Relationship between Immunoglobulin E, Eosinophil Count and Allergens in Allergic Patients (알레르기 환자에서 Immunoglobulin E와 Eosinophil count 및 Allergen과의 관련성)

  • As the allergic diseases in patients are increasing, it is important to find out the allergens. A multiple antigen simultaneous test(MAST) is a simple method for in vitro measurement of allergen-specific IgE antibodies. This study was performed to evaluate the relationship between allergen-specific IgE antibodies, serum total IgE and peripheral eosinophil count in the allergic patients. According to the results of the study, the total IgE positive rate(above class 2) from the inhalent is 96.97%, and that from food panel is 98.06%. The research showed that the positive rate of the allergen-specific IgE was House dust 51.52%, D. farinae 45.46%, Cat 31.99% in inhalent panel, and 55.34%, 42.72%, 34.96% in food panel. Serum total IgE was associated with allergy, however, allergy was not always associated with eosinophilia.

  • It is generally accepted that parasite-specific IgE plays a crucial role in host defense against helminthic parasites. However, the role of high levels of nonspecific IgE in helminthic infections is still controversial. To investigate the role of nonspecific IgE in primary infections with P. westemani the effect of anti-lgE mAb treatment on serum IgE, $Fc{\varepsilon}RII/CD23$ expression and worm burden in Parcgonimus-infected mice were examined. In mice treated with anti-lgE antibody, the total IgE levels were not detectable ($1{\;}{\mu\textrm{g}/ml}$) throughout the experiment compared with untreated infected mice. The mean percentages of $Fc{\varepsilon}RII/CD23$ positive splenic B cells in anti-lgE treated mice (ridge: 20.3 - 30.5) were also decreased throughout the experiment compared with untreated infected mice (range: 35.7-44.4). Reduction of the total IgE and expression of $Fc{\varepsilon}RII/CD23$ on splenic B cells resulted in decreased worm burden six weeks post infection. These results suggest that high levels of nonspecific IgE in mice with primary infections of P. westemnni play a harmful, rather than beneficial, role for the host, perhaps by interfering with CD23-dependent cellular pathways.

  • Paragonimus westermani is a common fluke in Korea. The present study aimed to determine serum total IgE and specific IgG levels in experimental paragonimiasis of rats. Each Wistar rat was inoculated orally with 20~25 metacercariae of P. westermani from Cambaroides similis. Before and after infection(1,2,3,4,6,8 weeks) of P. westermani, the blood was collected from the retro- orbital venous plexus of rats and kept serum at $-70^{\circ}C$. Serum total IgE and specific IgG levels were determined by the capture and conventional enzyme-linked immunosorbent assay, respectively. The results were as follows; 1. Serum IgE values were increased to 0. 18${\pm}$0.042 at 2 weeks, $0.28{\pm}0.151$ at 4 weeks and 0.43${\pm}0.055$ at 8 weeks after infection. The absorbances of non-infected rats ranged $0.07{\pm}0.021~0.12{\pm}0.025$. 2. Specific IgG values were slightly increased at 3 weeks ($0.20{\pm}$0.032) and gradually increased up to 8 weeks($0.31{\pm}0.067$) after infection. The absorbances of non-infected rats ranged $0.11{\pm}0.035~0.18{\pm}0.019$. The present results suggested that p. westermani could elevate serum IgE and specific IgG antibodies in Wistar rats which were not a good definitive host.

  • Background : The $\beta_2$ adrenergic receptor ($\beta_2$ AR) polymorphisms occurring at amino acid position 16 (Arg to Gly), 27 (Gln to Glu), 34 (Val to Met), and 164 (Thr to Ile) are known to be functionally relevant and also disease-modifying in subjects with asthma. However the contribution of these polymorphisms to the development of the asthmatic phenotype or other markers for allergic disease remains to be established. Methods : 109 patients with bronchial asthma and 42 healthy person were included. Serum total IgE, allergen specific IgE, and skin prick test were performed to all of the subjects. $\beta_2$ AR polymorphisms were checked by mutated allele specific amplification (MASA) method. Results : The results were as follows. The frequencies of $\beta_2$ AR polymorphisms in asthmatic patients and healthy person were not statistically different(p>0.05). There was no association between $\beta_2$ AR polymorphisms of amino acid position 16, 27, 34 and the existence of atopy among asthmatic patients(p>0.05). Between asthmatic patients with or without elevated IgE level and $\beta_2$ AR polymorphisms of amino acid position 16, 27, 34, there was no statistically significant association(p>0.05). Conclusion : There was no difference in frequency of the $\beta_2$ AR polymorphism between asthmatic patients and healthy person. In the bronchial asthma, association of $\beta_2$ AR polymorphism and atopy/serum total IgE was not found.

Clinical validation of ImmuneCheck IgE for the rapid detection of serum total IgE (총 IgE의 신속한 정량 측정을 위한 ImmuneCheck IgE의 임상적 유용성)

  • Purpose: Conventional serum IgE assay was costly, required the skills of expert, and relied heavily on expensive equipment. Quantitative measurement of total IgE using Point of Care Test (POCT) device can be the solution for these limitations. This study evaluated and validated the reproducibility of ImmuneCheck IgE. Methods: This study included 120 patients of allergic diseases such as allergic rhinitis, asthma, drug allergy, food allergy, atopic dermatitis, or anaphylaxis. The reliability of POCT ImmuneCheck IgE was evaluated by comparing results from the naked eye and from the Q-Reader. Intratest reproducibility and intertest correlation were analyzed using intraclass correlation coefficient (ICC). Results: Of the 120 enrolled patients, 51 were males and 69 were females. The ages ranged from 19 to 84 years, with an average age of 51.5 years. The concentration of serum total IgE measured by Phadia ImmunoCAP IgE ranged from 5.95 to 5,000 IU/mL. ICC for Intratest reproducibility of ImmuneCheck IgE by naked eye and by Q-Reader were 0.991 (P< 0.001) and 0.989 (P< 0.001), respectively. In addition, intertest correlation between ImmuneCheck IgE and Phadia ImmunoCAP IgE results of naked eye and Q-Reader were 0.968 (P< 0.001) and 0.948 (P< 0.001), respectively. Conclusion: The ImmuneCheck IgE was reproducible and highly correlated with conventional Phadia ImmunoCAP IgE assay. This result suggests that ImmuneCheck IgE can be a useful tool for rapid and precise detection of total IgE.

The Correlation between SCORAD Index and Serum Total IgE or Eosinophil Count in Individual Patients of Atopic Dermatitis (아토피피부염 환자에서 중증도 변화에 따른 SCORAD index와 혈청 총 IgE 및 호산구 수의 상관관계)

  • Objectives : This study was aimed to evaluate the correlation between changes in disease severity and serum total IgE or eosinophil count in individual patients of atopic dermatitis depending on the change of severity. Methods : We performed a retrospective chart review of 8 cases of atopic dermatitis who underwent blood tests more than five times. Disease severity was assessed by objective SCORing of Atopic Dermatitis index (OSI). Blood tests include serum total IgE, eosinophil counts, aspartate aminotransferase (AST), alanine aminotransferase (ALT), blood urea nitrogen (BUN) and serum creatinine (SCr). In each subject, on the day that the blood test was performed, OSI was also assesed by a same physician. Correlation between OSI and total IgE or eosinophil count was analysed by nonparametric correlation analysis. Results : There was a statistically significant positive correlation between OSI and total IgE in three cases and a positive correlation between OSI and eosinophil count in two cases. In other three cases, there were no significant correlation. There were no cases that OSI correlated with total IgE and eosinophil count at the same time. In all cases, AST, ALT, BUN and SCr were stable. Conclusions : We suggest that total IgE and eosinophil count may reflect the severity of atopic dermatitis differently in individual patients. We also suggest that total IgE and eosinophil counts may be used as biomarkers to predict the severity of atopic dermatitis in some patients depending on the underlying pathology.

The Study of IgG subclasses in Acute stage of Kawasaki Disease (급성기 가와사끼병 환아의 IgG 아형항체에 대한 연구)

  • Purpose : Kawasaki Disease(KD) is a febrile disease with acute multisystemic vasculitis which is seen commonly in early childhood. The cause and etiologic agents are still unknown to identify using ordinary bacterial and viral culture, but the clinical labaratory studies suggest that KD is one of autoimmune disorder caused by infectious agents, but that is not proved yet. The study was performed to investigate the IgG subclasses in acute stage of KB before treatment of IVIG(Intravenous immunoglobulin). Method : The 35 cases in acute stage of KD before treatment of IVIG who were hospitalized from jan. 1995 to dec. 1996. The obtained sera were measured level of total IgG, IgM, IgA, IgE and IgG subclasses IgGl, IgG2, IgG3, IgG4 by using EIA and SRID method. Results : 1) The sex ratio is male to female is 1.5: 1.0, and male is prone to infected. 2) Total IgG, IgM, IgA and IgE level is normal range with age adjusted, but few cases are shown slight high level of total IgG compare to normal range of age adjusted. 3) acute phage reactants such as CRP, C3, ESR are all increased in acute stage of cases. 4) IgG subclasses IgGl, IgG2, IgG3 are shown normal range of age adjusted, but remarkably low level of IgG4 in all of cases. Conclusions : The low level of IgG4 is able to increasing the incidence of KD, and may use early diagnostic tools combine with other clinical symptoms and signs. But the resulsts of reported studies are different to each other, so it needs more times and cases to get final evaluation of changes of serum immnunoglobulin levels correlate to increase the incidence of high risk group of KD patients.

The Convergence Study on the Effects of White Ginseng Complex Extracts on OVA-induced Allergic Asthma in Mice (백삼복합물이 난알부민으로 유도된 천식 마우스에서의 천식개선에 대한 융복합 연구)

  • The aim of the convergence study is to evaluate anti-asthma effects of white ginseng complex extract(WGCE) on OVA-induced allergic asthma in mice. WGCE was administered at 100 mg/kg and 300 mg/kg to mice, where asthma was induced by OVA. Th2 cytokine including IL-4, IL-5 and IL-13 were measured by Luminex. Also, OVA-IgE and eosinophil were measured by haemocytometer and BALF total cells were measured microscope. Production of IL-4, IL-5, IL-13 and OVA-IgE in serum was decreased, respectively, in comparison with control. The eosinophil in whole blood decreased significantly. In addition, WGCE groups showed a decrease in the BALF total cells. These results demonstrated that WGCE decreases the Th2 cytokine and asthma factors. Therefore, we strongly suggest that WGCE could be effectively used as a therapeutic drug based on its anti asthma factors.

Anti-atopic Activity of Sargassum micracanthum Ethanol Extracts (잔가시 모자반 에탄올 추출물의 항아토피 효과)

  • Atopic dermatitis (AD) is a common chronic inflammatory disease preceding the development of allergic disorders. The aim of this study was to evaluate the effect of Sargassum micracanthum ethanol extract (SMEE) on AD. AD was induced by spreading 2,4-dinitrochlorobenzen (DNCB) on the back sides of BALB/c mice. The efficacy of SMEE was tested by observing the skin clinical severity score, proliferations of Raw 264.7 cells and the secretion of cytokines and IgE. The secretion of IL-4, and IgE was significantly decreased by SMEE in a dose dependent manner, while IFN-${\gamma}$ was increased. In addition, SMEE alleviated the AD symptoms better when compared to the positive controls. In conclusion, these results suggest that SMEE has an inhibitory effect on AD, and may serve as a useful biomaterial for the development of cosmeceuticals.

Allergy Prevention Effect of Oral Administration of Quercetin on Picryl Chloride-induced Contact Dermatitis in BALB/c Mice (BALB/c mice에서 quercetin의 경구투여가 picryl chloride로 유발된 접촉성 피부 알레르기의 예방에 미치는 영향)

  • The number of patients with immune- mediated dermatitis such as contact dermatitis is increasing year by year. Allergic contact dermatitis is a complex phenomenon that involves resident epidermal cells, fibroblasts, and endothelial cells, as well as invading leukocytes that interact with each other under the control of a network of cytokines and lipid mediators. It is a cell-mediated immune reaction, which occurs after susceptible individuals are exposed to sensitizing chemicals, and characteristic eczematous reaction is seen at the point of contact with an allergen. In this study, we investigated the allergy prevention effects of quercetin on mast cell-mediated allergic inflammation in BALB/c mice. BALB/c mice were sensitized with 40 ${\mu}l$ of 1.5% picryl choloride (PCL) to the left and right ear each. Total serum IgE levels and histamine levels were measured by the sandwich ELISA method using mouse IgE, histamine measuring kit. For histopathological examination, paraffin sections were stained with hematoxylin and eosin(HE) or toluidine blue(TB). Ear swelling responses were much weaker in the high-dose group (100 mg/kg) than the control group (0 mg/kg). The number of mast cells showed a significant decrease in the high-dose group (100 mg/kg) compared to the control group (0 mg/kg). Degranulation of mast cells was also confirmed by Toluidine Blue (TB) staining method. Both total serum IgE and histamine levels were significantly decreased in the high-dose group (100 mg/kg) compared to other groups. These findings suggest a certain relationship between the elevation of IgE, histamine levels and the degranulation of mast cells. These results show that the pharmacological actions of quercetin indicate its potential activity for prevention of allergic inflammatory diseases through the down-regulation of mast cell activation.
