A Study of Conservative Treatment for Patients with Osteoarthritis of the TMJ (측두하악골관절염 환자의 보존적 치료에 관한 연구)

  • To evaluate the treatment outcome after conservative treatment in patients with osteoarthritis of the TMJ(OA), the subjects were chosen among the patients who presented to the Department of Oral Medicine of Pusan National University Hospital, diagnosed as osteoarthritis of the TMJ, and treated with conservative methods from 1994 to 2006. 101 patients with diagnosis of osteoarthritis of the TMJ were selected as the experimental group and 74 patients with diagnosis of masticatory muscle disorders(MMD) were selected as the control group. Subjective symptoms and clinical findings were investigated to evaluate and compare the subject's status at the first and the last visit. The results were as follows : 1. In the OA group, satisfactory treatment outcome was obtained with conservative methods. But, at the last visit, the symptoms of the OA group were not improved enough compared with those of the MMD group. 2. In the OA group, radiographic findings of panorama view and transcranial projection were positive by about 60 percent. But, computed tomography and single photon emission computed tomography were positive by more than 90 percent. 3. At the last visit of the OA group, the joint noise was reduced significantly. 4. In the majority of the OA group, satisfactory treatment outcome was obtained with conservative methods such as medication, physical therapy, and occlusal stabilization appliance. In the majority of the MMD group, satisfactory outcome was obtained with medication and physical therapy. 5. It took the OA group from 6 months to 2 years to be cured well while the MMD group within 6 months.6. The OA group needed more than 10 times' treatment for satisfactory outcome while the MMD group needed less than 10 times' treatment.

  • Rheumatoid arthritis(RA) is an of autoimmune inflammatory systemic disease. It is characterized by uncontrolled proliferation of synovial tissue and a wide array multisystem comorbidities. The disease may involve any joint of the body, but often statrs in the peripheral joints. It was reported that more than 50% of RA patients exhibit clinical involvement of TMJ. This report is a case report of dental management and progression for 16 months in patients who had severe bony change in TMJ involved rheumatoid arthritis Dental management was included palliative treatment such as interocclusal splints, physical therapy, mouth opening exercise. Although it was progressed rapidly osteolytic bone change during follow-up, no more advanced occulsal change and improved symptom and jaw motion. Further investigations about rule of dentistry in TMJ involvement in RA maybe needed.

  • Granulocytic sarcoma is a manifestation of myelogenous leukemia, which means a solid mass consisting of primitive precursors of the granulocytic series of white blood cells. We present CT and MR imaging findings of bilateral sino-orbital granulocytic sarcoma in a 22-month-old boy. The mass involved bilateral orbital fossa which resulted in bilateral proptosis. Moreover, the mass extended to the almost skull base including paranasal sinuses, maxilla, temporal bone, zygomatic bone, sphenoid bone, ethmoid, and palatine bone. The adjacent dura was continuously thickened and the lower half of cavernous sinus was also involved. The patient was diagnosed as AML (M5) with t(8,21) translocation through a chromosome study from the bone marrow.

The Reliability of the Conventional Tomographic Interpretation for the Patients with Temporomandibular Disorders (턱관절장애환자의 일반단층촬영 판독에 대한 신뢰도)

  • 본 연구는 측두하악관절장애의 증상이 있는 악관절의 통상적인 방사선 단층촬영술에 대한 해석과 관절강 측정에 대한 조사자내, 조사자간 신뢰도를 조사하였다. 신뢰도 검사는 조사자들 간에 판독의 일관성이 있는지와 일정 시간이 지난 후 반복 측정 시 판독의 정확성이 있는지를 알아보기 위하여 시행하였다. 조사자내 상관계수는 과두 형태에 대해서는 각각 0.812와 0.619, 골변화 형태에 대해서는 모두1.00, 골변화에 대해서는 0.846과 0.991, 전방관절강에 대해서는 0.919와 0.966, 상방관절강에 대해서는 0.864와 0.955, 그리고 후방관절강에 대해서는 0.718과 0.672였다. 통상적인 방사선 단층촬영술이 신뢰도가 있음을 보여주는 이 연구의 결과로 보아, 측두하악 관절을 평가하는데 있어서 통상적인 방사선 단층촬영술은 높은 진단학적 정확성과 조사자간 일치성을 가진다고 할 수 있다.

Clinical Study of Patient with Traumatic Temporal and Occipital Bone Fracture (외상성 후두골과 측두골 골절에 대한 고찰)

  • This study shows that oriental medical treatment affected a patient with headache, dizziness, dim eyes, facial palsy and hard-of-hearing caused by traumatic temporal and occipital bone fracture. Traumatic facial palsy mainly occures by temporal bone fracture. Facial palsy caused by traumatic temporal bone fracture corresponds to gu-an-wa-sha (口眼?斜)of oriental medicine. Functional disorder of an auditory organ corresponds to yi-rong(耳聾) and hyun-hoon(眩暈) of oriental medicine. In general, everyone consider surgical operation first of all, in the case of having traumatic facial nerve paralysis. But, this case shows that oriental medical treatments(acupuncture and herbal medicine) have a good effect on a patient with traumatic temporal and occipital bone fracture.

  • A rare case of telangiectatic osteosarcoma of the sphenoid bone was reported. The patient was a 27-year-old male and had suffered from left eye protrusion and diplopia for three months. Radiologically, a lobulated osteolytic lesion was located between sphenoid and left temporal bone. Pathologically, the tumor had multiloculated blood filled cystic vascular spaces and osteoid formation by malignant spindle osteoblast cells. The differential diagnosis of this rare entity from aneurysmal bone cyst was important radiologically and pathologically. This case is the second case of telangiectatic osteosarcoma of the sphenoid bone followed by a case of Whitehead RE and Melhem ER in 1998.

  • Osteoarthritis in patients with temporomandibular disorders(TMDs) induces pain, limitation of mouth opening, occlusal problems, and most commonly affects their life quality. Control method and progressive process of osteoarthritis are being extensively researched. The researchers focus on histologic changes, synovial changes, muscular and ligamental changes and observed reaction to pain. Therefore most of them developed the animal model for osteoarthritis in TMD patients. In this study, we applied several methods which induces osteoarthritis of temporomandibular joint(TMJ) in rats or mice. For locally induce osteoarthritis in TMJ, Monosodium iodoacetate(MIA) or interleukin-$1{\alpha}$(IL-$1{\alpha}$) were injected into TMJ joint space for 5 or 3 weeks. Other groups are chosen for osteoarthritis under systemic control including hormonal changes and aging. To observe cellular change, increased collagen, degenerative bony destruction and distribution of proteoglycans (PGs), safranin-O staining and Masson's trichrome staining were used.

Effect of Sex and Menopausal Age on Thickness and Density of Membranous Bone : Focused on Computed Tomography in Squamous Portion of Temporal Bone (성별과 폐경기 나이가 막성골의 두께와 밀도에 미치는 영향 : 측두골 비늘부위 전산화단층영상을 중심으로)

  • The purpose of this study was to investigate the effect of sex and menopausal age on the thickness and density of squamous portion of the temporal bone as the membranous bone. Patients who visited a general hospital in Chungnam and had a computed tomography (CT) examination of the head. A retrospective study was conducted with 120 subjects (30 men under 55 years old, 30 men over 56 years old, 30 women under 55 years old, and 30 women over 56 years old). Axial images of the squamous portion in the temporal bone were obtained from CT of the head. For this image, a slice sensitive profile (SSP) was acquired with an image analysis program and these were evaluated. The thickness was measured by using a digital ruler to measure the full width at half maximum (FWHM) of the SSP, and the density was measured in hounsfield unit (HU). These by gender were measured to be about 0.5 mm thinner in the temporal bone in men than in women, and there was a significant difference. The density was measured to be about 200 HU higher in women than in men of it, and there was a significant difference. As a result, it in women was thicker and had a lower density. The thickness of it in men and women over 56 years of age was 0.8 mm or more thicker in women and less than 400 HU in density. As a result, the women group over the age of 56 showed a distinct increase in thickness and decrease in density, different from other target groups. It is expected that the results of this study could be used as basic data for a new bone density measurement site study.

Comparative Analysis : The Patterns of Temporomandibular Disorder among Adolescents (청소년 측두하악장애 환자의 임상적 연구)

  • A retrospective study was conducted to determine the patterns of Temporomandibular Disorder(TMD) among adolescents (12-19 years) over last ten years. 174 patients who had visited the Oral medicine of Pusan National University Dental Hospital in 2000 and 491 patients in 2008 were found on chart review. The results obtained were as follows : 1. The number of adolescent patients was significantly increased in 2008 than in 2000, especially in male. 2. Bruxing, clenching, holding habits and bad sleep hygiene were highly increased more in 2008 than in 2000. 3. Osteoarthritis was significantly increased in 2008 than in 2000 and anterior disc dislocation without reduction was slightly increased. 4. The ratio of male to female adolescent patients with osteoarthritis was remarkably increased. Among the patients who had holding, bruxing, clenching habits, significantly increased osteoarthritis found to be common. This could imply holding, bruxing, clenching habits are prominently associated with increasing risk of developing osteoarthritis.
