Hemilaminectomy for Herniated Discs at the Cervicothoracic Junction in a Beagle Dog (편측추궁절제술을 이용한 비글견의 경흉연접부 추간판 탈출증의 치료)

  • A 7-year-old, female Beagle dog presented with a primary complaint of severe neck pain and paretic gait. A lesion was suspected between C6 and T2 on the basis of neurological examinations. Magnetic resonance imaging of the cervicothoracic spine revealed herniation of disc material from the C7-T1 intervertebral space resulting in marked spinal cord compression. Hemilaminectomy was performed to treat the intervertebral disc disease at C7-T1. Treatment was effective and the clinical condition of the dog was remarkably improved. This result suggests that hemilaminectomy is an effective option for surgical treatment of intervertebral disc disease at the cervicothoracic junction in dogs.

A Case Report of Prescribing Yanghyeolgeopung-tang(yangxuequfeng-tang) to Two Patients with Cervical Disc Herniation and Headache (양혈거풍탕(養血風湯)을 투여한 두통을 동반한 경추 추간판 탈출증 환자 치험 2례)

  • Objectives : The purpose of this study is to investigate the clinical application of Yanghyeolgeopung-tang(yangxuequfeng-tang) to two patients with cervical disc herniation and headache. Methods : The patients were hopitalized at the Dept. of Oriental Rehabilitation Medicine, Jaseng Oriental Medicine Hospital, and diagnosed as cervical disc herniation and treated mainly with herbal medicine; Yanghyeolgeopung-tang(yangxuequfeng-tang). Additional acupuncture and pharmaco-puncture was provided. NRS(Numeric Rating Scale) and NDI(Neck Disability Index) scores were used as outcome measures. Results : After taking Yanghyeolgeopung-tang(yangxuequfeng-tang), the patients' pain was controlled and the patients slept well after treatment. NRS and NDI scores both decreased. Conclusions : As seen in these two cases of cervical disc herniation with headache, Yanghyeolgeopung-tang(yangxuequfeng-tang) has a positive effect in controlling neck pain and headahce due to cervical disc herniation.

Experience of spinal stenosis, herniation of intervertebral disk patients with low back pain under non-surgical treatment (비수술적 요법을 받는 척추관협착증, 추간판탈출증 환자의 요통 경험)

  • The purpose of the study was to explore the process of experience of spinal stenosis, herniation of intervertebral disk patients with low back pain under non-surgical treatment. 10 participants attended in-depth individual interview. Data analyzed using the Grounded theory methodology of Corbin and Strauss(2015). A core category emerged as 'Compromise with my body in the swamp of pain'. Central phenomenon was 'Constrained life by severe pain'. The process of the low back pain experience included three phases: 'cognition', 'coping', and 'adaptation'. This finding will be helpful for understanding the low back pain experience and be as fundamental data as for developing and applying nursing intervention program according to the low back pain experience process.

Measurement of ROI Temperature in Herniation of Intervertebral Disc Patients Using DITI (디지털 적외선 체열진단기를 이용한 추간판탈출증 환자의 ROI 온도측정)

  • Among 45 patients of herniation of intervertebral disc with $L_4$ to $L_5$ herniation, who underwent infrared thermography, the number of female was larger than male and the age of 50s was highest. From the results measured from the distribution table, we have confirmed that there was no significant difference depending on sex and age (p> 0.05). The region of the highest ROI temperature for patients with $L_4-L_5$ intervertebral disc prolapse was the back of the posterior right tibia, and followed by the back of the left shin bone-below the front right knee-below the front left knee. There was a significant difference depending on the measured site. The average ROI temperature for patients was $30.30{\pm}0.50$ whereas that for normal persons was $31.20{\pm}0.58$, yielding the temperature difference of $0.66{\pm}0.59$ between the two groups. The ROI of patients was lower than $31.20{\pm}0.58$ (p <0.05) because the significance of the sample, which has been obtained from the results of a sample t-test, was less than 0.05 (p <0.05). From further researches, it may necessary to develope the methodology for correcting data regarding thermal environment and, in addition, to develope a new thermal index based on it. Therefore, we can confirm that pre-treatment for infrared thermography is very important in order to minimize the procedure for correcting data. It is required that radiologists who inspect disc herniations should carefully observe and consider the patients during their measurements.
