Lumbar Puncture in Patients Who Experienced First Seizure with Fever (열이 동반된 첫 경련시의 뇌척수액검사)

  • Purpose : We aimed to ascertain whether lumbar punctures should be done to prove febrile seizure, and to study what conditions can delay or abolish lumbar puncture. Methods : This retrospective study was done for four years, from January, 1996 to December, 1999. The subjects were the patients who visited Masan Samsung Hospital for the first convulsion with fever, and whose age ranged from 1-month to 5-years old. The total number of patients was 197. We investigated the rate of meningitis, which was classified by age, season, convulsion type, associated symptoms, and results of blood test. All data were tested chi-square. Results : The diagnosis rate of meningitis among all 197 patients was 7.6%, and of these only one patient was proved to have bacterial meningitis. The rate was high in the young(below 6 month), in males and in summer and autumn. Although the general appearance was good after resolution of the fever, the rate of meningitis was 6.5%. Patients with nausea and vomiting totalled 11%; 9.1% of them were diagnosed with meningitis. The diagnosis rate of meningitis in cases in which additional seizures took place were 33.3%. Conclusion : The ultimate goal of lumbar punctures in seizure patients with accompanying fever is to rule out bacterial meningitis, which, has been demonstrated to be rare. So we think that lumbar puncture can be postponed by close observation of pediatrics specialists and skilled nurses.

Adaptive Response to ionizing Radiation Induced by Low Doses of Gamma Rays in Human Lymphoblastoid Cell Lines (인체임파양세포에서 저선량의 감마선에 의해서 유도되는 적응 반응)

  • When cells are exposed to low doses of a mutagenic or clastogenic agents. they often become less sensitive to the effects of a higher dose administered subsequently. Such adaptive responses were first described in Escherichia coli and mammalian cells to low doses of an alkylating agent. Since most of the studies have been carried out with human lymphocytes, it is urgently necessary to study this effect in different cellular systems. Its relation with inherent cellular radiosensitivity and underlying mechanism also remain to be answered. In this study, adaptive response by 1 cGy of gamma rays was investigated in three human lymphoblastoid cell lines which were derived from ataxia telangiectasia homozygote, ataxia telangiectasia heterozygote, and normal individual. Experiments were carried out by delivering 1 cGy followed by 50 cGy of gamma radiation and chromatid breaks were scored as an endpoint. The results indicate that prior exposure to 1 cGy of gamma rays reduces the number of chromatid breaks induced by subsequent higher dose (50 cGy), The expression of this adaptive response was similar among three cell lines despite of their different radiosensitivity. When 3-aminobenzamide, an inhibitor of poly (ADP-ribose) polymerase, was added after 50 cGy, adaptive responses were abolished in all the tested cell lines. Therefore it is suggested that the adaptive response can be observed in human lymphoblastoid cell lines, which was first documented through this study. The expression of adaptive response was similar among the cell lines regardless of their radiosensitivity. The elimination of the adaptive response by 3-aminobenzamide is consistent with the proposal that this adaptive response is the result of the induction of a certain chromosomal repair mechanism.

The Study of the Fitness on Calculation of the Flood Warning Trigger Rainfall Using GIS and GCUH (GIS와 GCUH를 이용한 돌발홍수 기준우량 산정의 타당성 검토 연구)

  • Using geomorphoclimatic unit hydrograph(GCUH), we estimated the fitness to calculate the mountainous area discharge and flash flood trigger rainfall(FFTR). First, we compared the GCUH peak discharge with the existing report using the design storm at the Dukcheon basin. Second, we compared the HEC-HMS(Hydrologic Engineering Center-Hydrologic Modeling System) model and GCUH with the observed discharge using the real rainfall events at the Taesu stage gage. Third, GCUH and NRCS(Natural Resources Conservation Service) were used for calculating FFTR and proper calculation method was shown. At the Dukcheon basin, the comparison result of using design storm was shown in Table 11, and it was not in excess of 1.1, except for the 30 year return period. In case of real rainfall events, the result was shown in Table 12, and GCUH discharges were all larger than the HEC-HMS model discharges, and they were very similar to the observed data at the Taesu stage gage. In this study, we found that GCUH was a very proper method in the calculation of mountainous discharge. At the Dukcheon basin, FFTR was 12.96 mm in the first 10 minutes when the threshold discharge was 95.59 $m^3$/sec.

  • A 5-year-old, male Yorkshire terrier dog was presented with progressive seizure and anorexia. Definitive diagnosis of necrotizing meningoencephalitis(NME) was made based on characteristic clinical features, brain lesion with MRI, and histopathologic examination. The dog was treated with prednisolone for 20 days, firstly. Prednisolone and mycophenolate mofetil(MMF) were then administered for 40 days following the initial therapy. However, the clinical signs were not improved and seizure frequency was increased. This patient survived around 2 months after diagnosis. This case report described the clinical findings, imaging characteristics and pathologic features of NME in a Yorkshire terrier with trial treatment using MMF.

The Private Security Officer ' Actual Condition of First-Aid Education (민간경비원의 응급처치 교육에 대한 실태조사)

  • Since the establishment of Security Business Act in 1976, the Korean private security industry has rapidly grown in the qualitative and quantitative aspects. In the circumstance, various roles of private security guards have been demanded. A private security guard as the first contactor of the users of multi-use facilities should first find an accident field situation and make a response when an accident occurs, and should give first aid as most as possible until emergency medical workers arrive. However, there is a lack of first-aid education for the private security guards who are responsible for the safety of facility users. Therefore, the purpose of this study is first to find how many times private security guards have job and first-aid education, and analyze how the education count influences their first-aid ability; secondly to analyze how private security guards' satisfaction with first-aid education influences their first-aid ability. To achieve the purpose, this researcher conducted a questionnaire survey on first-aid ability of the private security guards working in the metropolitan areas. For data analysis, Stata se/ 14.0ver was applied. For the analysis on validity and reliability, exploratory factor analysis was conducted and a Cronbach's a value was drawn. To find the actual conditions of first-aid education, frequency analysis, inter-group difference analysis, and multi-regression analysis were conducted. According to the data analysis, there were not enough times of job education and first-aid education for private security guards, and their satisfaction with first-aid education and first-aid ability were difference depending on the job and first-aid education counts. Also, their satisfaction with first-aid education partially influenced their first-aid ability.

A Study on the Self-confidence in Performance and Education Demand of First Aid in Kindergarten and Daycare Center Teachers (유치원 및 어린이집 교사의 응급처치 수행자신감 및 교육요구도)

  • The aim of this study was to identify the self-confidence in performance and education demand of first aid in kindergarten and daycare center teachers. The data from a questionnaire were collected from teachers in kindergarten and daycare centers from March 5 to 31, 2015. The responses from 149 teachers were analyzed using SPSS 21.0. The results showed that self-confidence in the performance of first aid was mostly low (mean score 2.70), but was significantly high in cases of having a willingness to perform first aid (p=.002). The education demand for first aid was mostly high (mean score 4.04), and significantly high in the case of having experience of emergency situations (4.21, p=.006), and being educated in first aid while working in a childcare facility (4.14, p=.001). Therefore, it is important to educate teachers in first aid regularly after developing an optimized program considering the frequency of emergency situations, and level of self-confidence in the performance and education demand of first aid.

  • DCS (Distributed Control System), the main control system of power plants, is an automated system for enhancing operational efficiency by monitoring, tuning and real-time operation. DCS is becoming more intelligent and open systems as Information technology are evolving. In addition, there are a large amount of investment to enable proactive facility management, maintenance and risk management through the predictive diagnostics. However, new upcoming weaponized malware, such as Stuxnet designed for disrupting industrial control system(ICS), become new threat to the main control system of the power plant. Even though these systems are not connected with any other outside network. The main control systems used in the power plant usually have been used for more than 10 years. Also, this system requires the extremely high availability (rapid recovery and low failure frequency). Therefore, installing updates including security patches is not easy. Even more, in some cases, installing security updates can break the warranty by the vendor's policy. If DCS is exposed a potential vulnerability, serious concerns are to be expected. In this paper, we conduct the penetration test by using NESSUS, a general-purpose vulnerability scanner under the simulated environment configured with the Ovation version 1.5. From this result, we suggest a log analysis method to detect the security infringement and react the incident effectively.

Equine Motor Neuron Disease in a Jeju Pony (제주 조랑말에서의 말운동신경세포질환 일례)

  • Equine motor neuron disease (EMND) is a spontaneous neurologic disorder of horses, which results from the degeneration of motor neurons in the spinal cord and brain stem. An 8-year-old Jeju pony gelding presented with weight loss, muscle tremors, frequent recumbence, low head carriage, sweating, and standing with four limbs close together. The gelding has been on the same stable and limited access to pasture for several years. The gelding has been fed with dried hays and commercial concentrated feeds. Laboratory test revealed very low serum vitamin E level (0.14 ${\mu}g/mL$; reference range > 1.5 ${\mu}g/mL$), mildly elevated creatine kinase (402 IU/L; reference range 119-287 IU/L) and aspartate aminotransferase (876 IU/L; reference range 226-336 IU/L). Oral glucose absorption test showed decreased glucose absorption. Histopathologic examination using a biopsy specimen from sacrocaudalis dorsalis medialis muscle revealed atrophic and hypertrophic muscle fiber, centralization of nucleus, degenerating and necrotic muscle fibers. Taken together, the gelding was diagnosed as EMND. After oral vitamin E administrated for 5 weeks, the gelding showed normally improved stance, decreased periods of recumbency, improved head carriage and weight gain except consistent tremors. This is the first report that successfully treated EMND case occurred in a Jeju pony in Korea.

Risk Assessment of Marine LPG Engine Using Fuzzy Multicriteria HAZOP Technique (퍼지 다기준 HAZOP 기법을 이용한 해상용 LPG 엔진의 위험성 평가)

  • Liquefied petroleum gas (LPG) is an attractive fuel for ships considering its current technology and economic viability. However, safety guidelines for LPG-fueled ships are still under development, and there have been no cases of applying LPG propulsion systems to small and medium-sized ships in Korea. The purpose of this study was to perform an objective risk assessment for the first marine LPG engine system and propose safe operational standards. First, hazard and operability (HAZOP) analysis was used to divide the engine system into five nodes, and 58 hazards were identified. To compensate for the subjectivity of qualitative evaluation using HAZOP analysis, fuzzy set theory was used, and additional risk factors, such as detectability and sensitivity, were included to compare the relative weights of the risk factors using a fuzzy analytical hierarchy process. As a result, among the five risk factors, those with a major impact on risk were determined to be the frequency and severity. Finally, the fuzzy technique for order of preference by similarity to ideal solution (TOPSIS) was applied to select the risk rank more precisely by considering the weights of the risk factors. The risk level was divided into 47 groups, and the major hazard during the operation of the engine system was found through the analysis to be gas leakage during maintenance of the LPG supply line. The technique proposed can be applied to various facilities, such as LPG supply systems, and can be utilized as a standard procedure for risk assessment in developing safety standards for LPG-powered ships.

Development of a method to create a matrix of heavy rain damage rating standards using rainfall and heavy rain damage data (강우량 및 호우피해 자료를 이용한 호우피해 등급기준 Matrix작성 기법 개발)

  • Currently, as the frequency of extreme weather events increases, the scale of damage increases when extreme weather events occur. This has been providing forecast information by investing a lot of time and resources to predict rainfall from the past. However, this information is difficult for non-experts to understand, and it does not include information on how much damage occurs when extreme weather events occur. Therefore, in this study, a risk matrix based on heavy rain damage rating was presented by using the impact forecasting standard through the creation of a risk matrix presented for the first time in the UK. First, through correlation analysis between rainfall data and damage data, variables necessary for risk matrix creation are selected, and PERCENTILE (25%, 75%, 90%, 95%) and JNBC (Jenks Natural Breaks Classification) techniques suggested in previous studies are used. Therefore, a rating standard according to rainfall and damage was calculated, and two rating standards were synthesized to present one standard. As a result of the analysis, in the case of the number of households affected by the disaster, PERCENTILE showed the highest distribution than JNBC in the Yeongsan River and Seomjin River basins where the most damage occurred, and similar results were shown in the Chungcheong-do area. Looking at the results of rainfall grading, JNBC's grade was higher than PERCENTILE's, and the highest grade was shown especially in Jeolla-do and Chungcheong-do. In addition, when comparing with the current status of heavy rain warnings in the affected area, it can be confirmed that JNBC is similar. In the risk matrix results, it was confirmed that JNBC replicated better than PERCENTILE in Sejong, Daejeon, Chungnam, Chungbuk, Gwangju, Jeonnam, and Jeonbuk regions, which suffered the most damage.
