with splints insertion, especially with the splint registered in centric occlusion position.

  • Hemifacial microsomia is characterized underdevelopment of the TMJ, mandibular ramus, and associated muscles of mastication. The Maxilla and malar bones on the affected side frequently are underdeveloped. The contiguous parotid gland may be hypoplastic. Preauricular sinus tracts and tags may exist, along with underdevelopment of the associated external ear, and affected facial nerve and muscles of facial expression may also show dysfunction. Children exhibiting the more classic signs will be identified at birth. Little is known about the etiology of hemifacial microsomia. We have corrected surgically a 22-year-old woman with hemifacial microsomia. We have performed leveling Le Fort I osteotomy with iliac bone graft on the maxilla, reverse-L osteotomy and iliac bone graft on the right mandibular ramus, vertical ramus osteotomy on the left side, onlay bone graft on the right mandibular body, and augmentation genioplasty. The postoperative course was uneventful and restoration of facial asymmetry was achieved.

Effects of Masseter and Cervical Muscle Activity in Temporomandibular Joint Disorder (저작근 및 경부근 긴장도가 측두하악장애에 미치는 영향)

  • Objectives : The purpose of this study was to investigate the relationship between masseter and cervical muscle activity and temporomandibular disorder in female office workers. Methods : Experimental group of 24 healthy subjects complained of temporomandibular joint related to computer use which lasted more than 3 months in the past year and was present in the past 7 days as well as on the day of test. Control group of 20 healthy subjects had no complaints of minimal discomfort on the day of test, and had no discomfort in the past 7 days. If they had reported discomfort in the past 12 months, it was of a short duration(<3 months) and resolved at least 3 months prior to participation. Outcomes were assessed by meridian-electromyography(MEMG), whole spin x-ray, mandibular function impairment questionnaire(MFIQ), neck disability index(NDI), visual analog scale(VAS), Beck depression inventory(BDI), stress reaction inventory(SRI) and Holmes & Rahe social readjustment rating scale(SRRS). Results : The contraction power of masseter muscle, upper trapezius, sternocleido-mastoid muscle and erector spinae by MEMG was significantly higher in the experimental group. The muscle fatigue of masseter muscle and sternodeido-mastoid muscle by MEMG was significantly higher in the experimental group. SRI was significantly higher in experimental group. There was no significant difference between two groups in the Jackson's angle, Cobb's method and cranio-cervical posture. Conclusions : The results suggest that temporomandibular disorder related mental stress but physical stress does not change cervical structure significantly.

Usefulness of Bipedicle Temporalis-pericranial Flap for Closure of Frontoethmoidal Encephalomeningoceles (전두사골 뇌수막류의 폐쇄를 위한 양경 측두근골막피판의 유용성)

  • Purpose: To close anterior cranial base, various types of pedicle flaps have been developed previously. However, the results of those pedicle flaps were not constant. To solve such problem, the author designed bipedicle temporalis-pericranial (BTP) flap based on various types of existing flaps and this study intends to introduce this flap and present clinical application case. Methods: The pedicle of the proposed temporalis-pericranial flap is temporalis muscle. The point of this BTP flap is that because of both sides of the unilateral temporalis-pericranial flap are connected by midline pericranial tissue connected with dense vascular network communicate one another locally, that BTP flap can be safely elevated. The case is a 14 months old male patient of frontoethmoidal encephalomeningocele. Surgery was done in a way that after elevating BTP flap and removing encephalomeningocele, BTP flap was moved intracranially, and to prevent cerebrospinal fluid leakage, anterior cranial base was closed. Results: During 1 year and 6 month outpatient tracking observation, no particular finding like CSF leakage, meningitis or hydrocephalus was observed. Conclusion: The benchmarked BTP flap, effective in the treatment of frontoethmoidal encephalomeningocele, is one of the methods to close intracranium and extracranium.

USING OF THE TEMPORALIS MUSCLE AND FASCIA FLAP FOR MAXILLA RECONSTURCTION (상악 결손부 수복을 위한 측두피판의 이용)

  • The functional deformity following removal of the maxilla is considerable, especially following maxilla cancer excision. Rehabilitation of oral and maxillofacial region used to be very difficult with prosthesis or traditional flaps. Temporal muscle and fascia flaps have been described for immediate reconstruction following hemi-maxillectomy, but not total maxillectomy. The muscle and superficial fascia of the temporal area differ in their physical characteristics, vascular supply and clinical applications. Both can be employed independently or simultaneously as regional flaps in the reconstruction of a variety of complex craniofacial defects. Four case is presented in which 3 case maxilla cancer, one case non-union after maxilla fracture, and all case showed successful use of this flap. Only one patient developed partial necrosis of the flap ; significant necrosis did not occur in any other patient. This present paper reviews the anatomy, surgical technique and utilisation of temporal muscle and fascia flaps.

THE EFFECT OF COSTCHONDRAL GRAFT ON THE RECONSTRUCTION OF TEMPOROMANDIBULAR JOINT (측두하악 관절의 재건을 위한 늑연골 이식의 효과)

  • We performed costochondral grafting in mmandibular reconstruction including condyle replacement in 5 patients which have 4 cases of ameloblastoma, 1 case of resorption of left condyle due to osteomyelitis. We harvested the 6 th costochondral cartilage of unaffected side. No specific complications were appeared with unexpected fracture of costochondral junction and infection of operation site, These patients show 30-45mm at month opening, near normal of affected side, near normal or normal of unaffected side at lateral excursion. Postoperative coureses was uneventful with improved cosmetic and functional results. Our report is indicated that the costochondral graft is satisfactory to reconstruct mandibular defect including condyle replacement, especially in growing patients.

The Effects of Stabilization on the Electromyographic Activities of the Masseter and Anterior Temporal Muscles during Maximal Clenching (안정교합장치가 교근 및 전측두근 활성에 미치는 영향)

  • The author studied the changes of electromyographic activities of the masseter and anterior temporal muscles during maximal clenching before and after stabilization splint wearing. It also studied the changes of the symmetry of the muscle activity during maximal clenching. For this study, 15-healthy-female-students were selected. The obtained results were as follows : The EMG activities of right and left anterior temporal muscles and right masseter muscle during maximal clenching immediately after wearing of the stabilization splint were not changed compared with before wearing of the stabilization splint. The EMG activities of right and left masseter and anterior temporal muscles during maximal clenching were significantly increased after 1 week and 4 week-use of the stabilization splint(p<0.01). The asymmetric indices of the EMG activities of right and left masseter and anterior temporal muscles during maximal clenching immediately after wearing of the stabilization splint were not changed compared with before wearing of the stabilization splint. The asymmetric indices of the EMG activities of right and left anterior temporal muscles during maximal clenching were significantly decreased after 1 week and 4 week use of the stabilization splint(p<0.01). and that of masseter muscles showed a decreased tendency but there were no significant differences(p>0.05).
