급성 췌장염 진료 권고안 (Clinical Practice Guidelines for Acute Pancreatitis)

  • Acute pancreatitis is a common gastrointestinal disease that is associated with significant morbidity and consumes enormous health care resources. As such, it requires up-to-date evidence-based diagnosis and standard treatment guidelines with broad support from the clinician. Korean Pancreatobiliary Association has developed clinical practice guidelines for the diagnosis and treatment of acute pancreatitis to provide a framework for clinicians to manage acute pancreatitis and to improve national health care. The guidelines were divided into four parts: the diagnosis of acute pancreatitis, the assessment of the severity, the initial management, and the treatment of necrotizing pancreatitis and local complications of acute pancreatitis. In this article, we summarize and present the diagnosis and treatment guidelines for acute pancreatitis established in Korea.

MMR 2차 예방접종을 받은 후 이하선염 없이 발생한 볼거리 뇌염 1례 (Mumps meningoencephalitis without parotitis, after secondary vaccination of Measles-Mumps-Rubella (MMR) : A Case Report)

  • 볼거리(mumps)는 유행선 이하선염(epidemic parotitis)으로 불리며, 볼거리 바이러스에 의해 유발되는 바이러스성 감염질환이다. 볼거리는 췌장염, 고환염, 청력장애, 난소염, 이하선염 및 뇌수막염과 같은 합병증을 일으킬 수 있다. 볼거리로 인한 중추신경계의 침범은 볼거리로 진단된 환자의 65%에 달한다는 보고가 있으며, 그 중 대부분은 볼거리 뇌막염(mumps meningitis)이다. 이에 반해 볼거리 뇌염(mumps meningoencephalitis)은 볼거리 환자 6000명당 1명의 경우로 매우 드물다고 알려져 있다. 뇌염은 볼거리바이러스의 흔하지 않은 중추신경계의 합병증으로 홍역-볼거리-풍진 예방접종은 볼거리바이러스로 인한 치명적인 합병증을 예방해주고 경과를 가볍게 해준다고 알려져 있다. 저자들은 24세의 젊은 여자가 이하선염을 동반하지 않은 볼거리 바이러스로 인한 급성 뇌염을 경험하여 이에 대한 사례 연구를 하였다. 환자는 예방접종 시기에 맞춰 볼거리 예방접종을 받았다. 본 병원을 내원 시 환자는 인지능력의 저하를 보였으며 치료 후 빠른 회복을 보였고 합병증은 보이지 않았다. 이 연구는 볼거리 바이러스에 대한 2차 접종을 시기에 맞춰 받은 환자에서 이하선염의 동반 없이 뇌염증상이 발생한 환자에 대한 사례 연구이다.

에탄올에 의하여 유도된 마우스 췌장 선포세포의 염증성 손상에서 췌장분비 효소의 활성 및 세포 재생관련 유전자들의 발현에 미치는 EGCG의 영향 (Effects of (-)-Epigallocatechin-3-gallate on the Release of Pancreatic Enzymes and Expression of Regenerating Genes in Ethanol-injured Murine Pancreatic Primary Acinar Cells)

  • 본 연구에서는 마우스 췌장의 선포세포(primary acinar cell)에서 에탄올에 의해 유도된 염증성 손상에 미치는 녹차의 생리활성 물질인 EGCG의 효과를 췌장분비 효소의 활성과 선포세포에서의 기능 회복 관련 마크로 알려진 유전자들(AMPK, RKIP 및 Reg1)의 발현 조절 측면에서 조사하였다. 대조군에 비해 에탄올이 처리된 세포에서는 ${\alpha}$-amylase와 chymotrypsin의 활성이 증가되었지만, EGCG 처리에 의하여 그들 활성이 유의적으로 감소하였다. 세포 생존 및 보호와 관련 있는 AMPK 인산화 수준은 에탄올 단독 처리시 그 발현이 감소하였지만, EGCG 처리에 의하여 유의적으로 회복되었다. 세포 사멸과 세포독성 조절과 연관이 있는 RKIP 또한 그 발현 경향이 AMPK인산화 변화의 정도와 유사하였다. 또한 베타세포 재생에 관여하는 Reg1 발현은 에탄올이 처리된 선포세포에 EGCG를 처리하였을 경우 그 발현량이 유의적으로 증가하였다. 이상의 결과들을 종합하여, 항산화성 폴리페놀인 EGCG는 알코올성 췌장염으로 인한 세포의 손상이나 당뇨병 예방과 회복에 치료적 효과를 가질 수 있다고 제안한다.

  • To investigate the effects of trypsin inhibitors, aprotinin and urinary trypsin inhibitor (UTI), on the cute pancreatitis, this study was carried out in dogs of acute pancreatitis induced by oleic acid (0.28 mg/kg). Administration with aprotinin and UTI seemed to have a therapeutic effect on the clinical sign, ultrasonographic finding, histopathologic finding. But in amylase and lipase activity, there were no significant differences among three groups.

가성 낭종을 동반한 만성 췌장염 환자에 대한 증례보고 (A Case Report of Chronic Pancreatitis with Pseudocyst)

  • Objectives : The purpose of this case is to report the improvement after treatment on the patient who has chronic pancreatitis with pseudocyst. Methods: We treated the patient with oriental and western medical treatment from 22nd February 2007 to 24th March 2007. We checked the patient with VAS score and Abdomen CT. Results : After treatment Pain, dyspepsia, itching sense and general body weakness were improved, VAS score changed from 10 to 0 and cystic lesion decreased on Abdomen CT. Conclusion : This case shows oriental medical treatment has useful effect on chronic pancreatitis. More research of chronic pancreatitis is needed.

오리방풀로부터 분리된 ORI2의 췌장염 유발 콕사키바이러스B4 증식억제 (ORI2 is a Strong Inhibitor of Coxsackievirus B4 Replication)

  • The ORI2 (3-[3,4-dihydroxyphenyl]acrylic acid 1-[3,4-dihydroxyphenyl]-2-methoxycarbonylethyl ester) was purified from the extract of Isodon excisus. We confirmed the antiviral effect of ORI2 in a coxsackievirus-induced pancreatitis model. Coxsackievirus B4 (CVB4) is a common cause of pancreatitis and may be reason of the type-1 diabetes. Anti-enteroviral compounds were screened by HeLa cell survival assay. Purified natural compounds were added to HeLa cells cultured 96-well plates after $10^4PFU/ml$ CVB4 pre-incubation for 30 min. ORI2 significantly improved HeLa cell survival in a dose-dependent manner. In addition, ORI2 (1 mM) treatment was dramatically decreased virus protease 2A induced eIF4G-I cleavage and viral VP1 capsid protein production. HeLa cell virus titers and viral RNA replication were significantly decreased in ORI2-treatment in a dose dependent manner (1 mM~0.001 mM). These results demonstrate that ORI2 has a strong antiviral effect. It was significantly decreased virus replication. ORI2 may be developed as a potential therapeutic agent for CVB4.

개의 췌장염에 의한 만성 화농성 지방조직염증 (Chronic Suppurative Steatitis Secondary to Pancreatitis in a Dog)

  • A Maltese was presented with a subcutaneous mass at right flank, depression and hyperthermia. Ultrasonography revealed a hypoechoic mass with hyperechoic foci, a defect of abdominal wall and peritonitis. In cytology, fat necrosis was found. This dog was diagnosed as chronic suppurative steatitis secondary to pancreatitis and pancreatic herniation through surgery and histopathologic examination. Steatitis is recurrent inflammation and necrosis of adipose tissue. Also steatitis associated with pancreatic diseases such as pancreatitis and pancreatic tumor was reported and direct fat necrosis by pancreatic enzyme was supposed as the etiology. Steatitis secondary to pancreatitis was rarely reported in dogs and there was difficulty in diagnosis and treatment. This case has a limitation that pancreatitis was not diagnosed as underlying cause to steatitis before surgery. However, a defect of abdominal wall and peritonitis were observed on ultrasonography. Good prognosis could be achieved through prompt surgery and medical treatment.

이묘산(二妙散)과 구성약재(構成藥材)의 효능(效能) 비교(급성(急性) 췌장염(膵臟炎)을 중심으로) (The Comparison Between Single And Yimyosan in Acute Pancreatitis : Atractylodes chinensis(AC) and Phellodendron amurense Ruprecht(PAR))

  • Objectives : Atractylodes chinensis(AC) and Phellodendron amurense Ruprecht(PAR) have been used as anti-inflammatory medicines. However, the effects of AC, PAR and Yimyosan on AP were not examined. To prove this, We decocted the dried prescription of AC and PAR with boiling distilled water and freeze-dried to be powdered. AC, PAR and Yimyosan was administrated intraperitoneally. Methods : 1h after administration, cerulein was injected hourly six times. 6hrs after last cerulein injection, mice were sacrificed, then the pancreas and blood were harvested. Serum amylase and lipase, neutrophil infiltration, pancreatic cytokines were used as the parameter of severity of AP. Results : As a result of assessment the parameters of AP, AC alone treatment did not inhibit the severity of AP, however PAR treatment inhibited the severity of AP significantly. Yimyosan also showed the protective effects against AP at lower doses, however AC alone plus PAR alone extract did not show the protective effects significantly. Conclusions : In conclusion, PAR extract has a protective effects on AP, and the effects could be increased by co-treatment with AC.

십이지장 내강 내 게실로 인한 반복적인 췌장염 - 1예 보고 - (Recurrent Pancreatitis Caused by Intraluminal Duodenal Diverticulum in an 11-year-old Girl - a Case Report -)

  • An 11-year-old girl with a history of two previous attacks of acute pancreatitis was admitted to another hospital. Her epigastrium was tender, and serum amylase was 657 IU/L and lipase 3131 IU/L. Abdominal computed tomography scan suggested necrosis of 30% of the pancreas. Retrograde endoscopic cholangiopancreatography showed a diverticulum covered by normal duodenal mucosa at the second portion of the duodenum, which was separated from the adjacent duodenal lumen by a radiolucent band at UGI series. The apex of the diverticulum was incised endoscopically using a needle knife papillotome. A follow-up endoscopy on the next day noticed bleeding from the incised edge of the diverticulum. Endoscopic hemostasis with hemoclipping and injection of hypertonic saline-epinephrine solution was not successful. The patient was transferred to Kyungpook National University Hospital, and open duodenotomy and excision of the diverticulum were performed. She has recovered well and remains asymptoatic.

해죽순 물 추출물의 급성 췌장염 억제 효과 (Protective Effect of Nypa fruticans Wurmb. Water Extract on Acute Pancreatitis)

  • Nypa fruticans Wurmb. (NF) has been used as a folk remedy to treat inflammatory diseases in Asia. In this study, we aimed to investigate the protective effect of NF water extract on cerulein-induced acute pancreatitis (AP). To measure the protective effects of NF on AP, AP was induced via intraperitoneal injection of cerulein (50 ㎍/kg) hourly for 6 h in mice. NF water extract (100, 250, or 500 mg/kg) or saline was administrated to intraperitoneal injection 1 h before the first injection of cerulein. The mice were sacrificed at 6 h after the final injection of the cerulein. Pancreas, and blood sample were taken for further analysis. Administration of NF water extract inhibited the pancreatic injury, the elevation of pancreatic weight/ body weight ratio, and the elevation of serum digestive enzymes such as amylase and lipase during cerulein-induced AP in mice. Also, pancreas MPO activity, which represents neutrophil infiltration, was inhibited by administration of NF water extract. Taken together, administration of NF water extract reduces the severity of cerulein-induced AP, which suggests a clinical basis that NF could be a potential agent to prevent AP.
